1.Efficacy of pulsed dual-wavelength 595 and 1064 nm laser in treatment of ulcerated infantile hemangioma

Objective To observe the efficacy of pulsed dual-wavelength 595 and 1064 nm laser in treating ulcerated infantile hemangioma.Methods Lesions from 40 patients were treated with the laser.The distribution of lesion sites was in buttuck and perianal area (13 patients),vulvar lips (7),groin and inside thigh (5),scrotum (4),neck (3),scalp (2),lip (2),trunk and limbs (4).All lesions including ulcer and non-ulcer were conducted with 595 nm plused dye leaser at the first therapy.Later,595 nm pulsed dye leaser was continued or performed with Group 1 or Group 2 parameter of 595 and 1064 nm multiplex mode until the treatment was finished according to the thickness of the lesion.The frequency of treatment was one to six times.The interval of the treatment was four weeks.Results The ulcer from 39 lesions healed gradually in 1 to 2 weeks (average 9.5 days) after the first treatment.These ulcer lesions were healed four weeks after the first time of therapy.The recovery rate of ulcerated lesion was 97.5 %.For the whole lesion,after 1 to 6 treatments,13 lesions showed excellent result,19 lesions showed good result,and the effective rate was 80 %.Pain relieved 2 to 7days (average 3.5 days) after the first laser therapy in 35 patients who accompanied with pain.Pain score decreased from 1.875 to 0.125 before and after treatment,with significant difference(P<0.05).The tolerance of the treatment was good and no side effect was observed.Conclusions 595and 1064 nm dual-wavelength laser is a safe and effective tool for treating ulcerated infantile hemangioma.
2.Cost-effectiveness of integrated pharmaceutical care in community-based patients with chronic obstructive pulmonary disease

Objective To analyze the cost-effectiveness of integrated pharmaceutical care (IPC) in community-based patients with chronic obstructive pulmonary disease (COPD).Methods The COPD patients were divided randomly into intervention group (group A,n =79) and control group (group B,n =71).Group A received the IPC intervention measures,including serial lectures of pharmaceutical knowledge,home supervision and web-based communications for while group B group had none.Results As compared with group B,group A obviously improved on the knowledge of pharmaceutical treatment effect and side effect (95.1%,67.4% vs.63.1%,21.9%,x2 =12.445,P=0.000 vs.x2 =55.557,P=0.000).There were notable differences between two groups [ ( 1.77 ± 1.23 ) vs.( 2.42 ± 1.66 ) s,t =2.583,P=0.011],pulmonary rales [ (0.01 vs.0.01)s,Z =2.370,P=0.018],6-min walk distance (6 MWD) [ (457 ± 67 ) vs.(425 ± 72) m,t =2.760,P =0.007 ],vital capacity ( VC ) [ ( 1.60 ± 0.25 ) vs.( 1.49 ± 0.23 ) L,t =2.718,P =0.007 ) ] andquality-of-life items ( 48 ± 10 vs.52 ± 11,t =2.624,P =0.010) after intervention.Analyze of cost-effectiveness show that groupA was superior to group B [ total effect total cost(TE/TC =764.6/4936,15.49% )vs.(TE/TC =1509.4/4708),32.06%,x2 =6.183,P < 0.01 ].Conclusion IPC can improve the disease condition and achieve excellent cost-effectiveness for community-based COPD patients.
3.Analysis of the condition of Helicobacter pylori infection in one hundred and fifty-two patients with colorectal tumor

