1.Distribution of CAG repeat number within androgen receptor gene in Chinese Han nationality and its application in genetic diagnosis for Kennedy's disease

Objective To investigate the distribution of androgen receptor (AR) gene CAGrepeats in the Chinese Han nationality and its application in genetic diagnosis for Kennedy's disease (KD). MethodsRT-PCR,denaturing polyacrylamide gel electrophoresis (DPAGE) and gene sequencing were conducted for AR gene CAG repetition among 100 healthy controls and 28 patients diagnosed as motorneuron diseases,and the number of the repetition was counted. Results The healthy controls had a range of 15-31 times of CAG repetition,with an average of (23 ± 3) times.Among patients with motoneuron disease,3 cases with CAG repetition for more than 40 times (namely,46,47 and 47 times) were diagnosed as KD.The main clinical manifestations included slow progress of limb weakness,primarily in the proximal lower limbs,fatigue accompanied by myalgia,muscle jumping,muscle atrophy,elevated serum creatine kinase (CK) levels,neurogenic damage revealed by electromyogram (EMG) and androgen insensitivity.Conclusions The incidence of KDmay be underestimated in the Chinese population.Performing genetic diagnosis in patients with motor neuron disease for AR gene can improve clinical diagnosis and avoid misdiagnosis.
2.Effect of ru'ai shuhou recipe on immune response in HER2/neu tranagenic mice undergoing breast cancer carcinogenesis process.

OBJECTIVETo explore the immune response induced by HER2/neu oncogene in the breast cancer (BC) carcinogenesis process and the immunological mechanism of Ru'ai Shuhou Recipe (RSR) in the prevention and treatment of BC.
METHODSHER2/neu transgenic spontaneous breast tumor model mice were fed with RSR from 5 weeks old, the occurrence of breast tumor in them was observed, and the changes of T cell-mediated immune response and associated cytokines were detected during the carcinogenesis process, i. e., when mice aged between 15 and 25 weeks.
RESULTSRSR showed significant effects in postponing and reducing the carcinogenesis of primary breast tumor, up-regulating the amount of T cell in splenic lymphocyte in tumor-bearing mice, promoting the proliferation of T lymphocyte, and inducing the secretion of cytokines such as interleukin-2, interleukin-12 and interferon-y.
CONCLUSIONSA serial immune response reveals in the carcinogenesis process. The immunologic function of HER-2/neu transgenic mice is significantly different to that of the same strain non-transgenic mice. Effect of RSR in preventing and postponing breast cancer carcinogenesis is possibly realized through enhancing the anti-tumor immune response of transgenic mice themselves.

3.Application of grouping responsibility system in tile on-the-job training of emergency nurses

Objective To explore the effectiveness of a skill training method which brings in a system of grouping responsibility for the on-the-job training of the emergency nurses. Methods Nurses who enrolled in the training program at the emergency department during the period from 2008 to 2009 were randomly divided into two groups. The control group (group A) was trained in a traditional way. Grouping responsibility system was employed in the experimental group (group B). The differences of parameters such as training popularity rate, attainment rate, the capability of education and training, as well as the mastery of professional skills were compared between the two groups. Results All the parameters were significantly higher in the experimental group than the control group. There were significant differences between the two groups ( P < 0. 05 or P < 0.01). Conclusions The grouping responsibility training system showed obvious advantages considering its flexible training staff and time as well as differential treatment in education. The results supported the view that this training method presented strong practicability for specific skills of emergency department and should be extended in clinical application.
4.Changes of IK, ATP current density and allosteric modulation during chronic atrial fibrillation.

