Objective To study clinical manifestations, diagnosis, treatment, as well as effect of surgical operation, of primary mediastinal large B-cell lymphoma (PMBCL).Methods Data of 19 patients of PMBCL with complete clinical and pathological records hospitalized at Peking Union Medical College Hospital during 1998 to 2008 were retrospectively reviewed and analyzed.Kaplan-Meier method was used to estimate their survival time and survival rate in comparison of efficacy in those with surgical operation to those without it.Results Overall three-year survival rate was 54% for all the 19 patients, 55% for those with surgical operation and 54% without it, respectively, with an average survival time of 44 months, 44 months for those with surgical operation and 42 months without it, respectively, but not reaching at statistical significance.Symptoms of superior vena cava syndrome in five cases at admission relieved immediately after surgical operation, but symptoms of superior vena cava syndrome in three of nine cases in non-surgical group relieved gradually one to three weeks after chemotherapy.Conclusions Surgical operation has no significant effect on survival in patients with PMBCL, but it can relieve syndrome of superior vena cava syndrome and provide accurate pathological diagnosis.

OBJECTIVE:To study the bioequivalence of Alendronate Sodium Tablets in healthy volunteers. METHODS:According to a paired and cross-over design,a bioequivalence test was performed on two different alendronate sodium tablets,10 mg (Group 1) and 70 mg (Group 2). Then each group was subdivided into two groups,i.e. test preparation and reference preparation. The urine concentrations of alendronate at different time point within 48 h were determined by HPLC and the urine excretion rate -time curve was drawn,and the DAS software was used to compute the pharmacokinetic parameters and the relative bioavailability of the tested formulation. RESULTS:The 90% confidence intervals of urine excretion rate of the tested formulations were 82.4% ～122.6% (Group 1) and 85.4% ～124.6% (Group 2),respectively and the maximum excretory rates were 80.3% ～131.5% (Group 1) and 82.3～136.5% (Group 2),respectively,compared with the reference formulations. CONCLUSION:The two different Alendronate Sodium Tablets and their respective reference formulations were proved to be bioequivalent in healthy volunteers.

Objective To compare the surgical modes of cholecystectomy in patients with diabetes mellitus. Methods The intraoperative blood loss, operation time, duration of hospital stay and incidence of complications were respectively compared in 46 patients with diabetes receiving either laparoscopic cholecystectomy (LC, n=23) or open cholecystectomy (OC, n=23). Results In the Group LC and the Group OC, the intraoperative blood loss was (80?23) ml and (120?40) ml, respectively ( t = -4.518 , P = 0.000), the operation time was (45?20) min and (69?28) min, respectively ( t =-3.345, P =0.002), and the hospital stay was (6?2) d and (11?3) d, respectively ( t =-6.651, P =0.000). There were no statistical significances between the two groups in the incidence of complications. Conclusions LC can be performed safely in patients with diabetes mellitus. It offers the advantages over OC in less blood loss, shorter operative time and shorter hospital stay.

Objective To explore the effect of early enriched environment intervention on expression of neurofilament protein (NFP) in brain and the neurobehavior of filial rats with brain injury.Methods The pregnant Wistar rats of lipopolysaccharide (LPS) group were consecutively injected intraperitoneally with LPS on the 17th and 18th day of gestation while the control group only received an injection of same dose of 9 g/L saline.All premature rats,whose gestational age was less than 22 days,were removed from both groups;While the full-term newborn rats were chosen.After delivering,the uterus and placenta were taken out immediately to examine the infection situation by HE staining.Twenty-four hours after born,the brains of the newborn rats were taken out to observed the white matter damage by HE staining.LPS group was randomly divided into 2 groups:intervention group and non-intervention group.The intervention group was treated with neonatal handling and enriched environment from postnatal 8 days,while no management was performed in control group and non-intervention group.Ethological examination was tested with hanging test,and immunohistochemical technique was used to detect the expression of NFP,when the neonatal rats were 21 days old.Results There were a large amount of neutrophilic granulocyte in the uterus and placenta in LPS-treated group; In the 1-day-old rats in LPS group,brain tissue pathology test showed diffuse white matter lucencies.The scores of hanging test in control group was the highest,the ones in non-intervention group was the lowest among the 3 groups,and there was significant difference between them (Pa

ObjectiveTo study the effect of facilitation antagonism acu-therapy in early rehabilitation of patients with cerebral thrombosis.Methods60 patients with cerebral thrombosis were divided randomly into two groups, Group A received facilitation antagonism acu-therapy, Group B received traditional acu-therapy. Fugl-meyer(FMA), MBI, etc were applied to assess effect before and after treatment.ResultsBoth groups had a great improvement after treatment(P<0.01).The effect in Group A was better than that in Group B compared in two groups (P<0.01 or P<0.05).ConclusionThere is effective in early rehabilitation of patients with cerebral thrombosis using facilitation antagonism acu-therapy.It is better than traditional acu-therapy.

