Objective To investigate the accuracy of functional magnetic resonance imaging(fMRI) and contrast-enhanced ultrasound on evaluation of the breast cancer patients after neoadjuvant chemotherapy.Methods Forty-eight patients with breast cancer who were confirmed by biopsy,undergoing 4 cycles weekly dose-concentrated programs (wPC) neoadjuvant chemotherapy,evaluated the efficacy by fMRI and contrastenhanced ultrasound.Results In pathology,complete remission was 12 cases (25.0%,12/48),non remission was 13 cases (27.1%,13/48).In contrast-enhanced ultrasound,complete remission was 20 cases (41.7%,20/48),non remission was 10 cases (20.8%,10/48).In fMRI,complete remission was 17 cases (35.4%,17/48),non remission was 11 cases (22.9%,11/48).The P values more than 0.05 showed no statistically significant among three methods,the Kappa test showed that fMRI were superior to contrastenhanced ultrasound on evaluating the efficacy of neoadjuvant chemotherapy.Conclusions fMRI and contrast-enhanced ultrasound can be used in evaluating the efficacy of neoadjuvant chemotherapy,however,the data shows fMRI is superior to contrast-enhanced ultrasound.Contrast-enhanced ultrasound must be as the supplementary method.
3.MR molecular probes RGD-USPIO in evaluation effect of ucleoside combination on human hepatoma cell line Bel-7402 in vitro

Objective To evaluate the effect and the anti-tumor mechanism of nucleoside combination on human hepatoma cell line Bel-7402 with Arg-Gly-Asp sequence labeled by ultrasmall superparamagnetic iron oxide (RGD-USPIO).Methods The tumor cells Bel-7402 of logarithmic phlyhase were divided into experimental group and control group,treated with 1 mmol/L nucleoside combination and 1640 medium respectively.The two group were co-cultured for 48 h,and were added RGD-USPIO and co-cultured for 6 h.Then the two groups were proceeded with MR scanning,and the signal intensity of T2WI were measured.After extraction of the total RNA and protein of experiment group and control group,the expression of integrin avβ3 was detected using real-time PCR and Western blot.Results The T2WI signal intensity of experimental group (997.35±42.83) was higher than that of control group (241.05±15.36,t 28.79,P<0.01).Compared with control group,the expression of integrin αvβ3 mRNA in experimental group was (0.22±0.02) times (t=4.50,P<0.01).According to Western blot,the protein bands of experimental group were relatively lighter than that of control group,the expression of integrin αvβ3 in experimental group was lower (t =11.88,P<0.01).Conclusion Nucleoside combination has anti-tumor effect by inhibiting integrin ligand-receptor binding,and the anti tumor mechanism may be related to the induction of tumor cell apoptosis.MR molecular probes can conveniently and accurately evaluate the anti-tumor effect of nucleoside combination on Bel-7402 cells.
4.Correlation between sleep quality and anxiety in CKD patients

Objective To study the correlation between sleep quality and anxiety in patients with chronic kidney diseases (CKD).Method Toally 118 patients with CKD were investigated with Pittsburgh sleep quality index (PSQI) and self-rating anxiety scale (SAS).Results About 75.42% (89/118) patients had poor quality of sleep.The scores by PSQI and the dimensions were higher than those of the norms (all P<0.05),but the total scores by PSQI and the factors of sleep quality,time of falling sleep,duration of sleep,use of hypnosis drugs were all significantly lower than those of the insomnia group (all P<0.01).35.59% (42/118) of the patients had anxiety and the score of SAS was significantly higher than that of the national norm (P<0.01).The total scores of PSQI,sleep quality,sleep time,sleeping time,sleep disorder,daytime function were positively related to the score of SAS (P<0.01).Conclusions The quality of sleep is poor in the CKD patients and there is a positive correlation between the quality of sleep and the anxiety.It is necessary to strengthen the knowledge-related health education and to provide emotional support and individualized nursing services for CKD patients so as to relieve their anxiety and improve their sleep quality.
5.Progress on low protein diet for patients with chronic kidney disease

