1.Clinical evaluation of selective laser trabeculoplasty for residual primary angle-closure glaucoma

Background Selective laser trabeculoplasty(SLT)has been widely accepted for the treatment of open-angle glaucoma.However,there are few reports about its application in treating angle-closure glaucoma so far.Objective This study was to clinically evaluate the safety and efficacy of selective laser trabeculoplasty for primary angle-closure glaucoma with opened anterior angle follow-up surgery.Methods A case-observational design was adopted in this study.A total of 33 eyes from 23 angle-closure glaucomatous patients with opened 180° angle were enrolled.All the patients understood this trial and written informed consent was obtained prior to the medical treatment.The average preoperative intraocular pressure(IOP)and number of medication were(23.07±4.01)mmHg and 1.52±0.66,respectively.A frequency doubled Q-switched Nd:YAG laser was used to irradiate the trabecular meshwork with 70-80 laser spots over 180°.The same preoperative anti-glaucoma drugs were topically administered during the follow-up period.Visual acuity,IOP,slit lamp examination,gonioscopy and the coefficient of outflow facility were regularly assessed and performed after laser surgery.Results The IOP values of the 33 eyes were(18.78±4.87)mmHg,(18.83±3.95)mmHg,(19.78±4.48)mmHg,(19.63±3.68)mmHg,(19.11±3.36)mmHg,and(21.17±3.14)mmHg 1 day,l week,and 1,3,6,9 months after operation,showing significant decline in comparison with preoperation(P<0.01).The mean drug uses were 1.63±0.85,1.23±1.01,1.25±0.98,1.21±1.02,1.22±0.86,and 1.69±0.53 at the same time point after operation,respectively,and significant differences were found 1 week,1,3,6 months in postoperation in comparison with preoperation(P =0.020,0.026,0.020,0.031).Ciliary congestion and aqueous flare naturally disappeared 1 week after surgery.The coefficients of outflow facility were 0.11±0.09 and 0.23±0.15,respectively at pretreatment and 3 months after SLT with a statistically significant difference (P=0.002).Conclusions Selective laser trabeculoplasty is effective and safe for angle-closure glaucoma.However,it is essential to have 1/2 circle of anterior chamber open for SLT in angle-closure glaucoma.
2.In vitro study on bacterial growth enhancement in the presence of TNF-?,IL-1? and IL-6

Objective To conduct an in vitro study of the growth of Pseudomonas aeruginosa,Staphylococcus aureus and Acin- etobacter spp.,and evaluate their response to various concentrations of tumor necrosis factor(TNF)-?,interleukin (IL)-1?, and IL-6.Methods To monitor the growth of bacteria incubated with the cytokines TNF?,IL-1?and IL-6 that were added to RPMI 1640 medium in various concentrations (10,50.100,500 pg,1 and 10 ng) at 2,4 to 6,8 and 16-18 h.The bacterial concentration was estimated when the mixtures of cytokines and specific neutralizing monoclonal antibodies (MoAbs) were in- cubated.Results We found that all three bacterial species showed concentration-dependent growth enhancement when incubated with one or more tested cytokines.Blockade by specific neutralizing cytokine significantly inhibited cytokine-induced growth. When compared with control,the 6 h growth response was maximal with IL-1?for Staphylococcus aureus and Acinetobacter spp.,and with IL-6 for Pseudomonas aeruginosa.Conclusions In this study we provide additional evidence for a newly de- scribed mechanism for bacterial proliferation in the presence of exaggerated and protracted inflammation.The effect that cyto kine-induced growth enhancement inhibited by specific neutralizing cytokine MoAbs may be useful for antimicrobial therapy.
3.Influences of Deduhonghua -7 powder on matrix metalloproteinases and transforming growth factor beta expression in liver fibrosis

Objective To observe the therapeutic effect of Deduhonghua-7 powder on liver fibrosis and its correlation with matrix metalloproteinases ( MMPs) and transforming growth factor β1 ( TGF-β1 ) .Methods The rats were divided into 4 groups, blank group, model group, control group, experiment group, 8 rats each group.The CCl4 was used to induce rat liver fibrosis.The control group was given colchicine pill, CMC sodium cellulosate was used to prepare 2% of colchicine suspen-sion, once time per day, 0.2 mL · kg-1 each time.The experiment group was given Deduhonghua-7 powder, CMC sodium cellulosate was used to prepare 3% of Deduhonghua-7 powder suspension, once time per day, 0.2 mL· kg -1 each time; blank group and model group were given same volume of pure water.All rats were sacrificed after 40 days of medication.Western blot was acted to detect the matrix metalloproteinase-1 (MMP-1), matrix metalloproteinase-13 (MMP-13),tissue inhibitor of metalloproteinases-1 (TIMP-1) protein expression.The PCR was used to detect the changesoftype Ⅰ, Ⅲ collagen, TGF -β1 mRNA gene expression.Results The MMP-1, MMP -13 protein expression in rats of model group was down -regulated compared with blank group ( P<0.05 ).The TIMP-1 protein expression was up-regulated compared with blank group ( P<0.05 ) . The mRNA expressions of typeⅠprocollagen, typeⅢprocollagen and TGF-β1 were increased compared with blank group(P<0.05).The MMP-1, MMP-13 protein expression of control group and experiment group were significant-ly up-regulated compared with model group.The TIMP-1 protein expression significantly down-regulated compared with model group.The mRNA gene expression of type I procollagen, typeⅢprocollagen and TGF-β1 were down-regulated compared with model group ( P<0.05 ).Conclusion Deduhonghua-7 powder regulate and maintain the stabilization of MMP/TIMP to block the expression of TGF-βand inhibit the transcription of type I,Ⅲ collagen and TGF-β1 mRNA to reach the purpose of detention of occurrence and development of liver fibrosis.
4.Correlation between polymorphism of tumor necrosis factor-related apoptosis-inducing ligand gene 1525 locus polymorphism and nodular thyroid disease

