OBJECTIVE:To explore the role and breakthrough point of clinical pharmacist in the anti-infective therapy for a patient with purulent meningitis. METHODS:Clinical pharmacists provided pharmaceutical care for a patient with purulent meningi-tis,assisted physicians to adjust therapy plan of antibiotics,i.e. anti-infective therapy of vancomycin+ceftriaxone sodium,meropen-em+vancomycin,levofloxacin+vancomycin successively;disposed ADR induced by meropenem and levofloxacin. RESULTS:The physicians adopted the suggestions of clinical pharmacists,the infection symptom had been controlled,and ADR disappeared;and then the patient was discharged from hospital. CONCLUSIONS:Clinical pharmacists provide pharmaceutical care for patient with purulent meningitis,assist the physicians to develop safe and effective anti-infective therapy plan,and dispose ADR timely so as to promote rational drug use in the clinic.

Acute megakaryoblastic leukemia(AML-M7)is a rare neoplastic hematologic disorder,the pathogenesis is not clearly.Recent research has found many eytogenetic changes in this disease;and also some genes mutation,such as GATAI,p53,RUNXI play very important roles in AMKL.The aim of this article is to review the cytogenetic and molecule mechanism of acute megakaryoblastic leukemia.

Objective To observe the clinical efficacy of thrombolytic therapy combined with psychological intervention in treatment of patients with acute myocardial infarction.Methods 80 cases were treated in department of emergency, Hangzhou First People’s hospital were randomly divided into observation group and control group from February 2014 to February 2015.40 cases in each group.Observation group was given psychological intervention conventional therapy plus thrombolytic therapy, control group were treated with thrombolytic therapy alone, recorded and analysed of therapeutic effect of two groups of patients.Results NOS (nitric oxide synthase), ET (endothelin) values of observation group post-treatment were (45.9 ±5.1)U/mL,(87.3 ±7.1) pg/mL,which were better than control group (39.1 ±3.6)U/mL,(96.5 ±9.3)pg/mL (P<0.05); observation group total effective rate was 75.0% better than control group ( 70.0%) , but the two groups had no significant difference in efficiency.Conclusion Thrombolytic therapy combined with psychological intervention in the treatment of acute myocardial infarction is effective , precise, and can be effectively improve the symptoms of acute myocardial infarction, it is worthy of further research and application.

Objective To determine the nature of enlarged mediastinal lymph nodes,the study of CT-guided transbronchial needle aspiration biopsy(CT-TBNA) was conducted. Methods According to classification and locating standard of mediastinal lymph nodes,CT-TBNA was performed in 158 mediastinal lymph nodes of 73 patients,including 54 primary lung cancer with enlarged mediastinal lymph nodes,5 malignant lymphoma, 3 mediastinal abscess and 11 mediastinal inflammatory adenopathy. Results CT confirmed that the needles were inside all 158 lymph nodes. Adequate aspirates for histologic and/or cytologic diagnosis were obtained in 136 of 158 (86%) lymph node biopsies. No serious complications occurred. Conclusion Locating standard of CT-guided TBNA was reliable. Puncture was safe and cost-effective.CT-TBNA was important for ascertaining the nature of mediastinal adenopathy and staging of neoplasms.

The diarrheal rat model was established by orally administering senna. The preventive experiment was concurrent for 6 days. The treatment experiment modeling had lasted for 12 days. The administration started at the 7th day, and lasted for 6 days. During the experiment, efforts were made in symptom score and weighing. After the experiment, hearts, livers, spleens, kidneys, brains, adrenals and thymuses were collected and weighed, lactic dehydrogenase (LDH) and cardiac troponin-I (cTn-I) in serum were detected. The efficacy of aqueousextracts from Aconiti Lateralis Radix Praeparata in preventing and treating rats with diarrheal and its accompanying toxicity were respectively studied. The result shows that aqueous extracts from Aconiti Lateralis Radix Praeparata could improve syndromes of rats with diarrheal. The 50% effective doses (ED50) of preventive and treatment administrations were 1.420 4 g · kg(-1) and 1.048 9 g · kg(-1), respectively. Aqueous extracts from Aconiti Lateralis Radix Praeparata could decrease the ratio of heart to body weight, and increase serum LDH and cTn-I. It was concluded that aqueous extracts from Aconiti Lateralis Radix Praeparata had a specific preventive and treatment effect on rats with diarrheal caused by senna, but with specific toxicity on heart.

Methods: A total of 40 MHD patients [mean age (53.5 ± 7.1) years] who underwent normal hemodialysis more than 6 months were randomly classified into two groups, Levocarnitine supplemented group (LS-G) (n = 20; Levocarnitine supplementation after each normal hemodialysis session, at a dose of 1.0 g/day by intravenous administration) and control group (C-G) (n = 20; normal hemodialysis). Before treatment, one month and three months after treatment we respectively measured or observed the following items, the tolerance to hemodialysis, carnitine level in plasma, C-reactive protein, IL-6, TNF-α, percentage of neutrophil, and some relevant nutritional parameters, such as lipid profile, transferrin, total protein, albumin and prealbumin levels. Comparative analysis was conducted between the two groups. Results: In LS-G three months after treatment, the levels of carnitine, hemoglobin, and prealbumin in plasma were significantly increased (P<0.05), but C-reactive protein, neutrophil percentage, low-density lipoprotein and triglyceride were significantly decreased (P < 0.05) in contrast to those in C-G and before treatment. Transferrin, total protein, and albumin were elevated in LS-G, with no statistical significance. Conclusion: There was a significant improvement of lipid metabolism and nutritional status for the long-term maintenance hemodialysis patients with Levocarnitine supplementation. And this improvement is related to the decrease of inflammatory factors.
