1.Tuberculosis in Selected Drug Abuse Treatment and Rehabilitation Centers (DATRCs) in Luzon, Philippines

Background and Objectives:
Tuberculosis (TB) continues to be a public health concern in the Philippines. Vulnerable populations in congregate settings such as drug abuse treatment and rehabilitation centers (DATRCs) have higher risks of TB transmission and infection. With the Duterte administration’s intensified campaign against illegal drugs, government-retained DATRCs are filled to capacity. There is an identified need to profile drug users and dependents living in DATRCs. Furthermore, national guidelines for TB management specific to this population is absent. A study was conducted to determine the profile of admitted clients diagnosed with TB, TB prevalence, treatment outcomes and choice of TB diagnostic modalities in six (6) Luzon-based DATRCs from 2013-2015.
Methods:
All medical records of drug users admitted in this period were reviewed. Information on the total patient census was sought from DATRC heads and used to compute for prevalence.
Results:
A total of 347 records were obtained. Overall TB prevalence in the study sites was 7,216 per 100,000 population. The typical individual diagnosed with TB in a DATRC was male, with a mean age of 35 years, with at least high school education, and unemployed. Weight loss and fatigue were the most common symptoms reported. The majority (79.83%) completed TB treatment in the DATRCs. Sputum microscopy and chest x-ray were both used for TB diagnosis in 92.80% (322) of the cases.
Conclusion
The high burden of TB in Luzon-based, government-retained DATRCs is alarming and underscore the critical need for standards and guidelines in the National Tuberculosis Control Program addressing the unique context of these facilities.

Background and Objectives: Tuberculosis (TB) remains a public health problem in the Philippines despite trends indicating a decline in the burden of disease. Persons who use drugs who are confined in government-retained drug abuse treatment and rehabilitation centers (DATRCs) face an increased risk for TB because of the congestion in the facility coupled with the absence of guidelines on TB management specific to DATRCs. Thus, this study was conducted to document the current case finding and case holding practices as well as TB treatment pathway in six (6) Luzon-based, government-retained DATRCs.
Methods: Key informant interviews were conducted with DATRC personnel involved in TB diagnosis and management in six selected DATRCs in Luzon, Philippines. Interviews were transcribed and coded for thematic analysis. We compared the DATRC practices with the provisions of the 2014 National TB Control Program (NTP) Manual of Procedures. Results were validated through a workshop with (a) a group of physicians and rehabilitation practitioners assigned in other DATRCs; and (b) a group of experts who have experience in managing or overseeing DATRCs in the country.
Results: Two physicians and four nurses participated in the interviews. Variations in case finding and case holding practices in six DATRCs have been found. National guidelines exist for congregate settings but are more specific to jails/prisons, which are administratively and operationally different from DATRCs.
Conclusion: Development of special guidelines for TB case finding and case holding in DATRCs as well as staff training on the latest NTP Manual of Procedures are recommended. However, gaps and inequities posed by the current set-up underscore the need to address health system-wide factors affecting the practice and performance of these facilities.

3.Study on the distribution of hepatitis C virus genotypes in patients visiting one methadone maintenance clinic in Wuhan.

OBJECTIVETo study the prevalence of hepatitis C virus (HCV) infection and characteristics on molecular biology related to HCV among patients who were enrolled in a Methadone maintenance clinic in Wuhan.
METHODSSerum samples from 332 injection drug users (IDUs) were obtained and anti-HCV IgG was detected by enzyme linked immunosorbrent assay(ELISA), together with 86 anti-HCV positive specimens genotyped. A reverse transcriptase-polymerase chain reaction (RT-nPCR) assay using conserved primers deduced from the core-envelopel (C-E1) region of the HCV genome was employed to amplify a 474 bp fragment. Phylogenetic analysis of the C-E1 sequences was conducted by direct sequencing of the RT-nPCR products and alignment with determined by nucleotide sequencing followed by composition of a phylogenetic tree.
RESULTSThere were 313 cases (94.3%) appeared positive anti-HCV IgG in the 332 patients from a Methadone maintenance clinic in Wuhan. It was demonstrated that there were four different subtypes of HCV in that clinic in Wuhan, including 6a--71 cases (82.5%), 3b--7 cases (8.2%), 1a--5 cases (5.8%) and 1b--3 cases (3.5%).
CONCLUSIONInfection of 6a genotype HCV was predominant in patients from the Methadone maintenance clinic in Wuhan, followed by HCV 3b, 1a and 1b.

4.Factors associated with retention in a community-based methadone maintenance treatment among drug users in Urumqi, Xinjiang Uigur Autonomous Region.

