1.Comparative research of moxibustion and infrared method in testing heat-sensitive state at Xiaguan (ST 7) in primary trigeminal neuralgia.

OBJECTIVETo explore the possibility of the infrared objective displaying of heat-sensitive state of acupoint.
METHODSFifty-four qualified subjects were enrolled. In the natural state, thermal tomography (TTM) was adopted to shoot the thermograms on the face. After the thermograms collection, the thermo-sensitive detection with moxibustion was applied. The suspending moxibustion with moxa stick was used at Xiaguan (ST 7, affected side) for 10 mm. The cases with heat extension and transmission at the acupoint were recorded at the end of moxibustion. In following, the second infrared radiation detection was done so as to record the changes in the infrared images on the face before and after moxibustion and compare the difference in the heat-sensitive state at Xiaguan (ST 7, affected side) in the subjects between two detection methods.
RESULTSBefore moxibustion, the infrared radiation intensity at Xiaguan (ST 7, affected side) in the patients displayed mostly low temperature feature and its sensibility (true positive rate) was 65.6%, the specificity (true negative rate) was 68.2% and the accuracy was 66.7%. After moxibustion at Xiaguan (ST 7, affected side), there was an obvious block-diffused increment area of infrared radation along the cheek and its sensitivity (true positive rate) was 81.8%, the specificity (true negative rate) was 81.0% and the accuracy was 81.5% as compared with the moxibustion detection.
CONCLUSION(1) The heat-sensitive state at Xiaguan (ST 7, affected side) in the patients of primary trigeminal neuralgia can be objectively displayed by infrared imaging to a certain extent. (2) The acupoint heat-sensitization produced by moxibustion is not only the subjective sensation of the subjects, but also can be objectively displayed by infrared imaging to a certain extent.

2.Observation on the distribution of heat-sensitized acupoints in patients with primary trigeminal neuralgia.

OBJECTIVETo observe the distribution characteristics of heat-sensitized acupoints in the high incidence region of heat sensitization on the body surface of the patients with trigeminal neuralgia.
METHODSSixty cases of trigeminal neuralgia were collected and 60 healthy subjects were in the control. With moxibustion sensitization method to detect the heat-sensitized acupoints, the probability of the heat-sensitized acupoints in the high incidence region of heat sensitization on the body surface was analyzed and the correlation between the heat-sensitized acupoints and trigeminal neuralgia was discussed.
RESULTS(1) The occurrence rate of the heat-sensitized acupoints in the high incidence region of heat sensitization in patients with trigeminal neuralgia was 83.3% (50/60). (2) The occurrence rate of the heat-sensitized acupoints was high in the site of Xiaguan (ST 7) and Sibai (ST 2), and was secondarily high in the site of Jiachengjiang (Extra), Fengchi (GB 20) and Yuyao (EX-HN 4) in the high incidence region of heat sensitization in patients with trigeminal neuralgia.
CONCLUSIONThe heat-sensitized acupoints are highly correlative with trigeminal neuralgia, which provides the evidence for the further study on prevention and treatment of trigeminal neuralgia with heat-sensitive moxibustion.

3.Efficacy comparison of lumber disc herniation treated with mild moxibustion at Yaoyangguan (GV 3) under different conditions.

OBJECTIVETo compare the difference in clinical efficacy on lumber disc herniation (LDH) treated with Yaoyangguan (GV 3) between mild moxibustion under thermosensitive condition and that under non-thermo-sensitive condition.
METHODSFifty-seven LDH patients were selected as the study objects. Mild moxibustion at Yaoyangguan (GV 3) was applied for 45 min each time. Additionally, the conventional acupuncture was given, once a day, for 20 days. At the end of treatment, two groups were classified. A thermosensitive acupoint group (thermosensitive group) was composed of the cases with thermo-sensitization at Yaoyangguan (GV 3) and presenting for > or =4 times in the entire treatment. A non-thermosensitive acupoint group (tranquilization group) was composed of the cases without thermo-sensitization or the frequency of thermo-sensitization <4 times in the entire treatment. The modified Japanese orthopedics association scoring system (M-JOA) was adopted to observe the cases before and after treatment and 6 months after treatment in the two groups. The efficacy was compared between the two groups.
RESULTSAfter treatment and in 6 months after treatment, the score of M-JOA was apparently reduced as compared with that before treatment in the two groups (all P<0.05). The score reducing in the thermosensitization group was more obvious than that in the tranquilization group (both P<0.01). After treatment and in 6 months after treatment, the curative rate and remarkably effective rate were 89.7% (26/29) and 79.3% (23/29), which was better than 71.4% (20/28) and 60.7% (17/28, both P<0.05) in the tranquilization group separately.
CONCLUSIONMild moxibustion at acupoint under thermosensitive condition achieves the better short-term and long-term effects as compared with that under non-thermosensitive condition.

