1.Therapeutic effect of continuous renal replacement therapy on different types of cardiorenal syndrome

Objective:To observe therapeutic effect of continuous renal replacement therapy (CRRT ) on different types of cardiorenal syndrome (CRS) .Methods :According to patient′s conclition ,a total of 247 CRS patients were divided into type 1 group (n=47) ,type 2 group (n=51) ,type 3 group (n=55) ,type 4 group (n=49) and type 5 group (n=45) .All patients received CRRT ,echocardiography and plasma level of N terminal pro B type natriuretic peptide (NT‐proBNP) detect to evaluate cardiac function status ;the 24h urine volume and endogenous creatinine clearance rate (Ccr) were measured to assess renal function status before and after treatment . Results:Compared with before treatment ,on one week after CRRT ,LVEF ,urine volume and Ccr level significantly rose ,NT‐proBNP level significantly reduced ( P<0.05 or <0.01) .Compared with type 4 and 5 group ,there were significant rise in LVEF [ (48.98 ± 1.55)% ,(44.67 ± 1.48)% vs .(55.13 ± 4.27)% ,(53.73 ± 3.52)% ,(57.95 ± 2.89)% ] ,urine volume [ (1118.83 ± 168.09 ) ml , (1125.47 ± 177.28 ) ml vs . (1655.67 ± 198.37 ) ml , (1697.47 ± 171.27 ) ml , (1702.72 ± 179.28) ml] and Ccr level [ (40.11 ± 1.79) ,(41.11 ± 1.39) vs .(52.33 ± 2.49) ,(50.97 ± 2.11) , (51.32 ± 2.01 )] , and significant reduction in NT‐proBNP level [ (3738.19 ± 118.77 ) , (3378.38 ± 121.82 ) vs . (2137.51 ± 123.59) ,(2421.12 ± 121.22) ,(2139.81 ± 147.28)] in type 1 ,2 and 3 group ,P<0.05 all .Conclusion:CRRT is effective on different types of CRS ,but it′s best on type 1~3 than those of type 4~5 , which may be re‐lated to different pathological mechanisms of CRS different types .
2.Detection and Significance of Anti-β2 Glycoprotein 1 Antibodies in Female Infertility and Threatened Abortion

Objective To analysis the correlation of the anti-β2 glycoprotein 1 antibodies in female infertility and threatened a-bortion.Methods Selected 547 patients with female infertility,229 patietns with threatened abortion,229 patietns with ir-regular menses and 31 normal female between August 2013 to January 2014.All the anti-β2 glycoprotein 1 antibodies (Ig-G/Ig-M)were detected by ELISA.Detected the anti-β2 glycoprotein 1 antibodies (Ig-G/Ig-M)of 114 patients who treated with aspirin and prednisone combined with gamma globulin.Results The positive rate of anti-β2GP1 antibodies (Ig-G/Ig-M)of female infertility,threatened abortion,irregular menses and normal female were 0.36%,0.43%,1.6%,0% and 20.29%, 19.21%,8.3% and 3.2%,respectively.The positive rate of aβ2GP1-IgM of female infertility,threatened abortion,irregular menses were higher than normal female.The anti-β2GP1-IgM of the treated female infertility were 47.14± 34.85 RU/ml and 31.14±27.64 RU/ml,respectively.The anti-β2GP1-IgM of the treated threatened abortion were 37.75±31.20 RU/ml and 24.34±24.48 RU/ml,respectively.The levels of anti-β2GP1-IgM of the treated patietns were significantly lower than before.Conclusion There was a close relationship between anti-β2GP1-IgM antibodies and female infertility and threatened abortion,anti-β2GP1-IgM antibodies maybe one of the immune factors for female infertility and threatened abortion.
3.Study of bone mineral density and its influential factors in partial androgen deficiency in aging male patients

