1.Pulmonary epithelioid hemangioendothelioma:a case report and review of the literature

Objective To describe the clinical features of pulmonary epithelioid hemangioendothelioma and heighten the ratio of diagnosis.Methods With a case report and review of the related literature,the etiology,clinical manifestations,pathology diagnosis,differential diagnosis,management and prognosis of pulmonary epithelioid hemangioendothelioma were described.Results The etiology of this rare disease remained unknown.Symptoms were scanty and usually mild.Chest radiograph or computed tomography usually revealed multiple bilateral pulmonary nodules.Primitive lumen formed by a single cell was the pathologic feature.Immunohistochemical stains showed that the malignant cells are of the endothelial type.There was no effective treatment for this disease and its prognosis was unpredictable.Conclusion Pulmonary epithelioid hemangioendothelioma is rare and often misdiagnosed as other pulmonary diseases.
2.The changes of SARS-IgG,X-ray of lung and lung function in patients with severe acute respiratory syndrome .

Objective To investigate the changes of SARS-IgG in serum,X-ray of lung and lung function in severe acute respiratory syndrome(SARS)patients.Methods During the period of 2003-06~2004-04,the SARS-IgG in serum,X-ray of lung and lung function tests were checked up in 103 SARS patients by Guangzhou Institute of Respiratory Disease in regularly every 3 months after their discharge from hospital and the SARS-IgG level in serum was compared with another 20 healthy medical workers.Results 1.On the third day after the onset of SARS disease,the optical density(OD)value of SARS-IgG in serum was(0.063?0.012)which was significantly higher than the normal control group. Moreover,from then on,it increased step by step and went up greatly in the third week(0.180?0.202)and attained the highest level(1.730?0.331)in the fourth month. Then it decreased step by step and in the 17th month,it dropped to(0.668?0.109).2.The chest X-ray of 32.3%of 62 cases became normal in the first 3 months after onset of SARS and that of 56.5% did in the 17th month.However,if 22 cases were taken into account,who did not have a re-check,80% of the cases became normal and the left 5 cases were severe patients. 3.The lung function of 16 of 25 cases became normal in 6 months after onset and among the other 9 cases,the lung function of 4 cases had an improvement in the following 3 to 6 months.Conclusion The OD value of SARS-IgG in serum in the third day can be used as a screening test of SARS. Its remarkable increase in the third week will support the diagnosis.The chest X-ray and lung function of SARS patients will improve gradually and become normal in the final.
3.Pulmonary phaeohyphomycosis due to Exophiala jeanselmei:a case report and review of literature

Objective To better understanding the clinical presentations of phaeohyphomycosis,and improve the diagnosis and management of the disease.Methods We reported a case of pulmonary phaeohyphomycosis caused by Exophiala jeanselmei at the First Affiliated Hospital of Guangzhou Medical University in 2008,and reviewed the relevant literature.The clinical,radio-logical and etiological features were summarized based on this case and the other 23 phaeohyphomycosis patients reported in China from January 1995 to August 2013.Results 24 Chinese cases of phaeohyphomycosis have been reported to date,including 15 males and 9 females.The age of these patients ranged from 4 to 76 (mean 40.0±21 .8)years old.Seventeen patients were otherwise healthy.The other 7 patients had complications.Clinical presentations of phaeohyphomycosis vary widely,including cutaneous and subcutaneous infection in 18 cases,pulmonary and central nervous system involvement in two cases each,para-nasal sinus and palpebral conjunctiva infection one case each.The diagnosis of 18 cases were confirmed both microbiologically and histologically.One case was confirmed histologically alone.Five cases were identified microbiologically alone.The samples for culture were collected from skin abscess (1/5 ),pulmonary tissue (2/5 ),and cervical spinal fluid (2/5 ),respectively. Twenty-two strains of causative organisms were identified,7 of which were Exophiala jeanselmei .Twenty-three patients received treatment.They were cured by antifungal agents alone (18)or in conjunction with surgical resection (4 ),or assisted with XD-635AB-based photodynamic laser therapy (1).Specifically,10 pa-tients were cured by itraconazole alone.Conclusions In China, most patients of phaeohyphomycosis have concurrent conditions or have previously received immunosuppressive agents and cor-ticosteroids.Cutaneous and subcutaneous infection were most common,located mainly on limbs,face,chest and abdominal skin.The most frequently isolated pathogen is Exophiala jeanselmei ,followed by Phialophora verrucosa and Exophiala spinifera .Itraconazole therapy would be very effective.Susceptibility testing is very useful in case of refractory infection.

