1.Anti-HIV therapy in central nervous system:challenges and strategies

Combined antiretroviral therapy ( cART) is widely used for infections of human immune deficiency virus ( HIV) .However , some antiviral drugs can not reach the effective concentrations in central nervous system due to the hinder of blood-brain barrier ( BBB) , resulting in the formation of viral reservoir in central nervous system .BBB is formed by human brain microvascular endothelial cells ( HBMVECs ) , which are connected by tight junction and a thick basement membrane , and astrocytic end-feet.This paper reviews possible mechanisms of BBB hindrance and anti-HIV drug efflux by transport proteins , as well as effective methods to deliver antiretroviral drugs into brain , including the application of nano technology .
2.Observation of clinical effect of Xuebijing injection adjuvant therapy for severe lung infection

Objective To study the clinical effects and mechanism of Xuebijing injection combined with antibiotics in the treatment of severe lung infection in ICU.Methods 110 cases of ICU severe lung infection were randomly divided into control group(55 cases) and observation group(55 cases).The control group was administered with cefotaxime sodium and sodium benzene azole penicillin,while the observation group was co-administered with Xuebijing injection and cefotaxime sodium,sodium benzene azole penicillin.All treatment lasted for 7 days.Meanwhile,the serum levels of C-reactive protein (CRP),TNF-α,IL-6,COX-2 and SOD were measured before and after the therapy.Results After treatment,the levels of CRP and TNF-α,IL-6 were significantly reduced in the two groups[after treatment CRP,TNF-α,IL-6 levels of the control group:(46.50 ± 17.74) ng/L,(339.50 ± 112.61) ng/L,(141.20 ± 42.66) ng/L;and those in the observation group:(35.60 ± 16.89) ng/L,(268.20 ± 98.47) ng/L,(118.70 ± 39.81) ng/L;the control group:t =7.329,9.682,6.038;the observation group:t =11.012,14.335,14.335,all P < 0.01],and the reduced amplitude of the observation group was significantly lower than that of the control group (t =3.300,P < 0.01).The serum levels of COX-2 and SOD were significantly reduced [after treatment COX-2 and SOD levels of the control group:(189.50 ± 34.52) ng/L,(203.60 ± 67.26) U/mL;those of the observation group:(118.20 ± 25.36) ng/L,(162.30 ± 59.78) U/mL;COX-2:the control group:t =15.021,P < 0.01;the observation group:t =32.931,P < 0.01;SOD:the control group,t =4.183,P < 0.01;the observation group,t =7.682,P <0.01],and the reduced amplitude of the observation group was significantly lower than that of the control group(t =3.404,P <0.01).Conclusion Xuebijing injection combined with antibiotics in the treatment of severe lung infection in ICU has good effects,which is due to the inhibition of COX-2 and SOD to improve inflammation and oxidative stress damage in cells.
3.Application of hospital teaching rounds in clinical teaching practice in hospitals for infectious diseases

Great difficulties were existed in clinical teaching in hospitals for infectious diseases. Therefore,a centralized hospital teaching rounds,a new clinical teaching practice was taken in a hospital for infectious diseases. During seven years' practice,a basic mode of centralized hospital teaching rounds was established,which contains many aspects such as organizational form,time arrangement,case se-lection and operation procedures. Its effectiveness was evaluated accoding to the following aspects:number of people attending hospital teaching rounds,composition of these people,satisfaction of these people, comparison of ward rounds data recorded nowadays and 7 years before and assembling all the rounds records into a book. Results indicated that this measure overcame the limitations of the specialized hospi-tals to some extent and played an important role in improving clinical teaching quality.
4.Test Data Analysis of Blood Donors in Beijing 2001-2008

OBJECTIVE To provide current tendency of viral transmission by test-negative blood components changing among blood donors and to improve the safety of blood for transfusion.METHODS The test data of 1 608 816 blood donors in Beijing Red Cross Blood Center from 2001 to 2008 were analyzed.RESULTS Before voluntary blood donation,the positive rate of anti-HCV was 0.16%,that of anti-HIV was 0.005% and that of anti-TP was 0.15%.After voluntary blood donation,positive rate of these components increased,there were 0.45%,0.017% and 0.48%,respectively,but the positive rate of HBsAg and ALT was decreased.After voluntary blood donation,the positive rate of HBsAg,anti-HCV,anti-HIV and anti-TP were elevated,that of ALT was decreased in greatly.CONCLUSIONS As the increasing risk of viral transmission disease,it is more important for blood safety to screen volunteer blood donors.
5.Pathologic grading and clinical features of chronic hepatitis B.

