1.Progress in treatment of lower limb atherosclerosis occlusion

Surgery and endovascular techniques are the most important methods in treating lower limb atherosclerosis occlusion.Different strategies should be adopted according to the features of lesion in lower limb.This article reviews the latest clinical outcomes in treating lower limb atheroselerosis occlusion with surgery and endovascular techniques.
2.Application of high quality care in clinical nursing and its effect evaluation

Objective To explore the advantages and disadvantages of high quality care in clinical care. Methods 2000 cases of hospitalized patients from August 2010 to August 2011 in department of neurology were selected as research subject.The 2000 hospitalized patients were divided into the control group and the research group with 1000 cases in each group.The control group was given routine care,the research group selected high quality care.General activity of daily living (ADL) assessment and satisfaction degree of patients was surveyed at discharge for two groups and the results were compared. Results When patients were discharged,ADL scores and satisfaction degree of patients in the research group were higher than those of the control group. Conclusions Application of high quality care for patients can promote the improvement of activity of daily living and satisfaction degree of patients.
3.Infection status of pigs with Cryptosporidium parvum.

To investigate the infection status of pigs with Cryptosporidium parvum, 589 fecal samples were collected from pigs raised at farm in Chungcheongbuk-do and Chungcheongnam-do. Of the 589 pig fecal samples, 62 (10.5%) were positive for C. parvum. The area showing the highest positive rate was Dangjin-gun, Chungcheongnam-do (14.0%), and the lowest (0%) Salmi-myon, Chungcheongbuk-do. The positive rate of C. parvum in Judok-eup increased from 12.7% in the winter to 22.1% in the summer. The results of this study suggest that the pigs may be a source of human C. parvum infection.

4.Study on effect of astragalus polysaccharide on peripheral circulation MDSC in lung cancer and its clinical effect

Objective To investigate the effect of Astragalus Polysaccharide on peripheral circulation myeloid derived suppressor cells (MDSC) in lung cancer and its clinical effect. Methods 100 patients with advanced non small cell lung cancer were selected and divided into 2 groups, 50 cases in the control group treated with chemotherapy aalone, 50 cases in the experiment group received injection of Astragalus Polysaccharide on the basis of the control group, peripheral blood MDSC levels, peripheral blood T lymphocyte subsets levels, serum ARG I activity, TNF-α, IL-6 levels, the clinical effect and incidence of adverse reactions were compared after the treatment. Results Compared with the control group, peripheral blood levels of MDSC (Lin-HLA DR-\CD33+\CD11b+) was lower after treatment in the experiment group , peripheral blood levels of CD3+, CD4+T lymphocytes and CD4+/CD8+ were higher, and CD8+T lymphocytes level was lower after treatment, serum levels of ARG I activity, TNF-α, IL-6 levels were lower after treatment, (P<0.05) , the total effective rate in the control group(42.0%)was lower than the experiment group(64.0%), (P<0.05), the incidence of gastrointestinal reaction(40/24), 3 ~ 4 degree of leukocyte reduction(26/12), liver function damage(19/9), renal damage(17/7) in the control group were higher than the experiment group (P<0.05). Conclusion Astragalus Polysaccharide can significantly reduce the peripheral circulation MDSC (Lin-HLA DR-\CD33+\CD11b+) level in patients with lung cancer, improve T lymphocyte immune function, inhibit the activity of serum ARG I, reduce the level of TNF-αand IL-6, effectively improve the clinical efficacy, and reduce the toxic and side effects of chemotherapy.

6.Factors influencing postoperative prognosis in superficial femoral artery occlusive disease

Objective The purpose of this study is to determine whether the TransAtlantic InterSociety Consensus (TASC) criteria (Ⅱ-2007 versions), the Society of Vascular Surgery (SVS) runoff score or risk factors for peripheral arterial disease were correlated with postoperative outcome of superficial femoral artery occlusive disease. Methods From January 2006 to September 2009, patients who suffered from lower extremity atherosclerosis occlusion disease and underwent endovascular or surgical therapy in superficial femoral artery segment were reviewed retrospectively at Beijing Tongren Hospital. Femoralpopliteal artery lesions were graded according to the TASC Ⅱ criteria. Runoff scores were determined in infrapopliteal artery segment lesions. All patients were followed up. Kaplan-Meier method was applied to calculate primary patency rate, and COX regression analysis was used to determine if TASC Ⅱ classification,runoff score, or factors for peripheral arterial disease affected primary patency rate. Results 142 patients (197 limbs) were followed up after treatment at 1 month, 3 months, 6 months, and every 6 months thereafter. Median follow-up time was 13 months. By Cox regression analysis, TASC Ⅱ classification(RR =1.471,P = 0. 012 ), runoff score ( RR = 1.190, P = 0. 004 ), and type 2 diabetic mellitus ( RR = 2. 320, P =0.019) significantly affected primary patency. Conclusions Postoperative poor patency rates are associated with higher degree of the TASC Ⅱ lesions, poor initial runoff score, and type 2 diabetic mellitus in patients of superfical femoral artery occlusive disease.
7.Investigation of multiple fractures around the knee in adults