BACKGROUNDAtrial fibrillation (AF) is the most common supraventricular arrhythmia in clinical practice. Chronic atrial fibrillation (CAF) is associated with ionic remodeling. However, little is known about the activity of ATP-sensitive potassium current (IK, ATP) during CAF. So we studied the changes of IK, ATP density and allosteric modulation of ATP-sensitivity by intracellular pH during CAF.
METHODSMyocardium samples were obtained from the right auricular appendage of patients with rheumatic heart disease complicated with valvular disease in sinus rhythm (SR) or CAF. There were 14 patients in SR group and 9 patients in CAF group. Single atrial cells were isolated using an enzyme dispersion technique. IK, ATP was recorded using the whole-cell and inside-out configuration of voltage-clamp techniques. In whole-cell model, myocytes of SR and CAF groups were perfused with simulated ischemic solution to elicit IK, ATP. In inside-out configuration, the internal patch membranes were exposed to different ATP concentrations in pH 7.4 and 6.8.
RESULTSUnder simulated ischemia, IK, ATP current density of CAF group was significantly higher than in SR group [(83.5 +/- 10.8) vs. (58.7 +/- 8.4) pA/pF, P < 0.01]. IK, ATP of the two groups showed ATP concentration-dependent inhibition. The ATP concentration for 50% current inhibition (IC50) for the SR group was significantly different in pH 7.4 and pH 6.8 (24 vs. 74 micromol/L, P < 0.01). The IC50 did not change significantly in CAF group when the pH decreased from 7.4 to 6.8.
CONCLUSIONSDuring CAF, IK, ATP current density was increased and its allosteric modulation of ATP-sensitivity by intracellular pH was diminished.

5.The prognostic value of metabolic tumor volume in FDG PET/CT evaluation of post-operative survival in patients with esophageal squamous cell cancer

ObjectiveTo evaluate the prognostic value of MTV on 18F-FDG PET/CT in patients with esophageal cancer.MethodsForty-nine patients with esophageal cancer underwent 18 F-FDG PET/CT scan before surgery.The median follow-up time for the patients was 29 months (range,8- 57 months).The prognostic significance of MTV,age,sex,histologic grade,SUVmax of the primary tumor,tumor size measured on PET/CT,T stage,N stage,M stage,American Joint Committee on Cancer (AJCC) stage,number and location of lymph nodes metastases were assessed by Kaplan-Meier analysis and multivariate Cox model.ResultsIn the univariate analysis,AJCC stage (x2 =16.206,hazard ratio (HR) =1.177,P <0.001),N stage (x2 =9.536,HR =10.833,P =0.002),T stage (x2 =5.810,HR=2.397,P=0.016),number of lymph nodes metastases (x2 =11.423,HR =1.567,P =0.001 ),and MTV (x2 =3.872,HR =2.433,P =0.049 ) were significant predictors of survival.Multivariate analysis showed that MTV and AJCC stage were independent predictors of survival (x2 =4.525,HR1.170,P =0.033 ;X2=4.875,HR =3.071,P =0.027).Kaplan-Meier survival curves revealed longer survival time of low-MTV group as compared to high-MTV group ( Log-rank,x2 =4.186,P =0.041 ).ConclusionMTV on 18 F-FDG PET/CT may be an independent prognostic factor in patients with esophageal cancer.
6.Mid-term outcomes of a prospective phase Ⅱ trial of preoperative sandwich-like neoadjuvant chemoradiotherapy for locally advanced rectal cancer

Objective To evaluate the mid-to long-term survival benefits of preoperative sandwich-like neoadjuvant chemoradiotherapy (CRT) in patients with locally advanced rectal cancer (LARC).Methods A total of 45 LARC patients who underwent neoadjuvant sandwich CRT in the form of XELOX regimen prior to,concurrently with,and following volumetric modulated arc radiotherapy (VMAT) in 2012 were enrolled in this study.VMAT was given at a gross tumor volume dose of 50 Gy in 25 fractions,and a clinical target volume dose of 45-46 Gy in 25 fractions.Total mesorectal excision was performed 6 to 8 weeks after completion of VMAT.The overall survival (OS) and disease-free survival (DFS) were determined by the Kaplan-Meier method,and survival comparison and univariate prognostic analysis were performed using the log-rank test.Results The median follow-up time was 46.7 months.There was no local recurrence detected among the patients.The 3-year distant metastasis (DM) rate was 18%,and the 3-year OS and DFS were 96% and 84%,respectively.Univariate analysis indicated that perineural invasion,N1-N2 pathology (pathological stage Ⅲ),and Ca-199>35 U/ml before treatment were risk factors for DM (P=0.000,0.000,and 0.013,respectively).Conclusions The significant short-term efficacy of preoperative sandwich-like neoadjuvant CRT can be extended to a positive mid-term survival in LARC patients.However,further phase Ⅲ clinical studies will be needed to confirm this finding.
7.Molecular epidemiological characteristics of HIV-1 strains isolated from newly diagnosed MSM subjects (2006-2010) in Beijing, China.

