OBJECTIVE:
To investigate the expression and the relativity of RECK,RAGE and MMP-9 in nasopharyngeal carcinoma (NPC) tissues.
METHOD:
RECK, RAGE and MMP-9 were detected with immunohistochemical methods in 64 NPC specimens(30 cases with cervical lymph nodes metastasis, while the other 34 cases are not) and 30 specimens with chronic nasopharyngitis.
RESULT:
RECK hardly expressed in chronic nasopharyngitis and NPC tissues, However, strong expression of RECK could be seen in the surrounding inflammatory cells and matrix of cancer nests. The positive rates of RECK in NPC with and without cervical lymph nodes metastasis were 3.3% (1/30) and 11.8% (4/34), separately, There was no statistical significance between the groups (P > 0.05). The positive rates of RAGE in chronic nasopharyngitis and NPC tissues were 100% (30/30) and 70.3% (45/64) respectively, while MMP-9 were 46.7% (14/30) and 78.1% (50/64). The positive rates of RAGE in NPC with and without cervical lymph nodes metastasis were 86.7% (26/30) and 55.9% (19/34) separately, whereas MMP-9 were 90.0% (27/30) and 67.7% (23/34) respectively. They all showed statistical significance between the groups. MMP-9 had a negative correlation with RECK (r = -0.369, P < 0.05) and a positive correlation with RAGE in NPC (r = 0.471, P < 0.05).
CONCLUSION
The expression of RECK and RAGE in NPC were down-regulated, the expression of MMP-9 was up-regulated. While the expression of RECK in NPC with cervical lymph nodes metastasis was down-regulated, while RAGE and MMP-9 were up-regulated. The abnormal expression of the three genes may be related to the progression of NPC. RECK and RAGE may be involved in the invasion and metastasis of NPC by regulating the expression of MMP-9.

4.Suppression of EphB4 improves the inhibitory effect of mTOR shRNA on the biological behaviors of ovarian cancer cells by down-regulating Akt phosphorylation.

The aim of the present study was to examine the effects of suppression of EphB4 and/or mTOR on the biological behaviors of ovarian cancer cells, and the potential regulatory pathways. Antisense EphB4 vectors and shRNA vectors targeting mammalian target of rapamycin (mTOR) were constructed and transfected into A2780 and SKOV3 cells (two ovarian cancer cell lines). The effects of the antisense EphB4 vectors and the shRNA vectors on the proliferation, apoptosis and invasion of ovarian cancer cells were measured, and the expression of EphB4, mTOR and Akt detected. The results showed that transfection with mTOR shRNA could inhibit growth, induce apoptosis, and reduce invasive ability of ovarian cancer cells, which was accompanied by downregulation of EphB4, mTOR and Akt. The inhibitory effects on cell growth caused by mTOR shRNA alone were weaker than those by antisense pEGFP-C1-EphB4. In the antisense pEGFP-C1-EphB4-transfected cells, it was found that EphB4 knockdown could decrease the mTOR expression and slightly reduce the Akt phosphorylation. Significant suppressive effects on cell growth were observed in cells co-transfected with antisense pEGFP-C1-EphB4 and mTOR shRNA. In co-transfection group, the expression levels of EphB4, mTOR and Akt were distinctly lower than those in other groups. It was concluded that suppression of EphB4 may inhibit the growth of ovarian cancer cells by downregulation of the PI3K/Akt/mTOR pathway, and reverse Akt phosphorylation induced by mTOR shRNA. Inhibition of EphB4 and mTOR combined may cooperatively suppress the biological behaviors of ovarian cancer cells.
5.Collaboration between tertiary hospital and community health service center in comprehensive management of chronic liver diseases in the community

Chronic liver diseases, including chronic hepatitis, autoimmune liver disease, alcoholic liver disease, cirrhosis, fatty liver disease, drug-induced liver injury and so on, are common and important disorders in China. A hospital-community-patient-family integration management model for chronic liver diseases has been implemented since 2018 in Shuangshuinian Community Health Service Center collaborated with Sichuan Provincial People′s Hospital. Patients were taken care by general practitioners in the community health center under the guidance of hospital specialists, and with the active participation of patients and their family. This article introduces this integrated management model and the preliminary accomplishment to provide a reference for the management of patients with chronic liver diseases in the community.
6.Current knowledge, attitudes and behaviors of patients with non-alcoholic fatty liver disease

Objective To investigate the knowledge, attitude and behavior of patients with non-alcoholic fatty liver disease ( NAFLD) , and analyze the relevant factors and problems, in order to promote the establishment of patient′s health behavior and quality of life. Methods A total of 100 patients with non-alcoholic fatty liver disease in gastroenterology clinic had been selected by convenient sampling method, and were used self-designed questionnaire which contained the knowledge, attitude and behavior of patients with NAFLD to survey and analyzed them. Results A total of 100 questionnaires were surveyed and collected (100% collective rate). NAFLD patients had 59. 37% accuracy for fatty liver disease relevant knowledge, attitudes accounting for 89. 00% accuracy, behavior 44. 54%. There were statistically significant between patients′knowledge and attitude (r=0. 295, P<0. 05), knowledge and behavior (r=0. 295, P<0. 05), and without statistical significance between attitude and behavior (r =0. 700, P >0. 05). Conclusions The knowledge of patients with NAFLD still needs to be improved, and the attitudes and lifestyle behaviors need further guidance. Nurses play a significant role in improving the patient′s cognitive level and helping patients establish a correct attitude, correct bad habits, and improve quality of life.
7.Experimental study of controlled release microencapsulated Salmonella typhi capsular polysaccharide vaccines immunized mice.

