Objective A retrospective study of GCTTS in the hand was undertaken to determine whether specific clinic or pathologic factors were associated with an increased risk of recurrence, and to investigate the relationship between the recurrence of GCTTS and its biological indices. Methods 83 patients with histologically proven GCTTS were treated and followed up. The clinical materials were reviewed. Comparison was made between certain factors to determine which were associated with increased recurrence rates. Results The patients were followed for an average of 55 months. 22 patients (26.5%) recurred. Age, gender, site, the origin of tumors, size, bone erosion or destruction, cellularity and the mitoses were not risk factors for recurrence. Tumors without capsule had higher recurrence rate than that with capsule, and the difference was highly significant. The local recurrence rate of Nm23 positive group was 28.3%, and Nm23 negative group was 19.2%, there was no remarkable difference between the groups. The PCNA-LI was 0.448?0.130 in recurrent group and 0.358?0.147 in nonrecurrent group, the difference was statistically significant. The aneuploidy DNA content was presented in 50% of the recurrent tumors, and in 23.2% of nonrecurrent ones. The SPF was 6.98?4.64 in recurrent group and 4.70?2.49 in nonrecurrent group. These values were also significantly different. PCNA-LI, the aneuploidy DNA content and SPF were significantly higher in group without capsule than those in group with capsule. These values were not different between tumors with and without bone destruction, large and small diameter, high and low cellular tumors, as well as high and low mitoses tumors. Conclusion The high proliferative indices of recurrent GCTTS may explain its aggressive biologic behavior. Tumors without capsule possess higher proliferative indices than the ones with capsule, and they have higher recurrent rate. The proliferative indices of the tumors with and without bone distruction are comparable, and the difference of the recurrence rate among groups is not significant.

Objective To survey the double-preceptor system in standardized residency training of general practitioners (GPs).Methods A questionnaire survey was conducted among 48 residents,53 clinical and community preceptors during May and June 2013.Results The preceptors mostly agree with the necessity to carry out double-preceptor system.82.8% (24/29) clinical preceptors,95.8% (23/24) community preceptors and 89.6% (43/48) residents considered that it was very effective.Less than half of the clinical(13/29) and community (11/24) preceptors knew very clearly about their responsibilities and obligations ; 93.1% (27/29) clinical preceptors and 95.8 % (23/24) community preceptors hoped to receive corresponding training courses.72.4% (21/29) clinical preceptors and 75.0% (18/24) community preceptors communicated with residents frequently,75.9% (22/29) clinical preceptors thought residents were not actively involved was the main obstacle in the implementation of double-preceptor system. The guide contents were focused on clinical skills, scientific research and how to pass assessments. The main approaches were interviews, checking the quality of medical instruments and small lecture.Only 6.9% (2/29) clinical preceptors conducted outpatient teaching,83.3% (40/48)residents thought tutors did not knew clearly about their responsibilities and obligations was the main obstacle in the practice of doublepreceptor system.Regarding how to evaluate the double-preceptor system, clinical and community preceptors tended to use examination results and feedback of residents.Clinical preceptors, community preceptors and residents all gave low comments about the preceptor-resident contact handbook.87.5% (21/24) community preceptors thought lack of corresponding incentive mechanisms was the main obstacle in the practice of double-preceptor system. Conclusions Double-preceptor system has been widely recognized by both preceptors and students. In order to improve the system,we should conduct tutor training courses regularly and establish an effective mechanism to assess and evaluate tutors.

A total of 133 smokers visiting our smoking cessation clinic from February 2009 to September 2010 were studied. The data were collected during initial assessments and over a follow-up period of 24 months.Long-term abstinence rate was observed. And logistic regression was employed to identify the independent predictors of successful quitting. There were 132 males and 1 female with a mean age of 47 years. They smoked an average of 26 cigarettes per day.The mean duration of smoking was 25 years.Among them,57 smokers (42.9%) were severely nicotine dependent and 76 smokers (57.1%) were followed up for 24 months. And 20 smokers (26.3%) maintained 24 month abstinence. The predictor of avoiding relapse was immediate family members being nonsmokers.

In the middle of 1990 s, it was discovered that sirolimus had inhibitory effect on the proliferation of T lymphocytes , therefore, it is used for the treatment of anti-tumor cells. At present, the dosage forms of sirolimus are capsules , oral liquid and tab-lets.However, because of good fat-soluble and poor water-soluble property, its bioavailability is low with many side reactions .In order to provide reference for the further studies of sirolimus , the literatures in recent years were searched in Pubmed , Springer, CJFD,VIP Chinese periodical database and Wanfang data , and the research progress in the new dosage forms of sirolimus was summarized .

Objective To investigate the effect of Kanglaite injection (KLT) on immunological function of rat models with Lewis lung carcinoma. Methods Forty C57BL/6 mice were used to establish Lewis lung carcinoma models and divided randomly into the high dose(25 mL/kg), middle dose (12.5 mL/kg) and low dose (6.25 mL/kg) of KLT groups and model group(n=10). The mice in the KLT groups were sacrificed after injecting corresponding dose of KLT with intraperitoneal injection for 14 d. No treatment was performed on the rats in model group. The body weight, tumor and spleen weight was weighed, then the ratio of tumor restriction and the index of spleen was calculated. MTT colorimetric method and ELISA were used to detected activity of T cell proliferation and expression of IL-2 in spleen. The expression of NF-κB and IκBα protein was detected by Western blot. Results The ratio of tumor restriction in the high, middle, low dose of KLT groups decreased gradually. The indexes of spleen of the high and middle dose of KLT groups were higher than those in the model group (P<0.05 or P<0.01).Compared with the model group, the activity of T cell proliferation in the high, middle, low dose of KLT groups and the expression of IL-2 in the high and middle dose of KLT groups was increased significantly (P<0.05 or P<0.01). The expression of NF-κB protein in the nuclei of high, middle, low dose of KLT groups increased dose-dependently, and the expression of NF-κB and IκBα protein in the cytoplasm decreased dose-dependently. ConclusionKLT could enhance immunological function by effecting T cell proliferation, expression of IL-2, NF-κB and IκBα, while restricting tumor growth in Lewis lung carcinoma models.