This study aims to analyze the molecular epidemiological characteristics of HIV-1 strains prevailing among men who have sex with men (MSM) in Beijing, China. The pol gene fragments from 250 newly diagnosed HIV-1-infected MSM individuals during 2006-2010 in Beijing were amplified by RT-nested PCR, sequenced, and phylogenetically analyzed. HIV-1 pol gene from 189 individuals were amplified and analyzed; 81 (42. 9%), 3 (1. 6%), 2 (1.0%), 88 (46. 6%), and 15 (7.9%) individuals were infected with HIV-1 subtypes B, B', C, CRF01_AE, and CRF07_BC, respectively. The subtypes B and CRF01_AE could both be grouped into two clusters, and CRFO7_BC strains shared high homology and were presumed to originate from a common ancestor. The HIV-1 circulating in MSM in Beijing had a lower genetic diversity than in heterosexuals. The HIV-1 epidemic (2006-2010) in MSM in Beijing was actually a rapid spread of HIV-1 CRF01 AE and B, or rather native strains of the two viruses.

8.Clinical study on hybrid bioarttficial liver supporting system for acute on chronic liver failure patients

Objective To construct an hybrid bioartificial liver supporting system,and observe its effectiveness and safety on patients with acute on chronic liver failure.Methods Hybrid bioartificial liver supporting system (HBALSS) was constructed using bioreactor with HepG2 cells transfected with human augmenter of liver regeneration (hALR) gene.12 acute on chronic liver failure patients were divided into 2 groups randomly.The treatment group was treated with the hybrid bioartificial liver support system.The group underwent plasma exchange was used as control.Results In the treatment group,four patients recovered,one patient died of hepatic encephalopathy,one patient died of hepatorenal syndrome,one patient recovered,but died of gastrointestnal bleeding after 1 year.In control group,two patients recovered,one patient underwent orthotropic liver transplantation,and three patients died of liver failure.Conclusion The hybrid bioartificial liver supporting system with HepG2 cell line was established successfully and have certain safety and effectiveness on acute on chronic liver failure patients.
9.Effects of Sanqi Gel on expressions of cyclooxygenase-2 and transforming growth factor-β_1 during epidural adhesion

Objective To discuss the relevant mechanism of Sanqi Gel in the prevention of epidural adhesion. Method All male SD rats (n=72) were randomly divided into the normal saline water group, Zhanlianping group, carbopol gel group and Sanqi Gel group (each n=18). The model of laminectomy was established and observed at different time points after the laminectomy for 7, 14 and 21 days respectively by applying immunohistochemistry method. The expressions of transforming growth factor-β_1 (TGF-β_1) and cyclooxygenase-2 (COX-2) were analysed during the healing period.Result The different treating methods had different effects on the expressions of TGF-β_1 and COX-2 with different effective strength. The expression of COX-2 was higher significantly in the normal saline water group, carbopol gel group and Zhanlianping group than that in the Sanqi Gel group (P<0.05) 14 days after the operation. The expression of TGF-β_1 was higher significantly in the normal saline water group, carbopol gel group and Zhanlianping group than that in the Sanqi Gel group (P<0.05) at 3 time points (7, 14 and 21 days after the operation).Conclusion Sanqi Gel can prevent epidural adhesion through inhibiting the expressions of COX-2 and TGF-β_1.
10.Killing effect of double suicide genes mediated by retroviral vector on k562 cells.

The aim of study was to investigate the killing effect of double suicide gene system mediated by retroviral vector on K562 cells in vivo and ex vivo. CDglyTK gene was transfected into PA317 cells by using lipofectamine. K562 cells were infected with viral supernatant. K562/CDglyTK cells were treated with 5-fluorocytosine (5-FC) and/or ganciclovir (GCV). Mice were randomly divided into three groups: tumor formation, tumor inhibition and tumor therapy. Each mouse was implanted with K562/CDglyTK cells or K562 cells. The results indicated that the killing effect of 5-FC in combination with GCV on K562/CDglyTK was more significant than using 5-FC or GCV alone. In vivo study showed that after being injected subcutaneously with K562 cells and K562/CDglyTK cells, there was not obvious difference in tumor formation rate of mice, 5-FC + GCV could suppress tumor formation of the K562/CDglyTK cells. After being treated with 5-FC and GCV, the median tumor volume of mice implanted with K562/CDglyTK cells decreased obviously, compared with the control group. Their median survival was significantly prolonged. It is concluded that double suicide genes are more effective for killing effect on K562 cells in vivo and in ex vivo. It may be applicable to clinical gene therapy.