Nutrition therapy is considered as the basis for prevention and management of chronic kidney disease (CKD), throughout the three-tier prevention strategies of CKD. The primary objective is to delay the disease progression, correct metabolic disorders, and improve the outcomes of CKD. Low protein diet has been recognized as an important therapeutic measure in CKD, but the quantity, quality and source of protein are always the points of contention. Recently, both domestic and foreign guidelines have been updated on the amount of protein intake. In addition to quantity, attention has been paid to the type and diversity of proteins. With the rise of plant-based food consumption and the concept of vegetarian diet, the scientific community began to review the benefits of plant protein again, and a plant-based diet is recommend extensively. Whether the plant-based dietary pattern is also suitable for CKD patients who need a low-protein diet, and whether it could meet the nutritional needs of CKD patients are hot topics, this article reviews the recent progress of these research hotspots.

Halohydrin dehalogenase is of great significance for biodegradation of the chlorinated pollutants, and also serves as an important biocatalyst in the synthesis of chiral pharmaceutical intermediates. A putative halohydrin dehalogenase (HheTM) gene from Tistrella mobilis KA081020-065 was cloned and over-expressed in Escherichia coli BL21 (DE3). The recombinant enzyme was purified by Ni-NTA column and characterized. Gel filtration and SDS-PAGE analysis showed that the native form of HheTM was a tetramer. It exhibited the highest activity at 50 degrees C. The nature and pH of the buffer had a great effect on its activity. The enzyme maintained high stability under the alkaline conditions and below 30 degrees C. HheTM catalyzed the transformation of ethyl(S)-4-chloro-3-hydroxybutyrate in the presence of cyanide, to give ethyl (R)-4-cyano-3-hydroxybutyrate, a key intermediate for the synthesis of atorvastatin.

7. Pedicled anterolateral thigh flap with kiss technique for reconstruction of vulva

Objective:
To report the pedicled anterolateral thigh flap with kiss technique for reconstruction of vulva.
Methods:
From Mar.2015 to Mar.2017, 17 patients with vulvar tumor were treated with radical resection and transfer of the pedicled anterolateral thigh flap with kiss technique.
Results:
All the flaps survived completely with no complication. The contour and function of reconstructed vulva was satisfactory.
Conclusions
The pedicled anterolateral thigh flap with kiss technique for reconstruction of vulva has the advantages of reliable blood supply, low morbidity at donor sites, and good therapeutic effect.

Objective:
To evaluate the relationship between visceral pleural invasion (VPI) and other clinicopathological features in lung adenocarcinoma with tumor size ≤3 cm, and to investigate the impact of VPI on the patients′ prognosis.
Methods:
The clinical and pathological features were retrospectively reviewed in 231 cases of lung adenocarcinoma with the largest diameter of tumor ≤3 cm, following complete resection and systemic lymphadenectomy. VPI was divided into three grades, PL0, PL1 and PL2 according to modified Hammar classification for lung cancer upon elastic fiber staining. Survival analysis was performed by Kaplan-Meier method, and the risk factors for prognosis were explored by Cox proportional hazards model. Patient prognosis was evaluated by progression-free survival (PFS) and overall survival (OS).
Results:
In all 231 cases, the number of patients with VPI was 70 (30.3%), of which 61 cases were PL1 and 9 cases were PL2. The remaining 161 cases (69.7%) had no VPI (PL0). The tumor size (

