4.Comparison of Patient-Sitter Ward Nurses and General Ward Nurses on Work-Related Musculoskeletal Symptoms, Occupational Stress and Nursing Work Environments.

PURPOSE: This study aimed to compare work-related musculoskeletal symptoms, occupational stress and nursing work environments of nurses working in patient-sitter wards and general wards. METHODS: The study surveyed 240 nurses with more than one year of experience working in both patient-sitter wards and general wards. The collected data then was analyzed by SPSS statistics version 22. RESULTS: As for the musculoskeletal symptoms, the survey showed that 85.2% and 67.8% of the nurses had such symptoms respectively in patient-sitter wards and general wards. In terms of occupational stress, no significant difference was observed between the patient-sitter ward and the general ward (t=-0.23, p=.821). Lastly, the study showed that there is a significant difference in terms of work environment considering the scores recorded 2.65 and 2.55 points respectively in patient-sitter ward and general ward (t=2.53, p=.012). CONCLUSION: Follow-up research should look at ways to lower the rate of experiencing work-related musculoskeletal symptoms; analyze work performances and establish work standards to lower occupational stress; and devise measures to improve the work environment for the nurses working in patient-sitter wards.

7.Epidemiological Study on the Mumps Epidemic: In one Primary School in Hwasung-Kun, Kyonggi-Do.

In the last years, according to the increasing incidence of measles and other viral diseases, it have been needed to study on not only the fundamental epidemiological characteristics of mumps and measles but also judgement on the effectiveness of vaccine that meet our domestic situation. The author carried out questionnaire to obtain epidemiological characteristics and vaccine efficacy of mumps on epidemics of mumps in one primary school in Hwasung-kun, Kyonggi Do from Feb. to July, 1994. The results are summarized as follows: 1. Total students was 556, but 446(78.4%) responded to the questionnaire. Among that 446,130(29. 1%) have suffered from mumps. From 9 to 10 year old students had the high attack rates, especially 10 year olds has the highest(39. 7%). 2. From Feb. 2, 1994, first case was on, to July 6, 1994, the epidemic was continues, and attack rate was higest in May(50. 5%). 3. Among the 130 mumps patients, 5 have suffered from complication(3.8%) : encephalitis 3(2 in male, 1 in female). 4. Vaccination rate was 91%, and as ages were lowered, the rates were higher. 5. Attack rate was higher in group who had time interval after vaccination less than 5 years than total attack rate, and was higher in group who had been vaccined after 5 year old. 6. Attack rate in vaccinated group was 27.3%. 7. Vaccine efficacies, be judged by three methods of analysis according to case definition and vaccination status, were 37.4 39. 2%. 8. Factors that have significantly affected the mumps epidemic were vaccination status and vaccination age.

8.Fragile Self and Malevolent Others: Biased Attribution Styles in Individuals at Ultra-High Risk for Psychosis.

OBJECTIVE: Biased attribution styles of assigning hostile intention to innocent others and placing the blame were found in schizophrenia. Attribution styles in individuals at ultra-high risk (UHR) for psychosis, however, have been less studied especially for its association with various psychological factors. We investigated whether UHR individuals show increased hostility perception and blaming bias and explored the associations of these biased styles of attribution with the factor structure of multifaceted self-related psychological variables and neurocognitive performances. METHODS: Fifty-four UHR individuals and 80 healthy controls were assessed by evaluating resilience, self-perception, self-esteem, and aberrant subjective experiences of schizotypy (physical anhedonia, social anhedonia, magical ideation, and perceptual aberration), basic symptoms, and carrying out a comprehensive neurocognitive test battery. Attribution styles were assessed using the Ambiguous Intentions Hostility Questionnaire. RESULTS: UHR individuals, compared with normal controls, showed increased hostility perception and blaming bias. Factor analysis of self-related psychological variables and neurocognitive performances in the entire subject population showed a three-factor solution, which was designated as reflective self, pre-reflective self, and neurocognition. Multiple regression analysis in UHR individuals revealed that hostility perception bias was associated with reflective self and composite blame bias was associated with reflective and pre-reflective self. CONCLUSION: This study supports the emergence of attribution biases in the putative ‘prodromal’ phase of schizophrenia. The associations of biased attribution styles with multifaceted self-related psychological constructs suggest that psychosocial interventions for biased attribution styles in UHR individuals should focus not only on reflective self but also pre-reflective self-related psychological constructs.

BACKGROUND: This article reports annual data of intensive care units (ICU) module of the Korean Nosocomial Infections Surveillance (KONIS) system from July 2011 through June 2012. METHODS: We performed a prospective surveillance of nosocomial urinary tract infections (UTI), bloodstream infections (BSI), and pneumonia (PNEU) at 143 ICUs in 81 hospitals using the KONIS system. Nosocomial infection (NI) rates were calculated as the number of infections per 1,000 patient days or device days. Asymptomatic bacteriuria was excluded on or after October 1, 2011. RESULTS: A total of 3,374 NIs were found during the study period: 1,356 UTIs (1,336 cases were urinary catheter-associated), 1,253 BSIs (1,091 were central line-associated), and 765 PNEUs (481 were ventilator-associated). The rate of urinary catheter-associated UTIs (CAUTIs) was 2.26 cases per 1,000 device-days (95% confidence interval, 2.14-2.39) and urinary catheter utilization ratio was 0.85 (0.849-0.851). The rate of central line-associated BSIs was 3.01 (2.84-3.19) and the utilization ratio was 0.52 (0.519-0.521). The rate of ventilator-associated PNEUs (VAPs) was 1.70 (1.56-1.86) and the utilization ratio was 0.40 (0.399-0.401). Ventilator and urinary catheter utilization ratios were lower in the ICUs of hospitals with 400-699 beds than those in hospitals with 700-899 beds or more than 900 beds. Nevertheless, VAPs and CAUTIs were more common in hospitals with 400-699 beds. CONCLUSION: Nosocomial infection rates were similar to the findings of those of the previous period, July 2010-July 2011. Implementation of proven infection-control strategies are needed, especially in the hospitals having fewer than 700 beds.

BACKGROUND: The incidence of renal cell carcinoma (RCC) in patients receiving long-term hemodialysis or peritoneal dialysis patients is 100-fold higher than that in general population. But the clinical study about RCC in dialysis patients is not reported in Korea yet. This study was performed to evaluate the clinical characteristics of RCC in dialysis patients. METHODS: We retrospectively investigated medical records of dialysis patients who were diagnosed with RCC between patients January, 1993 and December, 2002. RCC diagnosed before first dialysis was excluded. Diagnosis of RCC was made by both radiologic and pathologic examinations. RESULTS: A total of 7 patients was diagnosed with RCC. Mean age was 46+/-9 years. Primary renal disease consisted of chronic glomerulonephritis (n=3), autosomal dominant polycystic kidney disease (n=3), and hypertensive nephropathy (n=1). Five patients were on hemodialysis, 2 were on peritoneal dialysis. Mean duration of dialysis was 82+/-46 months. The presenting symptoms were asymptomatic in 2 patients, sudden onset of flank pain due to spontaneous renal cyst rupture in 3, palpable abdominal mass in 2. Mean tumor size was 3.1+/-1.9 cm and multiple renal cysts were noted in all patients. According to the TNM classification for RCC, 6 patients had stage I and 1 patients had stage IV. Six patients with stage I were treated with nephrectomy. With a mean follow-up of 45+/-35 months after the operation, there has been no recurrence. CONCLUSION: Six patients out of total 7 patients had early stage tumor and all these patients were treated with nephrectomy and the prognosis was good. All RCC were associated with inherited or acquired multiple renal cysts.
