OBJECTIVE:To investigate the biliary aerobic bacterial infection in acute cholecystitis and their susceptibility to antibacterials.METHODS:Biliary samples of 198 patients undergoing acute cholecystitis surgery in our hospital from 2003 to 2007 were sujected to culture of biliary aerobic bacteria and drug sensitivity test.RESULTS:Of the total 198 bile specimens,147 were postive in aerobic bacteria culture,of which,21 were mixed infection.A total of 225 strains were isolated,with Escherichia coli,Klebsiella pneumonia and Enterobacter representing 42.7%,22.7% and 16.9% respectively.In the drug sensitivity,all the bacteria showed different degree of resistance to antibiotics,with cephalosporines showing the higest antibiotic sensitivity.Gram-positive bacteria were the most sensitive bacteria to vancomycin while Gram-negative bacteria were the most sensitive bacteria to imipenem.CONCLUSION:Biliary bacterial culture and drug sensitivity test are necessary in acute cholecystitis surgery and drugs should be used according to the results of drug sensitivity test.

Objective To explore the influence of optimizing practical teaching of on improving the application ability and clinical thinking of acupuncture professional students.Method The students of four 2012 acupuncture and massotherapy classes in the college were randomized into experiment and control groups. In the experiment group, practical teaching reform was implemented to adopt Whole course module teaching system, establish practice teaching resource database of and construct the training teaching mode of medical records as guidance, objective and “three observations, seven practices and one comparison”. In the control group, lessons were given in sequence of “meridian and acupoints first” and “needling and moxibustion methods next” of traditional course system, and the training teaching mode of teacher demonstration-student group practice-teacher comment and the teacher-led mode of operation assessment were used. At the end of term, grades were awarded by examining based meridian drawing and acupoint location, puncturing with filiform needles and based operation. Assessment was made by the teachers and experimental group students.Result There were statistically significant differences in the grades of meridian drawing and acupoint location, based puncturing with filiform needles and based operation between the experiment and control groups (<0.01). Both the teachers and experimental group students responded that the application ability was improved to a larger extent.Conclusion The practical teaching reform of is very helpful in improving students’ comprehensive quality of skills and application abilities. standardization of based based.

Objective:To evaluate the effect of the psychological intervention on heart intervention operation of the old patients.Methods:The supportive psychotherapy was employed to treat the old patients undergoing heart intervention operation.HAMA、HAMD measure forms and clinic self-edited questionnaire were adopted to carry out an investigation on 105 cases before and after heart intervention operation.Results:The psychological intervention significantly stabilized emotion of the patients;anxiety and depression were relieved after the treatment.Conclusion:The psychological intervention is helpful in treating the old patients undergoing heart intervention operation.It can promote the recovery and improve the emotion.

Objective: To evaluate the efficacy and safety of domestically made candesartan cilexetil in the treatment of mild to moderate hypertension.Methods: In randomized,double-blind and parallel control trai1,58 patients with mild and moderate essential hypertension randomly received either candesartan cilexetil at 8mg daily(=29) or irbesartan at 150mg daily(=29) for 2 weeks.The patients with poor prognosis in 2weeks(DBP≥90mmHg) received extra 1 tablet daily.Results:Significant differences in the overall response rate of candesartan or irbesartan monotherapy(75.86% VS.37.93%) was found at 2 weeks after treatment.No significant difference in the overall response rate of candesartan or irbesartan monotherapy(89.66% VS.79.33%) was found at 4 weeks after treatment.Significant differences in decreasing the range of systolic pressure in candesartan group or irbesartan group(27.52?11.90mm Hg VS.20.72?9.27 mm Hg) at sitting position and(24.59?11.87 mm Hg VS.15.90?16.22 mm Hg) at standing position were found in at 4 weeks(P

Objective:To determine the equilibrium solubility of zolmitriptan in various media and its partition coefficients for the n-octanol-water/buffer solution systems.Methods:A high performance liquid chromatography was established and used to detect the concentration of zolmitriptan;the partition coefficients for the n-octanol-water/buffer solution systems of zolmitriptan were determined by shaking flask method.Results:The equilibrium solubility of zolmitriptan in water was 12.6 g/L at 37℃.The equilibrium solubility of zolmitriptan were 1 382.0,553.0,85.2,71.5,47.1,11.5 and 4.85 g/L in buffer solutions with pH values of 3.6,4.5,6.0,6.8,7.4,8.0 and 9.0 respectively.Papp of zolmitriptan was 0.013 in water and were 0.04,0.06,0.14,0.20,0.23,0.25 and 1.44 in buffer solutions with different pH values.Conclusion:In the range of pH3.6 to pH 9.0,the equilibrium solubility of zolmitriptan was decreased while the apparent oil/water partition coefficient was increased with the increasing of pH values.

Objective To investigate the value of ultrasound in diagnosis of fetal heart malformation at 11-13+6 weeks gestation.Methods A total of 70 pregnant women were selected,based on the results of induction of labor and postnatal imaging examination, the effect of ultrasound on fetal heart abnormalities in 11-13+6 weeks was observed.The display of cardiac malformations with different cuts was compared.Results In 70 fetuses with cardiac malformations, malformation types included tetralogy of Fallot, double outlet right ventricle, complete endocardial cushion defect, transposition of the great arteries, persistent truncus arteriosus, aortic arch, venous drainage.Ultrasound diagnosis of 64 cases of fetal cardiac malformations, the diagnostic accuracy rate was 91.4%.Among them, the highest proportion of tetralogy of Fallot (42.9%), venous drainage was the lowest (2.9%).The diagnostic rate of 4CV+3VVT ultrasound for fetal heart malformation was the highest(91.4%), which was significantly higher than 4CV+VOTV(72.2%), 4CV+(AR-SAV)(68.6%) and 4CV, the differences were statistically significant (x2=7.643,5.324,4.447,all P<0.05).The diagnostic accuracy of 4CV+3VV ultrasound for fetal heart malformation was 88.6%, which was significantly higher than 4CV+ (AR-SAV) and 4CV section, the differences were statistically significant (x2=6.343,4.011,all P<0.05).Conclusion Ultrasound can be used to detect fetal heart malformation at 11-13+6 weeks gestation, among them, the accuracy of 4CV+3VVT ultrasound section is the highest.

Objective To evaluate the methods for improving diagnostic ratio of unruptured ectopic pregnancy with ultrasonography. Methods A total of 139 patients of small adnexal cysts with thick wall were predicted respectively with sonographic indexes. All patients (61 with unruptured ectopic pregnancy and 78 with small corpusluteum) were confirmed postoperatively or with long-term follow up, and the results were compared. Results The diagnostic sensitivity of with hyperecho or isoecho compared with endometrium, hyperecho compared with ovarian tissue, adnexal cyst without small follicle around was 86.89%, 93.44% and 96.72%, and the specificity of three predictors was 93.59%, 85.90% and 70.51%, respectively. Two sonographic indexes together had a sensitivity of 98.36% and a specificity of 94.87% for diagnosing ectopic pregnancy. Conclusion Observing the relation of adnexal cyst and ovarian and the echogenic of cyst wall are specific and sensitive for the diagnosis of ectopic pregnancy.
