1.CT Analysis of 143 Cases of Cervical Intervertebral Disc Herniation

Objective To evaluate the value of CT in diagnosis of cervical intervertebral disk herniation. Methods The observation of CT appearances of 146 cases and the size, type, position of cervical intervertebral disk herniation were analyzed. Results ①Cervical intervertebral disk herniation was always multi-segments,often lolated at C4,5and C5,6. ②C5,6was mostly seen (142 cases,97.3%). ③Among centric and lateral type,centic type was mostly seen { 135 cases,92.5%). ④Of 135 cases of centric type,54 cases were mild type (< 2 mm) ,66 cases were moderate type (2 ~ 4 mm) , 15 cases were severe type (> 4 mm). Conclusion CT scan is convenient, efficient,accuracte and the first choice method in diagnosing cervical intervertebral disk herniation.
2.Effects of three stains on the expression of tumor necrosis fator-αlpha in carrageenan-induced synovitis in Rats

Objective To explore the anti-inflammatory mechanisms of three stains. Methods Airpouch induced acute inflammation rat model was developed by subcutaneous injection of carrageenan.Inflammatory invasion and tumor necrosis fator-alpha (TNF-α) expression levels of the inflammed skins were examined by immunohistochemical methods and digital image analysis. The anti-inflammatory action of the 3 different statins was compared. Results Stains could inhibit inflammatory invasion and the expression of TNF-α. Conclusion The statins have demonstrated strong inhibition of inflammation.
3.Trend analysis of antibiotic resistance in Neisseria gonorrhoeae in Xi'an region, 2002-2009

Objective To monitor the antibiotic resistance in Neisseria gonorrhoeae, determine the prevalence of penicillinase-producing Neisseria gonorrhoeae (PPNG) and plasmid-mediated tetracycline-resistant Neisseria gonorrhoeae (TRNG) in Xi'an region, and to analyse the trends in antibiotic resistance in Neisseria gonorrhoeae. Methods In total, 647 strains of Neisseria gonorrhoeae were isolated from patients with gonorrhea in sexually transmitted disease (STD) clinic settings from 2002 to 2009. Agar dilution method was used to detect TRNG and determine the minimal inhibitory concentration (MIC) of antibiotics, and paper acidometric method to detect PPNG. Results Of these 647 strains, 216 (33.4%) were TRNG, 290 (44.8%)were PPNG. The prevalence of TRNG strains remained between 28.3% and 49.2% in 2002-2009, except for 17.3% in 2005; the prevalence of PPNG strains increased from 37.1% in 2002 to 64% in 2005, but declined from 2006 to 2009 (32.3%). The prevalence of resistance to spectinomycin maintained at a low level (0 to 2.8%) over these years, while that to ciprofloxacin remained higher than 80% from 2002 to 2009, and accounted for 100% in 2005, with the exception of 51% in 2006. Ceftriaxone resistance was observed in none of these strains except 4 isolates in 2003, but the susceptibility to ceftriaxone decreased yearly. Conclusions Neisseria gonorrhoeae is highly sensitive to spectinomycin, which should serve as the first treatment choice for gonorrhea.Full dose is necessary for the application of ceftriaxone in the treatment of gonorrhea. Ciprofloxacin should not be used to treat gonorrhea.
4.The protective role of peroxisome proliferator activated receptors-α and pathomechanism in D-galactosamine/lipopolysaccharide-induced acute liver failure in rats

