Objective To explore the value on the treatment of severe acute pancreatitis (SAP) by hemofiltration combined with Dan Sen and catheterizing drainage through laparoscope. Methods Thirty patients of SAP were divided into two groups. On the basis of routine treatment(supportive treatment,suppressing excrine of pancreas stilamin,trypsin inhibitor and antibiotics),hemofiltration,catheterizing drainage,stilamin and Dan Sen were added in group A,while only routine treatment were administered in group B. TNF,IL-6,IL-8,score of Balthazar CT and score of APACHEⅡ were studied in both groups. NNS could decrease blood viscosity and hematocrit, inhibit the aggregation of erythrocytes and platelet and increase the activities of superoxide dismutase (SOD) and glutathione peroxidase (GSH -Px). [Conclusion] NNS has an effect on AIS, which is similar to nimodipine. Its therapeutic mechanism may be related to the relief of cerebral ischemia by improving blood rheology and cerebral blood flow and protecting brain cells from injury by eliminating free radicals.
5.Protective Mechanism of Naoxing Nasal Spray on Ischemic Cerebral Injury

[Objective] To explore the protective mechanism of Naoxing Nasal Spray (NNS) on ischemic cerebral injury. [Methods] Seventy Wistar rats were randomized into 7 groups: high-dose NNS group (group A), moderate-dose NNS group (group B), low-dose NNS group (group C), nimodipine group (group D), model group (group E), solvent control group (group F) and pseudo-operation group (group G). Rat models of ischemia/reperfusion were established by blocking left middle cerebral artery. Plasma levels of tumor necrosis factor a (TNF-?) and interleukin 1? (IL-1?), malondialdehyde (MDA) content, activities of superoxide dismustase (SOD), glutathione peroxidase (GSH-Px) and nitric oxide synthase (NOS) in brain homogenate were detected. [Results] NNS decreased the contents of MDA, TNF-? and IL-1?, and increased the activities of SOD, GSH-Px and NOS. [Conclusion] NNS has protective effect on ischemic cerebral injury by promoting the clearance of free radicals, inhibiting the release of inflammatory mediator and increasing the synthesis of NO and thus to reduce injury of free radicals, inhibit the local inflammatory reaction and improve cerebral blood perfusion.
6.Staged resuscitation for uncontrolled hemorrhagic shock induced by simulated abdominal trauma in field battle

Objective To establish an abdominal-trauma-induced uncontrolled hemorrhagic shock (UHS) model, and explore the effect of staged resuscitation. Methods Indwelling catheters were inserted into left ventricle, femoral artery and femoral vein of 50 Sprague-Dawley rats, and then laparotomy was performed. 42 out of the 50 animals were subjected to vascular injury to simulate UHS by infra-renal arterial puncture with 25G needles, and the model was successfully established in 32 rats, for which the animals were divided into 3 groups: control group (n=10), with no resuscitation; hospital-resuscitation group (n=11), resuscitated in early injured stage; and staged-resuscitation group (n=11), resuscitated pre-hospital and then in-hospital. 8 out of the 50 rats, which were not subjected to vascular injury, served as sham-operated group. The mean aortic pressure (MAP), central venous pressure (CVP), serum lactic acid, alanine aminotransferase (ALT) and creatinine (Cr) were determined, and the volume of blood loss and survival time were recorded. Results The MAP decreased from 94.7mmHg to 28.9mmHg 15min after aortic injury in control group and rose slowly after continuous transfusion in staged-resuscitation group. Similar change was found in CVP. Serum lactic acid, ALT and Cr were increased in control group and hospital-resuscitation group at early injured stage, but the increase was delayed in staged-resuscitation group. The mean survival time in control group, hospital-resuscitation group and staged-resuscitation group were 76, 846 and 2480 min, respectively. The 72 h mortalities of the three groups were 100%, 72.7% and 36.4%, respectively. Conclusion Staged resuscitation which is characterized by slowly continuous transfusion at early aid stage cannot increase the blood pressure and circulating blood volume to normal level in UHS, but can reduce the level of serum lactic acid, ALT and Cr, and dose not increase the blood loss. The survival rate and mean survival time can be increased by this method.
7.Prognostic value of combining postprocedural fibrinogen with C-reactive protein levels in acute myocardial infarction patient underwent primary percutaneous coronary intervention

