1.Anatomic data and clinical significance analysis of 150 cases of type A aortic dissection

Objective To analyse of 150 cases of type A aortic dissection anatomic parameter and the relationship between anatomic parameter and clinic date and prognosis.Methods We identified 150 cases of type A aortic dissection who were diagnosed clearly.All patients were divided into groups by gender and surgical approaches.General clinic data and radiological data were recorded.Survival rate was evaluated by follow up 3 months after surgery.Results The aortic root diameter of group aortic root replacement was(53.25±13.17)mm,group aortic root sparing was (49.08±6.94)mm,there was significant difference between the two group(P<0.05);survival rate was 86.84% and 79.79%,respectively.There was no relevance between diameter of aortic arch and branch with the aortic arch surgery way.The starting diameter of two group were (54.47±9.69)mm and(54.48±8.30)mm,the diameter of aortic arch were (31.39±7.14)mm and(32.73±6.59)mm,respectively.The anonymous artery diameter were (14.65±3.00)mm and(14.28±2.99)mm,respectively.The left common carotid artery diameter were(9.81±2.33)mm and(9.56±2.10)mm,respectively.The start diameter of the left subclavian artery were(11.15±2.84)mm and(11.13±2.56)mm(all P>0.05).There were gender differences between type A dissection parameters of descending aorta,the start diameter of male and female's descending aorta were (41.09±8.86)mm and(37.44±5.60)mm,respectively.The descending aorta in parallel to the pulmonary artery diameter were(34.31±0.59)mm and(31.11±0.88)mm,respectively.Descending aorta diameter of the diaphragm were(31.45±6.50)mm and(28.46±5.20)mm,respectively(all P>0.05).Conclusion In patients who suffer from type A aortic dissection,Parameter of aortic root is one of factors which determine surgical approach to aortic root.When treating descending aorta,surgeon should consider the influence of gender.Our study provided data references for selection and design of endovascular stent-graft.
2.Advances in lymphatic targeted drug delivery system for treatment of tumor metastasis

Lymphatic metastasis is one of the main routes of tumor metastasis. The limitation of traditional medicine in the treatment of lymphatic tumor metastasis lies in the low concentration of the drug in lymphatic metastases resulting in poor efficacy. Nanocarrier-based drug delivery system plays an important role in enhancing drug targeting, improving drug bioavailability, and reducing side effects. This review introduces the composition and function of the lymphatic system as well as its role in tumor metastasis, enumerates the present therapeutic means and limitations of anti-tumor lymphatic metastasis, and focuses on the recent advances in the passive, active and antigen-presenting cell-mediated lymphatic targeted drug delivery systems in tumor metastasis are highlighted.
3.Preparation and evaluation of standards for whole blood trace elements detection

Objective To explore the methods of preparing whole blood control of seven trace elements (magnesium,manganese,iron,copper,zinc,lead,calcium) in laboratory and evaluate its performance.Methods Heparin sodium anticoagulant calf whole blood was used as substrateMetal salt or standard solution with target concentration of each element was added.And whole blood control product was made after process of anticorrosion,mixing and sub-packaging.Antibacterial effect was observed,uniformity and stabilitywasevaluatedaccording to CNAS-GL03 and matrix effects was evaluatedaccording to CLSI EP14.SDI (standard deviation index) and detection coefficient of variation (CV)were calculated to evaluateapplication effectiveness.Results Laboratory preparation of whole blood control reached target concentration,sterility tests was qualified,results of uniformity and stability indicated that the substrate was even and stable at least for one year.Besides,matrix effects of other six elements can be ignored except lead.Historical and inter-laboratory comparisons had shown that laboratory preparation of whole blood control has no obvious difference with commercial ones in performance.Conclusion The formulation and evaluation scheme of whole blood control of seven trace elements (magnesium,manganese,iron,copper,zinc,lead,calcium) was feasible and can be used as commercial ones for elementary tests in medical laboratory.
4.Recombinant Activated Factor VII as a Second Line Treatment for Postpartum Hemorrhage.

