1.Construction of Caveolin-1 RNA interfering eukaryotic expressing vector and its influence on biological behavior of human gastric cancer cells

Objective To design siRNA for Caveolin-1(CAV1) and construct eukaryotic expression vector Dsilence4.1-CMV-neo-CAV1, which is transfected into gastric cancer cell line SGC7901 to study the effects of inhibition of CAV1 expression with siRNA on biological behavior of gastric cell line SGC7901.Methods Design and chemosynthesize three pairs of double strands siRNA based on the neucleotide sequence of CAV1 gene provided by GeneBank.The siRNAs were transient transfected into SGC7901.The CAV1 expression of transfected cells were detected respectively by semi-quantity RT-PCR to sieve a valid siRNA sequence.Design and synthesized DNA sequence which can express small hairpin RNA(shRNA),construct RNA interfering eukaryotic expression vector of CAV1 gene.The SGC7901 Was stably transfected and was detected the expression of CAV1 gene by RT-PCR.The influence on the gastric cell growth and pmliferation after CAV1 was inhibited Was detected by cell growth curve, clone formation test and FCM.Results The CAV1 RNA interfering eukaryotic expression vector psilence4.1-CMV-neo-CAV1 was successfully constructed,which can significantly reduce the expression of CAV1 after stably transfected to target cells,and in contrast to Dsilence4.1-CMV-neo transfection group,PI and colony for mution rate were increased(t=7.98,P<0.05;t=13.19,P<0.01),the growth speed Was up.Conclusion The growth and proliferation ability of tumor is remarkablv accelerated by inhibiting the expression of CAV1 of gastric cell by RNAi.
2.Assessment of coronary artery fistula diagnosed by color Doppler flow imaging

Objective To evaluate the value of color Doppler flow imaging(CDFI) in the diagnosis of congenital coronary artery fistula (CAF).Methods Thirteen consecutive patients ranging in age from 8 to 68 years underwent conventional transthoracic echocardiography,which depicts their origin, course, and drainage site. Results Right CAFs were detected in 8 patients: four of them had drainages to the left ventricle, two to the main pulmonary artery, one to the right ventricle(RV) and the other to the right atrium(RA). CAFs arising from the left coronary artery were noted in five patients: three of them had drainages to the main pulmonary, and the other two to the RA and RV respectively. The origin and the course were precisely defined and the drainage site was clearly depicted in all thinteen patients. The diagnosis of the thirteen patients was confirmed by coronary angiography, and eight of them received surgical treatment. Conclusions CDFI offers a simple, useful and reliable technique in diagnosis of CAF.
3.Efficacy of ultrasound and microbubbles on augmentation phlebothrombosis inrabbit: an in vivo study

Objective To study the effects of diagnostic ultrasound combined with microbubble contrast agent on augmentation thrombolysis in vivo and to discuss the synergism and mechanism of combination of urokinase. Methods Thirty-two rabbits of femoral vein thrombus were divided into four groups: simple urokinase group, ultrasound and contrast microbubble group, ultrasound and contrast microbubble combined with urokinase group, control group. Then according to the best parameter combination which was definited in previous experiment, the experiments combination of urokinase were performed. Color Doppler was used to evaluate the recanalization of different group in 15 rain,30 min and 60 min. Results After the treatment of 15 rain and 30 min,the recanalization rate in ultrasound and contrast microbubble combined with urokinase group improved significantly compared with other groups (P<0.05);after 60 min, the recanalization grade of ultrasound and contrast microbubble and urokinase group was evidently better than other two group. The result of immunohistochemistry showed that disaggregation of clot and fibrous in this group at 60 min was better than the results at 15 min, 30 min. Conclusions Diagnostic ultrasound with microbubble contrast agent showed superior effects of thrombolysis and the synergism of combination of urokinase in vivo.
4.An Analysis on the Mental Health of Adolescent with Cochlear Implants

Objective To analyse the mental health of adolescents with cochlear implants and provide the ref‐erence for programming the personalize rehabilitation schedule of audio - speech -psychological .Methods The mental health test(MHT) was used to evaluate the mental health status of 60 adolescent with cochlear implants ,and compared with those of adolescent with normal hearing .Results The incidence of psychological problems was 3 .33% in adolescent with cochlear implants .The hypersusceptilility scores of the group of cochlear implants (5 .63 ± 2 .22) was higher than that of in normal hearing group(4 .05) in ,which was significantly different(P<0 .01) . The body syndrome scores in the cochlear implants group (4 .03 ± 2 .72) was lower than that of in normal hearing group(4 .99) ,which was significantly different(P<0 .01) .The phobic tendency scores in cochlear implants group (1 .95 ± 2 .01)was significantly lower than that of in normal hearing group (3 .06)(P<0 .01) .Conclusion The over‐all mental health of adolescent with cochlear implants is good .Cochlear implants can help them improve hearing and speech function ,and improve quality of life ,but they still need parents ,teachers and the community to pay more at‐tention and care to help them get through adolescence ,and return to mainstream society .
5.Clinicopathological features and prognostic study of 115 patients with combined hepatocellular and cholangiocarcinoma

