Objective To explore the effect of histamine in paraventricular nucleus (PVN) on the development of neurogenic pulmonary edema (NPE).Methods NPE models of rabbits were made and H 1 receptor antagonist (chlorpheniramine, H 1 group), H 2 receptor antagonist (Cimetidine, H 2 group) or normal sodium (NS group) were injected into PVN of rabbits utilizing the technique of stereotaxis. Translational medicine is a novel concept about combination of basic research and clinical application. The aim of translational medicine is to realize the translation of basic research into clinical practice. microRNAs (miRNAs) are non-coding single-stranded RNAs with 21-25 nucleotides in length as newly discovered factors in regulating gene expression. Recently, the key regulatory role of miRNA in the cardiovascular system has been elucidated and amount of remarkable results has been achieved, particularly in the regulation of cardiac arrhythmias. A series of studies demonstrate that miRNAs are involved in the regulation of expression of a variety of proteins associated with cardiac electrical activity, and are the potential targets of occurrence of cardiac arrhythmias and anti-arrhythmic drugs. miRNAs as a therapeutic target regulate the stability of mRNAs of target genes or play an inhibitory role in the translation process. Stability of the corresponding miRNA expression levels in the target organ may be a new approach for the disease therapy. Regarding the dysfunction of miRNA, we employed miRNA re-expression strategy and anti-miRNA strategy to correct target protein function and provide a new entry for the therapy of arrhythmia. With the technology of miRNA mimics and antagomirs, miRNAs are expected to treat various cardiovascular diseases and will provide a fresh impetus to achieve transform medicine.
4.Effects of curcumin pretreatment on expression of Nrf2 protein during ventilator-induced lung injury in rabbits

Objective To evaluate the effects of curcumin pretreatment on the expression of Nrf2 protein during ventilator-induced lung injury in rabbits.Methods Twenty-four healthy male New Zealand white rabbits,aged 3-6 months,weighing 2.5-3.0 kg,were randomized into 3 groups (n =8 each) using a random number table:two-lung ventilation (TLV) group; one-lung ventilation (OLV) group; and curcumin pretreatment group (group Cur).In group Cur,curcumin 40 mg/kg (dissolved in 2 ml of 1% sodium carboxymethylcellulose) was given via a gastric tube into the stomach twice a day for 7 consecutive days starting from 7 days before ventilation,while the equal volume of sodium carboxymethylcellulose was given via a gastric tube instead of curcumin in TLV and OLV groups.All the rabbits were tracheostomized,and a tracheal tube was inserted to perform TLV in TLV group,and a tracheal tube was inserted into the right bronchus to establish OLV in OLV and Cur groups.Volumecontrolled ventilation was used in the three groups and the ventilatory parameters were regulated to maintain SpO2 > 90 %.Immediately before beginning of ventilation (T0) and at 1,2 and 3 h of ventilation (T1-3),arterial blood samples were obtained for blood gas analysis and determination of PaO2.The oxygenation index was calculated.At the end of ventilation,the rabbits were sacrificed and right lungs were removed for determination of wet/dry lung weight ratio (W/D ratio).The right lower lobe was isolated and puhmonary specimens were obtained for determination of malondialdehyde (MDA) content and superoxide dismutase (SOD) activity (using colorimetric method) and the expression of Nrf2 and HO-1 protein (by Western blot) in lung tissues and for microscopic examination of pathological changes of the lung which were scored.Results Compared with group TLV,the W/D ratio,pathological scores,MDA content,and expression of Nrf2 and HO-1 were significantly increased,and the SOD activity and oxygenation index at T2,3 were decreased in OLV and Cur groups (P < 0.05).Compared with group OLV,the W/D ratio,pathological scores,and MDA content were significantly decreased,and the SOD activity,oxygenation index at T3,and expression of Nrf2 and HO-1 were increased in group Cur (P < 0.05).Conclusion Curcumin pretreatment reduces ventilator-induced lung injury through promoting the expression of Nrf2 protein in lung tissues in rabbits.
5.Diagnostic Value of Pathogenic Detection on Acute Respiratory Infection in Children by Multi-IgM Investigation

Objective To investigate the pathogens of acute respiratory infection of children.Methods A total of 159 children with acute respiratory infection who were hospitalized in our department from August 2005 to January 2006 were involved in this study.The serum IgM antibody of 18 pathogens were detected by indirect immunofluorescence test.The 18 pathogens included respiratory syncytial virus(RSV),adenovirus(ADV),influenza A(H1N1,H3N2)and B viruses,parainfluenza viruses(PIV) type 1,2,3 and 4,coxsackie virus B1(CBV1),coxsackie virus A7(CAV7),echovirus(ECHO7),haemophilus influenzae(HI),klebsiella pneumoniae(KP),bordetella pertussis(BP),bordetella parapertussis(BPP) and legionella pneumophila serotype 1 and 12.Results The evidence of specific IgM was obtained in 103 of 159 patients(64.78%).Influenza A was found in 66 cases(64.08%),influenza B in 49 cases(47.57%),enterovirus in 26 cases(25.24%),RSV in 18 cases(17.48%),PIV in 11 cases(10.68%),and co-infection in 66 cases(64.08%),1/ 3 of them were co-infected with influenza A and B.Conclusions Viruses are the most common agents of acute respiratory infection.Influenza virus is predominant among them.
6.Distribution and drug resistance of Gram positive bacteria of ten hospitals in Chongqing