OBJECTIVETo raise the accuracy of clinical diagnosis of chronic hepatitis B.
METHODSThe correlation between the clinical features, biochemical tests (the serum total bilirubin-TBil, albumin-ALB, prothrobin activity-PTA, alanine aminotransferase-ALT, albumin/globulin-A/G, and r-globulin-r-G) and histopathological data in 202 patients with chronic hepatitis B (CHB) were studied.
RESULTSSome of presenting symptoms and signs were obviously associated with histological grade and stage. The grade of necroinflammatory activity of CHB was associated with the rising TBil, ALT, GGT and the declining ALB, A/G and PTA. The coincidence of clinical diagnosis and pathology was highest in mild chronic hepatitis, 63.8%~79.0%; then was in marked chronic hepatitis, 40.0%~62.5%. The coincidence was lowest in moderate chronic hepatitis, 10.0%~28.2%.
CONCLUSIONSGreat attention should be paid to the significance of symptoms and signs, meanwhile the standard of clinical diagnosis for moderate chronic hepatitis might be relaxed somewhat.

7.Clinical application of conventional magnetic resonance imaging and diffusion-weighted imaging in differentiating histopathological types of small hepatocellular carcinoma

Objective To study the imaging apperances and the diagnostic value of conventional magnetic resonance imaging (MRI) and diffusion-weighted imaging (DWI) in differentiating histopathological types of small hepatocellular carcinoma (sHCC).Methods 40 sHCC confirmed by histopathology were classified into 4 groups according to their degree of differentiation:well (n=6),well-moderate (n=5),moderate (n=27) and moderate-poor (n =2).All patients received conventional MRI and DWI (1.5T,b =0 and 600 s/mm2) before the operation.The ADC values of the sHCC were measured and compared.Results On T1WI,32 lesions showed hypointensity,4 hyperintensity (well) and 4 isointensity (well-moderate =2,moderate =2).On T2WI,hyperintensity was observed in 39 lesions and isointensity in 1 lesion (well).Steatosis in the sHCC was seen in 17 of 40lesions (17/40,42.5 %,well=4,well-moderate=1 and moderate=12).A pseudocapsule was seen in 67.5 % sHCC (27/40,well=4,well-moderate=3,moderate=18 and moderate-poor=2).32 lesions showed hypervascularity on arterial phase,and 8 lesions showed hypovascularity (well=3,moderate =4,moderate-poor=1).On DWI,37 lesions showed hyperintensity,except for 3 lesions with welldifferentiated sHCC which showed isointensity (50%,3/6).The mean ADC values±S.D.of sHCC in the well,well-moderate,moderate and moderate-poor groups were (1.757 ± 0.337) × 10-3,(1.917±0.574)×103,(1.816±0.545)×103 and (1.723±0.217)×10-3,respectively.There were no significant differences among the 4 groups.Conclusion The imaging appearances of wellmoderate,moderate and moderate-poor sHCC on conventional MRI were classical which make diagnosis easy.Hyperintensity on DWI contributed to diagnosis.However,the imaging appearances of some well-differentiated sHCC were atypical.The lesions could be isointensity or hyperintensity on DWI.The combination of conventional MRI and DWI contributed to better diagnosis of sHCC,especial for atypical sHCC.
8.Construction of a GFP/Puro double-labeled lentiviral vector containing CK8 interfering RNA and its effect on cell apoptosis in vitro.

