To investigate the interrelationship between plasma renin activity(PRAXaldosterone (Ald),atrial natri-uretic peptide ( ANP ) and blood volume ( BV ) in adult nephrotic syndrome ( NS ) . Methods BV was assessed using 113mInCl dilution method. PRA、 Angiotensin Ⅱ (AngⅡ)、Ald、ANP were determined by radioirnmunoassay. 28 edema-tous NS patients,26 healthy controls and 18 cases of remission were involved. Results (1)During edematous period, PR, AngⅡ , Ald、 ANP were higher in NS than in controls. There was no difference of BV between NS and controls. Albumin ,PRA,AngⅡ 、Ald and ANP were not correlated with BV; (2)After remission, comparing with edematous period, Ald 、ANP deceased significantly whereas PRA、AngⅡ、Ald and total BV were not different; (3) Urine natrium excretion (UNaV) was negatively correlated with Aid. Conclusion Ald and ANP may be the major regulating factors of sodium excretion in nephrotic syndrome.

Objective To study the expression of basic fibroblast growth factor(bFGF)in the kidney tissues of chronic interstitial nephritis(CIN).Methods Expression of bFGF was assayed in 30 patients with CIN and 5 normal persons by SP method of immunohistochemical technique. The average gray value and average optical density of the bFGF positive parts and its ratio with the whole visual field had been measured with video analysis system. Results The expression of bFGF in the kidney tissues of CIN was significantly stronger than that in normal tissues. The average gray value, average optical density and the positive areas were significantly high compared with that of normal cases. Conclusion The bFGF can promote the proliferation of fibroblasts and monocytes in renal interstitial patients and lead to renal interstitial fibrosis in the end.
4.Effect of G3PD modifier protein on the renal functions in aged rats with gentamycin-induced acute renal failure
Chinese Journal of Geriatrics 2001;0(03):-
Objective To study whether G3PD modifier protein (MP) could repair tubular epidermal cell(TEC) and ameliorate renal functions in aged rats with gentamycin induced acute tubular necrosis(ATN). Methods Aged and young rats were randomly divided into 3 groups, namely aged and young normal groups, aged and young model groups and the aged and young MP groups. The animal models of acute renal failure (ARF) were induced by gentamycin (140 mg?kg -1 ?d -1 , ip?7 d). Nitric oxide (NO) concentration was determined by Cd activated cadmium reduction method, malonaldehyde(MDA) with thiobarbituric acid test, superoxide dismutase(SOD) with hydroxylamine test, serum creatinine (Scr) with picric acid method, and the renal histology was observed by light and electron microscopy. Results NO was significantly higher in the aged MP group than that in the aged model group 〔serum: (94.29?7.68)?mol/L vs (70.14?5.53)?mol/L, P

The study is to establish an HPLC method using fluorescence detector for the determination of doxazosin enantiomers and investigate their chiral inversion in vitro and in vivo. Ultron ES-OVM was taken as the chiral chromatographic column, and the column temperature was 30 degrees C. Isocratic elution using a mobile phase of phosphate buffer-acetonitrile (85 : 15, v/v) at a flow rate of 0.8 mL x min(-1) was done. The fluorescence detection was set at lambda(Ex) = 255 nm and lambda(Em) = 385 nm. Prazosin was used as the internal standard. (-) Doxazosin or (+) doxazosin added into rat plasma in vitro was determined after incubating in 37 degrees C water bath for 2, 5 and 10 days. (-) Doxazosin or (+) doxazosin was administered orally to the rats for one months. Plasma samples were taken at 8 h after the last administration. A good linear relationship was achieved when the concentration of doxazosin enantiomers was within the range of 4 - 2 000 ng x mL(-1). The average recovery for (-) doxazosin was 99.5% with RSD 3.6%, and for (+) doxazosin was 99.3% with RSD 4.3%. Chiral inversion was observed neither in vitro nor in vivo studies. The method is selective, accurate and reproducible, which is suitable for the detection of doxazosin enantiomers in rat plasma. The in vitro and in vivo studies indicate that chiral inversion occurs uneasily between (-) doxazosin and (+) doxazosin in the rat.

Objective: To explore the risk factors in patients with chronic heart failure (CHF) combining atrial ifbrillation (AF) and to identify the predictive factors for AF recurrence after cardio version. Methods: A total of 246 CHF patients treated in our hospital were divided into 2 groups: CHF+AF (AF) group,n=71 and CHF without AF (Non-AF) group,n=175. The baseline information was recorded. The risk factors for AF occurrence were screened by Logistic regression analysis. There were 66 AF patients with successful cardio verse and they were followed-up for 18 months, the predictors for AF recurrence were studied by Cox regression analysis. Results: The baseline information showed that age, CHF duration, left atria diameter (LAD), the levels of BNP, uric acid and serum creatinine in AF group were all higher than those in Non-AF group, allP<0.05. Logistic regression analysis presented that age (RR=1.89, 95% CI 1.08-3.31,P<0.05), CHF duration (RR=1.35, 95% CI 1.05-1.74,P<0.05), BNP level (RR=1.82, 95% CI 1.30-2.53,P<0.01), LAD (RR=1.51, 95% CI 1.09-2.08,P<0.05) were the risk factors for AF occurrence. Cox regression analysis indicated that BNP level (RR=1.30, 95% CI 1.09-1.54,P<0.01) and LAD (RR=1.74, 95% CI 1.30-2.34, P<0.01) were the predictors for AF recurrence. Conclusion: The elder age, longer CHF duration, increased BNP level and LAD were the risk factors for AF occurrence in CHF patients. The increased BNP level and LAD were the objective predictors for AF recurrence after cardio version.

Objective To evaluate the preoperative nutritional status of the patients which were preparing for cardiac valves replacement using nutrition risk screening 2002 (NRS2002),and analyze the evaluation effect and bring forward the corresponding nursing interventions. Methods 226 patients who would carry out cardiac valves replacement in the context of extracorporeal circulation were evaluated by NRS2002 preoperatively.88 patients had nutrition risk. Those elective 88 patients were randomly assigned into the experimental group and the control group, each had 44 cases. Then different nursing measures were implemented in each group and the clinical outcome was contrasted in the two groups. Results In the patients who were preparing for cardiac valves replacement,88 cases had preoperative nutrition risk score ≥3 points and 138 cases < 3 points.Preoperative nutrition risk score had positive correlation with heart function grade.Difference of the experimental group and the control group in length of stay, length of mechanical ventilation after surgery and incidence rate of general complications had statistical significance. Conclusions It is effective to analyze the preoperative nutritional status of the patients with cardiac valves replacement by NRS2002.It may improve prognosis effectively by providing preoperative nursing interventions for patients with nutrition risk.Besides,it provides theoretical basis for further studying the relation between nutrition support and clinical outcome of patients with nutrition risk.
