(1)H-NMR and (13)C-NMR assignments of 12-oleanene-3,11-dione (compound 1) were completely described for the first time through conventional 1D NMR and 2D shift-correlated NMR experiments using (1)H-(1)HCOSY, HMQC, HMBC techniques. Based on its NMR data, the assignments of 28-hydroxyolean-12-ene-3,11-dione (compound 2) were partially revised.

2.Inducement, Purification and Characterization of?-mannanase from Armillariella tabescens EJLY2098

Armillariella tabescens EJLY2098 was capable of secreting p-mannanase by konjac inducement. A 34 orthogonal design was applied to determine the optimum medium of inducing mannanase by Armillariella tabescens EJLY2098. The results suggested that Armillariella tabescens EJLY2098 secreted the high-activity enzyme in the optimum medium, which was composed of 2% konjac, 1% peptone, 25% potato juice,0.3% KH2PO4,15% MgSO4?7H2O, 0.01% VitB1. Purified by DEAE-anion exchange chromatography, two eluting peaks (P1 and P2) with the p-mannanase activity were obtained, and one of them (named?-mannanase P2) was a single band by the SDS-PAGE, and the molecular weight of?-mannanase P2 was 78. 9kDa. The isoelectric point of?-mannanase P2 was estimated to be 4.0-4. 1. The optimum activity for the enzyme was found at 60℃and pH4. 0 - 6. 0, and the enzyme was stable between pH4. 5 - 6. 0. The activity of?-mannanase P2 were enhanced by Na+ and Ba2+ . This?-mannanase can be used in feed industy. a new fungi secreting?-mannanase was obtained, providing an important base for cloning mannanase gene and constructing recombin microbe expressing?-mannanase .

This paper introduces the pelvic inflammatory disease treatment characteristics of TCM Master Xia Guicheng.He emphasizes that comprehensive regulation is needed for pelvic inflammatory disease needs,and clearing heat and detoxifying, removing dampness and blood stasis are not the only methods for regulating internal disease resistance.For the acute stage,the treatment should be based on blood-heat of lower Jiao,clearing heat and detoxifying,removing blood stasis to stop pain should be used.For the chronic stage,reinforcing healthy qi should be taking as the root treatment which advocates menstrual cycle therapy under the regulation of heart-kidney-uterus axis,and heart-kidney interaction,removing blood stasis to stop pain should be used.

This paper expounds the concept and method of TCM Master Xia Guicheng diagnosis and treatment of dysmenor-rhea,which is summed up as six kinds of methods.Master Xia believes that the cause of dysmenorrhea is generally attributed to qi stagnation or blood stasis,and the general treatment usually performs from six aspects including stopping pain,dredging channels,calming the heart,regulating liver,warming meridian and spasmolysis,which have achieved remarkable effects. The treatment root is focus on regulating the activities of yin-yang,qi and blood of the menstrual cycle rhythm,and to treat dysmenorrhea by improving female's imbalance of the body environment through balancing yin and yang,smoothing qi and blood circulation.

This paper introduces the philosophy and method of regulating and treating polycystic ovary syndrome(PCOS)byTCM master Xia Guicheng.Circular motion biological clock rhythm of female reproductive axis was analyzed with the conceptof waxing and waning,conversion of yin and yang in Tai Chi of Yi-ology.He proposes heart-kidney interaction,priority of balancingyin and yang,coordination of liver and spleen,qi and blood,and regulating the changes of menstruation cycle to treatmenstrual disorder.It is believed that the disorder of the circular motion biological clock rhythm of reproductive axis is thesource of PCOS.Dysfunction of main zang-fu organs is the key of PCOS,including lack of essence-generation source due to theinsufficiency of reproduction-stimulating essence,rhythm disorder due to non-interaction between the heart and the kidney,yindeficiency with yang hyperactivity due to insufficiency of liver-yin,phlegm and fat causing disease due to spleen and stomach.The dysfunction of storing and discharging of extraordinary fu organs is the apparent factor of menstruation disorder in PCOS.
6. Recurrent syncope related to catecholaminergic polymorphic ventricular tachycardia due to de novo RyR2-R2401H mutation

