1.Application of intrapericardial pneumonectomy in locally advanced lung cancer

Objective To probe into the indication, operative skill of intrapericardial pneumonectomy, the prevention and cure in postoperative complication of intrapericardial pneumonectomy for locally advanced lung canc-er. Methods The clinical data of 43 cases of locally advanced lung cancer undergoing intrapericardial pneumonec-tomy from February 1998 to November 2007 were retrospectively analyzed. Results No surgical death occurred. The main postoperative complication was arrhythmia, acute pulmonary edema and pneumonia, with incidence as 41. 86%. The 1-,3- and 5-year postoperative survival rate was 79.1% ,30.2% and 11.6%. Conclusions The intrap-ericardial pneumonectomy skill not only can improve resection rate, reduce exploration rate, but also offer more oppor-tunity of further therapy in a part of locally advanced lung cancer.
2.Early postoperative enteral nutrition with esophageal cancer in patients aged 70 years and older

Objective To investigate the significance of early postoperative enteral nutrition with esophageal cancer in the elderly. Methods A total of 62 cases of esophageal cancer patients aged 70 years and older were randomly divided into the enteral nutrition(EN)group and the parenteral nutrition(PN)group. In preoperative and the first day and eighth day of postoperative serum albumin,prealbumin,peripheral blood lymphocytes,serum immunoglobulin(IgG,IgM,IgA),complement C3 and C4 were measured,and the postoperative infectious complications and cardiovascular complications were observed. Results During the eighth day after operation,the statistic analysis of the data between EN group and PN group showed: tALB = 2. 535,tPA = 2. 880,tLymphocyte =2. 720,tIgA =2. 166,tIgM =2. 065 ,tC3 =4. 103 ,tC4 =2. 478,P <0. 05. The result indicated that the EN group was better than the PN group in improving the nutritional status of patients,strengthened the immune system. The complication comparison between the two groups after operation: x2 Infection = 4. 24,x2 Cardiovascular =15.55,P < 0. 05. The result indicated that the EN group had lower incidence of infectious complications and cardiovascular complications than the PN group. Conclusions Early enteral nutrition can significantly improve the nutritional status of elderly patients with esophageal cancer after postoperative,enhance immunity,and reduce the incidence of complications.
3.The effect of different nutrition support route on the cellular immune function of patients after esophagectomy

Objective To explore the nutrition support route of enteral nutrition (EN) and parenteral nutrition(PN) on the cellular immune function of patients after esophagectomy.Methods Sixty-two patients with esophageal surgery were randomly divided into EN group and the PN group,and each was 31 cases.Cellular immune indexes(including T lymphocyte subsets,natural killer cells,B lymphocytes) were measured at the preoperation,the postoperative 1st day and 8th day.The t test was preformed to compare the measurement data of two groups,and repeated measures analysis of variance was used to compare multiple time points of measurement data of two groups.Results The levels of CD3,CD4,CD8,CD4/CDS,NK cells and B cells in EN group at before operation were (60.46 ± 6.41) %,(39.03 ± 5.64) %,(25.06 ± 5.39) %,(1.60 ± 0.24),(13.86 ±4.91)%,(13.14 ± 3.81)% respectively,and those indices at 8th day after operation were (65.18 ±7.81)%,(40.45 ±6.82)%,(22.81 ±4.47)%,(1.78 ±0.16),(16.97 ±4.85)%,(15.69 ± 3.88)% respectively,and the differences were statistically significant(P < 0.05).The levels of CD4,CD4/CD8,NK cells and B cells in PN group at before operation were (59.78 ± 5.17) %,(39.10 ± 5.49) %,(24.94 ± 3.97) %,1.57 ±0.20,(14.02 ± 4.47) %,(12.96 ± 3.39) % respectively,significantly different from that of 8th days ((59.78 ±5.17)%,(39.71 ±5.63)%,(24.74 ±3.80),(1.62 ±0.15),(15.35 ±4.31)%,(14.33 ±3.43) % respectively),and there were significant difference compared to before operation in terms of CD4,CD4/CD8,NK cells and B cells.At the 8th day after operation,there was significant difference between in EN group and PN group in terms of CD3,CD8,CD4/CD8,NK cells and B cells(P <0.05).Conclusion Early EN for patients after esophageal carcinoma operation can significantly improve the cellular immune function of patients.

