1.Construction of clinical microbiological laboratory with fast-responsibility

Inadequate empirical antibiotic therapy and delayed target therapy have been demonstrated to be significantly associated with higher mortality.Because microbiological results based on the traditional culture and identification require quite a long time,empirical broad-spectrum antimicrobial therapy is often initiated according to the clinical presentation,which may resulting in the development of antimicrobial resistance.One of the strategies to alter the dilemma situation is that the initial empirical therapy is adjusted to definitive therapy as early as possible,which can only be done on the basis of rapid microbiological reports.Clinical microbiology laboratories should integrate a variety of rapid detection methods,improve working process,present reports as soon as possible,and bear fast-responsibility.
2.Adverse Drug Reactions in Our Hospital:Analysis of 125 Cases

OBJECTIVE:To study the characteristics and general pattern of the adverse drug reactions(ADR)occurred in our hospital.METHODS:A total of 125 ADR cases collected in our hospital during 2007 were analyzed statistically in respect of patients' age and sex,route of administration,serious ADR,drug combination,past ADR history,drug kinds,ADR-involved organs or systems and clinical manifestations etc.RESULTS:Of the total 125 ADR cases,11 categories or 62 kinds of drugs were involved,with antimicrobial drugs took the lead which caused ADRs in 65 cases(52.0%)followed by Chinese patent agents which caused ADR in 38 cases(30.4%).102(81.6%)ADR cases were induced by intravenous route.Lesions of skin and appendants were the most common presentations of the ADR,which appeared in 70 cases(52.24%).10(8.0%)were severe cases.CONCLUSION:Importance should be attached to the reporting and monitoring of ADR so as to lessen or avoid the occurrence of ADR in the clinic meanwhile promoting rational use of drugs and ensure medication safety.
3.Clinical Observation of Kudiezi Injection Combined with Alprostadil in the Treatment of Posterior Circula-tion Ischemic Vertigo

OBJECTIVE:To observe the clinical efficacy and safety of Kudiezi injection combined with alprostadil in the treat-ment of posterior circulation ischemic vertigo. METHODS:Totally 180 patients with posterior circulation ischemia were randomly divided into group A,group B and group C. All patients were given aspirin,atorvastatin,nutrition nerve,antihypertensive,hypo-glycemic and other conventional treatment. On this basis,group A was given Alprostadil injection 10 μg,adding into 0.9% Sodium chloride injection 100 ml,iv,once a day;Kudiezi injection 30 ml,adding into 0.9% Sodium chloride injection 250 ml,iv,once a day. Group B was given Alprostadil injection(the same usage as group A);group C was given Kudiezi injection(the same usage as group A). The course was 14 d. The clinic data was observed,including clinical efficacy;blood flow velocity in left vertebral ar-tery(LVA),blood flow velocity in right vertebral artery(RVA),blood flow velocity of basilar artery(BA),changes of peak laten-cy(PL)and incubation period latency(IPL)before and after treatment;and the incidence of adverse reactions. RESULTS:The to-tal effective rate in group A was significantly higher than group B and C,with significant difference(P<0.05). After treatment,the blood flow velocity of LVA,RVA and BA in each group were significantly higher than before,group A was higher than group B and C;PL-Ⅰ,PL-Ⅲ,PL-Ⅴ,IPL-Ⅰ-Ⅲ and IPL-Ⅲ-Ⅴ were significantly lower than before,group A was lower than group B and C,with significant differences(P<0.05);however,there were no significant difference between group B and C(P>0.05). There were no obvious adverse reactions during treatment. CONCLUSIONS:Kudiezi injection combined with alprostadil has better effica-cy than only Kudiezi injection or alprostadil in the treatment of posterior circulation ischemic vertigo,with good safety.

The ways and key factors were thus put forward for public health-oriented popular science information service in medical library by analyzing the advantageous medical information resources, advanced tech-nical equipments and professional staff in medical library.
5.Pathogenic significance and possible pathogenic mechanism of human endogenous viruses in development of schizophrenia.

The association between psychogenic illness and human endogenous viruses (HEVs), including human endogenous retrovirus and Borna disease virus, remains unclear. As the component of human genome, HEVs may become the joint of various pathogenic factors of schizophrenia (SZ), such as heredity, environment, and immunity. In this review, we strive to uncover the clinical and laboratory evidence for the roles and possible pathogenic mechanism of HEVs in the development of SZ.

8.The best cut point of IFG and effects of its change on the statues of metabolism among Chongqing adults in China

Objective To find the best cut point of FPG to predict diabetes(DM) and impaired glucose tolerance(IGT) and to study its effects on the metabolism status.Methods The ROC analysis of FPG in 3189 citizens from some area of Chongqing without a previous history of diabetes was done by OGTT in a cross-section study.The metabolic features of impaired glucose regulation(IGR) and its subcategories were analyzed according to 5.6 mmol/L and 6.1 mmol/L as the FPG threshold for IFG.Results The areas under the ROC curve were 0.899 for diabetes and 0.728 for IGT.The cut point of FPG with the best equilibrium between sensitivity and specificity was 5.6 mmol/L for diabetes and 5.2 mmol/L for IGT.Among people whose 2 hPG
9.Brief Introduction on High-value Drug Insurance Access Mechanism in Australia and Its Enlightenment on Orphan Drug Medical Insurance in China

OBJECTIVE:To introduce high-value drug insurance access mechanism in Australia,and to provide suggestion for improving orphan drug medical insurance access in China. METHODS:By literature study and policy analysis,access principle, risk-sharing mechanism and restricted use principle of high-value drug insurance access in Australia were summarized and analyzed, so as to evaluate the effects of high-value drug insurance access mechanism. RESULTS & CONCLUSIONS:The inclusion of high-value drug in Australian National Formulary considers not only the cost-effectiveness ratio,but also the drug’s clinical effica-cy,irreplaceability and the social principles;risk-sharing mechanism and restricted use principle achieve a good balance between the accessibility of patient’s medication and the sustainability of health insurance fund;remarkable results are achieved in guarantee-ing patient’s medication and strengthening basic medical insurance. It is recommended to perfect orphan drugs medical insurance ac-cess standard,introduce risk-sharing mechanism and standardize the use of orphan drugs and reimbursement in China.
10.Comparative Analysis on Contents of Phenolic Acids in the Decoction of Xanthii Fructus before and after Frying

Objective:To examine the content changes of neochlorogenic acid, chlorogenic acid, 1, 5- dicaffeoylquinic acid and total phenolic acids in the water extract from raw and fried Xanthii Fructus. Methods:The stir-frying method was used to process fried Xanthii Fructus from different habitats. The contents of neochlorogenic acid, chlorogenic acid and 1,5-dicaffeoylquinic acid in the wa-ter extract were determined by HPLC, the total phenolic acids content was determined by UV. Results:The contents of neochlorogenic acid, chlorogenic acid, 1,5-dicaffeoylquinic acid and total phenolic acids in the water extract from fried Xanthii Fructus were all in-creased. Conclusion:Fried Xanthii Fructus can increase the contents of effective ingredients in the decoction resulting in the enhanc-ment of clinical curative effect.
