1.Myocardial Nutrition Study after Myocardial Revascu-Larization by Laser.

The myocardial nutrition after myocardial revascu-larization was studied in 14 rabbits with the aid of Nd-YAG laser, before Ligation of the anterior descending branch of the left coronary artery (only ligated LAD in 4 rabbit), the rabbits were seperatively Killed 3 or 5 weekafter operating. Myocardial samples were abtained in the parti of 0 - 2,2 - 4,4 - 6mm from laser hole. Sample numbers were 30 in myocardial revascularization group (MRG); 20 in myocardial Infarction group(MIG); 20 in normal group (NG). Amount of ATP and CP in myocar-dia were determined with luciferase. Resalts.ATP and CP near laser holes were normal 3 week late. This far from laser holes was lower than that of normal myocardia 5 week late. Recover of CP amount was slower than that of ATP amount. Inclusion, myocardial revascularization can play role in myocardial nutrition.
2.Clinical significance of acanthosis nigricans in children and adolescents with obesity induced metabolic complications.

PURPOSE: This study investigated the clinical significance of AN in children and adolescents with obesity induced metabolic complications. METHODS: Forty-nine patients who had obesity induced metabolic complications were participated in this cross-sectional study. Obesity induced metabolic complications are as follows: hypertension, dyslipidemia, impaired fasting glucose (IFG), impaired glucose tolerance (IGT), nonalcoholic steatohepatitis (NASH), homeostasis model assessment of insulin resistance (HOMA-IR)>3.16. Clinical characteristics, such as, age, percentage-weight-for-height (PWH), pubertal status, blood pressure (BP), fasting plasma insulin level, fasting and post-oral glucose tolerance test 2-hour glucose levels, liver function test, lipid profile, HOMA-IR were compared according to the presence of AN. RESULTS: Sixty-five percent of patients had AN, 57.1% NASH, 57.1% dyslipidemia, 55.1% hypertension, 46.9% IFG, 24.5% HOMA-IR>3.16 and 16.2% IGT. The patients who were moderately to severely obese with AN had higher incidence of IGT and HOMA-IR>3.16. The patients with AN had significantly higher diastolic BP (79.4+/-6.9 vs 75.4+/-5.6 mmHg), fasting levels of plasma insulin (10.6+/-6.0 vs 6.2+/-5.4 microIU/mL), HOMA-IR index (2.6+/-1.4 vs 1.4+/-1.3) and PWH (42.4+/-13.0 vs 34.3+/-1.8%). The increasing tendency for the presence of AN was significantly related to the cumulative number of obesity induced metabolic complications. Binary logistic regression analysis revealed that the presence of AN was significantly associated with fasting plasma insulin level, PWH and IFG. CONCLUSION: AN could be useful as a clinical surrogate of obesity induced metabolic complications.

3.Efficacy and safety analysis of combination use of Amiodarone and Wenxin granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmia

Objective To investigate the efficacy of Amiodarone combined with Wenxin granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmia. Methods A total of 80 patients with unstable angina pectoris complicated with ventricular arrhythmias from February 2014 to May 2016 were randomly divided into the observation group and the control group, with 40 patients in each group. Two groups were treated with conventional dilatation vascular anticoagulation and antiplatelet treatment, on the basis of routine treatment, the observation group was given Amiodarone combined with Wenxin granule, and the control group was given Amiodarone plus conventional treatment, the effects of the treatment in two groups were compared. Results After a course of treatment, the average times within 24 hours of myocardial ischemia and angina occurred of patients in the control group were less than the control group, the cumulative dosage of nitroglycerin is less than the control group (P<0.05), at the same time, the control group during the follow-up period, the incidence of ventricular fibrillation and ventricular rate was lower than the control group. The incidence of adverse reactions was also lower than that of the control group. Conclusion Compared with the control group and the control group, the efficacy and safety of Amiodarone combined with Wenxin granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmias are better than Amiodarone alone. After combination treatment, the times of partial ventricular arrhythmias and angina pectoris in patients were reduced with less adverse reactions and better safety, compared with single use of Amiodarone.
4.Vaginal Delivery in Hirschsprung's disease complicating pregnancy.

Bowel obstruction is a rare complication of pregnancy. The usual causes include previous abdominal surgery, volvulus, intussusception, colonic neoplasm, or the enlarging uterus. Bowel obstruction secondary to uncorrected Hirschsprung's disease as a complication of pregnancy is difficult to diagnosis, its occurrence can have grave implications for both mother and fetus, and anticipation of dystocia. Hirschsprung's disease is diagnosed and treated in the neonatal period. Persistence of Hirschsprung's disease into adulthood is very rare and confirmed by rectal biopsy providing the absence of the ganglion cell in Auerbach and Meissner's plexus. We experienced vaginal Delivery in Hirschsprung's disease complicating pregnancy and report our own case study with a brief literature review.

5.Pineal Germinoma with Extracranial Metastases: Case Report.

Pineal germinoma is the commonest pineal region tumor of childhood and adolescence. Metastatic germinoma most commonly occurs via the cerebrospinal fluid (CSF), and it is usually limited to the cerebrospinal axis. Extacranial hematogenous metastasis is known to be very rare. We report here on a case of pineal germinoma with gradual extracranial metastases that occurred both through the CSF pathway and by hematogenous spread. The patient had multifocal CSF seeding after his surgery for pineal germinoma, and the left iliac metastasis and lung metastasis then occurred.

