1.Progress on Toric intraocular lens

With the increasing perfection and improvement of microsurgery technique in modern ophthalmology, especially the popularization of micro-incision cataract extraction and the rise of femtosecond-laser-assisted cataract extraction, the cataract extraction and intraocular lens implantation is gradually transformed from vision rehabilitation surgery into refractive surgery and people also pursue better postoperative visual experiences. However, the preoperative and postoperative corneal astigmatism is always the main factor affecting the postoperative visual effect. ln recent years, the development of design and materials of the intraocular lens ( lOL) provided necessary basic conditions for clinical application of the Toric lOL. The Toric lOL is primarily characterized by effective correction of cataract patients′postoperative corneal astigmatism, which results in the enhancement of patients′postoperative visual function. ln this paper, the design, classification and clinical application of Toric lOL were analyzed, and current problems and prospect were discussed.
2.Determination of Isomeric Impurity in Fasudil Hydrochloride by HPLC

Objective:To establish an HPLC method for the determination of isomeric impurity in fasudil hydrochloride. Meth-ods:The chromatographic method was carried out on a Kromasil 100-5 Phenyl C18 column with phenyl bonded silica as the filler (250 mm × 4. 6 mm, 5 μm), and phosphate buffer (10 mmol· L-1 ammonium dihydrogen phosphate, adjusting pH to 4. 0 with 1% phos-phoric acid) -acetonitrile (80∶ 20) was used as the mobile phase. The detection wavelength was 275nm and the column temperature was 40℃. The flow rate was 1. 0 ml· min-1 , and the injection volume was 10 μl. Results:Fasudil hydrochloride and its derivative was linear within the range of 0. 148-2. 960 μg·ml-1(r=1. 0000) and 0. 101-2. 014μg·ml-1(r=0. 999 9), respectively. The av-erage recovery of isomer impurity in fasudil hydrochloride was 101. 9% with RSD of 0. 98%(n=9). Conclusion:The method is sim-ple,accurate and reproducible,which can be used for the quality control of fasudil hydrochloride and its isomer.
4.Preliminary Prooerties of Meridian System as a Complex Network

The system of meridians and acupoints in the traditional Chinese medication (TCM) builds up a complex network. We analyze the features and properties of the system of meridians and acupoints as a complex network based on knowledge of TCM and mathematics. The system shows some important features of complex networks such as the small-world effect and the scale-free property. A preliminary discussion on the robustness of the meridian system as a complex network, and the biomedical functions of such robustness is also given.
5.Effect of lentivirus-mediated RNA interference silencing IL-1 type Ⅰ receptor on signal transduction in a rat model of osteoarthritis

Objective To observe the effects of lentivirus-mediated RNA interference silencing IL-1 type Ⅰ receptor on the expression of mitogen-activated protein kinase (MAPK) in a rat model of osteoarthritis (OA), and to explore the role of IL-1 and MAPK in OA. Methods Forty SD rats were randomly divided into four groups(A-D, each group: n = 10). The rats (A-C group) were underwent unilateral anterior cruciate ligament transaction and partial excision of the medial meniscus. At day 7 and 9 after operation, the rats of group A were respectively received 40 μL of lentivirus-mediated RNAi silencing IL-1 type Ⅰ receptor by intra-articular injection, while the rats of group B were received lentivirus-mediated RNAi non-silencing IL-1 type Ⅰ receptor, and the rats of group C were received saline. The rats of group D were taken for normal control. All rats were sacrificed four weeks after the surgery. The knees were harvested to observe macropathologic changes of the joint cartilage. ERK1/2, JNK1/2 and p38 proteins in the cartilage were detected by Western blot. Results Cartilage degradation in group A was milder than that in group B and C (P<0.05),but was worse compared to group D (P<0.05). Level of p38 expression in cartilage in group A was lower than that in group B and C (P<0. 05), but had no significant difference compared to group D (P>0.05). Levels of ERK1/2 and JNK1/2 expressions in cartilage in group A were lower than that in group B and C(P<0.05),but was significantly increased compared to group D(P<0.05). Conclusions Lentivirus-mediated RNAi silencing IL-1 type Ⅰ receptor can inhibit the expression of MAPK, in particular,p38 protein was strongly inhibited.
6.Expression of endonuclease G in the brain tissue of rats with chronic fluorosis

