1.Knowledge, Attitude, and Practice on the Basic Life Support among Medical Interns of the Philippine General Hospital

Objective. This study aimed to assess the knowledge, attitude, and practice on the Basic Life Support (BLS) among the Philippine General Hospital medical interns, compare the scores between trained and untrained interns, and determine the proportion of respondents with updated BLS training.
Methods. This was a cross-sectional study using a thirty-five item questionnaire on the knowledge, attitude, and practice of medical interns at the Philippine General Hospital on the BLS.
Results. A total of 262 interns were included in the study. Although the mean knowledge scores of untrained and trained medical interns did not differ significantly (7.00 vs. 7.30, p=0.1637), more than 20% committed mistakes on questions on the following: jaw-thrust, rescue breaths, defibrillation, and CPR check. Generally, interns followed the BLS guidelines in their practice and had a positive attitude towards BLS. However, only 37% of trained participants had training within the past two years.
Conclusion. Most medical interns have a fair knowledge on BLS regardless of training status and adhere to them in practice. Since the training duration did not affect their knowledge scores, initial training in medical school and continuous exposure in the wards may have provided adequate BLS reinforcement. Recertification may not be needed during their formative years in medical school.

2.The magnitude of delay in non-metastatic breast cancer treatment in a tertiary hospital: An analysis from 2012 to 2018

Background and Objective:
The burden of treatment delay in breast cancer is high, especially among developing countries. Despite adversely affecting morbidity and mortality, treatment delay remains unexplored in the Philippines. This study aimed to determine treatment delays among breast cancer patients in a tertiary hospital during surgery, neoadjuvant chemotherapy, and adjuvant chemotherapy, and to identify predictors of delay.
Methods:
A cross-sectional study was conducted among breast cancer patients seen between January 1, 2012 to December 31, 2018. The following outcomes were investigated: ≥90 days from initial diagnosis to surgery, ≥8 weeks from diagnosis to initiation of neoadjuvant chemotherapy, and >120 days from diagnosis to initiation of adjuvant chemotherapy. Summary statistics were reported as percent for categorical data and as mean for continuous data. The individual correlations were performed using Chi-square for qualitative data and t-test for quantitative data while predictors were determined through logistic regression.
Results:
A total of 324 patients were included in this study. The majority of the patients were less than 65 years old living in urban areas. More than half of the patients were overweight or obese, hypertensive, and diabetic. The following delays were observed: 61.1% (n = 198) with any type of delay, 23.8% (n = 53) with delay in surgery, 53.8% (n = 120) with delay in adjuvant chemotherapy, and 74.3% (n = 75) with delay in neoadjuvant chemotherapy. The patients noted to have any type of delay were more likely to be hypertensive (p = 0.046) and residing in urban areas (p = 0.041). There were no differences in the distribution of age, body mass index, and presence of co-morbid conditions such as hypertension, diabetes mellitus, coronary artery disease, and heart failure among those with any form of delay compared with no delay.
Conclusions
The present study shows the presence of treatment delay among breast cancer patients and may be used to enact policy changes to optimize breast cancer care delivery. Further studies may be done to identify other factors affecting these delays and policy changes are recommended to address these gaps in surgery and chemotherapy administration among breast cancer patients.

Objectives:
Adherence to clinical practice guidelines (CPG) has been shown to reduce inter-physician practice
variation and improve quality of care. This study evaluated guideline adherence of physicians in two tertiary public hospitals to local CPG on COVID-19.
Methods:
This was a multicenter, retrospective chart review, rapid assessment method study. Guideline adherence and non-adherence (overuse and underuse) to 15 strong recommendations in the prevailing Philippine COVID-19 Living Recommendations were assessed among a sample of patients admitted in two centers from July to October 2021. Differences in adherence across COVID-19 disease severities and managing hospital units were analyzed.
Results:
A total of 723 patient charts from two centers were reviewed. Guideline adherence to dexamethasone use among patients with hypoxemia is 91.4% (95% CI 88.6 to 93.6) with 9.2% overuse. Tocilizumab was underused in 52.2% of patients with indications to receive the drug. There was overuse of empiric antibiotics in 43.6% of patients without suspicion of bacterial coinfection. Lowest adherence to antibiotic use was seen among patients with critical disease severity and those managed in the intensive care unit. None of the other non-recommended treatment modalities were given.
Conclusion
Management of COVID-19 in both centers was generally adherent to guideline recommendations. We detected high underuse of tocilizumab probably related to the global supply shortage during the study period and high overuse of antibiotics in patients without suspicion of bacterial coinfection. While the results of this study cannot be generalized in other healthcare settings, we recommend the application of similar rapid assessment studies in guideline adherence evaluation as a quality improvement tool and to identify issues with resource utilization especially during public health emergencies
