1.A comparison study on cholecystectomy in diabetic patients

Objective To compare the surgical modes of cholecystectomy in patients with diabetes mellitus. Methods The intraoperative blood loss, operation time, duration of hospital stay and incidence of complications were respectively compared in 46 patients with diabetes receiving either laparoscopic cholecystectomy (LC, n=23) or open cholecystectomy (OC, n=23). Results In the Group LC and the Group OC, the intraoperative blood loss was (80?23) ml and (120?40) ml, respectively ( t = -4.518 , P = 0.000), the operation time was (45?20) min and (69?28) min, respectively ( t =-3.345, P =0.002), and the hospital stay was (6?2) d and (11?3) d, respectively ( t =-6.651, P =0.000). There were no statistical significances between the two groups in the incidence of complications. Conclusions LC can be performed safely in patients with diabetes mellitus. It offers the advantages over OC in less blood loss, shorter operative time and shorter hospital stay.
2.Clinical effect and safety of atropine gel combined with anisodamine eye drops on the treatment of iridocyclitis

Objective To investigate the clinical effect and safety of atropine gel combined with anisodamine eye drops on the treatment of iridocyclitis.Methods A total of 174 cases with iridocyclitis in Zhejiang Hospital from May 2013 to July 2015 in this study were selected and divided into the control group and research group with 87 cases in each group.The patients in the control group were treated with tropicamide mydriatic,the patients in the research group were treated with atropine gel combined with anisodamine eye drops mydriatic.Serum levels of inflammatory factors, serum immunoglobulin and complement levels were observed and recorded before and after treatment,the clinical efficacy and incidence of adverse reactions were compared.Results The effective rate in the control group(85.06%) was lower than the research group(94.26%),the difference was statistically significant(P<0.05);compared with the control group,serum levels of TNF-α,IFN-γ,IL-23,CRP were lower in the research group after treatment, serum levels of IgG,IgE were lower,and serum levels of IgA,C3 were higher after treatment,the difference was statistically significant(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups.Conclusion The clinical effect of atropine gel combined with anisodamine eye drops in the treatment of iridocyclitis was exactly , can effectively reduce the inflammatory response,improve the immune status,and high security.
3.Screening for microdeletions of AZF region of Y chromosome in patients with spermatogenetic malfunction and its significance

Objective To investigate the condition of microdeletions of AZF region of the Y chromosome in Chinese patients with spermatogenetic malfunction. Methods Six Y chromosome specific sequence-tagged-sites(STS) in AZF region were screened and the microdeletions were determined by PCR in 27 patients with spermatogenetic malfunction. Results Microdeletions in genomic DNA were observed in 2 cases who presented with azoospermia and limited to AZFc subregion.The total deletion rate was 7.4%.The deletion patterns of the 2 cases were DAZ(sY254,sY255) and DAZ plus sY157,respectively. Conclusions Like other races, microdeletions of the Y chromosome in Chinese people may be one of the causes of spermatogenetic malfunction.Therefore prior to assistant reproduction,screening for microdeletions of the Y chromosome in patients with spermatogenetic malfunction is necessary.
4.Clinical effect of ultrasound-guided percutaneous transhepatic gallbladder drainage in treatment of acute cholecystitis

Objective To investigate the clinical effect of ultrasound-guided percutaneous transhepatic gallbladder drainage (PTGBD) in the treatment of acute cholecystitis.Methods A retrospective analysis was performed for the clinical data of 81 patients with acute cholecystitis who underwent ultrasound-guided PTGBD in General Hospital of Lanzhou Petrochemical Company from March 2013 to February 2016.The changes in related parameters after the placement of drainage tube were analyzed.The t-test was used for comparison of continuous data between groups.Results All the 81 patients underwent a successful one-time puncture.After the surgery,1 patient experienced tube dislodgement and 2 experienced obstructed bile drainage,while no patient experienced serious complications such as bleeding,bile leakage,and hemopneumothorax.Within 12 hours after surgery,there was a significant improvement in pain in the right upper quadrant and significant reductions in the major axis and radial width of the gallbladder (t =13.28 and 9.54,P =0.023 and 0.041),as well as significant reductions in white blood cell count and neutrophil count (t =8.70 and 8.03,P =0.028 and 0.034).Of all patients,38 achieved symptom remission after the placement of drainage tube and refused selective surgical treatment,and 43 underwent cholecystectomy within 3 months after surgery.Conclusion PTGBD is simple,convenient,visible,and safe and can effectively reduce patients' pain,increase the success rate of the treatment of acute cholecystitis,improve patients' clinical outcomes,and reduce the incidence of complications.It also provides the conditions of selective surgery for critically ill patients with acute cholecystitis,and therefore,it holds promise for clinical application.
5.Effect of sequoyitol on expression of NOX4 and eNOS induced with glucose in human umbilical vein endothelial cells.

OBJECTIVETo investigate the protective effect and mechanism of sequoyitol (Sep) on high glucose-induced human umbilical vein endothelial cells (HUVECs) injury.
METHODSHUVECs were cultured with high glucose (30 mmol/L) in the presence or absence of sequoyitol (0.1, 1 and 10 micromol/L) for 24 h. Cell proliferation was measured by BrdU marking and cell cycle was detected by flow cytometry. 2', 7'-dichlorofluorescein diacetate was used to evaluate intracellular reactive oxygen species (ROS) levels. The NO, malonydialdehyde (MDA) and H2O2 levels were determined by colorimetric method according to the manufacturer's instructions. The expression of endothelial nitric oxide synthase (eNOS) and NADPH oxidase 4 (NOX4) were measured by real-time PCR and Western blot.
RESULTSIn the present study, we found that sequoyitol pretreatment for 1 h significantly decreased cell injury, promoted cell proliferation. Meanwhile sequoyitol significantly down-regulated NOX4 expression and decreased the level of ROS, MDA and H2O2 and obviously increased NO levels and up-regulated eNOS expression.
CONCLUSIONSequoyitol alleviates high glucose-induced cell injuries in HUVECs via inhibiting oxidative stress and up-regulating eNOS expression.