Objective To investigate the possible mechanism of endonuclease G (Endo G)-mediated non-Caspase-dependent apoptotic pathway in brain neuronal apoptosis in chronic fluorosis rats.Methods Sixty Sprague Dawley (SD) rats (half male and half female) were randomly divided into two groups:control group fed with tap water with fluoride content < 0.5 mg/L and fluorine group in which sodium fluoride was added into drinking water with fluoride content of 50.0 mg/L.Both groups were fed with standard food with fluorine content < 0.5 mg/kg.The experiment period was 10 months.At the end of the experiment,all the animals were sacrificed,and brain tissue was taken.Flow cytometry was used to examine apoptosis rate,immune-histochemistry was employed to detect the distribution of Endo G in brain tissue;Western blotting was used to test the protein expression of Endo G.Results Compared to the apoptosis rate of control group [(1.3 ± 0.6)%,(1.9 ± 0.3)%],the apoptosis rate in hippocampus and cortex of rats with chronic fluorosis [(2.6 ± 0.6)%,(3.1-± 0.7)%] was significantly increased (t =3.1,3.4,all P < 0.05).The Endo G positive neurons and their degree of staining in CA1,CA2,CA3 and CA4 of hippocampus,frontal cortex as well as the upper layer of parietal cortex [(11.1 ± 2.2),(10.2 ± 1.9),(9.8 ± 3.1),(9.9 ± 1.6),(10.6 ± 2.9),(8.2 ± 2.4),(11.1 ± 2.8) scores] in rats with chronic fluorosis were significantly higher than those in the control group [(5.8 ± 1.8),(6.7 ± 2.6),(5.2 ± 2.4),(7.2 ± 2.1),(7.7 ± 2.6),(6.1 ± 1.9),(8.1 ± 2.6) scores,t =2.9,2.5,2.4,2.3,2.2,2.5,2.3,P < 0.01 or < 0.05].The protein level of Endo G in the mitochondria of rat brains with chronic fluorosis [(86.4 ± 7.2)%,(83.9 ± 6.8)%] was significantly lower than that of control group [(100.0 ± 6.1)%,(100.0 ± 5.5)%,t =2.6,2.3,all P < 0.05].Meanwhile,the protein level of Endo G in the nucleus of neurons from chronic fluorosis rats [(117.5 ± 6.4)%,(115.2 ± 6.2)%] was significantly higher than that of the control [(100.0 ± 5.2)%,(100.0 ± 5.5)%,t =2.5,2.2,all P < 0.05].Conclusion The high expression of Endo G and nuclear transfer are related to the neuron apoptosis in chronic fluorosis rat,which may be one of the mechanisms of brain injury of the disease.
7.Diagnosis and treatment of mucinous tumor of the bile duct: our experience on 11 patients

Objective To study the diagnosis,treatment and therapeutic results of 11 patients who suffered from mucinous tumor of the bile duct.Methods Eleven patients who were diagnosed to suffer from mucinous tumor of the bile duct were retrospectively studied.Three patients who presented with obstructive jaundice were diagnosed on ERCP,and 8 patients who had extra-and intrahepatic cholangiolithiasis were diagnosed by biopsy during choledochoscopy.Results One of the 3 patients who underwent ERCP died from obstructive jaundice after failed drainage of bile using endoscopic nasobiliary drainage (ENBD).The remaining two patients underwent laparoscopic common bile duct exploration and T tube drainage.The eight patients who had extra-and intrahepatic cholangiolithiasis were diagnosed by biopsy during choledochoscopy.Conclusions The clinical presentation of mucinous tumor of bile duct is non-specific and the preoperative misdiagnosis rate is high.Common bile duct exploration,T tube drainage and long-term T tube drainage is a good way to treat mucinous tumor of the bile duct.
8.Mechanics effect of lumbar physiological curvature adjustment by three-dimensional swirl angle program control:A randomized controlled analysis