[Objective]To investigate the morbidity,classification and treatment of multiple fracture around the knee in adults,and to analyze the significance of AO classification in the treatment.[Method]Multiple fractures in 167 patients from 2003 to 2007 were retrospectively analyzed and grouped according to their ages,occurring years,mechanism of injury and treatment.AO/OTA classification was assessed from the preoperative X-ray result,and healing situation was evaluated according to postoperative X-ray.[Result]All 167 multiple fractures around the knee accounted 23.69% for all lower limb fractures,with 89.2% of male and 10.8 of female.For the patient's age analysis,30-40 years old age group accounted for the largest proportion (29.34%).For the injury mechanism,traffic accident accounted for the largest proportion (53.29%).Distal femur + proximal tibia fracture accounted for 44.3%.patella + distal femur fracture accounted for 31.1%.Proximal tibia + patella accounted for 19.8%.Distal femur + proximal tibia + patella accounted for 4.8%.According to AO classification,A3 fracture was the most common in distal femoral fracture group (25.4%).B1 fracture was the most common in proximal tibia fracture group (21.7).C3 fracture was the most common in patella fracture (31.2).Tthe final good-to-excellent rate was 85.63%.[Conclusion]Distal femoral fracture combined with proximal tibia fracture was the most common in multiple fractures around the knee.The common fracture type is supracondylar fracture in femur,transverse patella and tibia.Application of nail,plate,wire and tension band,et al.can achieve good fixation.
8.A case of Diphyllobothrium latum infection.

Human diphyllobolthriasis is chiefly caused by infection with Diphyllobothrium latum(D. latum) through eating raw or unproperly cooked fish such as salmon, trout or perch. D. latum is worldwidely distributed, but not a common parasite of humans, and its prevalence in Korea is not high. The first worm-proven case of D. latum infection was first reported in 1971 and about 30 cases have been reported in Korea until now. However, with the increasing popularity of eating raw salmon, trout and perch, this tapeworm infection tends to increase in Korea. Most of the patients infected by D. latum have not showed the specific symptoms and only non-specific gastrointestinal symptoms(anorexia, nausea, vomiting, diarrhea, abdominal pain). We report a case which includes a chain whitish, threadlike tapeworm segments spontaneously discharged in the patients stool. On reviewing the literature, the present case is the first report of D. latum infection by eating cultured salmon. The patient was treated with 600mg single dose of Praziquantel.

10.Effects of intermittent change the angle of operating table on the skin in pressure areas of patients in prone position

Objective:To evaluate the effects of intermittent change the angle of operating table on the skin in pressure areas of patients in prone position.Methods:A total of 150 hospitalized patients for elective surgical treatment in Yantai Yuhuangding Hospital from January 2019 to June 2019 were selected as subjects. The patients were divided into control group, 15° test group and 30° test group by random number table method, with 50 patients in each group. The control group received routine intraoperative pressure ulcer prevention measures, the 15° test group on the control group, the itinerant nurses adjusted the angle of the operating table according to the left 15°-supine-right 15° every half hour, and the 30° test group on the control group, the itinerant nurses adjusted the angle of the operating table according to the left 30°-supine-right 30° every half hour. The incidence of surgically related pressure ulcers and doctors' satisfaction with the surgical position of the three groups of patients were evaluated.Results:Pressure ulcers occurred in 8 patients of the control group, the incidence of pressure ulcers was 16%. Pressure ulcer occurred in 0 patient of the 15° test group, the incidence of pressure ulcers was 0. Pressure ulcers occurred in 2 cases in the 30° test group, the incidence of pressure ulcers was 4%. The difference was statistically significant ( χ2 value was 10.120, P<0.05). Further comparison showed that the incidence of pressure ulcers was statistically significant between the control group and the 15° test group ( P<0.016 7), while the incidence of pressure ulcers was not statistically significant between the control group and the 30° test group, between the 15° test group and the 30° test group ( P>0.016 7). The results of the postoperative doctors' satisfaction with the surgical body position were (27.880 ± 1.637), (27.520 ± 1.693), (26.920 ± 1.947) points in the control group, the 15° test group and the 30° test group, there was a statistically significant difference among the three groups ( F value was 3.779, P<0.05). Further comparison showed that there was no statistically significant difference in the score of surgical position satisfaction between the control group and the 15° test group, between the 15° test group and the 30° test group ( P>0.05). The satisfaction score of surgeons in the control group was higher than that in the 30° test group, and the difference was statistically significant( P<0.05). Conclusions:Intermittent adjustment of surgical position can reduce the incidence of pressure ulcers and improve the quality of nursing in the operating room without affecting the convenience of the surgeon. The optimal effect was achieved by adjusting the angle of the operating bed according to the left-leaning 15°-supine-right-leaning 15°at an interval of 30 minutes.