OBJECTIVETo explore the factors associated with retention in a community-based methadone maintenance treatment(MMT) among drug users in Urumqi.
METHODSWith national MMT guideline( drift) for heroine addicted drug users, local heroine-dependent people were admitted to community-based MMT program affiliated to Xinjiang Uigur Autonomous Region Center for Mental Health. Data on outpatients' social-demo characteristics, baseline behaviors on drug use and daily stabilized dose of drugs were entered to MMT database.
RESULTSUp to 10, Feb. , 2006,353 persons withdrew MMT treatment among 709 heroin dependant drug users. Between 11, Aug., 2005 and 10, Feb., 2006, with median duration as 77 days, cumulative drop-out rate of 90 days and 180 days after first dose of MMT were 73.8% among 455 and 99.4% among 355 drug users, respectively. The incidence of drop-outs was 29.8 per 100 person-month. The median length of stay (days) in MMT was 68.0 (95% CI: 59.0- 78.0). Correlates of retention were found as: Being Uigur(HR = 1.35;95% CI :1.09-1.67), duration of drug use (HR =0.74; 95% CI:0.55-0.99) and stabilized dose(HR = 0.60;95% CI: 0.48-0.74) was found in multiple Cox proportional hazard regression model.
CONCLUSIONRetention of MMT among drug users in Urumqi was low. Uigur people should be given individual counseling to help them increase the compliance rate. Within the ranges of clinic dosage, adjustment of the methadone dose on an individual base might serve as an appropriate approach to increase the effectiveness of the program.

5.Evaluation of first 8 pilot methadone maintenance treatment clinics in China.

OBJECTIVETo evaluate the effectiveness of the first eight pilot methadone maintenance treatment (MMT) clinics in China.
METHODSA questionnaire survey of the clients at the 8 pilot MMT clinics was performed at entry, 6 month and 12 month follow-up. Drug using behaviors, drug related crime behaviors, and relationships in families were compared among at entry, 6 and 12 months follow-up.
RESULTSThere were 585, 609 and 468 clients involved at baseline, 6 month and 12 month follow-up surveys, respectively. At entry, 6 month and 12 month follow-up, the proportion of clients whose injection of drugs reduced from 69.1% to 8.9% and 8.8%, and the frequency of injection in the past month had reduced from 90 times per month to 2 times per month. Employment rate increased from 22.9% to 43.2% and 40.6%, and self-reported criminal behaviors reduced from 20.7% to 3.6% and 3.8%. At 12 month follow-up, 65.8% of clients reported a healthy family relationship, increased from 46.8% at entry, 95.9% of clients reported that they were satisfied with the MMT service.
CONCLUSIONPilot MMT program reduced drug use, drug injecting behaviors, drug related criminal behaviors, and improved relationship with family members. Therefore, MMT clinic should be considered as a platform for providing comprehensive services to drug users.

6.Herpes simplex virus-2 infection and related factors among female drug abusers in the women's compulsory drug rehabilitation center of Shandong province.

Objective: This study aimed to understand the herpes simplex virus-2 (HSV-2) infection and related factors among female drug abusers in the women's compulsory drug rehabilitation center of Shandong province and to provide reference for the prevention and control of HSV-2 in these settings. Methods: We screened all of 451 female drug abusers in the women's compulsory drug rehabilitation centers in of Shandong province and conducted a study using both questionnaire investigation and serological tests for HSV-2, HIV and syphilis. We also used EpiData 3.1 software to establish a database and SPSS 20.0 software to conduct the χ(2) test and multivariate logistic regression analysis. Results: A total of 451 female drug abusers were under study. We noticed that the rates for HSV-2 infection, HIV infection and syphilis infection appeared as 72.1% (325/451), 2.2% (10/451) and 33.5% (151/451) respectively. Results from univariate analysis showed that factors as: awareness on AIDS, having temporary sex partner after using the drug, having multiple sex partners after using the drug, providing commercial services or having temporary sex practice before being detained, with syphilis infection etc., were associated with HSV-2 infection. Data from the multivariate analysis showed that the OR (95%CI) value of HSV-2 infection was 2.90 (1.19-7.06) for those who providing commercial service, when comparing to those who did not. Compared to those who did not suffer from syphilis infection, the OR (95%CI) value of HSV-2 infection for those with syphilis infection was 2.75 (1.63-4.63). Conclusions: The rate of HSV-2 infection was high in the women's compulsory drug rehabilitation center of Shandong province. We should enhance measures and promote condom use to prevent from HSV-2 and other sexually transmitted diseases among them.