Objective To investigate the influence of tissue-specific growth hormone receptor (GHR)deficiency in type 1 diabetes in the mice at the gene level using pancreaticβcells combined with streptozotocin (STZ)-induced type 1 diabetes model.Methods The experiment was divided into four groups:knockout mice group (LLc knockout group), using the homozygotes (LLc:LL+Cre) producted by pancreaticβ cell-specific expressed recombinant enzyme mice (RIP-Cre)and Cre-LoxP system modified GHR mice (Floxed,LL);LL control group, containing Floxed GHR allele homozygous mice (LL);LLc STZ group and LL STZ group (STZ was used for inducing type 1 diabetes model mice). The mice with feeding glucose≥25 mmol · L-1 were considered to be successful models.The Glucose Tolerance Test (GTT),pancreas tissue HE staining and immunohistochemistry were performed in the mice.Results The blood glucose of the mice in LL STZ group and LLc STZ group and LLc STZ group were increased after inj ection of STZ and the models achieved the diagnostic criteria for diabetes 1 6 d later.The results of GTT showed that compared with LLc control group and LLc knockout group, the blood glucose levels of the mice in LL STZ and LLc STZ groups were increased (P<0.05).There was no significant change of morphology and structure of islets between LL control group and LLc knockout group detected by HE staining. The immunohistochemistry results showed that the insulin level of the mice in LL STZ group was significantly reduced compared with LL control group;the insulin level of the mice in LLc STZ group was reduced compared with LLc control group.Conclusion Pancreaticβcell GHR gene knockout has no effect on the blood glucose and the function ofβcells in the mice with STZ-induced type 1 diabetes.

Objective To observe the efficacy and side effects of combined therapy with victoria A acid,arsenic trioxide and chemotherapeutics for newly diagnosed acute promyelocytic leukemia (APL). Methods 40 cases of patients were treated with combined therapy of ATRA, As2O3 and chemotherapy. The efficacy and side effects were analysed. Results 92.5 % (37/40) achieved complete remission(CR). The CR time was 27(22-61) d by average; leukocytes≥ 10×109/L group complete remission was 72.7 % (8/11), leukocytes < 10×l09/L group complete remission was 100 % (29/29). There was a significant different (x2 =8.550, P =0.004). No severe side effects was observed except one who developed retinoic acid syndrom. Conclusion The combined regimen of ATRA and As2O3 with chemotherapy for acute promyelocytic leukemia should be the first choice management.

Objective To investigate the effects of music therapy in treating cognitive dysfunction in early stroke patients. Methods Forty patients who had suffered a stroke in the previous four weeks were randomly di-vided into two groups of 20. One received music therapy as part of their cognitive training ( observation group) , and the other did not (control group). Before and after 6 weeks of treatment, cognitive functioning was evaluated using the second Chinese edition of Loewenstein's occupational therapy cognitive assessment (LOTCA). The patients' a-bility in the activities of daily living was also evaluated using the modified Barthel index (MBI). Results After treatment, the total LOTCA and MBI scores in both groups were significantly higher than before. Both LOTCA and MBI scores in the observation group were significantly higher than in the control group, and there were also signifi-cant differences between the observation and control groups on all of the LOTCA sub-items except spatial percep-tion. Conclusions Music therapy can improve the effects of rehabilitation on cognition and the ADL ability of early stroke patients.

Objective To explore a novel quantitative detection method for the concentration of specific IgG (sIgG) in food intolerance, taking egg sIgG detection for the example. Methods A total of 173 patients underwent food allergen sIgG detection were included in this study, and 78 healthy subjects were used as negative controls. The microtiter plates were coated with biotinylated bovine serum albumin (BSA) and linked with streptavidin. Then, the biotinylated egg antigen and specimen were successively added into the wells of plate.After washing, enzyme labeled anti-human IgG was added to establish an antigen indirectly coated liquid-phase reaction patterns of ELISA method. The concentration of the biotinylated allergens and enzyme labeled antibody were optimized and the reaction conditions were determined. This method was used to detect the sIgG in serum samples. Results The biotinylated egg white was selected as antigen, the optimal dilution rate was 1∶2 000,and the most suitable enzyme labeled antibody dilution ratio was 1∶12 000 in this method. The within-run and the between-run coefficients of variation were 4.83%-8.55%and 4.88%-7.93%respectively. The specificity is preferable, the species-crossed reaction rates with crab, cow milk and goat milk were＜10%. There was a good correlation between the assay developed in this study and the food intolerance detection kit provided by United States BIOMERICA Inc ( =0.977X+8.45, r=0.961, P＜0.05). Conclusion The detection method can be used to detect serum sIgG for egg intolerance patients with easy operation, highly accuracy and specificity. Furthermore, it showed the capacity of excellent repeatability and flexibility potential, which provided a good foundation for developing a kind of“personalized”random combination of food varieties ELISA kit.