Objective To investigate the effects of ?-2-macroglobulin on the aging process of human diploid fibroblasts. Methods pIRES-A2M sense and antisense vectors were constructed and transferred into 2BS cells mediated by lipofectamine.2BS/A2Ms and 2BS/A2Ma cell lines were verified by Southern and Northern blot analysis respectively.Cell growth curve,the population doublings,cell cycle analysis,staining of senescent-associated-?-galactosidase and expression of p16 and p21 in transfected cells were measured. Results Southern and Northern blot analysis verified that the exogenous cDNAs were integrated into genomic DNA in the transfected cells.The ultmost population doublings of 2BS/A2Ms cells were slightly higher than normal 2BS cells.Cell growth curve,cell cycle analysis,staining of senescent-associated-?-galactosidase and the population doublings all revealed that 2BS/A2Ms cells demonstrated obvious difference compared with 2BS/A2Ma cells((P0.05). Conclusions The aging process of 2BS cells is influenced slightly by expression of A2M.
6.Expression of let-7a2 in lung adenocarcinoma

Objective To explore the expression and significance of let-7a2 in lung adenocarcinoma.Methods To adapt technology of realtime PCR with reference of U6, the expression of let-7a2 in tumor tissues and normal tissues was detected in 15 cases of resected lung adenocarcinoma. The laboratorial data were analysed by measure of 2~(-△△CT) method. The analysed data were managed by statistics. Results The expression of let-7a2 in lung adenocarcinoma tissue was significantly lower than that in lung normal tissue (0.8733±0.4821 vs 2.4527±1.0111). This difference possess markedness significance(t=6.816, P<0.001).Conclusion The expression of let-7a2 in lung adenocarcinoma is obviously lower and is concerned with the course of lung cancer evolution.
7.Meta-analysis of Effectiveness and Safety of Inhaled Furosemide in the Adjuvant Treatment of Bronchial Asthma

OBJECTIVE:To evaluate the effectiveness and safety of inhaled furosemide in the adjuvant treatment of bronchial asthma systematically,and to provide evidenced-base reference for clinical treatment. METHODS:Retrieved from PubMed,Co-chrane library,CJFD,Wanfang database and VIP,randomized controlled trials(RCTs)about routine treatment+inhaled furosemide (trial group)vs. routine treatment(control group)in the treatment of bronchial asthma were collected. After quality evaluation and data extraction according to Cochrane systematic evaluation method 5.1.0,Meta-analysis was performed with Rev Man 5.3 soft-ware. RESULTS:A total of 13 RCTs were included,involving 708 patients. The results of Meta-analysis showed that:obvious ef-fective rate [RR=1.53,95%CI(1.28,1.84),P<0.001],total effective rate [RR=1.34,95%CI(1.23,1.45),P<0.001],the inci-dence of laryngeal discomfort [RR=10.79,95%CI(1.47,78.99),P=0.02] and gastrointestinal adverse reaction [RR=10.80,95%CI (1.43,81.53),P=0.02] in trial group were all significantly higher than control group,with statistical significance. There was no sta-tistical significance in the incidence of dry mount [RR=3.71,95%CI(0.81,16.86),P=0.09] and electrolyte disorder [RR=2.38, 95%CI(0.10,56.53),P=0.59] between 2 groups. CONCLUSIONS:Based on conventional treatment,the inhaled furosemide has good therapeutic efficacy and safety for bronchial asthma,and the laryngeal discomfort and gastrointestinal reaction should be strengthened.
8.Advances of studies on minimal persistent inflammation of allergic rhinitis

The minimal persistent inflammation(MPI)of allergic rhinitis(AR)is asymptomatic inflammation that occurs locally in the nasal mucosa after exposure to low doses of allergens.With the study of MPI,researchers have realized that this local sub-clinical inflammation in patients with AR not only causes chronic damage to the nasal mucosa,but also increases the hypersensitivity and hyperresponsiveness of allergic airway disease.The concept of MPI has changed our strategy for the treatment of AR,which re-quires that we should not only intervene in the symptomatic exacerbation period,but also intervene in the asymptomatic MPI period.This article systematically reviews the immunological mechanism,adverse effects,and treatment strategies of the MPI,to provide bet-ter treatment for the MPI of AR and reduce the recurrence of AR.
9.Hepatotoxic mechanism of diclofenac sodium on broiler chicken revealed by iTRAQ-based proteomics analysis

