1.Progress of diagnosis and treatment of exudative erythema multiforme in children

Exudative erythoma multiforme is a related to the immune function of children with skin and mucosa,damage to its diversity of characteristics of acute non suppurative inflammation,one of the aller-gic diseases.The disease and its pathogenesis is complex,the body can cause multi system and multi organ damage,the cause has not yet entirely clear.In recent years,due to our understanding of the exudative erytho-ma multiforme gradually thorough,both domestic and abroad on the level of diagnosis and treatment of the disease have greatly improved,the prognosis of patients with exudative erythoma multiforme has improved significantly.The progress of the pathology,clinical manifestation,diagnosis and treatment of exudative ery-thoma multiforme were summarized.
2. Investigation of Drug Zero Bonus Present Situation in a County -level Public Hospital in Henan Province

OBJECTIVE: To study the effect of "cancel drug addition" policy in county public hospitals, and to provide reference for further improvement of relevant policies. METHODS: This paper compares the data of medical expenses control, hospital income and expenditure, compensation and so on before and after the policy of "canceling the drug addition" of a county public hospital to study the actual effect of policy implementation. RESULTS: The proportion of drugs after the implementation of the decline, from the policy before the implementation of 43.6% to the policy objectives (30%). The total cost of drug growth rate is lower than before the implementation of the policy, the slowdown in growth; outpatient average medical expenses, inpatient per capita reduction in drug costs, the difference was statistically significant. The average cost of medical care was increasing, and the growth rate dropped first and then rose, which was close to the level before the implementation of the policy. At the same time, the income structure of the county-level hospitals tends to be reasonable, but the balance of payments has dropped significantly. The increase in hospital investment and adjust the price of medical services increased hospital income is not enough to make up for the "cancellation of drug additions" brought about by the loss. CONCLUSION: "Cancellation of drug addition" policy in the county-level public hospitals since the implementation of the control of medical expenses have a certain role in the control of drug costs growth effect is obvious. Patient medical expenses are falling. In order to consolidate the effectiveness, but also the establishment of multi-share long-term compensation mechanism, while paying attention to the increase in medical staff income.
3.The value of BAEF in early diagnosis of acute bilirubin encephalopathy in newborn

Objective Acute bilirubin encephalopathy in neonates is the most serious complication of neonatal hyperbilirubinemia, is one of the main causes of neonatal death and disability. Clinical early diagnosis, early treatment can improve the prognosis in children. Methods Brainstem auditory evoked potential (BAEF) was detected on two patients (40 patients with ABE, 40 cases of normal controls, all full-term) in the state of sleep in children and analysis the difference between the two groups ,all testing was completed by experienced Department of ENT full-time technician in charge,SPSS15.0 statistical analysis software was took for data analysis (using rank sum test method). Results There was significant difference between the two groups of neonatal latency of wave I, latency of waveⅤ, interpeak time , acute bilirubinⅠ-Ⅴencephalopathy group was significantly longer than that of the control group. Conclusions The BAEF detection is the sensitive index of brainstem damage , can objectively and sensitively reflect the function of the central nervous system , can reflect the functional status of cochlear and brainstem structures , often brainstem was slightly damaged but no clinical symptoms and signs , BAEP has changed significantly , so the conventional BAEP examination performed on patients with hyperbilirubinemia help to find bilirubin brain damage as early as possible,and prevent the occurrence of bilirubin encephalopathy.
4.Efficacy of ultrasound and microbubbles on augmentation phlebothrombosis inrabbit: an in vivo study

Objective To study the effects of diagnostic ultrasound combined with microbubble contrast agent on augmentation thrombolysis in vivo and to discuss the synergism and mechanism of combination of urokinase. Methods Thirty-two rabbits of femoral vein thrombus were divided into four groups: simple urokinase group, ultrasound and contrast microbubble group, ultrasound and contrast microbubble combined with urokinase group, control group. Then according to the best parameter combination which was definited in previous experiment, the experiments combination of urokinase were performed. Color Doppler was used to evaluate the recanalization of different group in 15 rain,30 min and 60 min. Results After the treatment of 15 rain and 30 min,the recanalization rate in ultrasound and contrast microbubble combined with urokinase group improved significantly compared with other groups (P<0.05);after 60 min, the recanalization grade of ultrasound and contrast microbubble and urokinase group was evidently better than other two group. The result of immunohistochemistry showed that disaggregation of clot and fibrous in this group at 60 min was better than the results at 15 min, 30 min. Conclusions Diagnostic ultrasound with microbubble contrast agent showed superior effects of thrombolysis and the synergism of combination of urokinase in vivo.

