1.Observation of the curative effect of irinotecan HCL trihydrate combination with cisplatin in treatment of non-small cell lung cancer

Objective To explore the clinical curative effect of irinotecan HCL trihydrate combination with cisplatin in treatment of non.small-cell carcinoma(NSCC)by observing the clinical curative effect,adverse reactions and the improvement of the quality of life post therapy.Methods 120 patients with NSCC were divided into two groups.60 cases in control group were treated with cisplatin and topotecan while ifinotecan HCL tfihydrate combination with cisplatin chemotherapy for 60 case of patients in observation group.The curative effect and Kamtfsky point were observed.Results The effective rate of observation group was 56.7%,which was significantly higher than that of the control group(41.7%),and differences between the two groupswas statistically significant(P<0.05).Average survival time for treatment group was(9.3±4.2)months that was longer than that of the control group[(7.2±3.2)months](P<0.05).The result of Kamofsky classification standard after chemotherapy also indicated the same situation(P<0.05).Conclusion Irinotecan HCL trihydrate combining with cisplatin in treatment of non-small cell lung cancer had a effective therapy result and could reduce the toxicity.as well as improving the quality of life of the patients.

3.The analysis ofβ-lactam antibiotics interference in fructosamine testing

Objective To evaluate the interference of 13 kinds of β‐lactam antibiotics in fructosamine detection .Methods A se‐ries of solution of antimicrobial drugs were formulated with water for injection ,and added into the mixture fresh serum volume by 5% .Interfere with antimicrobial drugs on the test results of fructosamine were evaluated .Results When the blood concentrations of cefathiamidine and ticarcillin sodium/clavulanate potassium were up to 37 .5 mg/L and 247 .5 μg/mL respectively ,they began to produce positive interference on fructosamine detection .However ,the degree of interference was not correlated to the serum concen‐tration of fructosamine .Conclusion Cephathiamidine and ticarcillin sodium/clavulanate potassium have positive interference on fructosamine detection .
4.Analysis of BMI,waistline,glucose and lipid metabolism status of high -normal blood pressure that involved Uygur,Kazak and Han nationalities in some areas of Xinjiang

Objective To explore body mass index(BMI),glucose and lipid metabolism status of high -nor-mal blood pressure that selected Uygur,Kazak and Han nationalities in parts of Xinjiang.Methods Some representa-tive regions were selected,including Fuhai County of Altay,Xinyuan County of Yili,Yanqi County of Korla and Pasto-ral of Urumqi South Mountain in August 2012 -2014 August,each region was randomly selected two survey points respectively,400 -450 people were expected to conduct on -site investigations in every survey point,and 1 046 cases were collected containing Uygur,Kazak and Han nationalities who were conformed to diagnostic standard,206 cases in which the Kazak (115 men and 91 women),Han 518 cases (277 men and 241 women),322 cases of Uygur (174 men and 148 women).in order to realize the BMI,waistline,lipid metabolism and fasting glucose status.Results (1)BMI was descending:Kazak >Uygur >Han,between the three groups respectively were statistically significant differences[(26.07 ±3.23)kg/m2 ,(25.85 ±4.09)kg/m2 ,(24.63 ±3.00)kg/m2 ,F =19.829,P <0.05],sub-group analyzes:BMI for men descending:Kazak >Uygur >Han,comparison between groups of Han,Uygur and Kazak provided results which were statistically significant(F =6.752,P <0.05);but statistical significance merely existed in Han and Kazak female comparison(F =2.248,P <0.05).Waist comparison showed a decreasing sequence:Uygur>Kazak >Han,there were no statistically significant difference between every three groups[(86.32 ±8.63)cm, (85.50 ±10.11)cm,(84.06 ±7.57)cm,F =1.675,P >0.05],gender comparison was found the same characteristics. (2)Blood fat comparison between different ethnic:Descending order of serum TG:Kazak >Han >Uygur,comparison among groups of Han,Uygur and Kazak differences were statistically significant(F =5.829,P <0.05),comparison among groups of Han,Uygur and Kazak differences were statistically significant(F =5.829,P <0.05).Comparison of Serum TC:Han >Uygur >Kazak.Statistical significance indwelled in Han,Uygur and Kazak comparison (F =5.298, P <0.05);The result of comparing serum HDL -C,LDL -C in different ethnic groups:there was no statistically significant difference between different group (F =1.026,0.744,all P >0.05).Descending order was presenting in fasting glucose:Han >Uygur >Kazak.Statistical significance appears in Han,Uygur and Kazak comparison (F =8.989,P <0.05).Conclusion High -normal blood pressure of Uygur,Kazak and Han nationalities who appeared glucose metabolic disorders,BMI and waistline increase(overweight and obesity),combined with the characteristics of different national regional life,it is of great significance to prevent and delay the occurrence of hypertension through strengthening the management and intervention of high -normal blood pressure crowd,giving medical care to the people suffering from overweight,obesity,and glucose metabolic disorders,and making full preparation for the primary of hypertension.

