1.The clinical study of CD64 in infected children treated in ICU of Shanghai Children Hospital

Objective To evaluate the values of CD64 expression in diagnosis of infected patients referred to intensive care unit.Method Sixty febrile children referred to the hospital intensive care unit from 2009.11 to 2010.03 were enrolled for a retrospective study.Fever was defined as a body temperature reaching 38℃ or higher with specifically bacterial infection or highly suspected with bacterial infection or viral infection.There were 28 patients with bacterial infection and 32 with viral infection.The non-infectious diseases such as juvenile rheumatoid arthritis and Kawasaki disease were excluded.The controls were 50 healthy children asking for physical examination.On admission,CD64 were measured by using flow cytometry,and blood routine examination,ESR,PCT,blood cultures and sputum cultures were simultaneously detected in all febrile patients.Data were statistically analyzed by using SAS 16.0 software.Data are given as means±SE.Categorical variables were analyzed using X2 test and continuous variables were compared by applying paired 1-tailed t test,Significance level was set at less than 0.05.Results of them,57.1%bacterial infection patients and 71.9%viral infection patients contracted pneumonia.CD64 in bacterial infection patients、viral infection patients and the subjects of control group were(12.6±9.7),(5.4±2.42)and (2.9±0.77),respectively.The CD64 in the bacterial infection patients were significantly higher than those in the virus infection patients(F=11.002,P=0.004).Conclusions CD64 in infected children referred to a hospital intensive care unit can be clearly distinguished between bacterial infections and viral infections, providing an important guidance and a flexible strategy for clinical treatment and determine the timing of withdrawal.
2.Application and Practice of Intelligent Management and Solution for Clinical Rational Drug Use in Our Hos-pital

OBJECTIVE:To improve the level of rational drug use,and to reduce the workload of clinical pharmacists. METH-ODS:Established“Intelligent Management and Solution for Clinical Rational Drug Use”of our hospital was introduced,including post-prescription review(all prescriptions review)system, self-maintenance system for rational drug use knowledge base,pre-pre-scription review (rational drug use real-time review) early warning system. The effects of intelligent system were evaluated. RE-SULTS & CONCLUSIONS:After the application of the system,the following aspects could be achieved,including“all prescrip-tions review”,“multi-prescriptions/longitudinal review”,system knowledge base real-time update and autonomous definition of re-view rules,effective communication and intervention between physicians and pharmacists before printing prescription by Yaozhidao instant messaging system. Through related trial evaluation,the proportion of false positive prescriptions decreased by 17.65% after 4 months of the application of the system;drug consultation prescriptions review average score of outpatient pharmacists increased from 81 to 94.85;the time of prescription review decreased by 4.72-5.46 times. Through mandatory interception of unqualified pre-scriptions,severe warning,timely prompt and pharmacists’intervention,the rate of unqualified prescriptions pre-reviewed by the system in respiration department decreased from 32% in Jan. 2015 and 21% in May to 4.62% in Dec. So,the application of the system promotes rational drug use in outpatient department,reduces the workload of pharmacists and ensures the safety of drug use comprehensively.
3.Correlation of IL-28B gene polymorphism and natural outcome of hepatitis C

Objective:To explore the association between interleukin(IL)-28B single nucleotide polymorphisms and natural outcome of hepatitis C virus.Methods:The IL-28B rs12979860 locus was genotyped in 266 HCV infected volunteer blood donors(107 spontaneous cleared and 159 chronic infection) and 97 healthy controls using Sanger sequencing assay.The difference in rs12979860 genotypes and allele frequencies between the six groups(107 spontaneous cleared and 159 chronic infection,266 HCV infection and 97 healthy controls,159 chronic infection and 97 healthy controls) were analyzed by statistics.Results:159 HCV chronic infection,107 spontaneous cleared and 97 healthy controls,were shown more CC genotype,accounting for 83.6%,95.3%and 86.6%,respectively, while the CT genotype accounted for 16.4%,4.7%and 13.4%respectively.No TT genotype was found.The CC/CT genotype was not significant difference between HCV infection and healthy controls,chronic infection and healthy controls(χ2=0.204,P=0.652;χ2=0.406,P=0.524),but between chronic infections and spontaneous clearance had statistically significant(χ2=8.474,P=0.004),the frequence of C allele in spontaneous cleared was higher than HCV chronic infection(χ2=7.949,P=0.005).Conclusion: The gene polymorphism of IL-28B rs12979860 is not related to HCV susceptibility,but there are differences in chronic infection and spontaneous cleared,showing the C allelic in favor of HCV spontaneous cleaed.
4.Experimental study on transcription regulation of mouse hepcidin gene by NF-kB.