Objective To determine the role and mechanism of peroxisome proliferator activated receptors (PPAR) α in a mouse model of D-galactosamine/lipopolysaccharide (D-GalN/LPS)-induced acute liver failure(ALF).Methods Firstly,C57BL/6 mice were randomly divided into control group(n =8),ALF 2h group(n =8),ALF 4h group (n =8),ALF 6h group (n =8).Secondly C57BL/6 mice were randomly divided into control group(n =8),ALF group(n =8),WY14643 group(n =8).To induce ALF,the mice were injected intraperitoneally with D-GalN (700 mg/kg) and LPS (10 μg/kg).WY14643 (6 mg/kg),the selective agonist of PPAR α,was administered via tail vein two hours prior to D-GalN/LPS exposure.Two,four,and six hours after D-GalN/LPS treatment in the first study,mice were anesthetized and blood was collected,6h after D-GalN/LPS treatment in the second study,blood was collected.The liver tissue was harvested for histology and mRNA extraction.Serum levels of ALT and AST were measured to evaluate the hepatic damage.Inflammatory cytokines (TNFα,IL-1β,IL-6) and chemokines (CXCL-1,CXCL-10) were detected by real-time quantitative PCR.Differential protein expression of p-NF-κBp65,p-JNK,p-ERK,p-p38 in inflammatory pathways was detected by Western blotting.Significance of inter-group differences was assessed by one-way ANOVA,and pairwise comparison was performed by the least significant difference test.Results The gene and protein expression of PPAR α were gradually reduced during the development of ALF.Compared with the model group,the liver architecture was better preserved almost with normal morphology in WY14643-treated mice.Serum ALT and AST levels in WY14643-treated group were significantly lower [ALT:(555 ±62)U/L vs (2 898 ±822) U/L,P <0.05; AST:(791 ±58) U/L vs (3 013 ±997)U/L,P < 0.05].The expression of proinflammatory cytokines and chemokines was significantly suppressed during the activation of PPAR α.In the second study,the levels of gene expression of proinflammatory cytokines and chemokines were detected in control group,ALF group and WY14643 group respectively as followings:TNFα (0.161 ± 0.085,7.996 ± 1.068,3.346 ± 0.94,P < 0.05),IL-1β(0.041 ±0.002,3.657 ±0.904,0.176±0.089,P<0.01),IL-6 (0.018 ±0.008,1.762 ±0.589,0.163±0.0487,P <0.05),CXCL-1 (0.063 ±0.008,7.881 ±0.966,2.737 ±0.864,P <0.01),CXCL-10 (0.054 ±0.005,5.671 ±0.948,2.578 ±0.804,P <0.05).Conclusion Our findings first demonstrate that PPARα protects liver from injury in an ALF mouse model by suppressing inflammatory response,indicating PPARα as a potential new therapeutic target for ALF.
5.Development and application of special position mattress for endoscopic retrograde cholangiopan-creatography

Objective To design a kind of special support mattress for the endoscopic retrograde cholangiopancreatography (ERCP), and to improve the comfort of the patients during the procedure, and evaluate the efficacy and safety during clinical application. Methods By 3D modeling, we designed a special memory foam mattress for ERCP. From January 2014 to September 2014, 424 patients who underwent ERCP treatment without general anesthesia and could take semi-prone position, were divided into special mattress group (210 cases) and ordinary mattress group (214 cases) according to the random number table method. The satisfaction of the patients and the operators of the two groups were evaluated and compared. Results The operation times of the two groups were (32.9±18.5) min and (33.2±20.1) min, respectively, and there was no significant difference between the two groups (P>0.05). The overall satisfaction ratings of the patients in the two groups were (7.9±1.6) points and (6.3±1.3) points, respectively (P<0.01), and the discomfort ratings of the patients in the two groups were (1.3± 0.9) points and (3.8 ±1.2) points (t=116.45, 246.91, P<0.01). The operator satisfaction scores of two groups were (8.5±1.3) points and (8.5±1.8) points, respectively, and there was no significant difference (P>0.05). No mattress related complications were found. Conclusions The special memory foam mattress for ERCP can alleviate the discomfort and improve the tolerance during the procedure. Therefore, it is worth promoting the application.
6.Clinical significance of FoxP3 and the correlation of FoxP3 expression with epithelial-mesenchymal transition in breast cancer

Objective: This study aims to investigate the correlation between the expression of FoxP3, TGF-β1, and epitheli-al-mesenchymal transition (EMT) in breast cancer and to determine the clinical significance of FoxP3. Methods: The expression of FoxP3, TGF-β1, E-Cadherin, N-Cadherin, Vimentin, and Fibronectin protein were detected in the cancer cells of 74 cases with breast carcinoma via immunohistochemistry. The correlation of FoxP3 protein with clinico-pathologic features of breast carcinoma and the re-lationships among the expressions of FoxP3, TGF-β1, and epithelial-mesenchymal transition (EMT) were analyzed. Results:The ex-pression rates of FoxP3, TGF-β1, and EMT are 36.5%(27/74), 39.2%(29/74), and 40.5%(30/74), respectively. The FoxP3 protein ex-pression in breast cancer is correlated with lymph node metastasis (P<0.05) but not with other clinico-pathological features (P>0.05). The expression of FoxP3 is also correlated with the expression of TGF-β1. Furthermore, TGF-β1 can induce EMT (P<0.05). Conclu-sion:The expression of FoxP3 is correlated with lymph node metastasis and EMT in breast cancer. Therefore, FoxP3 may be a marker for predicting metastasis.
7.Expression of glucose transporter-1 and its correlation with basic fibroblast growth factor and proliferating cell nuclear antigen in epithelial ovarian neoplasm