Objective To explore the characteristics and its influential factors of bone mineral density (BMD) changes in partial androgen deficiency in aging male patients.Methods 186 aging male patients with partial androgen deficiency and 125 healthy persons were selected, and their BMD in the first to fourth lumber spine and the proximal left femur was measured with the dual-energy X BMD measuring instrument.The biochemical and bone metabolic markers and sexual hormones were collected, and the relationship between changes of BMD in the patients with partial androgen deficiency, age, BMI, sexual hormones and bone metabolic markers were observed by multivariate stepwise regression analysis.Results There was no significant difference in BMD at the lumbar spine between two groups( P > 0.05 ), but BMD in PADAM group was lower than control groups at the hip[Neck: (0.831 ±0.136) g/cm2 vs (0.954 ±0.143) g/cm2,War's: (0.712 ± 0.127 ) g/cm2 vs ( 0.811 ± 0.149 ) g/cm2,Troch: ( 0.697 ± 0.124 ) g/cm2 vs ( 0.764 ±0.131 )g/cm2, P < 0.05 )].The incidence of PADAM and control group with osteopenia were 48.9% and 36%, and the osteoporosis was 33.3% and 20.8%, respectively.The bone mineral density was positively correlated to BMI at the first to fourth lumber spine and the proximal left femur in the patients with PADAM and negatively correlated to age and serum level of androgen.Conclusion BMD in PADAM group was significantly decreased, and age and low BMI and androgen deficiency wwere the risk factors of low BMD in PADAM patients.
4.Clinical research of Salmeterol fluticasone propionate and Tiotropium bromide for improvement of pulmona-ry function in patients with chronic obstructive pulmonary disease and bronchial asthma

Objective To observe the improvement of Salmeterol fluticasone propionate and Tiotropium bromide for inhalation in the treatment of chronic obstructive pulmonary disease ( COPD ) with bronchial asthma patients.Methods 89 patients were randomly divided into the observation group with 46 cases and control group with 43 cases,both groups were given conventional therapy.The control group was given Salmeterol fluticasone propionate 50μg/500μg,twice a day;the observation group was given Tiotropium bromide 18μg, twice a day, based on the therapy of control group.The two groups were treated for 14 days,and then curative effect,score of SGRQ,pulmonary function and blood gas analysis index were evaluated, and adverse reactions were recorded.Results The total effective rate of treatment group was 86.1%,higher than that of the control group(68.6%) (Z=2.68,P<0.05);Compared with the control group after treatment,functional parameters[ FEV1 ( L) ,FEV1 (%) and FEV1/FVC(%) ] of the observation group increased significantly,and the observation group were all higher than that of the control group [(2.27 ±0.45) vs (1.92 ±0.36),(64.27 ±16.11) vs (53.64 ±15.55),(54.85 ±14.81) vs (45.33 ± 13.23)],the differences were all statistically significant(t=4.20,3.16,3.19,all P<0.05);PaO2 of the two groups patients were increased and PaCO2 were decreased,scores of SGRQ were decreased obviously,and the change of the observation group was obviously[(47.61 ±3.64) vs (49.34 ±4.23),(67.33 ±5.56) vs (63.66 ±5.21), (45.34 ±3.72) vs (50.65 ±4.16)],the difference was statistically significant(t =2.13,3.32,6.54,all P<0.05) .Conclusion Salmeterol fluticasone propionate and Tiotropium bromide for inhalation can improve lung func-tional parameters and respiratory conditions of the parents with COPD and bronchial asthma obviously, reduce the score of SGRQ,curative effect is distinct,worthy of clinical popularization and application.
5.Effects of Ginkgo biloba extract 761 on learning and memory in chronic alcoholic rats