OBJECTIVETo construct a GFP/Puro double-labeled lentiviral expression vector for CK8 silencing and assess the effects of CK8 silencing on cell apoptosis.
METHODSThe siRNA sequences of CK8 were inserted into the lentiviral expression vector GV248 and transfected into 293T cells with the packaging plasmids PMD and SPA. The lentivirus was collected at 24 and 36 h post-transfection. Flow cytometry was used to detect the virus titer and the positive cells were selected with puromycin. The knockdown of CK8 was examined by Western blotting. The effect of CK8 down-regulation on cell apoptosis induced by cisplatin was detected with Annexin V/PI staining.
RESULTS AND CONCLUSIONWe successfully constructed CK8 interference lentiviral vector and obtained a stable cell line with CK8 knock-down that was sensitive to cisplatin-induced apoptosis.

9. Analysis on on-site rescue and traumatic features of victims involved in gas explosion accident in Hangzhou

Objective:
To investigate the situations of on-site rescue and traumatic features of victims involved in gas explosion accident in Hangzhou, so as to provide more data support for emergency medical rescues of the similar incidents of massive casualty.
Methods:
Two medical workers with a certain clinical experience were sent to Hangzhou 120 emergency medical centers to collect data of the on-site rescue on 21st July, 2017, including ambulance call-outs, on-site command and traffic conditions, and on-site triage and evacuation of the victims. They were then sent to the hospitals receiving the victims to investigate the situations of these victims including the general information (such as gender, age, admitted hospitals, and number of admission, discharge, and transferring in the first two weeks after the accident) and injury assessment [such as injury position and type, injury severity evaluation by New Injury Severity Scoring (NISS), and burn severity evaluation for victims with burns].
Results:
(1) A total of 15 ambulances reached the accident site for rescue. The traffic and transportation were jammed and interrupted after this accident. On-site triage and distribution were disorderly conducted. (2) Clinical data of 53 victims were collected, including 24 males and 29 females, with the age of 8 to 70 (34±14) years old. They were sent into 6 hospitals in Hangzhou. Two victims died on the day of accident. Up to two weeks after this accident, 28 (52.8%) victims were discharged from the hospitals and received follow-up in outpatient department. Five victims with severe injuries were transferred to the other hospitals. (3) Based on the results of NISS, the injury severities were mild in 29 (54.7%) cases, moderate in 9 (17.0%) cases, serious in 3 (5.7%) cases, and severe in 12 (22.6%) cases. Those 2 dead victims were classified into the severe category due to the highest NISS score of 75. For all of the victims, skin and soft tissue defects were most common. Six (11.3%) victims were combined with burns. According to the classification of burn severity, there were one case of mild, one case of serious, and 4 cases of severe.
Conclusions
The gas explosion accident in Hangzhou caused massive casualties with complex injuries. The local emergency medical rescue responded quickly, but during the rescue process, lots of aspects should be further improved.
10.Research hotspots and frontiers of childhood hypertension

Objective:
To analyze the research status, hotspots and frontier progress of hypertension in children in English and Chinese literature, so as to provide reference for the early prevention of hypertension in children.
Methods:
The Web of Science core collection database and CNKI database were searched to collect the literature related to the study of hypertension in children from 2000 to 2021, and the CiteSpace 5.8.R3 and VOSviewer 1.6.18 visualization tools were used to analyze the literature characteristics including publications, authors, regions, institutional cooperation, research hotspots and frontiers.
Results:
A total of 22 687 English studies and 4 440 Chinese studies were finally included. According to the analysis results, the number of articles published on hypertension in children was on the rise. The published English articles were mainly University of Toronto and University of Colorado. The main publishing institution of Chinese articles was the First Affiliated Hospital of Peking University. The United States and China took the lead in the number of core journals published in the field of hypertension in children, the United States ranked first in terms of the influence of publications. Keyword co occurrence analysis showed that the high frequency keywords in the English literature included prevalence, risk, obesity, risk factor, body mass index, insulin resistance, overweight, metabolic syndrome, cardiovascular disease and mortality. Chinese high frequency keywords in the literature include obesity, risk factors, adiposis, influencing factors, overweight, prevalence, diabetes, treatment, health education and body mass index. The analysis of keywords showed that 25 burst terms were obtained separately in English and Chinese literature.
Conclusion
In recent years, the research interest on hypertension in children continues to grow and keeps updated, with the research scope expanding significantly, regarding obesity, diabetes and cardiovascular diseases.