ObjectiveTo study tumor necrosis factor-related apoptosis-inducing ligand(TRAIL) 1525 locus polymorphism in patients with nodular thyroid disease and investigate the relation between individual gene polymorphism and thyroid diseases.Methods A total of 125 patients were diagnosed with nodular thyroid disease at the Department of Endocrinology,the First Affiliated Hospital of Inner Mongolia Medical College.Among these patients,67 cases were nodular thyroid goiter and 58 cases were nodular thyroid adenoma,54 males,71 females,and average age was 41.05 ± 14.42. Patients with nodular thyroid goiter were grouped into toxic and non-toxic and thyroid adenoma were grouped into high-functioning or non-high-functioning.A total of 100 healthy subjects.47 males,53 females,average age 42.35 ± 16.52 were as control group.According to the principle of informed consent,venous blood was collected,polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method for TRAIL gene 1525 locus polymorphism determination was performed to calculate genotype of the TRAIL gene (homozygous GG,heterozygous GA,mutationhomozygous AA) and the gene frequency (G,A),and relative degree of risk(odd ratio,OR) was compared.ResultsNodular goiter group TRAIL gene 1525 locus genotype frequencies(GG:40.3%,AG:44.8%,AA:14.9%),allele frequencies(G:62.7%,A:37.3%) were compared with that of the control group(GG:17.0%,AG:65.0%,AA:18.0%; G:49.5%,A:50.5%),the differences were statistically significant(x2 =11.376,5.633,P < 0.01 or < 0.05 ).Adenomas group 1525 locus genotype frequencies of the TRAIL gene(GG:44.8%,AG:38.0%,AA:17.2%),allele frequencies(G:63.8%,A:36.2%) were compared with that of the control group,the differences were statistically significant(x2 =15.342,6.054,P < 0.01 or < 0.05).Allele frequencies of thyroid goiter group and denoma group were compared with that of the control group,OR values were 1.714 and 1.797(all P < 0.05) and 95% confidence intervals were 1.097 - 2.679 and 1.124 - 2.874.The difference of 1525 locus genotype or allele frequency distribution in toxic and non-toxic nodular group,high functioning and non-high-functioning adenomas group were not statistically significant (x2 =3.714,2.792; 1.103,2.020; all P > 0.05).ConclusionTRAIL gene 1525 locus polymorphism is significantly associated with nodular thyroid disease.

BACKGROUND: Chinese traditional osteopathy is long in history, unique in manipulation and miraculous in therapeutic effect. But people understand it more m perception rather than in theory, more in application rather than in development. There is little research truly on the bioseience.OBJECTIVE: To probe into the macro-idea of Chinese traditional osteopathy, micro-mechanisms on characters and mathematics-physics models, aiming to provide new principles and approaches of treatment for the daily increased bone trauma, fracture and sport injury.SETTING: Physics and machine-electron college of a university, and its affiliated hospital.METHODS: Based on the natural concept of "integration between heaven and human being" and new concept of holistic medicine in Chinese traditional osteopathy, the macro-idea and characters of reduction and union of fracture are generalized from the characters of natural therapy and the biomechanical mechanisms and characters of reduction and union of fracture are summarized from the micro-reaction of bone repair and union so as to discover biomechanical mechanisms and characters of reduction and union of fracture and further to set up biomechanical models and mathematics-physics expressions during the treatment.RESULTS: Chinese traditional osteopathy envelopes macro-idea of "initiative reduction-functional union" in fracture and micro-mechanism on "stress adaptability-functional adaptability" of bone repair and union.CONCLUSION: Chinese traditional osteopathy compiles with the natural,green and non-traumatic therapy in bio-natural law of bone repair and union and supports the theme of "high thought and high skill".
6.Sleep Disorders Documented by Night Polysomnography in Patients with Chronic Insomnia Disorder