Objective To investigate the effects of extracts of Ginkgo biloba leaves ( EGb761 ) on learning,memory and the hippocampal long-term potentiation (LTP) in chronic alcoholic rats.Methods 72 SD rats were randomly divided into control group,chronic alcoholism model group,low-dose EGb761 (100 mg/kg) and high-dose EGb761 (200 mg/kg) groups on average.55% alcohol was given ( 10 ml/kg) by gavages once every day for 6 weeks to create the model of chronic alcoholism.In low-dose and high-dose EGb761 groups,EGb761 was used for gastric infusion 30 minutes before model.The control group and the model group were infused with the same volume of distilled water.Morris water maze (MWM) was used to study the spatial learning and memory in rats.Electrophysiological methods were used to observe the changes of LTP through the comparison of field excitatory postsynaptic potential (fEPSP) slope in hippocampal CA3 region before and after high frequency stimulation (HFS).Results ( 1 ) The escape latency (EL) of model group ( (93.31 ± 14.21 ) s) was longer than that of the control group( (35.28 ±6.51 )s).The mean slope of fEPSP (relative to baseline) after HFS in model group was (96.91 ±7.65) %,in control group was ( 177.22 ± 10.12) % ; Compared with control group,the ability of learning and memory as well as LTP obviously descended in model group.(2) Compared with model group,EGb761(100,200 mg/kg) could shorten the EL.The slope of fEPSP after HFS in either low-dose (( 126.16 ± 5.84)% )or high-dose (141.31 ± 5.75)% EGb761 groups was enhanced significantly compared with the model group (96.91 ± 7.65 ) %,P =0.034,P =0.009 ).Conclusion EGb761 can significantly improve the alcohol-induced memory impairment by means of accelerating the recovery of the pathological synaptic plasticity.
6.Stratified analysis on correlation between abnormal blood lipid in senile people and coronary atherosclerotic cardiopathy

BACKGROUND: Hyperlipemia is one of the important risk factors of coronary atherosclerotic cardiopathy. Great attention has been paid on how to explain the correlation between blood lipid and coronary heart disease and the correlation of characteristics and level of blood lipid with coronary heart disease.OBJECTIVE: To probe into the correlation among the characteristics, the level of blood lipid and coronary heart disease in senile people.DESIGN: Case-control observation and stratified analysis.SETTING: Second Cadre Ward of General Hospital of Shenyang Military Area Command of Chinese PLA.PARTICIPANTS: Totally 105 inpatients were selected in coronary heart disease group, diagnosed as coronary heart disease, tallied with WHO Diagnostic Standard on Coronary Heart Diseases, in Second Cadre Ward of General Hospital of Shenyang Military Area Command of Chinese PLA from January 2001 to January 2002. Totally 99 inpatients, by checked, without coronary heart disease and cerebral vascular disorder were selected in the control in the same period.METHODS: 2 mL venous blood was collected in fasting in the morning and the serum was collected after centrifuged at 3 000 r/minute, for 5 minutes. Auto-biochemical analyzer and enzyme kit were used to determine cholesterol, triglycerin, high density lipoprotein cholesterol and low density lipoprotein cholesterol. Compared with the data in the control, the ranges of cholesterol, triglycerin and low density lipoprotein were calculated successively and divided by 10; at the class intervals of 0.53, 0. 39 and 0.29 mmonl/L, the frequency table was worked out. According to the frequency table, P25 (the 25th percentile), P50 (the 50th percentile), P75 (the 75th percentile) and P90 (the 90th percentile) of cholesterol, triglycerin and low density lipoprotein cholesterol were calculated successively. The stratified groups were divided based on P < 25, P25 to PS0, PS0 to P75, P75 to P90 and > P90, by which, the cases belonging to 5 groups were determined in coronary heart disease group and the control respectively. The exposure factor was determined by > P90, compared with other 4 groups, Menta-Haensecel method was used to calculate x2. If the exposure factor was related with coronary heart disease, the relevant coefficient and 95% confidence interval should be calculated. In accordance with the standards (Proposal on Prevention of Abnormal Blood Lipid and Targets of Abnormal Blood lipid on 2-grade Prevention of Coronary Heart Disease), the cases beyond the targets and morbidity were determined in two groups.MAIN OUTCOME MEASURES: Determination of blood lipid, abnormal cases and morbidity in two groups; cases beyond targets of blood lipid and morbidity; stratified comparisons of percentiles in two groups.RESULTS: According to intention measurement, 204 cases entered result and the control, there was no significant difference in cholesterol, triglycerin cases presented hypercholesterolemia, 34.3% cases hypertriglycerlipidemia and 11. 4% cases low density lipoprotein-hypercholesterolemia. It was indicated that the blood lipid was in the normal range among 64. 8% to 88.6% comparison between the cases with cholesterol ≥5.47 mmonl/L and the cases with cholesterol varied from 3.85 to 4.63 mmol/L, the morbidity of coronary heart disease in the former was 2.98 times as high as the latter (x2 = 6.29, 95% confidence interval was 1.27 -6.98) . In the comparison between the cases with cholesterol ≥ 5.47 mmol/L and the cases with cholesterol varied from 4.64 to 5.05 mmol/L, the correlation with coronary Lipid for 2nd Prevention of Coronary Heart Disease, among the patients with coronary heart disease, cholesterol ≥ 4.68 mmonl/L in 54. 2% cases,triglycerin ≥ 1.70 mmol/L in 34. 3% cases and low density lipoprotein ≥ 2.6 mmol/L in 52. 3% cases.CONCLUSION: For senile patients with coronary heart disease, the satisfactory level of cholesterol is 3.85- 4.63 mmol/L. For those with cholesterol ≥ 5.47 mmol/L, compared with those varied from 3.85 to 4.63 mmol/L, the morbidity of coronary heart disease in the former is possibly 2.98 times as same as the latter(x2 = 6.29, 95% confidence interval was 1.27 - 6.98).
7.Effects of compound Daqiqi decoction combined cisplatin on Bcl-2/Bax expression of nude mice ovarian cancer subcutaneous transplanted tumor.