Objective To evaluate the prognostic value of combining postprocedural fibrinogen with high-sensitivity C-reactive protein (hs-CRP) in first time ST elevated myocardial infarction (STEMI) patients who had underwent successful primary percutaneous coronary intervention (PCI).Methods A total of 183 consecutive patients who had their first acute STEMI attack and underwent successful primary PCI were enrolled. Fibrinogen and hs-CRP levels were measured within 12 hours after PCI. All patients were followed up for 2 years. The primary end point was death of any cause. The secondary end point was a combined end point of death, non-fatal MI, heart failure (NYHA Ⅲ~Ⅳ),myocardial ischemia confirmed by stress test and revascularization. 8.Malnutrition related factors in patients with gastric cancer

Malnutrition is one of the most important factors for the prognosis of patients with gastric cancer,but the mechanism of malnutrition is unclear at present. These patients are at risk of nutritional deple-tion from anorexia,dysphagia,and may be associated with the presence of two or more of the following cyto-kines with metabolic activity such as TNF-α,IL-1,IL-6,interferon,leptin,nuclear factor-κB and so on, which are secreted by the host-tumor interaction. The cytokines constitute a sophisticated regulatory network. Therefore,the study of the relationship between malnutrition and related factors in patients with gastric cancer may improve the nutritional status and prognosis of these patients.
9.Investigation of status quo of self-perceived burden of young and middle-aged patients with first-attack of ischemic stroke and analysis of its influencing factors

Objective To know the self-perceived burden (SPB) of young and middle-aged patients with first-attack of ischemic stroke and to analyze the influential factors of SPB as well as its relevance to the quality of life.Methods The patients' general information questionnaire,SPB Self-evaluation Scale,QOL-BREF were adopted to investigate young and middle-aged patients with first-attack of ischemic stroke.Results 100 patients were included,66 were male,34 were female.The Minimum age was 19.4 years old.The maximum age was 59 years old.The mean age was (50.80 ± 7.98) years old.The average SPB score was (24.21 ± 7.39) points,which was in the medium level.31 cases (31%) didn't have obvious SPB; 65 cases (65%) suffered mild-to-moderate SPB; 4 cases (4%) claimed they had severe SPB.There was negative correlation between the SPB and their quality of life.The multi-factor analysis showed that gender,the patients' understanding of disease,and caregivers' health situation were the influential factors of SPB.Conclusions Since self-perceived burden is common in young and middle-aged patients with ischemic stroke,clinical nurses should assess patients' self-perceived burden so as to provide targeted nursing intervention and health education,thereby to ease the burden on patients' self feeling,eventually to improve the level of their mental and physical health.
10.Detection and Significance of Anti-β2 Glycoprotein 1 Antibodies in Female Infertility and Threatened Abortion

Objective To analysis the correlation of the anti-β2 glycoprotein 1 antibodies in female infertility and threatened a-bortion.Methods Selected 547 patients with female infertility,229 patietns with threatened abortion,229 patietns with ir-regular menses and 31 normal female between August 2013 to January 2014.All the anti-β2 glycoprotein 1 antibodies (Ig-G/Ig-M)were detected by ELISA.Detected the anti-β2 glycoprotein 1 antibodies (Ig-G/Ig-M)of 114 patients who treated with aspirin and prednisone combined with gamma globulin.Results The positive rate of anti-β2GP1 antibodies (Ig-G/Ig-M)of female infertility,threatened abortion,irregular menses and normal female were 0.36%,0.43%,1.6%,0% and 20.29%, 19.21%,8.3% and 3.2%,respectively.The positive rate of aβ2GP1-IgM of female infertility,threatened abortion,irregular menses were higher than normal female.The anti-β2GP1-IgM of the treated female infertility were 47.14± 34.85 RU/ml and 31.14±27.64 RU/ml,respectively.The anti-β2GP1-IgM of the treated threatened abortion were 37.75±31.20 RU/ml and 24.34±24.48 RU/ml,respectively.The levels of anti-β2GP1-IgM of the treated patietns were significantly lower than before.Conclusion There was a close relationship between anti-β2GP1-IgM antibodies and female infertility and threatened abortion,anti-β2GP1-IgM antibodies maybe one of the immune factors for female infertility and threatened abortion.