OBJECTIVETo investigate the involvement of NF-kappaB (NF-kB) regulation of hepcidin gene transcription in acute phase response and its molecular mechanism.
METHODSFirst, a mouse model of acute phase response was established by intraperitoneal injection of LPS. The relationship between hepcidin expression and dose or time of LPS injection was assessed. Then, electrophoretic mobility shift assay (EMSA) was performed to explore the possibility of the involvement of NF-kB in regulation of hepcidin gene transcription. Next, pAVU6+27-NF-kB, NF-kB p65-specific siRNA expression vector was constructed and transfected into mouse primary hepatocytes using DOTAP liposomal transfection reagents. Hepcidin expression changes after silencing of NF-kB p65 and hepcidin expression after LPS induction were tested.
RESULTSHepcidin expression showed a time and dose-dependent manner with regard to LPS injection. At 10 h after 50 microg LPS injection, hepcidin expression reached its peak. The result of EMSA exhibited an evident lag band at -53 - -64 bp, indicating regulation of hepcidin gene expression by NF-kB. After mouse primary hepatocytes were transiently transfected with NF-kB p65-specific siRNAs, Western blot showed that inhibition rate of NF-kB expression was 50%-67%. Hepcidin expression of transfected hepatocytes dropped down obviously in comparison with that of untransfected hepatocytes, and could not be induced by LPS.
CONCLUSIONTranscription factor NF-kB is likely to be an important molecule in transcription regulation of hepcidin gene. As a key component, p65 subunit binds to hepcidin gene at -53 - -64 bp, and upregulates hepcidin expression.

5.Expression of PTN gene in glioma, hepatoma and laryngeal squamous cell carcinoma

Objective:To investigate the role of pleiotrophin (PTN) gene in carcino genesis using cDNA microarray and in situ hybridization. Methods:The expression of PTN gene in 5 cases of glioma, 10 laryngeal squamous cell carcinoma, 6 cases of hepatocarcinoma, and normal controls were detected by BioDoor 4096 type cDNA microarray and in situ hybridization. Results: The expression of PTN gene in carcinoma samples were significantly higher than in normal controls by cDNA microarray, the results was the same as by in situ hybridization. Conclusion: cDNA microarray is an effective technique in analysis of functional study of associated genes in carcinoma. High expression of PTN gene might be correlated with mechanism of multiple carcinoma. [
6.Continuous Improvement Practice and Effectiveness of Electronic Pharmaceutical System on Tollying in Outpatient Pharmacy of Our Hospital

OBJECTIVE:To optimize the workflow of electronic pharmaceutical system(ePS),and improve the work efficiency of outpatient pharmacy. METHODS:The problems about the application of ePS on tollying in outpatient pharmacy of our hospital were analyzed. The improvement measures were put forward and implemented. The effectiveness was summarized by evaluating relevant work indicators 6 months before and after optimization. RESULTS:Referring to main problems during the application of ePS on tollying as repetition of tallying drugs,complexity of stocking checking,low efficiency in the management of drug expiry date,deviation in the tracking of"batch/expiry",a series of measures were carried out in outpatient pharmacy of our hospital,including optimizing tallying process,updating tracking process,adding the function of alerting the near-expiry drug, etc.The system optimization shortened the time of replenishment(26.32 s vs. 7.78 s,improving by 3.38 times)and stock checking [(165.50±5.75)s vs.(146.70±4.54)s,shortening by 11.36%],realized accurate tracing of drug information"batch/expiry"and improved drug management [(58.00±1.88)s vs.(18.00±1.23)s,shortening by 68.97%]. CONCLUSIONS:The optimization of ePS on tallying in our hospital improves work efficiency of pharmacists and guarantees the accuracy of tracing drug"batch/expiry".
7.Construction of the grid management system of nursing quality

