2.Outcomes of subtotal proctocolectomy for familial adenomatous polyposis: a follow-up study of 21 clinical cases

Objective:To evaluate the short-and long-term outcomes of subtotal proctocolectomy for familial adenomatous polyposis(FAP).Methods: Twenty-one FAP patients who had undergone subtotal proctocolectomy during 1985-2000 in our department were followed up colonoscopically to observe whether there was any recurrence of polyps in postoperatively residual colon or/and rectum.Results: Adenomatous recurrence with variant degrees was detected in the residual colon or/and rectum in all 21 patients,the recurrence rate being 100%.Polypous growth was denser in the residual rectum than in the proximal residual colon.The adenomatous polyp in one patient cancerized 4 years after the initial operation.The cumulative occurrence of colorectal cancer was 4.8%.Conclusion: Subtotal proctocolectomy is a simple procedure for FAP with fewer complications,but there is a risk of polyps cancerization in the residual colon or/and rectum.

4.Familial adenomatous polyposis-associated desmoid tumors

Desmoid tumor(DT),a rare benign tumor sometimes with local invasion,is more frequently seen in patients with familial adenomatous polyposis(FAP) than in the general public,which might be associated with surgical trauma,genetic predisposition and hormonal factors.Fifty percent of FAP-associated DTs are intra-abdominal,usually present as non-tender,slowly growing masses.The symptoms include abdominal pain,vomiting and diarrhea,etc.Mesenteric DT can cause obstruction and ischemia of intestine,hydronephrosis and fistula.Diagnosis can be made through biopsy or CT scanning.Surgery should be considered for the abdominal wall DTs and medicine is the first choice for mesenteric DTs.Surgery should be chosen with caution.

Objective:To observe the biological behaviors of colorectal cancer LS174 cells before and after pcDNA3.0-hugl-1 transfection,so as to investigate the association of hugl-1 with colorectal cancer.Methods:The eukaryotic expression vector pcDNA3.0-hugl-1 was constructed and transfected into LS174 cells.RT-PCR and Western blotting methods were used to analyze the expression of hugl-1 mRNA and protein in LS174 cells before and after transfection.Soft agar colony formation assay,wound-healing experiment,adhesion assay and Matrigel invasion assays were used to study the effects of hugl-1 expression on the proliferation,adhesion,movement and invasion in LS174 cells.Results:The recombinant plasmid pcDNA3.0-hugl-1 was successfully constructed.RT-PCR and Western blotting showed that the hugl-1 expression was higher in cells transfected with pcDNA3.0-hugl-1 than in those un-transfected or empty vector-transfected cells (P
6.Endoscopic ballon dilatation for benign colorectal anastomosis strictures

Objective To study the effeciency of endoscopic balloon dilat ation(EBD) for benign structures of colorectal anastomosis. Methods Fifteen cas es with benign stenosis of colorectal anastomosis(BCAS) were treated with endosc opic balloon dilatation,of which sigmoidectomy was performed in one case,and ant erior resection was performed in 14 case;stapler was used in 13 case,and hand su tura was used in 2 cases.Diameters and lengths of strictures were ranging from 0 .2~ 0.7 cm and 0.5~ 2.0 cm respectively. Results Strictures of all 15 patients were dilated successfully.There were no complications in this series.Follow- u p has been from 3~ 30 months without recurrence of symptoms. Conclusion EBD is safe,efficient and well tolerated,and should be considered as the first choice o f treatment of BCAS.

Objective Analysis the features of extracolonic cancer of HNPCC.Method 32 families with HNPCC were registered and followed up.Results Among 12 families with typical HNPCC,8 cases in 6 families and 1 case of nontypical HNPCC families developed extracolonic cancer. Endometrial and gastric cancers are the most common cancers.Conclusions The extracolonic cancers are the important part in the cancer spectum of HNPCC. Among Chinese families, endometrial and gastric cancers are more common.
10.Clinical safety of preoperative lymphatic chemotherapy in the treatment of rectal cancer

Objective To investigate the clinical safety of preoperative lymphatic chemotherapy in the treatment of reetal cancer.Methods The regional and systemic symptoms,postoperatwe stoma healing,haematogenesis.functions of hean,liver and kidney after lymphatic chemotherapy,and the level of CD3+,CD4+,CD8+,CD4+/CD8+,CD(16+56)+in blood 30 minutes before and 48 hours after lymphatic chemotherapv were detected.Results There were no significant effects of lymphatic chemotherapy on the regional and systemic symptoms,postoperative stoma healing,haematogenesis and the functions of heart,liver and kidney.The level of CD4+/CD8+48 hours after lymphatic chemotherapy was significantly increased(t=7.145,P<0.05),while no significant changes of CD3+,CIM+,CD8+,CD(16+56)+were detected(t=1.782,1.151,1.184,0.955,P>0.05),when compared with those 30 minutes before lymphatic chemotherapy.Conclusions Preoperative lymphatic chemotherapy is safe and can enhance patients'immunity in early stage.