5.Efficacy and safety of SpyGlass system combined with electrohydraulic lithotripsy during endoscopic retrograde cholangiopancreatography in treatment of difficult common bile duct stones

ObjectiveTo investigate the efficacy and safety of SpyGlass system combined with electrohydraulic lithotripsy during endoscopic retrograde cholangiopancreatography (ERCP) in the treatment of difficult common bile duct stones. MethodsA total of 20 patients who used the SpyGlass system combined with electrohydraulic lithotripsy during ERCP for the treatment of difficult common bile duct stones in Department of Hepatobiliary Surgery, The First Affiliated Hospital of Chongqing Medical University, from December 2019 to May 2020 were enrolled. Liver function, blood amylase, and blood lipase were measured after surgery; the patients were observed in terms of body temperature, abdominal pain, hematobilia, infection, and perforation; the presence or absence of residual common bile duct stones was reexamined after surgery. ResultsOf all 20 patients, 18 underwent successful lithotripsy on the first attempt without serious complications, and 2 underwent ERCP-guided basket extraction and biliary tract cleaning with balloon due to residual stones found in the reexamination after surgery and achieved complete removal of stones. No biliary tract perforation or massive hemorrhage was observed; there was mild pancreatitis after surgery, which was improved after pharmacotherapy; some patients had increased levels of blood amylase and lipase, without any clinical symptom; there was a significant improvement in liver function after surgery. Abdominal color Doppler ultrasound performed for all patients at 3 months after cholecystolithotomy showed no residual common bile duct stones. ConclusionSpyGlass system combined with electrohydraulic lithotripsy during ERCP has a high first-attempt success rate of complete removal of stones in the treatment of difficult common bile duct stones, without marked surgical complications.
6.Self-microemulsifying drug delivery system of patchoulic alcohol to improve oral bioavailability in rats.

OBJECTIVETo improve the oral bioavailability of patchoulic alcohol in rats by using self-microemulsifying drug delivery systems (SMEDDS).
METHODPatchoulic alcohol was separated and purified from patchoulic oil, and the SMEDDSs including patchoulic alcohol or patchoulic oil were optimized by pseudo-ternary phase diagrams via central composite design-response surface methodology. Pharmacokinetics of both SMEDDSs and patchoulic alcohol itself in rats were investigated.
RESULTThe patchoulic alcohol SMEDDS (Cremophor EL-Tween 80-PEG 400-isopropyl myristate-patchoulic alcohol, 2:2:0.8:1.95:0.65) was considered as the optimized formulation. The mean drop size of the system was 30. 1 nm after diluted 100 folds in water. The average self-microemulsifying time was 142 s. Patchoulic alcohol SMEDDS and patchoulic oil SMEDDS showed no signficant difference in Tmax compared with patchoulic alcohol with around 60 minutes, while the AUCs of both SMEDDSs (2001 745.6 +/- 329 663.6) and (1594 005.6 +/- 280 150.3) microg x min x L(-1) were significantly higher than that of patchoulic alcohol (1 163 153.3 +/- 232 324.3) microg x min x L(-1).
CONCLUSIONSMEDDS can effectively improve the oral bioavailability of patchoulic alcohol in rats.

7.Dynamic Characteristics of Lenke3 Type Idiopathic Scoliosis

Objective To investigate dynamic response of the finite element model of Lenke3 type scoliosis. Methods The finite element model was established based on CT scanning images from a patient with Lenke3 type scoliosis, and validation of the model was also conducted. Modal analysis, harmonic response analysis and transient dynamic analysis were carried out on the model. Results The first order natural frequency of this model was only 1-2 Hz.The amplitude of the finite element model was the largest at the first natural frequency. At the same resonance frequency, the amplitude of the thoracic curved vertebra was larger than that of the lumbar curved vertebra.The amplitude from T6 vertebra to L2 vertebra decreased successively. Conclusions The degree of spinal deformity may affect the perception of spine vibration, and the higher the degree of spinal deformity, the higher the sensitivity to vibration. The first natural frequency is most harmful to Lenke3 type scoliosis patients. Under cyclic loading, the thoracic curved vertebra is more prone to deformation than the lumbar curved vertebra. The closer to T1 segment, the greater the amplitude of the vibration is.
8.CT and MRI features of intraosseous myofibroma/myofibromatosis in children