OBJECTIVETo determine the compound Daqiqi decoction( CDQD) combined cisplatin plays on the cell apoptosis of subcutaneous transplanted ovarian cancer in nude mice, to provide theory evidence for clinical treatment.
METHODMaking the models of subcutaneous transplanted ovarian cancer in nude mice, and divide the 40 mice with tumor into 5 groups (n = 8), the model control group, CDQD low dosed group, CDQD high dosed group, cisplatin group, cisplatin combined CDQD group, killing all the mice after 3 weeks' treatment and stripping tumors. Measure the volume and weight of the tumor and calculate tumor growth curve damps, the inhibition rate. Examining the expression of the Bcl-2(B-cell leukemia /lymphoma 2) and Bax (Bcl-2 associated x protein) mRNA and protein by the RT-PCR and the Western blot.
RESULT(O)The tumor weight shows that there was certain lighter effect in each CDQD group, and compared with cisplatin groups has no statistical significance, the cisplatin combined CDQD group is obviously lighter than that of the other group(P <0.01). The tumor growth curve damps shows that compared with model control group, the treatment group tumors had some extent narrowing (P < 0.01). (2) RT-PCR results: Bax (Bcl-2 associated x protein) mRNA expression shows that compared with model control group, the treatment group has increased (P < 0.01), and the cisplatin combined CDQD group compared with the other group is the highest, there was significant difference with the rest of the treatment group (P < 0.01). In the Bcl-2 mRNA express lowest in cisplatin combined CDQD group (P < 0.01), there has no difference between CDQD high dose group and cisplatin group. (Western blot shows: compared with model control group, the Bax protein of treatment group has increased expression (P < 0.01), the expression of Bax protein in cisplatin combined CDQD group is the highest(P < 0.01). There has no difference between CDQD high dose group and cisplatin group. The Bcl-2 protein expression of the cisplatin combined CDQD group is the lowest (P < 0.01), there has no difference between CDQD high dose group and cisplatin group.
CONCLUSIONThe effect of CDQD on subcutaneous transplantation ovarian tumor has promoting apoptosis function, its mechanism may be related to downgrade the Bcl-2 expression, higher expression of bax, stimulation on the apoptosis of tumor cells; cisplatin combined CDQD have synergistic effect.