Objective To investigate the distribution and drug resistance of the gram positive bacteria isolates from hospital in Chongqing during 2006-2011 .Methods Non-repetitive gram positive bacterias were collected retrospectively from ten hospitals in Chongqing .Antimicrobial susceptibility testing of all clinical isolates were carried out using Kirby-Bauer(KB) .Results 14 473 strains were isolated .There were 4 840 strains of Staphylococcus aureus (33 .4% ) ,2 566 strains of Staphylococcus epidermidis (17 .7% ) ,1 822 strains of Staphylococcus haemolyticus (12 .6% ) ,1 325 strains of Enterococcus (E .faecalis/E .faecium )(9 .2% ) . Staphylococcus was highly sensitive to nitrofurantoin ,rifampicin ,piperacillin/tazobactam ,linezolid ,teicoplanin and vancomycin ,but their resistance rate to ampicillin/sulbactam ,oxacillin ,penicillin and clarithromycin was high .The resistance rate of E .faecium to the majority of antimicrobial agents was obviously higher than that of E .faecalis ,excluding imipenem ,chloramphenicol and tetracy-cline .Vancomycin and linezolid still remain the strongest antibacterial activity to Enterococcus .Conclusion The distribution and drug resistance of pathogenic bacteria in Chongqing present regional particularity ,and antibiotic resistance is severe .It is significant to strengthen the epidemiology study of pathogenic bacteria and to develop policies to intervene in the use of antimicrobial agents .
7.Analysis of the Prevalence and Economic Burden of Diabetes in Guandu District of Kunming

Objective To analyze the prevalence and economic burden of diabetes in Guandu district of Kunming. Methods We used probability proportional to size (PPS) sampling method to select representative sample of 4595 residents aged 18 or over from this district. Each participant received face to face questionnaire interview and physical examination. We applied different methods to measure the direct,indirect and intangible costs of diabetes. Results In the study population, the overall prevalence of diabetes was 6.2%,and females had higher prevalence of diabetes than males (6.2%vs. 5.6%, <0.05) . The DALY/1000 population of diabetes was 3.52, among which males and females were 5.18 and 6.70, respectively. Mean unit direct costs, indirect costs and intangible costs of diabetes were 3464.49 Yuan,84.48 Yuan and 4 045.97 Yuan,respectively. The total economic burden of diabetes was 401.84 million Yuan. Intangible costs represented the largest component of economic burden of diabetes,followed by direct costs. Conclusion The huge economic burden of diabetes has become the cause for concern in Guandu district. Effective measures are needed to reduce the economic burden of diabetes.
8.Drug Resistance and Detection of ?-Lactamase from Chryseobacterium spp:A Five Year Surveillance

OBJECTIVE To investigate the isolation and drug resistance of Chryseobacterium spp in our hospital,and to explore the mechanisms of drug resistance.METHODS Bacteria were identified in our hospital for the last five years(Jan 2001-Dec 2005) and the antimicrobial susceptibility was tested by Kirby-Bauer plate dilution method.Forty-three isolates of C.meningosepticum,16 isolates of C.indologenes and 10 isolates of C.gleum were isolated and selected for further studies.Minimum inhibitory concentrations(MICs) against 14 antibiotics were determined by the agar dilution method.Extended-spectrum ?-lactamase(ESBL) and carbapenemase were detected by three-dimensional test and 2-mercaptopropionic acid inhibitory test.RESULTS One thousand and one hundred twenty-eight Chryseobacterium and others strains in total were isolated during the described period.Among them C.meningosepticum,C.indologenes,C.gleum,and other Chryseobacterium species were 88.3%,8.0%,2.9%,0.6% and 0.2%,respectively.The resistant ratios against antibiotics containing enzyme inhibitors were lower than other antibiotics.The MIC50 and MIC90 against most antibiotics were high except for quinolones.As for carbapenemase,the positive rate was 60.5%,68.8% and 90.0% in C.meningosepticum,C.indologenes,and C.gleum,respectively.CONCLUSIONS Chryseobacterium are highly resistant against a variety numbers of antibiotics.Nevertheless,there exists a significant difference in the resistance against different antibiotics for different species of Chryseobacterium.The major drug resistant mechanism in Chryseobacterium is due to the production of ?-lactamases,especially metallo-?-lactamases.

10.Comparison of postoperative cognitive function in elderly patients undergoing sevoflurane-based anesthesia versus propofol-based anesthesia

Objective To compare the postoperative cognitive function in elderly patients undergoing sevoflurane-based anesthesia versus propofol-based anesthesia.Methods Sixty-two patients of both sexes,aged 65-80 yr,weighing 48-90 kg,of ASA physical status Ⅰ or Ⅱ,were randomly allocated to either sevoflurane-based anesthesia group (group S,n=31) or propofol-based anesthesia group (group P,n =31).At 1 day before operation and 7 days after operation,cognitive function was assessed by MiniMental State Examination,Digit Span Test (forward test and backward test),Digit-Symbol Substitution Test,Trail Making Test A and Word Recognition Test,and the scores were recorded.Results There were no significant differences between the two groups in the scores of each test used for assessment of cognitive function.Compared to group P,Mini-Mental State Examination scores,forward test scores and backward test scores obtained from Digit Span Test,Digit-Symbol Substitution Test scores,and Word Recognition Test scores were significantly decreased at 7 days after operation,and no significant change was found in Trail Making Test A scores in group S.Conclusion Postoperative cognitive function is decreased under sevoflurane-based anesthesia when compared with that under propofol-based anesthesia in the elderly patients.