The Lianhua Qingwen (LHQW) capsule is a popular traditional Chinese medicine for the treatment of viral respiratory diseases.In particular,it has been recently prescribed to treat infections caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2).However,due to its complex composi-tion,little attention has been directed toward the analysis of chemical constituents present in the LHQW capsule.This study presents a reliable and comprehensive approach to characterizing the chemical constituents present in LHQW by high-performance liquid chromatography-Q Exactive-Orbitrap mass spectrometry (HPLC-Q Exactive-Orbitrap-MS) coupled with gas chromatography-mass spectrometry(GC-MS).An automated library alignment method with a high mass accuracy (within 5 ppm) was used for the rapid identification of compounds.A total of 104 compounds,consisting of alkaloids,flavonoids,phenols,phenolic acids,phenylpropanoids,quinones,terpenoids,and other phytochemicals,were suc-cessfully characterized.In addition,the fragmentation pathways and characteristic fragments of some representative compounds were elucidated.GC-MS analysis was conducted to characterize the volatile compounds present in LHQW.In total,17 compounds were putatively characterized by comparing the acquired data with that from the NIST library.The major constituent was menthol,and all the other compounds were terpenoids.This is the first comprehensive report on the identification of the major chemical constituents present in the LHQW capsule by HPLC-Q Exactive-Orbitrap-MS,coupled with GC-MS,and the results of this study can be used for the quality control and standardization of LHQW capsules.
10.Central nervous regulatory mechanism of myocardial ischemia-reperfusion injury in rats: excitability of neurons in hypothalamic paraventricular nucleus

Objective To evaluate the relationship between excitability of neurons in hypothalamic paraventricular nucleus (PVN) and central nervous regulatory mechanism of myocardial ischemia-reperfusion (I/R) injury in rats.Methods Clean-grade healthy adult male Sprague-Dawley rats,in which PVN catheters were successfully implanted,aged 6-8 weeks,weighing 260-300 g,were divided into 4 groups (n =6 each) using a random number table method:sham operation group (S group),myocardial I/R group (I/R group),γ-aminobutyric acid group (GABA group) and L-glutamic acid group (L-Glu group).Myocardial ischemia was induced by occlusion of the left anterior descending coronary artery for 30 min followed by 120-min reperfusion.Normal saline 2.4 μl/h was infused for 50 min via the PVN catheter starting from 10 min before ischemia in S and I/R groups.GABA 30 μmol/L and L-glutamic acid 30 μmol/L were infused for 50 min via the PVN catheter at a rate of 2.4 μl/h starting from 10 min before ischemia in GABA and L-Glu groups.The heart rate (HR) and mean arterial pressure (MAP) were recorded at 10 min before ischemia (T0),beginning of ischemia (T1),30 min of ischemia (T2) and 120 min of reperfusion (T3).Rate-pressure product (RPP) was calculated.Blood samples were collected at T3 for determination of plasma cardiac troponin I (cTnI) concentrations by chemiluminescence assay.Rats were then sacrificed and myocardial specimens were obtained for measurement of myocardial infarct size (IS) and area at risk (AAS),and IS/AAR percentage was calculated.PVN tissues were taken to detect the expression of cfos by Western blot.Results No myocardial infarction was found in group S,and myocardial infarction was marked in the other three groups.Compared with group S,the plasma cTnI concentrations were significantly increased,and the expression of c-fos in PVN was up-regulated in I/R,GABA and L-Glu groups (P<0.05),and MAP,HR and RPP were significantly decreased at T1-3 in I/R and GABA groups and at T3 in group L-Glu (P<0.05).Compared with group I/R,IS and IS/AAR percentage were significantly decreased,the plasma cTnI concentrations were decreased,the expression of c-fos in PVN was down-regulated,and HR,MAP and RPP were decreased at T1,2 in group GABA,and IS and IS/AAR percentage were significantly increased,the plasma cTnI concentrations were increased,the expression of c-fos in PVN was up-regulated,and HR,MAP and RPP were increased at T1,2 in group L-Glu (P<0.05).Conclusion Excitability of neurons in hypothalamic PVN is involved in central nervous regulatory mechanism of myocardial I/R injury in rats:decreased excitability can attenuate myocardial I/R injury and increased excitability aggravates myocardial I/R injury.
