1.The result of endoscopic vein harvesting in patients undergoing coronary artery bypass grafting

Objective To evaluate clinical results of endoscopic vein harvesting(EVH) in patients who underwent coronary artery bypass grafting(CABG).Methods From July 2013 to October 2014,500 patients who underwent isolate CABG in our institution were divided into two groups:EVH group (n =250) and open vein harvesting(OVH) group(n =250).The surgical result and perioperative complications were compared between the two groups.Results The procedures were completed successfully in all the patients in both groups.There was no significant difference existed in the length of the harvested vein and vein injury between the two groups(P > 0.05).Similar results were obtained in the in-hospital mortality.Compared to group OVH,group EVH showed significantly shorter harvesting time,smaller incision,and lower rates of perioperative leg complications (P < 0.01).Group EVH had a less bed staying time after operation and a shorter post-operation hospital stay (P < 0.05).EVH group achieved an improved patient satisfaction and postoperative pain when compared with OVH in CABG(P <0.05).Conclusion EVH can provide adequate vein graft for CABG,and also significantly reduce wound related complications when compared with OVH.EVH is recommended for vein harvesting to improve patient satisfaction.
2.Application of high quality care in clinical nursing and its effect evaluation

Objective To explore the advantages and disadvantages of high quality care in clinical care. Methods 2000 cases of hospitalized patients from August 2010 to August 2011 in department of neurology were selected as research subject.The 2000 hospitalized patients were divided into the control group and the research group with 1000 cases in each group.The control group was given routine care,the research group selected high quality care.General activity of daily living (ADL) assessment and satisfaction degree of patients was surveyed at discharge for two groups and the results were compared. Results When patients were discharged,ADL scores and satisfaction degree of patients in the research group were higher than those of the control group. Conclusions Application of high quality care for patients can promote the improvement of activity of daily living and satisfaction degree of patients.
3.Cross-sectional survey of adolescent sexual psychology and knowledge

BACKGROUND: The population of male and female adolescents in China accounts for 1/5. Adolescents have much psychological bewilderment,troublesome and even diseases on sexual issues.OBJECTIVE: To investigate the development status quo and the regulation of adolescents' sexual physiology, sexual psychology, sexual knowledge and sexual education.DESIGN: A cross-sectional survey by employing students of junior high school as subjects.SETTING: A clinic for adolescent of a tertiary hospital.PARTICIPANTS: Totally 960 students aged between 14 and 19 years old of three junior high schools in Dalian city were selected as subjects between November and December of 2001.METHODS: Self-complied questionnaire was used in the survey based on the sexual psychological education situation scale for junior and senior high school students. Classes were randomly selected for anonymous survey based on voluntary principle.Answers to sex-related questions and differences between male and female students.RESULTS: The average age of menarche in female students was 12.96years old and the average age of first emission in male students was 13.89years old, which were consistent with the results of similar national researches. The incidence and frequency of masturbation and sexual impulse was significantly higher in males than females. There were significant differences in sexual concept and the understanding of sexual knowledge between males and females. The desire to affiliate with the opposite sex was far more than the real occurrence.CONCLUSION: The understanding of sexual knowledge in adolescent is not satisfied. Students are also not satisfied with the sexual education provided by their schools. Hence, school sexual education for adolescents should be enhanced with the involvement of community.
4.Comparison between nuclear matrix protein 22 (NMP 22) test and voided urine cytology for detection of bladder cancer

Objective To compare the sensitivity and specificity of NMP 22 test with voided urine cytology in detection of bladder cancer and to evaluate their clinical values. Methods For 155 patients suspected with bladder cancer NMP 22 and cytology were conducted in the same voided urine samples.Of them 95 patients with bladder transitional cell carcinoma were confirmed histologically.The sensitivity and specificity of NMP 22 and urine cytology were analyzed.60 patients without bladder cancer were selected as control. Results The overall sensitivity and specificity of NMP 22 test were 65.3% and 70.0%,respectively,those of urine cytology were 43.2% and 83.3%,respectively.There was no significant difference between the specificity of urine cytology and that of NMP 22 test;however,NMP 22 was significantly more sensitive than urine cytology in detection of any stages and grades of bladder cancer(P

6.Percutaneous Fixation of Distal Tibia Comminuted Fracture by Using Normal Anatomical Plate

Objective To explore efficacy of minimally invasive percutaneous plate osteosynthesis (MIPPO) for the treatment of distal tibia comminuted fracture by using normal anatomical plate. Methods Between January 2007 and July 2008,18 cases of distal tibia facture were treated by MIPPO using anatomical plate. The clinical data of the patients were reviewed. Results A mean of 6.5 cm incision (5.0-8.5 cm) was made in the patients; the intraoperative blood loss ranged from 60 to 300 ml (mean,145 ml);and the operation time ranged from 30 to 120 min with a mean of 63 min. After the surgery,2 patients developed mild skin necrosis,and was then cured by conventional therapy; no patient had nonunion of the fracture,failure of internal fixation,or delayed wound healing. The 18 cases were followed up for a mean of 8 months (range,4 to 10 months); all of them were healed clinically and could walk without crutch in 4 months postoperatively. According to Johner-Wruhs score system,11 were excellent and 7 were good; the excellent-good rate was 100%. Conclusions MIPPO with anatomical plate is an optimal treatment for distal tibia comminuted fracture with advantages in protecting the soft issues and bony blood supply,promoting the wound-healing process,and reducing the rate of complications.
7.The neurological manifestations of chronic mercury poisoning