Salmonella Typhi capsular polysaccharide vaccines were encapsulated in the Micro-particles made from polyethylene glycol-poly-DL-lactide (PELA). BALB/c mouse were divided into three groups with 20 mice in each. Mouse were immunized respectively with controlled release microencapsulated Salmonella Typhi capsular polysaccharide vaccines and Salmonella Typhi capsular polysaccharide vaccines by oral and subcutaneous administration. The mice blood and salvia were collected at the 2nd, 4th and 8th weeks respectively for the titrating of IgG and sIgA antibodies by RIA. At the 8th week, live typhoid bacteria were injected into the immunized mice for the calculation of the rate of immunization protection. The IgG titers of the controlled release microencapsulated Salmonella Typhi capsular polysaccharide vaccines group were higher than those of the other groups(P < 0.05). The IgA titers of the low groups of controlled release microencapsulated Salmonella Typhi capsular polysaccharide vaccines (oral and subcutaneous) were higher than those of the group of Salmonella Typhi capsular polysaccharide vaccines (P < 0.05). The immunization protection rates of the three groups were 40%, 100% and 60% respectively. The controlled release microencapsulated Salmonella Typhi capsular polysaccharide vaccines possess the advantages of releasing slowly in vivo and persisting long time immunogenicity.

9.Construction of the evaluation index system for comprehensive budget management in public hospitals

Objective:To construct an evaluation index system for comprehensive budget management at public hospitals and conduct empirical research for reference in evaluation of comprehensive budget management at public hospitals.Methods:From May to June 2022, based on the Implementation Measures for the Comprehensive Budget Management System of Public Hospitals and its interpretations, literature analysis, and expert interviews, a preliminary evaluation index system was developed for comprehensive budget management of public hospitals. Then the Delphi method was applied for two rounds of expert consultation, with experts invited to evaluate the importance of indexes, while the analytic hierarchy process was used to calculate the weight values of indexes. Based on such a comprehensive budget management evaluation index system, a questionnaire was developed, and tertiary public hospitals were selected based on the principle of convenient sampling. Then a questionnaire survey was conducted from June to October 2022. The questionnaire was filled out by hospital level leaders(chief accountants), finance/audit leaders, and directors of finance/audit/operation management departments from the sample hospitals with professional knowledge and rich operational management experience in budget management. The comprehensive budget management level of the sample hospital was thus evaluated based on the survey results. Results:The comprehensive budget management evaluation index system for public hospitals was composed of 2 first-level indexes, 4 second-level indexes, and 20 third-level ones. The weights for the 2 first-level indexes of target building and management orientation were both 0.500, while the weights for the 4 second-level indexes, namely strategic implementation, comprehensive coverage, performance evaluation, and adaptation matching were all 0.250. Among the third-level indexes, the weights of budget execution, system improvement, system establishment, organizational construction, and revenue and expenditure business ranked top five, with weights of 0.070, 0.069, 0.066, 0.066, and 0.064, respectively. A total of 95 hospitals were selected for questionnaire survey, and the evaluation found 46 of them with a comprehensive budget management level of " excellent" and " good" , 27 with a " moderate" level, and 22 with a " poor" and " very poor" level. Scores of the third-level indexes were between 39-94 points, while the score of 6 third-level indexes were below 60 points, namely interconnection, budget analysis, compatibility and adaptation, system improvement, participation level, and assessment application.Conclusions:The evaluation index system for comprehensive budget management at public hospitals in this study proves rational and practical, serving as a reference for the evaluation of comprehensive budget management at public hospitals and related researches.

Objective To evaluate the effect of conversion from mycophenolic acid (MPA) to mizoribine (MZR) in renal transplant recipients with gastrointestinal tract (GI) symptoms.Methods A total of 355 renal transplant recipients with GI symptoms caused by MPA administration were enrolled from April 2015 to March 2017 in 25 different renal transplant centers in China.The symptomatic improvement of GI before (baseline) and after conversion to MZR (1,2,4 weeks) was assessed by each item of GI symptoms indication.In addition,the efficacy and safety of the conversion therapy during 12 months were determined.Results Patients showed improvement in GI symptoms including diarrhea,abdominal pain,abdominal distention and stomachache after conversion to MZR 1,2,4 weeks (P<0.05).In patients with different severity of diarrhea,conversion to MZR therapy significantly improved diarrhea (P<0.05).During 12 months,no patient experienced clinical immune rejection.We did not observe any infections,leucopenia and other serious side effects.Conclusion MZR could markedly improve GI symptoms caused by MPA administration in renal transplant recipients.