Objective To assess clinicopathological features and prognosis of patients with combined hepatocellular and cholangiocarcinoma (cHCC-CC). Methods Clinicopathological and follow-up data of 115 cHCC-CC patients confirmed pathologically in Liver Cancer Institute of Fudan University from 1995 to 2007 were analyzed. Kaplan-Meier method was used to calculate 1-,3- and 5-year survival rates and tumor-free survival rates. Survival curves were analyzed using the log-rank test. The factors that impacted the prognosis of cHCC-CC were estimated. Results In 115 cases, one was Allen's type A, one was Allen's type B, and the other 113 were Allen's type C. Being with male in predominance, most of the cHCC-CC patients had liver cirrhosis background. They presented with elevated AFP or CA19-9, vascular invasion, resembling hepatocellular carcinoma(HCC)as well as lymph nodes metastasis. One-, 3-, 5-year survival rates of 115 patients were 68. 1%, 38. 1% and 33.6%, respectively, with median survival time of 13.0 months. Whereas the 1-, 3-, 5-year survival rates in radical resected patients were 78.4 % ,44.4 % and 44.4 % ,respectively, with median survival time of 16.0 months. Tumor free survival time at 1-, 3- and 5-year was 57.8 %, 12.6 % and 0.0 %,respectively,with median recurrent time of 10.0 months. One-, 3-, 5-year survival rates of 10 nonsurgical patients were 10/10,10/10 and 0/10,respectively, with median survival time of 5.3 months.TNM stage was independent factor for prognosis of the patients after resection. Whereas the lymph nodes involvement was independent factor for the tumor free survival time of radical resected patients.Conclusions Although clinicopathological characteristics of cHCC-CC are more similar to those of HCC, the prognosis of cHCC-CC is more unfavorable than that of HCC. TNM stage is an independent determinant of long time outcome for patients after resection.
6.Clinical significance of serum interleukin-8 and myocardial enzyme levels in the diagnosis and treatment of children with bronchopneumonia

Objective To investigate the association of serum interleukin-8 (IL-8) and myocardial enzyme levels in children with bronchopneumonia.Methods Serum IL-8 contents were measured by the radioimmunity method and serum myocardial enzyme contents were measured by the Olympus AU640 in 60 children with bronchopneumonia (mild 30 cases and severe 30 cases) as well as in 30 controls.Results Serum IL-8 contents were significantly higher in children with severe bronchopneumonia and mild bronchopneumonia than those in controls [( 2.54 ± 0.65),( 1.28 ± 0.53) μ g/L vs.(0.43 ± 0.08) μ g/L] (P <0.01),and serum myocardial enzyme contents (α-hydroxybutyrate dehydrogenase,MB isoenzyme of creatine kinase,creatine kinase,aspartate aminotransferase) were significantly higher in children with severe bronchopneumonia and mild bronchopneumonia than those in controls (P<0.01).Conclusions IL-8 and myocardial enzyme may play a role in children with bronchopneumonia.Determination of serum IL-8 and myocardial enzyme levels might have important prognostic values in children with bronchopneumonia.
7.The ratio of urine albumin to creatinine used in screening for albuminuria among the middle-aged and elderly at high-risk for diabetes in an urban community with casual and morning urine specimens

Objective To compare the value of clinical application of the ratio of urine albumin to creatinine (ACR) in the community-dwelled middle-aged and elderly at high-risk for diabetes in screening for albuminuria with casual and morning urine specimens. Methods Totally, 443 middle-aged and elderly people (234 men and 209 women) at high-risk for diabetes, with an average age of (59±8) years, were recruited from Changfeng Community of Shanghai in screening for albuminuria with casual and morning urine specimens collected on the same day, as well as another two morning urine specimens collected at an interval of two weeks, for determination of urine ACR. Albuminuria was defined as ACR equal to or more than 30 mg (albumin)/g (creatinnine) in two or more of the three urine specimens collected. Results Overall prevalence of albuminuria was 6. 3% ( 28/443 ) in the middle-aged and elderly according to diagnostic standard mentioned above, significantly higher in casual urine specimens than in morning urine specimens (14. 9% vs. 5.9%, P <0. 05). Urine ACR of casual urine specimens was significantly higher than that of morning urine (7. 1 mg/g vs. 3. 2 mg/g, P<0. 001 ). There was no significant difference in sensitivity and positive predictive value (PPV) between casual and morning urine specimens in screening for albuminuria (96. 4% vs. 92. 9%, and 99.7% vs. 99. 5%, respectively, P >0. 05). Specificity and negative predictive value (NPV) of casual urine specimens was significantly lower than those of morning urine specimens (90.6% vs. 100.0%, and 40.9% vs. 100.0%, respectively, P<0.05). The optimal cut-off value of ACR by casual urine specimens was 40. 0 mg/g in screening for albuminuria, based on receiver operating characteristic (ROC) curve, with sensitivity of 96. 4%, specificity 95.2%, PPV 57.5% and NPV 99. 8%. The middle-aged and elderly with negative albuminuria in morning urine but positive in casual urine had more risk factors for cardiovascular and chronic kidney diseases than those with negative albuminuria in both morning and casual urine specimens. Conclusions Morning urine specimen is better than casual urine for albuminuria screening in the middle-aged and elderly people with risk factors for diabetes in urban community, but casual urine used in screening for albuminuria is very convenient for collection. It is suggested that urine ACR of 40. 0 mg/g be used as an optimal cut-off value for casual urine in albuminuria screening among them.
8.Cut-off value of waist circumference for identifying metabolic syndrome in middle-aged and elder residents in Changfeng community of Shanghai

