Rectal syphilis, known as a great masquerader, can be difficult to diagnose because of its variable symptoms. Gastroenterologists should be aware of the possibility of rectal syphilis when confronted with anorectal ulcers, and should gather a detailed history about sexual preferences and practices, including homosexuality. We report a case of primary rectal syphilis mimicking rectal cancer on radiologic imaging. In this report, we described the clinical, endoscopic, and radiologic features of this rare case.

A 30-yr-old man was referred for suspicious rectal cancer because of ulcerated lesions in the rectum and a palpable mass in left inguinal area. Sigmoidoscopy showed two indurated masses and histologic evaluation of biopsy revealed obliterative endarteritis with heavy plasma cell infiltration. Both venereal disease research laboratories (VDRL) and fluorescent treponemal antibody absorption (FTA-ABS) tests were positive. After injection of penicillin G benzathine for 3 weeks, the rectal chancre and the palpable mass disappeared.

We report three cases of solitary rectal ulcer syndrome (SRUS). The diagnosis was established according to histopathologic criteria. But, the initial clinical diagnosis was carcinoma, non-specific ulcer and localized proctitis respectively. SRUS is considered as one of functional disorder in pelvic floor which might go underdiagnosed due to unfamiliar concept in Korea. So we should consider SRUS to be one of the differential diagnosis in cases of complaining anorectal symptoms.

In medical practice, enemas are known to incidentally bring about colitis by mechanical, thermal, or direct chemical injuries. Coffee enema is told to ameliorate the constipation in alternative medicine. We hereby report a case of acute colitis resulting from coffee enema, which was presented with severe abdominal pain and hematochezia.

Although the chronic prostatitis is the most common infectious disease in the urinary tract in male, it is confusing us to diagnose and treat thoroughly. It is usually diagnosed by careful history, urinalysis, rectal palpation, and wet smear of prostatic secretions. But sometimes these findings are not identical to the true conditions of patients. 53 patients suspected of chronic prostatitis were studied by cultures of prostatic secretion and semen. And they compared to each other. The results were as follows: 1. Semen cultures, in 10 cases of the control group, showed positive findings in two cases. One was staphylococcus epidermis and the other was Gram(+) bacillococcus, but their colony counts were less than 100. 2. The positive findings on prostatic secretion culture were 37 cases (69. 8%) and on semen culture were 28 cases(52. 9%) 3. The etiologic organisms of chronic prostatitis by semen and prostatic secretion cultures revealed staphylococcus epidermis, staphylococcus aureus, streptococcus, Gram(-) diplococcus and Gram(+) bacillococcus in order of frequency. 4. Pathologic findings on prostatic biopsy according to different etiologic organisms revealed nothing remarkable except nonspecific chronic inflammation in each group. 5. Although there is no evidence the semen culture is superior to the prostatic secretion culture for diagnosis of the chronic prostatitis, it is more useful in the patient who may have an acute exacervation of chronic prostatitis, the patient whose prostatic juice can't be obtained by massage, and the patient who can't endure pain because of rectal pathology such as anal fissure or severe hemorrhoids.

OBJECTIVETo study the expression of arginase-1 (Arg-1), glypican-3 (GPC3), hepatocyte paraffin antigen 1 (HepPar-1) and alpha-fetoprotein (AFP) in hepatocellular carcinoma (HCC), benign liver lesions (BLL) and metastatic carcinoma (MC), and their applications in diagnosis and differential diagnosis.

METHODSImmunohistochemical study (EnVision method) for Arg-1, GPC3, HepPar-1 and AFP was carried out in three groups of liver lesions, including 85 cases of HCC, 35 cases of BLL and 19 cases of MC. The relationship between expression of Arg-1, GPC3, HepPar-1 and AFP and clinicopathologic features in HCC was also analyzed.

RESULTSThe positive expression rate of Arg-1 was 90.6% (79/85) in HCC and 100% (35/35) in BLL. Arg-1 expression was observed in 1 of the 19 cases of MC studied. The positive expression rate of GPC3 was 82.4% (70/85) in HCC, 5.3% (1/19) in MC and 0 (0/35) in BLL. The positive expression rate of AFP was 47.1% (40/85) in HCC and 0 in BLL or MC. The positive expression rate of HepPar-1 was 72.9% (62/85) in HCC, 100% (35/35) in BLL and 2/19 in MC. Arg-1 has a higher sensitivity in highlighting hepatocellular lesions than AFP and HepPar-1 (P=0.000 versus P=0.002). The specificity of GPC3 expression in HCC was 98.1%.

CONCLUSIONSArg-1 is a sensitive hepatocellular marker in delineation of liver lesions.GPC3 is a relatively specific marker in diagnosis of HCC.