Objective To study the effect of RNA interference (RNAi) on HYAL1 gene mRNA expression and the invasive potential of human breast cancer cell lines. Methods Chemically synthesized double stranded RNA (dsRNA) targeting HYAL1 was transfected into human breast cancer cell lines MDA-MB-231, MDA-MB453S, ZR-75 and ZR-75-30 using SiPORT Lipid. The transfection efficiency was observed under fluorescence confocal microscopy. Expression of HYAL1 mRNA was detected by reverse transcription polymerase chain reaction (RT-PCR). OBJECTIVETo study the role of interleukin 17 (IL-17) in the pathogenesis of chronic otitis media with effusion (COME).METHODSThe expression of IL-17 in middle ear effusion (MEE) and blood plasma were measured in 30 patients (48 ears) by means of Cytometric Bead Array (CBA), as well as in 20 normal volunteers.RESULTSCompared with the control group, the level of IL-17 significantly increased in the peripheral blood of COME patients (P<0.05). What was more, the level of IL-17 in the MEE was higher than that in peripheral blood of COME patients (P<0.05).CONCLUSIONIL-17, as an important immunoregulatory mediator, may play an important role in chronic course of COME.
7.Effect of Comprehensive Psychotherapy on Psychology of Cancer Patients

OBJECTIVE:To study the effect of comprehensive psychotherapy on psychology of cancer patients received chemotherapy or radiotherapy. METHODS:77 cancer patients received chemotherapy or radiotherapy were divided into two groups. Both groups received routine chemotherapy or radiotherapy. Intervention group(n=40)were additionally given comprehensive psychotherapy,i.e. cognitive therapy,psychological persuasion and relaxation training. Self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were applied to evaluate all patients one week before intervention and one month after comprehensive psychotherapy. RESULTS:SAS and SDS scores of intervention group were significantly lower than that of control group after treatment(P0.05). CONCLUSION:Comprehensive psychotherapy can improve the mental state of patients with can-cer.
8.Antibiotic Resistance Analysis of Haemophilus Influenzae and Moraxella Catarrhalis Strains Isolated from Children in Sichuan Province from 2013 To 2016

Objective To investigate the changing antibiotic resistance profile of Haemophilus influenzae and Moraxella catarrhalis strains collected from children in Sichuan province from 2013 to 2016,provide some reference for rational utilization of clinical antimicrobial agents.Methods Collected the infection data of Haemophilus influenzae and Moraxella catarrhalis strains isolated from children which reported in Sichuan Province Drug Resistance Monitoring Report from 2013 to 2016.The experimental results were analyzed by WHONET5.6 software.Results The prevalence of H.influenzae increased with time from 8.95% in 2013 to 16.6% in 2016.The prevalence of M.catarrrhalis increased with time from 4.16% in 2013 to 6.34% in 2016.Among the 15 896 clinical strains of H.influenzae,the highest resistance rate was to ampicillin,which was 71.6% in 2016.The resistance rate to cefaclor also increased from 26.1% in 2013 to 59.5% in 2016 for increase of 33.4%.The insensitivity rate to azithromycin increased from 8.3% in 2013 to 25% in 2016.However,the insensitivity rate to ceftriaxone and moxifloxacin decreased in recent years and the susceptibility rate to ceftriaxone,cefotaxime,levofloxacin and moxifloxacin were higher than 90% in each year.The resistance rate of H.influenzae strains from children were higher than the stains from all patients.The insensitivity rate to azithromycin in strains from children and all patients increased from 8.3%,10.2% in 2013 to 25%,22.1 % in 2016,respectively.The 5 625 clinical strains of M.catarrrhalis re mained highly susceptible to the amoxicillin-clavulanic acid,ceftriaxone,cefotaxime,levofloxacin,ciprofloxacin (greater than 90%).The resistance rate to cotrimoxazole increased from 15.1 % in 2013 to 59.1 % in 2016.Conclusion H.influenzae are still susceptible to the third generation cephalosporins (greater than 90 %),which can be used as the first choice in clinical practice.Nearly 70 % of these strains were resistant to ampicillin and cotrimoxazole,which is inappropriate for clinical therapy.The resistance rate to cotrimoxazole in the M.catarrrhalis strains from children increased from 15.1% in 2013 to 59.1 % in 2016,and the resistance rate to the other test drug in M.catarrrhalis did not change much in the-year period.
9.Establish research model of post-marketing clinical safety evaluation for Chinese patent medicine.

The safety of Chinese patent medicine has become a focus of social. It is necessary to carry out work on post-marketing clinical safety evaluation for Chinese patent medicine. However, there have no criterions to guide the related research, it is urgent to set up a model and method to guide the practice for related research. According to a series of clinical research, we put forward some views, which contained clear and definite the objective and content of clinical safety evaluation, the work flow should be determined, make a list of items for safety evaluation project, and put forward the three level classification of risk control. We set up a model of post-marketing clinical safety evaluation for Chinese patent medicine. Based this model, the list of items can be used for ranking medicine risks, and then take steps for different risks, aims to lower the app:ds:risksrisk level. At last, the medicine can be managed by five steps in sequence. The five steps are, collect risk signal, risk recognition, risk assessment, risk management, and aftereffect assessment. We hope to provide new ideas for the future research.

10.Histologic analysis and long-term effect of acellular dermal matrix combined with autologous thin split-thickness skin graft.

OBJECTIVETo evaluate the long-term therapeutic effect and histologic result of ADM combined with autologous thin split-thickness skin graft.
METHODS23 patients were treated with acellular dermal matrix (ADM) combined with autologous thin split-thickness skin graft. The patients were followed up at 3, 6, 12, and 18 months after operation. The histological analysis was also performed.
RESULTS3, 6, 12, 18 months after operation, the composite skin grafts became smooth with no hypertrophic scar and hyperpigmentation. It was soft and elastic. The joints could move randomly. The histologic study showed the composite skin graft had a similar appearance as the normal skin.
CONCLUSIONAs for the treatment of wound, the composite skin graft with ADM is smooth and soft with good elasticity after transplantation, but it has no perspiration.