Objective To study the expression of glucose transporter-1 (GLUT_1) and its correlation with basic fibroblast growth factor (bFGF) and proliferating cell nuclear antigen (PCNA) in epithelial ovarian neoplasm. Methods Streptavidin-peroxidase complex technique was used to examine the expression of GLUT_1, bFGF and PCNA protein in six cases of normal ovarian tissue, 20 cases of benign epithelial tumors, seven cases of borderline tumor and 44 cases of epithelial ovarian carcinoma. Results In normal ovary and benign ovarian tumor, GLUT_1 was not detected,but in borderline ovarian tumor and cancer, the positive expression ratio of GLUT_1 was 6/7 and 91%(40/44), respectively. The intensity of GLUT_1 in ovarian epithelial neoplasm was significantly higher than in borderline tumors. The staining intensity of GLUT_1 was significantly correlated with the histological grade of the tumor (r_S=0.499, P=0.001), and was positively correlated with the clinical stage, cancer invasion and lymph node metastasis. GLUT_1 staining was intense in cytoplasmic membrane, and was stronger in areas far away from blood vessels and near the necrotic center. GLUT_1 expression level did not show any association with histology type (P=0.513). bFGF positive rate in tumor was 57%(25/44). AlM: To evaluate the effect of rigid gas permeable contact lens ( RGP ) in correcting high myopia and astigmatism.METHODS: Forty-one patients ( 65 eyes ) with myopia (-9. 03 ± 6. 19DS, maximum -23. 00DS) and astigmatism (-1. 41 ± 1. 32DC, maximum -5. 50DC) were fitted with RGP after strict routine ophthalmological examination, objective refraction and subjective refraction. All these patients were followed after 1wk, 1, 3mo and half one year.RESULTS:Sixty-five eyes were fitted with RGP (-9. 92± 5. 96DS). RGP base curve ( BC) was majorly located within the range 7. 20 ~8. 25mm. 46. 2% eyes with RGP achieved 1. 0 ( BCVA ) and 80. 1% achieved above 0. 6 ( BCVA) . However, with spectacles, the percent was 28%(1. 0) and 60% (>0. 6), respectively. BCVA of RGP was 0. 81 ± 0. 22, but BCVA with spectacles was 0. 66 ± 0. 28, there was statistical significance (P<0. 01). There were 40 eyes (62%) fitted with RGP whose vision were enhanced more than 1 line, 24 eyes ( 37%) whose vision were not changed and 1 eye (2%) whose vision were dropped 1 line.CONCLUSlON:RGP effectively improves visual acuity of high myopia and astigmatism compared with spectacles.

10.Effects of Mycobacterium vaccae vaccine on immunological function in elderly patients with stable chronic obstructive pulmonary disease

Objective To investigate the effects of Mycobacterium vaccae vaccine on immunological function and clinical character in elderly patients with stable chronic obstructive pulmonary disease (COPD). MethodsA total of 100 elderly patients with stable COPD were randomly divided into immunotherapy group (group A, n= 50) and non-immunotherapy group (group B, n= 50), and normal control group (group C, n = 50). The levels of peripheral blood T-lymphocyte subsets (CD3+ , CD4+, CD8+ , CD4+/CD8+ ratio), natural killer cells (NK cells), immunoglobulins (IgG, IgA, IgM) and cytokines (IL-6, IL-8, TNF-a) were measured respectively before and after therapy. Group A and B were followed up for 1 year, then the times of acute outbreak and hospitalization of patients in the two groups were also compared. Results The levels of CD4 + ,CD4+/CD8+ ratio and NK cells in group A, B were significantly lower before therapy (P<0. 05~0. 01=, and the levels of IL-6, IL -8, TNF-a and IgA were significantly higher than in group C (P<0. 01=. After treatment with Mycobacterium vaccae vaccine in group A, the levels of CD4+ , CD4+/CD8+ ratio and NK cells were significantly higher (P<0. 05-0. 01= and IL-6, IL-8, TNF-a and IgA were significantly lower than before treatment (all P<0. 01=. These levels showed no significant changes in group B after treatment (P>0. 05). After 1-year follow-up, the times of acute outbreak and hospitalization on patients were statistically lower in group A than in group B (P< 0. 01 ).ConclusionsMycobacterium vaccae vaccine can improve cellular immunity function and reduce the times of acute outbreak and hospitalization in patients with stable COPD, so it has a higher clinical application value.