Background:
At the therapeutic doses, diclofenac sodium (DFS) has few toxic side effects on mammals. On the other hand, DFS exhibits potent toxicity against birds and the mechanisms remain ambiguous.
Objectives:
This paper was designed to probe the toxicity of DFS exposure on the hepatic proteome of broiler chickens.
Methods:
Twenty 30-day-old broiler chickens were randomized evenly into two groups (n = 10).DFS was administered orally at 10 mg/kg body weight in group A, while the chickens in group B were perfused with saline as a control. Histopathological observations, serum biochemical examinations, and quantitative real-time polymerase chain reaction were performed to assess the liver injury induced by DFS. Proteomics analysis of the liver samples was conducted using isobaric tags for relative and absolute quantification (iTRAQ) technology.
Results:
Ultimately, 201 differentially expressed proteins (DEPs) were obtained, of which 47 were up regulated, and 154 were down regulated. The Gene Ontology classification and Kyoto Encyclopedia of Genes and Genomes pathway analysis were conducted to screen target DEPs associated with DFS hepatotoxicity. The regulatory relationships between DEPs and signaling pathways were embodied via a protein-protein interaction network. The results showed that the DEPs enriched in multiple pathways, which might be related to the hepatotoxicity of DFS, were “protein processing in endoplasmic reticulum,” “retinol metabolism,” and “glycine, serine, and threonine metabolism.”
Conclusions
The hepatotoxicity of DFS on broiler chickens might be achieved by inducing the apoptosis of hepatocytes and affecting the metabolism of retinol and purine. The present study could provide molecular insights into the hepatotoxicity of DFS on broiler chickens.
10.Safety and efficacy of stereotactic surgery in refractory mental disorders

Objective:To evaluate the clinical efficacy and safety of stereotactic surgery in patients with refractory mental disorders.Methods:A retrospective analysis was performed; clinical data, postoperative complications and medication intake of 149 patients with refractory mental disorders accepted stereotactic surgery in Department of Neurosurgery, People's Hospital, Wuhan University from August 2019 to December 2023 were collected. Outcomes were assessed at 1, 6, 12, and 24 months after surgery by Clinical Global Impression-Global Improvement (CGI-GI). Before and 1, 6, and 12 months after surgery, severities were assessed by Clinical Global Impression-Severity of Illness (CGI-SI); cognition was assessed by Montreal Cognitive Assessment (MoCA); positive and negative symptoms were evaluated by Positive and Negative Symptom Scale (PANSS); psychotic symptoms were evaluated by Brief Psychiatric Rating Scale (BPRS) and Symptom Checklist 90 (SCL-90); obsessive-compulsive symptoms, depressive symptoms, anxiety symptoms and manic symptoms were assessed by Yale-Brown Obsessive-Compulsive Symptoms Scale (Y-BOCS), Beck Depression Inventory (BDI)-II, Beck Anxiety Inventory (BAI), and Young Mania Rating Scale (YMRS), respectively; social functioning and quality of survival were evaluated by Social Disability Screening Schedule (SDSS) and World Health Organization Quality of Life-Bref Form (WHOQOL-BREF).Results:(1) Increased sleep was noted in 47 patients and fatigue in 38 patients within 1 week after surgery. Behavioral laziness and emotional apathy were still presented at 1 month after surgery in 6 patients, and complications disappeared in the rest patients. Mildly reduced initiative was presented at 12 months after surgery in 5 patients. (2) CGI-GI indicated that 149 patients were followed up 1 month after surgery with an overall efficiency of 85.90%; 135 patients were followed up at 6 months after surgery with an overall efficiency of 83.21%, 106 patients were followed up at 12 months after surgery with an overall efficiency of 79.24%, and 63 patients were followed up at 24 months after surgery with an overall efficiency of 80.95%. (3) Compared with those before surgery, significantly lower BPRS scores, significantly lower PANSS positive, negative, and overall scores, statistically lower BAI, BDI-II, YMRS, and MOAS scores, significantly lower Y-BOCS obsessional thinking, compulsive behavior and total scores, significantly higher WHOQOL-BREF (physical and psychological domains) scores, and significantly lower SDSS and SCL-90 scores were noted in patients at 1, 6, and 12 months after surgery ( P<0.05). (4) At 12 months after surgery, withdrawal drug was noted in 13 patients, reduced drug in 38, same dose in 52, and increased drug in 2 patients. Conclusion:Stereotactic surgery can obviously improve obsession, anxiety, depression, mania and aggression, and modify social functioning and quality of survival in patients with refractory mental disorders, enjoying good safety.