8.Study on the safety margin of esphagus in the radical gastrectomy of adenocarcionma of esophagogastric junction

Objective To discuss decision of the safety margin of esophagus in the radical gastrectomy of adenocarcionma of esophagogastric junction.Methods The length of esophageal resection margin of intraoperative,postoperative and pathological large slice under the microscope of 50 cases adenocarcionma of esophagogastric junction underwent radical gastrectomy were measured,and the proximal invasive length of esophagus in large pathological slice were also measured,and the relationship between it and pathological factors was analyzed.Determined the minimum safety cut edge distance.Results In 46 of 50 patients,proximal intramural spread of the tumor was observed.The invasive length of tumor from 0.1 cm to 3.5 cm.The invasive length of tumor was <1.0 cm with 11 patients,1.0-1.5 cm with 19 patients,1.5-3.0 cm with 15 patients,in only 1 patient was the distance of spread 3.5 cm.Invasive length of adenocarcionma of esophagogastric junction had a significant difference between the groups in different pathological factors(P<0.001).Conclusion The length of 3.5 cm is the safety margin of esophagus in the radical gastrectomy of adenocarcionma of esophagogastric junction(SiewertⅡ,Ⅲ type) by abdominal incision.
9.Risk factors for infants and young children with wheezing in Zhongshan city

Objectives To analyze the risk factors associated with infant wheezing in Zhongshan city. Methods A multi-center, large sample of case-control study was applied and the data related to risk factors was collected by questionnaire survey. T test and chi-square test were firstly used for univariate analysis, and then the multivariate stepwise logistic regression was used to analyze the independent risk factors associated with infant wheezing. Results A total of nine factors were found rele-vant to infant wheezing by univariate analysis including parental allergic history, way of birth, respiratory syncytial virus infec-tion, Mycoplasma pneumoniae infection, personal allergic history, like crying, parents have constant disagreements, home near the road, and factory around (P<0.05). Parental allergic history (OR=3.441, 95%CI:1.914-6.186, P<0.001), respiratory syncy-tial virus infection (OR=2.910, 95%CI:1.793-4.723, P<0.001), Mycoplasma pneumoniae infection (OR=2.277,95%CI:1.110-4.667, P=0.025), home near the road (OR=2.036, 95%CI:1.280-3.239, P=0.003) and like crying (OR=1.521, 95%CI:1.049-2.206, P=0.027) were approved to be the independent risk factors of infant wheezing in ZhongShan. Conclusions Nine factors have relationship with infant wheezing, including parental allergic history, respiratory syncytial virus infection, Mycoplasma pneumoniae infection, home near the road, like crying, personal allergic history, and that the former five factors are the indepen-dent risk factors.
10.Activation of renal tubular epithelial cells by monocytes/macrophages and the relative mechanisms

AIM: To observe the direct effects of peripheral blood monocytes/macrophages (MO/MAC) on renal tubular epithelial cells (RTEC),and further probe into the possible mechanisms. METHODS: Conditioned medium(M-CM) of human peripheral blood MO/MAC was collected and added to HK-2,a human renal proximal tubular cell line.After incubation with M-CM for 24 hours,HK-2 cells were detected for DNA synthesis by -TdR incorporation,osteopontin (OPN) and ?-smooth muscle actin (?-SMA) expression by Western blot,and fibronectin(FN) secretion by ELISA.Furthermore,anti-TGF?_1 neutralizing antibody and interlukin-10(IL-10) were used separately to antagonize the effects of M-CM on HK-2 cells. RESULTS: ①DNA synthesis,?-SMA expression and FN secretion were all increased in HK-2 cells when incubated with M-CM.②When adding with anti-TGF?_1 neutralizing antibody (5 mg/L) in the M-CM,the degree of upregulation of ?-SMA and FN in HK-2 cells was much lower than that stimulated by M-CM alone.③M-CM added with IL-10 (20 ?g/L) had a weaker ability to induce the increasing in ?-SMA expression and FN excretion in HK-2 cells, compared with M-CM itself alone.M-CM from MO/MAC preincubated with IL-10 caused a lower upregulation of ?-SMA expression in HK-2 cells than M-CM from non-preincubated MO/MAC. CONCLUSION: MO/MAC can directly induce proliferation,transdifferentiation and extracellular matrix secretion in RTEC.TGF?_1 and proinflammatory cytokines secreted by MO/MAC might be involved in the aboveeffects.