Objective To explore an optimal cut-off value of waist circumference(WC)for identifying metabolic syndrome(MS)in middle-aged and elder residents in an urban community.Methods Database of 1558 residents aged 40-79 years at high-risk in screening for diabetes from Changfeng community,Putuo District,Shanghai during 2005 to 2007 was used to analyze sensitivity and specificity of varied cut-off values of WC for identifying two or more components of MS in men and women,respectively.Receiver operating characteristics(ROC)curve was used to explore the optimal value.Results Optimal cut-off value of WC was 85.0 cm for men and 80.0 cm for women,respectively,for identifying two or more components of MS bv ROC curve,with sensitivity of 72.3%and 74.9%,and specificity of 50.2%and 62.2%,respectively.Based on that optimal cut-off value of WC and definition of International Diabetes Federation(IOF),prevalence of MS was 38.8%in this population,40.7%in men and 37.8%in women,respectively.Conclusion The optimal cut-off value of WC was 85.0 cm in men and 80.0 cm in women,respectively,for identifying MS in the middle-aged and eldedy in an urban community of Shanghai.
9.The clinical feature and diagnosis of transient intrahepatic cholestasis in early stage of orthotopic liver transplantation

Objective To discuss the feature of transient intrahepatic cholestasis in early stage of orthotopic liver transplantation. Methods Based on the review of early (within 1 month) postoperative hyperbilirubinemia in consecutive 200 patients undergoing liver transplantation, we summarized the occurrence, development and outcome of early postoperative intrahepatic cholestasis. Results Early transient intrahepatic cholestasis was identified in 112 patients. The characteristic of early intrahepatic cholestasis is that DBIL and?-GT increasingly elevated from the second or third day postoperatively, with a peak on the 7 - 14th d, then descended to normal level on approximately 21 -28th day. The average peak level of DBIL and?-GT were( 157. 32?82. 08)?mol/L and (172?80) IU/L respectively. During the period of DBIL and?-GT ascending, AST and ALT kept descending, and within 1 week it could fall to normal level. Acute rejection, drug toxicosis and bile duct obstruction were excluded. Conclusions Postoperative early transient intrahepatic cholestasis associated with ischemia-reperfusion injury has its special clinical process and most patients recover themselves without the need for special therapy.
10.Use of autologous left renal vein as a graft vessel for reconstruction after portal vein-superior mesenteric vein resection in pancreaticoduodenectomy

Objective:To study the use of left renal vein as a graft vessel in reconstruction after portal vein/superior mesenteric vein (PV-SMV) resection in pancreaticoduodenectomy.Methods:A retrospective study was conducted on 5 of these patients who underwent surgery from July 2008 to December 2017 at Chinese PLA General Hospital. The operative, complication and follow-up data were analysed.Results:There were 4 males and 1 female, with an average age of 57 (33-72) years. The mean operative time was 6.8 (5.4-9.1) h and the mean tumor size was 3.8 (2.8-4.8) cm. The average length of the PV-SMV defect left after resection was 3.8 (3.2-4.6) cm. The average length of the left renal vein used was 3.4 (3.0-4.1) cm. The operations were carried out in 3 patients with pancreatic cancer and in 2 patients with colon cancer pancreatic metastasis. The average postoperative hospital stay was 12 (10-25) days. Perioperative complications included 1 patient each with ascites, diarrhea and delayed gastric emptying. The creatinine levels ranged from 70-98 μmol/L preoperatively, with a transient creatinine rise to 80-156 μmol/L after operation and became 62-107 μmol/L upon discharge from hospital. The follow-up time was 4.3-17.8 months. Two patients died of recurrence/metastasis at 14.2 and 17.8 months after surgery.Conclusions:The left renal vein has the appropriate diameter and rich collateral branches. It has a sufficient length and it is conveniently located in the surgical field. This study showed that there was a minimal effect on renal function after its excision, and it can be used as a graft vessel for reconstruction in pancreaticoduodenectomy after PV-SMV resection.