4.Collagen spatial distribution in rapid atrial pacing dogs with or without superior vena cava and aortic root fat pad

Objective To observe the collagen spatial distribution,collagen volume fraction (CVF)and Cx40,Cx43mRNA expressions in rapid atrial pacing dogs post vagal denervation by removing fat pad located between the medial superior vena cava and aortic root(SVC-Ao fat pad).Methods Twentyfour dogs were randomly divided into unpaced sham operation group(S group,n=8),Keeping SVC-Ao fat.Pad group(K group,n=8)and Removing SVC-Ao fat pad group(R group,SVC-Ao fat pad was removed by surgical excision before pacing,n=8).K and R groups were paced for six weeks.Six weeks later,all dogs were sacrificed,left atrium(LA),right atrium(RA),left atrial appendage(LAA),right atrial appendage(RAA)and atrial septum(AS)were collected and stained with HE or Masson Trichrome or frozen in liquid nitrogen for quantifying the expression of Cx40,Cx43 mRNA by Real-time quantitative Reverse Transcription Polymerase Chain Reaction(RT-PCR).Results Spatial distribution of collagen fibers as well as CVF between S and R group were similar(all P>0.05).CVF was significantly higher in K group colnpared to R group,especially at LAA and AS locations(all P<0.05),Cx40mRNA expression in K group was significantly decreased in LA,RA,and significantly increased in LAA,RAA and AS compared those in S group(all P<0.05),significantly lower in LA and RA while significantly higher in LAA and RAA compared to R group(all P<0.05).Cx43mRNA expression in K group was significantly reduced in LA,RA,LAA and RAA while significantly increased in AS compared to S group(all P<0.05),significantly higher in LA,RA,RAA and AS while significantly lower in LAA compared to R group.Conclusion Pacing induced collagen remodeling and modulation on Cx40mRNA and Cx43 mRNA expressions could be partially attenuated by removing SVC-Ao fat pad suggesting vagal nervation plays a key role in the initiation and preservation of atrial fibrillation.

The academic thought of professor - on "no allergy without any deficiency" was explored. Theory and the clinical application were elaborated on allergic diseases treated with heat-sensitive moxibustion. It is believed that the critical pathogenesis of allergic diseases is deficiency, particularly the deficiency of the lung, the spleen and the kidney. The invasion of exogenous factors or the disturbance of the retained pathogens in the body may induce allergy. Regarding the therapeutic method, the warming method is applicable for the deficiency and the heat-sensitization counteracts allergy. The sensitized points are detected in accordance with the two-step location method and they are stimulated with the suspended moxibustion. The feeling of heat-sensitive moxibustion is a sign of activation of endogenous regulatory function in the human body. It is a kind of external therapy for the internal disorders, directly acting on the pathogenesis, strengthening the antipathogenic and removing the allergic factors. This therapy is a new endogenous anti-allergic approach.
6.Clinical characteristics of patients with MitraClip operation and predictors for the occurrence of afterload mismatch

Objective To explore the risk factors related to afterload mismatch(AM)after transcatheter mitral valve repair(MitraClip).Methods This was a retrospective cohort study.48 patients hospitalized in the Department of Cardiovascular Medicine,the First Affiliated Hospital of Sun Yat-sen University from December 2021 to December 2023,who underwent MitraClip due to severe mitral regurgitation(MR)were included.Preoperative clinical data,laboratory tests,and preoperative and postoperative color Doppler echocardiographic examination results of surgical patients were collected.AM was defined as the left ventricular ejection fraction(LVEF)decreased by 15%or more after surgery compared with the one before(dLVEF≤-15%).Patients were divided into AM group and non-AM group according to whether afterload mismatch occurred.Univariate and multivariate logistic regression were used to analyze the risk factors of postoperative AM.Results Among 48 patients who underwent MitraClip,14 of them(29.2%)developed afterload-mismatched.For those without AM,their overall LVEF was improved after the operation;for patients in both AM group and non-AM group,their overall left ventricular end-diastolic diameter(LVEDd),left ventricular end-diastolic diameter volume index(LVEDVi)was reduced compared with the preoperative ones.Univariate regression analysis showed that C-reactive protein levels(OR 1.98,95%CI 1.02-3.83),platelets(OR 2.22,95%CI 1.08-4.53),systemic immune inflammation index(OR 1.96,95%CI 1.03-3.71)were associated with an increased risk of AM in patients undergoing MitraClip(all P<0.05),while those with larger right atrial diameter(OR 0.35,95%CI 0.13-0.93)or moderate to severe tricuspid regurgitation(OR 0.19,95%CI 0.05-0.81)were less likely to develop into AM(both P<0.05),which is still satisfied after adjustment.Conclusions For patients who underwent MitraClip,C-reactive protein levels,platelets and systemic immune inflammation index(SII)are associated with an increased risk of afterload mismatched,while those with larger right atrial diameter or moderate to severe tricuspid regurgitation were less likely to develop into AM.

This study aims to compare the internal chemical composition and appearance indifferent growth patterns and years of Saposhnikovia divaricata decoction pieces,which was applied to explore the effect of growth patterns and years on its quality. The appearance characteristic data of 55 batches of different growth patterns and years of S. divaricata were collected using PANTONE color card.High performance liquid chromatography( HPLC) was used to determine the contents of prim-O-glucosyl-cinmifugin,cimifugin,4-O-β-D-glucosyl-5-O-methylvisamminol and sec-O-glucosylhamaudol. The content of alcohol soluble extract and water-soluble extract were determined by hot-dip method. The content of volatile oil was determined by steam distillation. The correlation between growth patterns and years and the contents of 4 chromones,extracts and volatile oil were analyzed by modern statistical methods. Also,the method of comprehensively evaluating the quality of Chinese herbal pieces was developed by combining the growth patterns and years,appearance and chemical indexes. MTT assay was used to evaluate the effects on the survival rate of RAW264. 7 cells at four different concentrations of chromones and LPS was used to stimulate well-growing RAW264. 7 cells to establish an inflammatory model. The contents of NO and TNF-α in cell supernatant were detected by NO test kit and ELISA method.