Objective To explore the relation between the infection of Helicobacter pylori (H .pylori)and colorectal tumor.Methods From January 2012 to January 2013,the medical data of 152 patients with colorectal tumor were collected.According to the findings of colonoscopy examination, the patients were divided into the colorectal adenoma group (84 cases)and the colorectal carcinoma group (68 cases).A total of 88 healthy individuals were also enrolled as control.The differences of H .pylori infection were compared among the colorectal adenoma group,the colorectal carcinoma group and the healthy control group.The distribution of H .pylori infection in different age,gender and nation was analyzed in colorectal tumor group.According to the location,maximum diameter,number,type of pedicle and pathological type of adenoma,the colorectal adenoma group was divided into subgroups and the condition of H .pylori infection was compared among the subgroups.Chi-square test was used in the statistical analysis.Results The positive rates of H .pylori infection of colorectal adenoma group, colorectal carcinoma group and healthy control group were 70.2% (59/84),72.1 % (49/68)and 53.4%(47/88),respectively,where the former two groups were both higher than that of the healthy control group,and the differences were statistically significant (χ2 =5 .147 and 5 .637,both P <0.05).Regarding the H .pylori infection,there was no difference in age,gender and ethnicity among 152 patients with colorectal tumor (all P > 0.05 ).Among 84 patients with colorectal adenoma,there was no statistical difference in the positive rate of H .pylori infection in different location,maximum diameter,number, type of pedicle and pathological type of adenoma (all P > 0.05).Conclusion H .pylori may have promoting effects on the genesis and development of colorectal tumor.
4.Alkanethiol modified magnetic silver flower nanoparticles applied to detection of chloramphenicol

Objective To prepare novel alkanethiol modified magnetic silver flower nanoparticles as SERS substrate to chloramphenicol for Raman detection and to determine their enhancement effect.Methods An alkanethiol was chosen as a surface modifier of the substrate and was self-assembled onto the magnetic silver flower nanoparticle surface.The chloram-phenicol molecules were enriched to the surface of the substrate by hydrophobic interaction and the effect for detection of chloramphenicol SERS signal was enhanced.Results It was found that the 1-hexanethiol-modified SERS substrate was able to lead to stronger enhancement than 1-dodecanethiol and octadecanethiol.Fe3 O4@SiO2-Ag-C6 was used to detect the chloramphenicol (10 -3 -10 -10 mol/L) and chloramphenicol in milk (10 -3 -10 -9 mol/L) by surface-enhanced Raman spectroscopy.The detection limits were 0.1 nmol/L (32 ppt) and 1 nmol/L (323 ppt) respectively.Conclusion Alkanethiol modified magnetic silver flower nanoparticles are a highly active SERS substrate, which can be used for detection of low concentrations of analytical substances.

Objective To study the changes and significance of Bid gene expression in hypoxic-ischemic brain damage (HIBD) in neonatal rats.Methods The animal models of HIBD were made. Total RNA from ipsilateral cerebral hemisph ere was extracted.RT-PCR was used to evaluate the levels of Bid gene expression at different time points after hypoxic-ischemic. Results The levels of Bid mRNA were higher in HIBD rats than those in normal rats.The ex pression of Bid mRNA in ipsilateral hemiphere following cerebral hypoxic-ischem ic began to increase at 6 h, increased to the highest level at 24 h, and then gr adually returned.Conclusion The upregulation of Bid mRNA can play a role in the induction of apoptosis fol lowing cerebral hypoxic-ischemic. The postpartum depression outcome and the effect of psychological intervention were studied in order to reduce the occurrence and development of the postpartum depression. A survey of 4000 women within 4-6 weeks postpartum in 80 communities in Shenzhen, China was performed using random cluster sampling method. By employing Edinburgh Postnatal Depression Scale (EPDS) as a screening tool, the positive women (defined as EPDS ≥10) were randomly divided into intervention group and control group at a ratio of 1:2. The women in the intervention group were treated by means of mailing postpartum depression prevention and treatment knowledge manual, face-to-face counseling, and telephone psychological counseling interventions aiming at individual risk factors, while those in the control group were treated with conventional methods. EPDS scores were assessed in these two groups again at 6th month postpartum. Totally, 3907 valid questionnaires were obtained. All the 771 positive women were divided into two groups: 257 in the intervention group, and 514 in the control group. At 6th month postpartum, the EPDS scores in the intervention group were decreased significantly, from baseline stage (12.84±3.02) to end stage (3.05±2.93), while EPDS scores in the control group were reduced from 12.44±2.78 to 6.94±4.02. There were significant differences in the EPDS scores at end stage between the two groups (t=13.059, P<0.001). Psychological intervention can reduce postpartum depression, with better maternal compliance. It is feasible and necessary to establish postpartum depression screening and psychological intervention model in community-hospital and include the postpartum depression screening, intervention, and follow-up into the conventional healthcare.