Objective:
To explore the clinical and molecular genetic features of a Chinese patient with catecholaminergic polymorphic ventricular tachycardia (CPVT).
Methods:
Clinical data including resting electrocardiography, echocardiography and treadmill exercise testing of a patient with CPVT admitted to our department in March 2013 were analyzed, and the peripheral venous blood samples of the patient and his family members and 400 ethnicity-matched healthy controls were obtained. All exons and exon-intron boundaries of the six CPVT-related genes including RYR2, CASQ2, TRDN, CALM1, KCNJ2 and ANKB were sequenced to detect the variants related to CPVT. The relationship between the genotypes and phenotypes was analyzed to direct the target therapy.
Results:
Recurrent syncope induced either by exercise or extreme frightened fear was observed in this patient. There was no positive family history of syncope or sudden death. The resting electrocardiography and echocardiography of the patient were normal, while the exercise testing revealed bidirectional and polymorphic ventricular tachycardia. A cardiac ryanodine receptor gene mutation (R2401H) was identified in this patient, while this mutation was absent in his parents and sister and 400 controls. No variant was detected in the remaining five candidate genes. Treatment with high dose of metoprolol succinate (118.75 mg/d) was effective and patient was free of syncopal attack during the 2 years follow-up.
Conclusion
This is the first report on RyR2-R2401H mutation in Chinese patient with CPVT, and high dose of metoptolol is the effective therapy option for CPVT related to RyR2 mutation.
7.Possible role of DNA polymerase beta in protecting human bronchial epithelial cells against cytotoxicity of hydroquinone.

OBJECTIVETo explore the toxicological mechanism of hydroquinone in human bronchial epithelial cells and to investigate whether DNA polymerase beta is involved in protecting cells from damage caused by hydroquinone.
METHODSDNA polymerase beta knock-down cell line was established via RNA interference as an experimental group. Normal human bronchial epithelial cells and cells transfected with the empty vector of pEGFP-C1 were used as controls. Cells were treated with different concentrations of hydroquinone (ranged from 10 micromol/L to 120 micromol/L) for 4 hours. MTT assay and Comet assay [single-cell gel electrophoresis (SCGE)] were performed respectively to detect the toxicity of hydroquinone.
RESULTSMTT assay showed that DNA polymerase beta knock-down cells treated with different concentrations of hydroquinone had a lower absorbance value at 490 nm than the control cells in a dose-dependant manner. Comet assay revealed that different concentrations of hydroquinone caused more severe DNA damage in DNA polymerase beta knock-down cell line than in control cells and there was no significant difference in the two control groups.
CONCLUSIONSHydroquinone has significant toxicity to human bronchial epithelial cells and causes DNA damage. DNA polymerase beta knock-down cell line appears more sensitive to hydroquinone than the control cells. The results suggest that DNA polymerase beta is involved in protecting cells from damage caused by hydroquinone.

8. Risk adjustment and its application in refined hospital management and assessment

Objective:
To study new ways and tools for assessing the inpatient disease management and improving refined management of the hospital.
Methods:
By using homepages of medical records of the patients discharged from 21 tertiary general hospitals in a city in 2016, we completed the modeling and predicted value calculation within each DRGs with the Disease Management Intelligent Analytic & Evaluation System (DMIAES System).
Results:
2 192 predication models were built, to compute the theoretic values of the mortality rate, length of stay, medical fee, medical service fee, and drug cost of each inpatient. Such values were compared with the observed results to gain the O/E index. If O/E is less than 1, it indicates that the inpatient′s disease management is good and better than expected. On the other hand, O/E index greater than 1 indicates poorer disease management than expected and rooms of further improvement. With the help of O/E index, we made multidimensional comparisons assessment and analysis of different hospitals, clinical disciplines, diseases and doctors.
Conclusions
The DMIAES System can take risk factors of inpatients′ outcomes into account, assessing the major indicators of inpatient outcomes by means of big data and modelling. This approach proves effective in enabling administrators and doctors to rapidly analyze problems for identifying solutions and enhancing management, thus having great potential in hospital management, supervision and assessment.

Objective:To investigate the effect of total ginsenoside ginseng root on the learning and memory impairment and anxiety of hindlimb suspension rats by detecting the performance of rats in the water maze, elevated plus maze, and the expression of hypothalamic-pituitary-adrenal (HPA) axis, inflammatory factors and tryptophan pathway related factors through the intervention of ginsenosides in hindlimb suspension rats. Method:The Wistar male rats were divided into normal group, hindlimb suspension model group, Huperzine A group (0.1 mg·kg-1), and total ginsenoside ginseng root low and high dose groups (100, 200 mg·kg-1), with 8 rats in each group. Except for the normal group, the rats in the other groups maintained a -30° hindlimb suspension state for 24 h. The normal group and the model group received intragastric administration of 10 mL·kg-1 pure water . After 28 days of continuous administration, the water maze and elevated plus maze behavioral tests were performed. After the tests, blood was taken from the abdominal aorta, and the rat brain cortex was peeled off on ice, quenched with liquid nitrogen, and stored at -80 ℃ for later use.