BACKGROUND: To investigate the sleep disorders related to chronic insomnia and compare the characteristics of each group. METHODS: We registered 191 patients (female 56.0%, age 61.7 years) who have complained about symptoms of insomnia for more than three months and assessed sleep disorders related chronic insomnia by analyzing their polysomnography (PSG) parameters, demographics, and questionnaires (Insomnia Severity Index, Beck Depression Inventory, Pittsburg Sleep Quality Index, and Epworth Sleepiness Scale, and World Health Organization Quality of Life Assessment Instrument abbreviated version). RESULTS: Patients were categorized into groups of primary insomnia (PI, 51.8%, n=99), comorbid insomnia with obstructive sleep apnea (CIO, 38.7%, n=74), and comorbid insomnia with periodic limb movement disorder (CIP, 9.4%, n=18). CIO was older and more obese than PI and CIP. The proportion of males was the highest in CIO. Arousal index of PSG was higher in CIO and CIP than in PI. Other parameters and the results of questionnaires were not different among groups. CONCLUSIONS: About a half of patients with chronic insomnia symptoms (48.2%) had considerable sleep disorders associated with insomnia. Our study suggests that the PSG as well as history taking and demographics are necessary to clarify the relevant conditions of chronic insomnia disorder for appropriate treatment.

7.Brucella meningoencephalitis in children with status epilepticus A case report and literature review

Abstract: Objective To investigate the clinical characteristics and incidence of Brucella encephalitis and meningitis in children. Methods We report the clinical data of a child with Brucella melitensis meningitis in children, and summarize the incidence, diagnosis methods and treatment of Brucella encephalitis or meningitis in children, taking into account the relevant domestic and foreign literature from January 2014 to December 2020. Results A 4-year-old girl was admitted to the hospital with status epilepticus on March 15, 2021 because of interrupted right limb numbness for 16 hours and convulsions for 2 hours. She had 2 non-febrile convulsions three months before admission and was diagnosed with epilepsy. This incident was acute, accompanied by low fever, with epilepsy as the main manifestation. Cerebrospinal fluid test suggested central nervous system infection, but the nature of infection could not be determined by routine and biochemistry of cerebrospinal fluid.The cerebrospinal fluid next generation sequencing confirmed that the pathogen of the infection was B. melitensis, which was further verified by the peripheral blood antibody test. After effective antibiotics combined with a full course of treatment, the patient recovered after six months of treatment. A total of 60 articles were retrieved in the database, including 29 in Chinese. During this period, a total of 7 cases of brucellosis in children with nervous system involvement were reported, one of which was a case report, and the other 6 cases were mentioned in the comprehensive analysis of children with brucellosis. Conclusions Brucella encephalitis or meningitis in children has a low incidence and various clinical features, which are easy to be misdiagnosed or missed.
8.Association between Maternal Adverse Childhood Experiences and Attention-Deficit/Hyperactivity Disorder in the Offspring: The Mediating Role of Antepartum Health Risks

Objectives:
This study aimed to examine the effect of maternal adverse childhood experiences (ACEs) on the attention-deficit/hyperactivity disorder (ADHD) symptoms in the offspring and to examine the mediating role of antepartum health risk on the intergenerational transmission of maternal ACEs.
Methods:
The participants consisted of 461 mother-child dyads. Mothers completed the ACEs questionnaire and Diagnostic Predictive Scales. Multivariate logistic regression analysis was used to estimate the risk of ADHD symptoms in the offspring of mothers with ACEs and the mediating effect of antepartum health risks by path analysis.
Results:
In all, 35.4% (n=163) had at least one maternal ACE, and 11.1% (n=51) had three or more. Compared to the non-ADHD symptom group, the group of offspring with ADHD symptoms showed a significant association with maternal ACE score (p<0.001) and antepartum health risks (p<0.001). Multivariate analysis further showed a significant association between the sum of maternal ACEs [odds ratio (OR)=1.264, 95% confidence interval (CI)= 1.060–1.516, p=0.009], antepartum health risks (OR=1.236, 95% CI=1.036–1.475, p= 0.019), and ADHD symptoms in the offspring. In the mediation model in which the mother’s ACE score affected the offspring’s ADHD symptoms, partial mediation through antepartum health risks was found to be significant (B=0.041, 95% CI=0.011–0.124).
Conclusion
Maternal ACEs are significantly related to the incidence of ADHD symptoms in the offspring and antepartum health risks exert an indirect effect. These findings suggest that maternal ACEs have a negative impact on the offspring’s brain development through intergenerational transmission, resulting in neurodevelopmental disorders such as ADHD.
9.Alitretinoin-induced Pseudotumor Cerebri: A Case Report

Purpose:
We present a case of alitretinoin-induced pseudotumor cerebri.Case summary: A 28-year-old woman presented with a 1-month history of bilateral papilledema and intermittent blurred vision in the left eye. She had been taking alitretinoin (9-cis-retinoic acid, AlitocⓇ; GlaxoSmithKline) for atopic dermatitis and hand eczema. Although visual acuity remained intact in both eyes, mild central scotoma was detected bilaterally. Optical coherence tomography revealed increased retinal nerve fiber layer thickness in both eyes. Enhanced magnetic resonance imaging revealed no abnormalities except for bilateral flattening of the posterior sclera. Cerebrospinal fluid exhibited an elevated opening pressure (220 mmH2O) and a normal composition. Following the discontinuation of alitretinoin, the intermittent blurred vision, papilledema, and central scotoma improved significantly within 2 months.
Conclusions