7.Development of Medical Rehabilitation System for Persons with Disabilities.

Recently the numbers of persons with disabilities and aged are rapidly increasing every year. And the scope of persons with disabilities is much more diversified. According to the recent report of the desire for the persons with disabilities, it is noted that medical rehabilitation service is most insufficient. But compared to the numbers and needs of persons with disabilities, the numbers of medical rehabilitation facilities are still much insufficient. The important factors to be considered in rehabilitation hospital are accessibility, comprehensiveness, quality, continuity and efficiency. In Korea, the current conditions of medical rehabilitation service are as follows: 1) increase of medical rehabilitation demand, 2) shortage of rehabilitation hospital beds, 3) insufficient medical rehabilitation insurance system, 4) absence of continuum care, 5) accessibility problem etc. Considering these aspects, the government is planning to build the regional rehabilitation center each district and meet the need for persons with disabilities by the second five-year plan of the welfare for persons with disabilities. Finally I suggest some a plans to establish medical service provision system for the disabled. First of all, regional rehabilitation center should be established in each province, so as to provide better and comprehensive medical services to the disabled. Second, medical services in public and private health sector should be expanded. Third, the need for the community rehabilitation committee that links the hospital and the community should be emphasized.

8.Family Function and Self-esteem among Young and Middle-aged Adults with Physical Disabilities.

PURPOSE: This study examined family function and self-esteem among young and middle-aged adults with physical disabilities. METHODS: The subjects of this study consisted of 116 adults with physical disabilities. Data were collected from 4 Community rehabilitation centers in C area from October to November 2008. All subjects were questioned about family function and self-esteem by the APGAR scale and the Rosenberg Self-esteem Scale. SPSS/WIN 12.0 was used for descriptive analysis, t-test, one-way ANOVA, and Pearson correlation. RESULTS: Subjects had a moderate level of family function (5.31) and self-esteem (28.13). There were significant differences of family function according to age, monthly income, economic status, and disability site. There were significant differences in self-esteem according to age, gender, monthly income, and economic status. The level of family function and income were related to self-esteem. CONCLUSION: These findings suggest that there is a necessity to improve family function and self-esteem among young and middle-aged adults with physical disabilities. Nursing interventions should take into account the role of family function in promoting self-esteem in people with physical disabilities.

9.A Study on the Pathological Syndromes and the Insights of the Patients in the Community Mental Health Centers.

PURPOSE: This study is a descriptive research in which the pathological syndromes of the schizophrenic patients in the community mental health centers, the degrees of the insights, and the correlations between them were examined. METHOD: The subjects included 114 SPR pt. people who were enrolled in community mental health centers located in D City. The research was conducted during the period from Aug. 1st, 2002 to Jul. 30th, 2004. The instruments were PANSS developed by Kay(1987) and SUMD developed by Amador(1993). RESULTS: Among the pathological syndromes, positives ones were 12.64+/-3.80 on the average, negative ones were 32.00+/- 5.36, and general pathological syndromes were 30.50+/-6.24. In the evaluation of the insights, the mean score was 11.81+/-5.09. There were some significant correlations between positive syndromes and general pathological ones (r=.572, p=.000), and negative syndromes and general pathological ones(r=.262, p=.029), while there was no significant correlation between the insights and the sub-measures of general pathological syndromes. CONCLUSION: Therefore, the schizophrenic patients taking advantage of the community mental health centers have more negative syndromes than those in hospitalized, and rehabilitation programs are needed to help them continuously. And the further study of the correlations between the pathological syndromes and the degrees of the insights are required, and still, it should be analyzed what effects the insight acquirement brings about to the improvements of the pathological syndromes after the application of the insight-oriented programs.

The purpose of this study is to develop the standards of care in rehabilitation nurse specialist. This study was a descriptive survey. The data were collected 143 nurses who were worked in rehabilitation unit at 4 university hospital and 3 rehabilitation center in Korea from Aug. to Nov. 2000. The questionnaire was consisted of 78 items developed by the standards of care in rehabilitation nurse specialist in A.R.N. and the practice contents of care at rehabilitation unit in Korea. Collected data were analyzed by frequency, percentage, mean, S.D. ANOVA The results are as follows 1. For the adequency of the practice contents of rehabilitation nursing, area of skin care is the highest score, and safety, elimination, emotion, respiration, was ordered. 2. The adequency of the practice contents of rehabilitation nursing according to age, educational level. position. clinical experience, clinical experience in rehabilitation unit were significant difference.