Objective To investigate the CT and MRI features of intraosseous myofibroma/myofibromatosis in pediatric patients.Methods The retrospective analysis involved the examination of clinical data and imaging findings from 15 children who were diagnosed with myofibroma/myofibromatosis of bone invasion through pathological means.Subsequently,the imaging characteristics were summarized.Results CT examinations were conducted on a total of 15 patients,with 2 of them also received enhanced scans.Additionally,MRI examinations were conducted on 5 patients,with 3 of them also underwent enhanced scans.Eleven patients were diagnosed with solitary type myofibroma,with 7 cases localized in the skull and the remaining lesions observed in the maxillofacial bone.Three patients exhibited the multicentric type without any involvement of visceral organs,while one patient presented with the multicentric type accompanied by visceral involvement.The lesions exhibited a uniform soft-tissue density on plain CT scan,predominantly located between the inner and outer layers of the bone.Additionally,they displayed swelling changes and osteolytic bone destruction,with some lesions showed residual bone shell.On MRI,the lesions exhibited a uniform signal,demonstrated an isointense or slightly hypointense signal on T1WI and an isointense or slightly hyperintense signal on T2WI.The lesions displayed significantly heterogeneous enhancement on CT and MRI.Conclusion The imaging manifestations of intraosseous myofibroma/myofibromatosis in pediatric patients exhibit certain characteristics,and the residual bone shell in the lesion is helpful for diagnosis,however,distinguishing it from Langerhans cell histiocytosis of the bone remains challenging,necessitating the reliance on pathological diagnosis.
9.Accuracy of prediction amount of length of gastric tube placed through nose in critically ill children

Objective:To improve accuracy of prediction amount of length of gastric tube placed through nose and reduce occurrence of adverse events thorough clinical observation of measurement of length of nasogastric tube placement in critically ill children.Methods:Using the convenient sampling method, critically ill children who were hospitalized and needed a nasogastric tube in Pediatric Intensive Care Unit (PICU) of Beijing Children's Hospital Affiliated to Capital Medical University were selected from April to September 2019. The prediction method of "nos-ear-xiphoid (NEX) increased by 5 cm" (NEX+5 cm) was adopted. The values of placed length in this study were collected and compared with those predicted by traditional measurement method (namely NEX) , improved "nose-ear-mid-umbilicus" (NEMU) and formula method.Results:A total of 52 critically ill children were enrolled in this study. The length of placed nasogastric tube was 31.5 (28.3, 35.8) cm, and 43 cases (82.7%) were determined to be qualified by X-ray. The length of gastric tube required to be placed in children was 27.0 (24.1, 31.0) cm according to the NEX method, and the length of gastric tube required to be placed in children was 26.1 (22.5, 29.0) cm measured by the formula method. Both were shorter than that measured by NEX+5 cm, and the differences were statistically significant ( P<0.01) . The NEMU method measured the length of gastric tube to be inserted into the child to be 31.0 (28.3, 36.0) cm. Compared with the length measured by NEX+5 cm, and the difference was not statistically significant ( P>0.05) . Conclusions:This study uses NEX+5 cm to predict the actual length of the gastric tube inserted through the nose. The accuracy is relatively high and the operation method is simple. It is necessary to consider individual differences in clinical applications, especially the large variability in infants and young children. After catheterization, abdominal ultrasound, X-ray and other auxiliary examination methods should be used to determine the location of catheterization, and individualized catheterization programs should be given to children according to different therapeutic objective.
10.Antitumor Study of Neoantigen-reactive T Cells Co-expressing IL-7 and CCL19 in Mouse Lung Cancer

Background and objective Neoantigen reactive T cell(NRT)has the ability to inhibit the growth of tumors expressing specific neoantigens.However,due to the difficult immune infiltration and the inhibition of tumor micro en-vironment,the therapeutic effect of NRT in solid tumors is limited.In this study,we designed NRT cells(7×19 NRT)that can express both interleukin-7(IL-7)and chemokine C-C motif ligand 19(CCL19)in mouse lung cancer cells,and evaluated the difference in anti-tumor effect between 7×19 NRT cells and conventional NRT cells.Methods We performed next-generation sequencing and neoantigen prediction for mouse Lewis lung carcinoma(LLC),prepared RNA vaccine,cultured NRT cells,constructed retroviral vectors encoding IL-7 and CCL19,transduced NRT cells and IL-7 and CCL19 were successfully ex-pressed,and 7×19 NRT was successfully obtained.The anti-tumor effect was evaluated in vivo and in vitro in mice.Results The 7×19 NRT cells significantly enhanced the proliferation and invasion ability of T cells by secreting IL-7 and CCL19,achieved significant tumor inhibition in the mouse lung cancer and extended the survival period of mice.The T cell infiltration into tumor tissue and the necrosis of tumor tissue increased significantly after 7×19 NRT treatment.In addition,both 7×19 NRT treatment and conventional NRT treatment were safe.Conclusion The anti-solid tumor ability of NRT cells is significantly enhanced by the arming of IL-7 and CCL19,which is a safe and effective genetic modification of NRT.