Carcinosarcoma is a rare malignant neoplasm consisting of both epithelial and nonepithelial components, that is, carcinoma and sarcoma in the same tumor. Since Queckenstedt reported the case of carcinosarcoma of the stomach in 1904, only 30 cases have been reported. We present a case of carcinosarcoma and adenocarcinoma of the stomach in 63-year-old man with the review of the literature.

7.Brain Magnetic Resonance Angiography in Children with Chronic Headache.

PURPOSE: Headaches occur frequently in the pediatric population and have a significant impact on their quality of life. Several mechanisms are currently thought to contribute to headache pathogenesis. Our aim was to investigate the association of chronic headache in children and cerebral vascular anomaly by performing brain magnetic resonance angiography (MRA). METHODS: We retrospectively reviewed medical records and MRA of 44 patients with chronic headache who visited the pediatric clinic of Young Dong Severance hospital from January 2006 to April 2008. RESULTS: The number of enrolled patients was 44. Eight(18.2%) of them showed abnormal brain MRA findings. Abnormal findings included 5 cases of unilateral A1 hypoplasia, 1 case of unilateral vertebral artery hypoplasia, 1 case of unilateral A1 and P1 hypoplasia, 1 case of Moyamoya disease. Among the 44 patients, 21 had complained only headache and 23 had complained other neurological symptoms with headache. The ratio of abnormal MRA between the two groups showed no statistical difference. CONCLUSION: Vascular anomaly could be a possible pathogenesis and risk factor in chronic headache in children. Further investigation and long term follow up is needed.

8.Blood Vessels of the Peri-Implant Mucosa: a Comparison Between Flap and Flapless Procedures.

BACKGROUND: The vascularity of the peri-implant tissue is a very important parameter in establishment and maintenance of a healthy tissue after dental implant insertion. OBJECTIVE: The purpose of this study was to compare the vascularity of the peri-implant mucosa between flap and flapless implant surgeries by using a canine mandible model. STUDY DESIGN: In six mongrel dogs, bilateral, edentulated, flat alveolar ridges were created in the mandible. After 3 months of healing, two implants were placed in each side by either the flap or flapless procedures. After another healing period of 3 months, biopsies were obtained, prepared for light microscopy and exposed to morphometric measurements. RESULTS: The supracrestal connective tissue lateral to the implant was found to be more richly vascularized in the flapless group than in the flap group. CONCLUSION: These results suggest that the flapless procedure may have an effect on increasing the vascularity of the peri-implant mucosa.

9.Clinical manifestations and neuroimaging findings of schizencephaly in children.

PURPOSE: Schizencephaly is a uncommon congenital brain anomaly characterized by congenital clefts spanning the cerebral hemispheres from pial surface to lateral ventricles and lined by gray matter. In this study, we investigated the clinical manifestation and radiologic findings of pediatric schizencephaly. METHODS: The data of 13 patients who were diagnosed with schizencephaly in Severance Childrens Hospital and Yongdong Severance Hospital from January 2005 to December 2007 were analyzed retrospectively. RESULTS: The mean age at diagnosis was 9.08+/-2.67 months old and ranged from 1 to 30 months. The ratio of male to female patients was 3.33:1. Five (38.5%) patients had bilateral clefts, while 8 (61.5%) had unilateral clefts. Five (38.5%) patients had closed lip clefts, and 4 (30.8%) had opened lip clefts. Four (30.8%) patients had multiple clefts. Associated anomalies showed in all cases. The clinical features consisted of mild unilateral weakness in 7 (53.8%) cases and a hemiparesis was present in 3 (23.1%) patients. A tetraparesis was in 3 (23.1%) patients. There was no difference in motor deficit between unilateral and bilateral clefts. Delayed development was observed in all cases. Epilepsy was present in 7 (53.8%) patients, 5 patients with unilateral clefts and 2 patients with bilateral clefts. Three (42.8%) patients showed intractable seizures. CONCLUSION: Schizencephaly showed variable clinical manifestations and radiologic findings in association with the types and locations of the clefts. It is necessary to diagnose schizencephaly early and to detect the development of epilepsy. Intensive and large studies of the correlation of clinical outcomes and radiologic findings should be continued for more effective treatment.

10.A case of Wernicke-Korsakoff syndrome associated with severe hyperemesis gravidarum.

Hypermesis gravidarum is a complication defined as vomiting severe enough to require hospital admission during early pregnancy. Thiamine deficiency is known to lead to certain neurological seguelae including Wernicke-Korsakoff syndrome. Wernicke's encephalopathy is an illness of acute onset characterized by global confusion, paralysis of eye ball movements, and gate ataxia due to a deficiency of thiamine. The immediate administration of thiamine prevents progression of the disease and reverses brain lesions that have not yet progressed to the point of fixed structural change. We have experienced a case of Wernicke-Korsakoff syndrome associated with hyperemesis gravidarum, which seemed to be developed by prolonged thiamine-free fluid therapy. We emphasize the need for thiamine supplementation in hyperemesis gravidarum patients.