6.Technology Optimization of Ultrasonic-enzyme-assisted Extraction of Sanguinarine from Zanthoxylum nitidum

Objective To optimize the technology conditions of ultrasonic-enzyme-assisted extraction for sanguinarine from Zanthoxylum nitidum.Methods Extraction rate of sanguinarine determined by HPLC was served as an index.The applicability of the extraction solvent added with acid and enzymatic hydrolysis pretreatment to the ultrasonic-enzyme-assisted extraction of Zanthoxylum nitidum was investigated.Ultrasonic power,extraction frequency and solvent volume were optimized by orthogonal experiment.Finally,ultrasonic extraction time was optimized in dynamic process.Results The optimal process was as follows:Zanthoxylum nitidum powder was extracted 3 times by ultrasonic-wave (250 W) with 40% ethanol (0.2%hydrochloride) as solvent (extracted for 15 rmin with 6-fold solvent at the first time,then extracted for 12 min with 3-fold solvent at the second and the third time,respectively).The extraction rate of sanguinarine was 88.6%.Conclusion The process is economic,efficient,energy-and time-saving,and provides experimental basis for industrial production of sanguinarine.
7.Patterns,effects and direction of the reform for public hospitals' internal operating mechanism in pilot cities

A summary of the reform for public hospitals'internal operating mechanism in 17 pilot cities identified its four main patterns,the internal decision-making machanism,human resource management mechanism,compensation allocation system,and business mechanism.It also analyzed the initial outcomes and problems encountered in the reform.On this basis,the authors proposed the direction for the reform pattern in designing the internal operating mechanism for reference of a wholescale reform of public hospitals in China.
8.Role of GABAA receptors in uninjured dorsal root ganglion in a rat model of neuropathic pain

Objective To evaluate the role of GABAA receptors in uninjured dorsal root ganglion(L4DRG)in a rat model of neuropathic pain.Methods Thirty adult female SD rats weighing 200-250 g were randomly divided into 3 groups(n = 10 each): control group(group C),muscimol group(group M)and bicuculline group(group B).Neuropathic pain was produced by L5 spinal nerve ligation.Normal saline 50 μl,GABAA receptor agonist-muscimol 50 μl or GABAA receptor antagonist- bicucullin 50 μl was injected into the L4 DRG.The thermal pain threshold and mechanical pain threshold were measured and recorded from 1 day before operation to 10 days after operation.Results Compared with group C,the mechanical pain threshold was significantly increased(P < 0.05),while no significant difference was found in thermal pain threshold in group M(P > 0.05),and the thermal pain threshold and mechanical pain threshold were significantly decreased in group B(P < 0.05).Conclusion Activation of GABAA receptors in uninjured DRG is involved in mechanical hyperalgesia in a rat model of neuropathic pain,but it dose not play a leading role.
9.Change in GABA receptor-activated current in dorsal root ganglion neurons freshly isolated from rats with neuropathic pain

Objective To investigate the change in GABA receptor-activated current in dorsal root ganglion (DRG) neurons in rats with neuropathic pain. Methods Twenty adult SD rats of both sexes weighing 100-150 g were randomly divided into 2 gorups: sham operation group (group S, n = 5) and neuropathic pain group (group NP, n= 15). Neuropathic pain was induced by ligation of right L5 spinal nerve. The animals were sacrificed at 5 days after operation. The L5 DRG( neurons in group NP and L3-5 DRG neurons in group S were immediately isolated. Whole-cellpatch- clamp technique was used. The extracellular solution contained GABA 100μmol/L.The frequency and amplitude of the GABA-activated current in DRG neurons and the changes in action potential (threshold potential, rheobase and overshoot) and resting potential before and after GABA administration were recorded. Results GABA 100μmol/L induced rapid inactivation of inward current in most neurons. Compared with the baseline before application of GABA, in group S GABA induced depolarization,increased resting potential and decreased amplitude and rheobase of action potential in large and medium DRG neurons, while in group NP GABA increased resting potential but induced no significant change in threshold potential and rheobase and overshoot of action potential. The frequency and amplitude of GABA-activated current and the degree of change in resting potential and rheobase and overshoot of action potential were significantly lower in group NP than in group S.Spontaneous discharge occurred in small DRG neurons in both groups. No GABA-activated current was observed in all DRG neurons with spontaneous discharge. Conclusions Neuropathic pain is induced by decreasing GABA-mediated inhibition signals in large and medium DRG neurons leading to increased excitability of neurons.
10.The application of Montreal classification in inflammatory bowel disease

Objective To investigate the clinical features of Crohn's disease(CD)and ulcerative colitis(UC)according to the Montreal classification.nethods The clinical data of 110 cases of CD or UC were reviewed.The age at diagnosis.location and clinical behavior were assessed with the Montreal criteria.Results CD patients diagnosed at an age younger than 16 years were rare(3.6%),the majority of the CD patients was diagnosed at 17-40 years old(65.5%).Although ileocolon lesions were most common in the patients diagnosed at 17-40 years old(37.3%),yet ileum lesions were a little more than those of other parts of digestive tract in the patients diagnosed after 40 years old(14.5%),the difference was not significant(P=0.054).Stricture frequently occurred(50.4%),especially when the lesions were located at ileum or ileocolon.Perforation rarely happened(5.3%).There was no significant difference between different location groups for clinical behaviors(P=0.096).The incidence of stricture or perforation was almost same among different age groups(P=0.984).UC patients mostly presented with mild or moderate symptoms even in the group with extensive lesion.UC patients with severe symptoms were rare(6.8%).There was no significant difference in severity between the groups with different extent of lesion (P=0.056).Conclusions The majority of CD patients was diagnosed at 17-40 years old.Stricture is much more than perforation,penetrating,occurring mostly at ileum.UC patients mostly present mild or moderate symptoms,no matter how extensive the lesion is.The extent of the lesion detected with coloscopic examination is not correlated to the severity of clinical manifestations.
