1.Clinical study on gut barrier dysfunction and bacterial translocation in patients following gastrointestinal surgery

Objective To investigate gut barrier dysfunction and bacterial translocation (BT) from gastrointestinal tract in postoperative patients using polymerase chain reaction (PCR) and plasma D-lactate measurement, and to study the relationship between BT and infections complications. Methods 34 patients undergoing selective gastrointestinal operations were observed. Blood samples were collected prior to and 2, 24, 48 h after surgery for bacterial culture, plasma D-lactate, and LPS measurement. PCR was used to detect microbial DNA. PCR was performed after DNA extraction, with target ?-lactosidase gene of E. coli and 16S rRNA gene of most pathogenic bacteria. All the patients were observed for 30 days for monitoring infectious complications. Results No bacterial DNA was detected before surgery, but it was positive in 7 patients (20.6%) after operation. 6.Analysis on distribution and drug resistance of pathogenic bacteria from blood culture during 2009~2013

Objective To analyze the distribution and drug resistance characteristics of pathogenic bacteria isolated from blood cultures in the infected patients in our hospital during 2009-2013 to provide the newest evidence for the clinical anti-infection ther-apy.Methods The bacterial identification and drug susceptibility test were performed by applying the VITEK-32 System.The dis-tribution situation and the drug susceptibility test results of pathogens isolated from blood culture specimens in our hospital during this period were analyzed.Results 2 301 strains of positive bacteria were isolated from 14 006 cases of blood culture during these five consecutive years.The positive rate was 16.4%.Among them,1 303 strains were Gram positive bacteria(56.6%),954 strains of Gram negative bacilli(41.5%)and 44 strains of fungi(1.9%).Gram-positive bacteria were mainly Staphylococcus aureus,coagu-lase negative staphylococci(CNS),etc.Gram-negative bacteria were mainly E.coli,Klebsiella pneumoniae,etc.E.coli and Klebsiella pneumoniae were highly sensitive to carbapenem,amikacin,cefoxitin and antibacterial drugs containing enzyme inhibitor.The detec-tion rates of extended-spectrumβ-lactamase(ESBL)-producing Escherichia coli and Klebsiella pneumoniae were 55.8% and 18.4%respectively.Acinetobacter baumannii in blood culture had serious resistance to most of antibacterial drugs.Conclusion The kinds of pathogens isolated from blood culture are complex with different sensitivity to antibacterial drugs.Timely understanding the re-sults of blood culture has the important significance to adjust the treatment scheme in clinic and timely discover drug-resistant strains for serve clinic better.

9.Prokaryotic expression and polyclonal antiserum preparation of Schistosoma japonicum Mago nashi like protein

Objective To express Schistosoma japonicum Mago nashi(SjMago)gene,and prepare its specific polyclonal antibody.Methods SjMago gene was amplified by PCR from Schistosomulum cDNA library and subcloned into pET28a(+)vector,its recombinant proteins were expressed with IPTG.Rabbits were immunized with the polyacrylamide gel particles containing the recombinant proteins for polyclonal antibody preparation,the sera were detected for antibody specificity by Western blot and titer by ELISA assay.Results SjMago prokaryotic expression plasmid was successfully recombined and the target proteins was induced by IPTG in a molecular weight of 17 X 103,the high titer(1∶40 960)polyclonal antibody was isolated from the immunized rabbit,specific rotein band was detected by Western blot.Conclusion SjMago protein has been successfully expressed and its specific polyantibody is prepared,which lays the foundation for further study.
10.Distribution and drug resistance of Gram positive bacteria of ten hospitals in Chongqing

Objective To investigate the distribution and drug resistance of the gram positive bacteria isolates from hospital in Chongqing during 2006-2011 .Methods Non-repetitive gram positive bacterias were collected retrospectively from ten hospitals in Chongqing .Antimicrobial susceptibility testing of all clinical isolates were carried out using Kirby-Bauer(KB) .Results 14 473 strains were isolated .There were 4 840 strains of Staphylococcus aureus (33 .4% ) ,2 566 strains of Staphylococcus epidermidis (17 .7% ) ,1 822 strains of Staphylococcus haemolyticus (12 .6% ) ,1 325 strains of Enterococcus (E .faecalis/E .faecium )(9 .2% ) . Staphylococcus was highly sensitive to nitrofurantoin ,rifampicin ,piperacillin/tazobactam ,linezolid ,teicoplanin and vancomycin ,but their resistance rate to ampicillin/sulbactam ,oxacillin ,penicillin and clarithromycin was high .The resistance rate of E .faecium to the majority of antimicrobial agents was obviously higher than that of E .faecalis ,excluding imipenem ,chloramphenicol and tetracy-cline .Vancomycin and linezolid still remain the strongest antibacterial activity to Enterococcus .Conclusion The distribution and drug resistance of pathogenic bacteria in Chongqing present regional particularity ,and antibiotic resistance is severe .It is significant to strengthen the epidemiology study of pathogenic bacteria and to develop policies to intervene in the use of antimicrobial agents .