ObjectiveTo analyze the neurological manifestations of chronic mercury poisoning for the improvement of clinical understanding of this disease.MethodsThe clinical data of 8 cases diagnosed as chronic mercury poisoning admitted in the department of neurology of Xuanwu hospital during the past 5 years were collected and analysed.Results Neurological manifestations of chronic mercury poisoning involved psychological problems,sleep disorders,ataxia,extremities weakness and atrophy,tremor,peripheral neuropathy and paresthesia.ConclusionsThere are various kinds of neurological manifestations of chronic mercury poisoning.Identification of these manifestations in clinical practice and early diagnosis are beneficial for the relief of pains in the patients.

9.Prevalence of diarrhea caused by Cryptosporidium parvum in non-HIV patients in Jeollanam-do, Korea.

The present study investigated the prevalence rate of Cryptosporidium parvum as a cause of diarrhea. We examined 942 stools of unidentified reasons occurring in patients in whom no immunosuppression had been detected. We examined the stools for Cryptosporidium parvum via modified acid-fast staining. The clinical records of all of the positive patients were then analyzed. Nine (1%) of the stools among the 942 diarrheal patients were positive for C. parvum. The positive rate in the males was 1.1% (6/522) and the positive rate of the females was 0.7% (3/420). Age distribution revealed that the highest positive rates were in patients in their sixties, with a positive rate of 2.5% (4/158). In the clinical tests, levels of c-reactive protein, erythrocyte sedimentation rates, and neutrophil proportions were normally increased in the peripheral blood, whereas the lymphocyte proportion exhibited a tendency towards decrease. The pathological findings were compatible with an inflammatory reaction in the host.

10.Inhibitory effects of midazolam on amygdala kindling in rats and maximal electroshock seizure in mice

BACKGROUND: Typically antiepileptic drugs, such as phenobarbital, fenitoina sodica and diazepam, can inhibit amygdala kindling effect in rats. However, whether midazolam has the same effect is still unclear.OBJECTIVE: To investigate the inhibitory effect of midazolam on amygdala kindling onset in rats and maximal electroshock seizure (MES) in mice and effeots of antiepileptic drugs.DESIGN: This study was divided into three subexperiments, including the effects of midazolam on amygdala kindling onset, independent activities and incidence convulsion. All the three subexperiments were completely randomized,infra-group control or self-control studies.SETTING: Medical College of Qingdao University.MATERIALS: The experiment was carried out in the Comprehensive Experimental Room, Affiliated Hospital of Medical College of Qingdao University from August 2004 to March 2005. Nine Wistar rats weighing (250±10) g and 120 Kunming mice weighing (20±5) g and either gender were provided by Animal Center of Qingdao Institute of Drug Control.Midazolam (5 g/L) was provided by Xuzhou Enhua Drugs Co., Ltd. (batch number: 20030706).METHODS: ① Establishment of amygdala kindling models: Nine kindled rats were randomly selected and intraperitoneally injected with 0.25, 0.5 and 1 mg/kg midazolam, respectively. Quadri-pathway biological signal processing system (SMUP-PC) was used to measure discharge duration (ADD) and Racine's stage. ② Sixty mice were randomly divided into 5 groups, including saline group, 40 mg/kg phenobarbital group, 0.5, 1.0 and 1.5 mg/kg midazolam groups with 12 mice in each group. And then, numbers of activities in a unit time (times per 5 minutes) were determined by XZC-4A mini-animals independent activity instrument. ③ MES models were established to calculate incidence of convulsion.MATN OUTCOME MEASURES: Effects of midazolam on ADD, Racine's stage, numbers of independent activities and incidence of convulsion.RESULTS: All the 9 rats and the 120 mice were involved in the final analysis. ① Effect of midazolam on amygdala kindling onset: After intraperitoneal injection of 0.5 and 1.0 mg/kg midazolam, ADD and Racine's stage were obviously lower than those before administration (P < 0.05-0.01). After intraperitoneal injection of 0.25 mg/kg midazolam, ADD was obviously lower than that before administration (P < 0.05), but there was no significant difference in Racine's stage. ②Effect of midazolam on independent activities of mice: Numbers of independent activities were lower in the phenobarbital group and 0.5, 1.0 and 1.5 mg/kg midazolam groups than those in the saline group (P < 0.01), while numbers of independent activities were higher in 0.5 mg/kg midazolam group than those in the phenobarbital group (P < 0.05). ③Effect of midazolam on maximal electroshock seizure: Incidence of convulsion was lower in the phenobarbital group and 0.5, 1.0 and 1.5 mg/kg midazolam groups than that in the saline group (P < 0.05-0.01), while Incidence of convulsion was higher in 0.5 mg/kg midazolam group than that in the phenobarbital group (P< 0.05).CONCLUSION: Midazolam can significantly inhibit amygdala kindling onset, reduce numbers of independent activities,and antagonist MES in mice.