AIM:To explore the regulation of lumbar physiological curvature by angulation traction and longitudinal rotation and the clinical significance of repairing spinal balance on lumbar disc herniation(LDH). METHODS:①Totally 71 patients with LDH diagnosed by CT at the 451 Hospital of Chinese PLA were enrolled from October 2005 to January 2006. Three-dimensional swirl angle program control restitution bed was applied. This bed was composed of fixing chest swirl section,magnetic heat lumbar flexure massage section and traction haunch angulation section with the pull strength of 20-90 kg. ②The patients were divided into experimental group(n =38) and control group(n =33). ③All patients of the two groups received supine genuflex traction for 20 minutes and combined with longitudinal rotation for 10 minutes. Patients in the experimental group received 20? traction from below lumbar to bed,while patients in the control group received plane traction combined with longitudinal rotation. ④Lumbar function was determined by criterion of the lumbar disease made by Japanese Shaping Association to observe improved indexes and times of treatment. Objective To discover biological markers of male infertility.Methods Two-dimensional electrophoresis and Mass-Spectrum techniques(MS+MS/MS) were used for analyzing the seminal plasma from idiopathic male infertility and the control.Results Serum albumin and prostatic acid phosphatase were reduced in seminal plasma from idiopathic male infertility,while Cathepsin B and Zn-alpha-2-glycoprotein were increased.ConclusionIdiopathic male infertility was potentially associated with disorder of sperm capacitation and seminal immune function.
10.Evaluation of right ventricular global and regional function in patients with metabolic syndrome by two-dimensional speckle tracking imaging

Objective To evaluate the right ventricular global and regional strain and strain rate in patients with metabolic syndrome(MS) by two‐dimensional speckle tracking imaging(2D‐STI) ,and explore the correlation between different MS criteria and global strain and strain rate of right ventricle . Methods Forty‐four MS subjects ,35 pre‐MS subjects and 40 healthy subjects were enrolled . The basal ,middle and apical systolic longitudinal peak strain( Ss) and strain rate( SRs) ,early diastolic and late diastolic peak strain rate(SRe ,SRa) of right ventricular free wall were measured using 2D‐STI and global strain and strain rate ( GSs ,GSRs ) ,early diastolic and late diastolic peak strain rate ( GSRe ,GSRa ) of right ventricle were calculated . The correlation between different MS criteria and GS ,GSR of right ventricle were analyzed . Results Compared with control group ,Ss ,SRs ,SRe and SRa in basal and middle segments in MS group and Ss ,SRs in basal segment in Pre‐MS group decreased( all P < 0 .05) . Ss and SRs in basal segment in MS group were significantly lower than those in Pre‐MS group(all P <0 .05) . Compared with control group , GSs ,GSRs ,GSRe and GSRa in MS group and GSs ,GSRs in Pre‐MS group decreased ( all P <0 .05) . GS and GRS in MS group and Pre‐MS group had no statistical significance( all P > 0 .05) . There were good correlation of waist circumference(WC) ,systolic blood pressure(SBP) ,fasting blood‐glucose with GSs( r =-0 .73 ,-0 .69 ,-0 .46 , P < 0 .01) and GSRs( r = -0 .63 ,-0 .78 ,-0 .48 , P < 0 .01) . The multiple regression analysis of MS criteria showed that waist circumference and systolic blood pressure were independently associated with GSs and GSRs of right ventricle(β= -0 .44 ,-0 .77 , P <0 .01) . Conclusions 2D‐STI could accurately assess right ventricular global and regional function in MS patients .Among all MS risk factors ,waist circumference and systolic blood pressure are the most responsible for right ventricular function .