Objective To explore the methods of nursing quality management in order to provide safety,high quality and satisfactory nursing for patients,Methods To establish one grid management model of nursing quality combined with " double three-level".Then,the report of security events,the implemention of nursing consultation,the incidence of adverse events,and patients' satisfaction were observed and compared before and after the management model.Results Mter implementing the grid management model of nursing quality,satisfaction average scores of hospitalized patients,emergency department patients,discharged patients were ( 98.22 ± 1.12 ),( 91.97 ± 1.67 ),(97.16 ± 2.41 ) respectively,and before it was implemented the average scores respectively were ( 91.59 ± 1.67 ),( 87.92 ± 1.98 ),( 87.89 ± 1.81 ),and the differences were statistically significant ( t =3.57,2.69,4.72,respectively ; P < 0.05 ).There was a significant change in the report of security events,implemention of nursing consultation and checking.The incidence of administration errors,falling down and pressure sores were significantly reduced.Conclusions Construction of the grid management system of nursing quality can ensure the implemention of nursing core institution,improve risk awareness of nurses,improve the nursing quality and increase patients' satisfaction.
8.Palliative primary tumor resection provides survival benefits for the patients with metastatic colorectal cancer and low circulating levels of dehydrogenase and carcinoembryonic antigen

Background: It remains controversial whether palliative primary tumor resection (PPTR) can provide survival benefits to the patients with metastatic colorectal cancer (mCRC) who have unresectable metastases. The aim of this study was to evaluate whether PPTR could improve the survival of patients with mCRC. Methods: We conducted a retrospective study on consecutive mCRC patients with unresectable metastases who were diagnosed at Sun Yat?sen University Cancer Center in Guangzhou, Guangdong, China, between January 2005 and December 2012. Overall survival (OS) and progression?free survival (PFS) after first?line chemotherapy failure were compared between the PPTR and non?PPTR patient groups. Results: A total of 387 patients were identified, including 254 who underwent PPTR and 133 who did not. The median OS of the PPTR and non?PPTR groups was 20.8 and 14.8 months (P < 0.001), respectively. The median PFS after first?line chemotherapy was 7.3 and 4.8 months (P < 0.001) in the PPTR and non?PPTR groups, respectively. A larger proportion of patients in the PPTR group (219 of 254, 86.2%) showed local progression compared with that of patients in the non?PPTR group (95 of 133, 71.4%; P < 0.001). Only patients with normal lactate dehydrogenase (LDH) levels and with carcinoembryonic antigen (CEA) levels <70 ng/mL benefited from PPTR (median OS, 22.2 months for the PPTR group and 16.2 months for the non?PPTR group; P < 0.001). Conclusions: For mCRC patients with unresectable metastases, PPTR can improve OS and PFS after first?line chemo?therapy and decrease the incidence of new organ involvement. However, PPTR should be recommended only for patients with normal LDH levels and with CEA levels <70 ng/mL.

Objective:
To analyze satisfaction with follow-up management of HIV/AIDS cases conducted by Community Health Service Center (CHS) and related factors in Hongkou district, Shanghai.
Methods:
Out of 302 HIV/AIDS cases followed up by CHS in Hongkou district from 2012 to 2016, 263 HIV/AIDS cases were recruited to participate in a cross-sectional study assessed by self-designed questionnaire-based interview during October 1, 2016 and October 20, 2016, with efficiency of 87.1%. Information of basic characteristics including sociodemographic, routes of infection, CD4+T cell counts, diagnose of AIDS and status of receiving ART were collected, as well as satisfaction with follow-up management conducted by CHS including service professionalism, service attitude, service environment, referral service, privacy protection, health education, psychological support, and care and assistance. Factors related to satisfaction were analyzed using multiple binary logistic regression.
Results:
Among 263 HIV/AIDS cases, the average age was 42.0±13.5, 93.2% (245 cases) were male and the proportion of overall satisfaction was 72.2% (190 cases). Out of 8 items of satisfaction, service attitude and health education got the highest score with a total number of 235 (89.4%) answering "very satisfied" or "satisfied" , while care and assistance got the lowest score with a total number of 69 (26.2%) answering "dissatisfied" or "very dissatisfied" . Compared to HIV/AIDS ≤30 years old and receiving ART, those who were 31-50 years or>50 years old and not receiving ART were less likely to be satisfied, the OR (95%CI) were 0.36 (0.15-0.89), 0.32 (0.10-0.97) and 0.11 (0.01-0.90), respectively. Conclusion Satisfaction with follow-up management conducted by CHS in Hongkou district is relatively high. Age, residence, status of receiving ART and cumulative time of following up by CHS are significantly associated with satisfaction, suggesting that CHS should improve their professional abilities during follow-up management, as well as be more targeted and focus on different aspects.