6.Case-control study of abnormal glucose and lipids in obesity-related hypertensive patients

Objective To observe the changes of Lipids,FBS,ISI and lipoprotein lipase in obesity-related hypertensive patients and investigate the relationship of abnormal glucose and lipid with obesity-related hypertension as well as metabolic syndrome.Methods A total of 122 obesity-related hypertensive patients and 122 matched normal individuals were enrolled.Blood pressure,BMI,WHR and serum lipids and glucose,serum and adipose tissue LPL were measured.Results The TG,FBS and INS in obesity-related hypertensive patients were significantly higher than those in normal control group(P(0.05)).Correlated analysis showed that serum LPL mass was respectively correlated with BMI(r=-0.64,P
7.Effect of sequoyitol on expression of NOX4 and eNOS in aortas of type 2 diabetic rats.

The aim of the present study is to investigate the effects of sequoyitol (Seq) on expression of eNOS and NOX4 in aortas of type 2 diabetic rats. Type 2 diabetic rats induced by high fat and high sugar diet and low dose of streptozotocin (STZ, 35 mg x kg(-1)) and were administered Seq (12.5, 25 and 50 mg x kg(-1) x d(-1)) for 6 weeks. The fasting blood glucose (FBG) and body weight were tested. Acetylcholine (Ach) induced endothelium-dependent relaxation and sodium nitroprusside (SNP) induced endothelium-independent relaxation were measured in aortas for estimating endothelial function. Aortic morphological change was observed with HE staining. The level of serum insulin was measured by radioimmunoassay. The total antioxidative capacity (T-AOC), malondialdehyde (MDA) and NO levels in aortas were determined according to the manufacturer's instructions. In addition, the expressions of eNOS and NOX4 in aortas were measured by immunohistochemisty, real-time PCR or Western blotting. The results showed that Seq significantly decreased FBG and insulin resistance, and improved aortic endothelium-dependent vasorelaxation function. The expressions of NOX4 and MDA content were obviously decreased, while the expression of eNOS, the levels of NO and T-AOC increased significantly in aortas of diabetic rats with Seq treatment. In conclusion, Seq protects against aortic endothelial dysfunction of type 2 diabetic rats through down-regulating expression of NOX4 and up-regulating eNOS expression.

8.Comparison of two different types of laryngeal mask airway in gynecologic laparoscopic surgery

Objective To compare new type streamlined liner pharynx airway with standard type Ⅰ laryngeal mask airway in the clinical effectiveness in gynecologic laparoscopic surgery.Methods Seventy three cases undergone gynecologic laparoscopic surgery were divided into two groups randomly.The observation group(40 cases) and the control group(33 cases) were maintained by streamlined liner pharynx airway(SLIPA) and standard type Ⅰ laryngeal mask airway respectively.The success rate of placement,throat hemorrhage and pain before/after anesthesia and other indicators.were strictly observed and recorded during anesthesia.Results The success rate of placement was significantly higher in the observation group than in the control group(P<0.05).The incidence of throat hemorrhage and throat pain after surgery were significantly lower in the observation group(P<0.05).Conclusion The streamlined liner pharynx airway could be easyly and safely placed and causes less tissue injury and complications in gynecologic laparoscopic surgery.SLIPA was worthy of being recommended in clinical surgery.
9.Data mining-based analysis on rules of acupoints selection in RCT literature of acupuncture treatment of Bell palsy

Objective:To analyze the rules and characteristics of acupoints selection in acupuncture RCT literature on therapy of Bell palsy.Methods:Acupuncture Prescription Database was established through collecting RCT literature of acupuncture treatment of Bell palsy.The characteristics and rules of acupoints selection were investigated by means of data mining method.Results:After data mining-based analysis on acupuncture therapy of Bell palsy,we found that points on Yangming Meridians were frequently used,especially ST 4.Selection of local acupoints and distal points were important.Conclusion:According the means of data mining,the results revealed that acupoints selection along related meridian was the important principle for Bell palsy,and the special acupoints were the main part of prescriptions.
10.Development and application of special position mattress for endoscopic retrograde cholangiopan-creatography

Objective To design a kind of special support mattress for the endoscopic retrograde cholangiopancreatography (ERCP), and to improve the comfort of the patients during the procedure, and evaluate the efficacy and safety during clinical application. Methods By 3D modeling, we designed a special memory foam mattress for ERCP. From January 2014 to September 2014, 424 patients who underwent ERCP treatment without general anesthesia and could take semi-prone position, were divided into special mattress group (210 cases) and ordinary mattress group (214 cases) according to the random number table method. The satisfaction of the patients and the operators of the two groups were evaluated and compared. Results The operation times of the two groups were (32.9±18.5) min and (33.2±20.1) min, respectively, and there was no significant difference between the two groups (P>0.05). The overall satisfaction ratings of the patients in the two groups were (7.9±1.6) points and (6.3±1.3) points, respectively (P<0.01), and the discomfort ratings of the patients in the two groups were (1.3± 0.9) points and (3.8 ±1.2) points (t=116.45, 246.91, P<0.01). The operator satisfaction scores of two groups were (8.5±1.3) points and (8.5±1.8) points, respectively, and there was no significant difference (P>0.05). No mattress related complications were found. Conclusions The special memory foam mattress for ERCP can alleviate the discomfort and improve the tolerance during the procedure. Therefore, it is worth promoting the application.