BACKGROUND: Severe or massive postpartum hemorrhage (PPH) has remained a leading cause of maternal mortality for decades across the world and it results in critical obstetric complications. Recombinant activated factor VII (rFVIIa) has emerged as a gold standard adjunctive hemostatic agent for the treatment of life-threatening PPH refractory to conventional therapies although it remains off-licensed for use in PPH. We studied the effects of rFVIIa on coagulopathy, transfusion volume, prognosis, severity change in Korean PPH patients. METHODS: A retrospective review of medical records between December 2008 and March 2011 indicating use of rFVIIa in severe PPH was performed. We compared age, rFVIIa treatment, transfusion volume, and Sequential Organ Failure Assessment (SOFA) score at the time of arrival in the emergency department and after 24 hours for patients whose SOFA score was 8 points or higher. RESULTS: Fifteen women with SOFA score of 8 and above participated in this study and eight received rFVIIa administration whereas seven did not. Patients' mean age was 31.7 ± 7.5 years. There was no statistically significant difference in initial and post-24 hours SOFA scores between patients administered rFVIIa or not. The change in SOFA score between initial presentation and after 24 hours was significantly reduced after rFVIIa administration (P = 0.016). CONCLUSIONS: This analysis aimed to support that the administration of rFVIIa can reduce the severity of life-threatening PPH in patients. A rapid decision regarding the administration of rFVIIa is needed for a more favorable outcome in severe PPH patients for whom there is no effective standard treatment.

6.Original Articles: Immunogenicity and Safety of a 20 microgram Dose of the Green Cross Recombinant Hepatitis B Vaccine ( HG -II ) in Healthy Adults.

BACKGROUND/AIMS: The introduction of Hansenula polymorpha for recombinant hepatitis B vaccine production allowed high product yield with plasmid stability and less glycosylation than conventional Saccharomyces cerevisiae system. A Green Cross HG-II vaccine formulated from HBsAg produced by a recombinant strain of the yeast H. polymorpha was evaluated for immunogenicity and safety in an open label triaL METHOFD: A 20 ug dose of Green Cross HG-II vaccine was administered intramuscularly at 0, 1 and 6 months at the deltoid region in 118 healthy adults seronegative for HBV markers. The anti-HBs titers were determined at one month after administration of the third dose of vaccine by radioimmunoassay. RESULTS: The seroconversion rate was 96.8% (90 out of 93), with seroprotective rate of 95.7% (89 out of 93). The geometric mean titers(GMT) of the anti-HBs response was 153.1mIU/ml in seroconverters. An age-dependent effect was observed in the anti-HBs response. But sex-dependent effect was not prominent. Reactogenecity was in incidence and general reactions were short-lasting and a mainly mild in severity. CONCLUSIONS: The results of this study have shown that the Green Cross HG-II vaccine is safe and clinically well tolerated, a nd that it may provide protection against HBV infection.

7.Effect of Vertical Displacement of Bilateral Horizontal Muscles in AV Syndrome.

We reviewed 20 cases for the surgical effect of the vertical transposition of the horizontal rectus muscle for AV syndrome without significant oblique dysfuction. We performed vertical tranposition of the horizontal rectus muscle with standard recession and resection procedure and followed up at least six months. Sixteen cases were V syndrome and four cases were A syndrome. Medial rectus was moved toward the apex of the A or V, and the lateral rectus was moved to the opposite side of the apex of the A or V. The amount of transposition was adjusted according to the amount of vertical incomitance. If the vertical incomitance was 10delta, 15delta, 20deltaand 25delta, vertical transposition of rectus muscle was 1/4 TW[tendon width], 1/2 TW, 3/4 TW amd 1 TW, respectively. We achieved an average of 12.9deltareduction by 1/4 TW transposition, 14.4deltareduction by 1/2 TW transposition, 13.3delta reduction by 3/4 TW transposition, 19.0deltareduction by 1 TW transposition. We also achieved an average of 5.2delta/mm effect by 1/4 TW transposition, 2.9delta/mm effect by 1/2 TW transposition, 2.4 delta/mm effect by 3/4 TW transposition, 1.9delta/mm effect by 1 TW transposition. The vertical transposition of the horizontal rectus muscle for AV syndrome without significant oblique dysfuction showed good result and the amount of correction per mm of vertical transposition seemed to decrease as the amount of transposition increased, even though the total amount of correction showed a trend toward increasing [Fig.1, 2].

10.Borneol is an inducer of rat hepatic CYP2D activity in vivo.

Borneol is a traditional Chinese medicine. In the past few years, many studies showed that borneol can improve the bioavailability of other drugs, promoting drugs to cross the blood-brain barrier, so the potential drug interactions between borneol and other medicines have attracted great attention, but the influence of borneol to CYP450 and its isoforms are rarely reported. In this research, male Wistar rats were orally administered by borneol for 7 days, then the mRNA and protein expression and the activities of CYP2D were detected, we also compared the pharmacokinetic parameters of CYP2D's specific substrate between control group and borneol group. The results show that borneol (33, 100 and 300 mg x kg(-1) x d(-1)) have no significant effect on CYP2D, while the activites of CYP2D increased 1.71, 1.97 and 2.89 times comparing to the control group. At the same time, borneol (300 mg x kg(-1) x d(-1)) caused the C(max) decreased 10.6% (P > 0.05), AUC(0-∞) decreased 27.5% (P < 0.01), CL/F increased 41.1% (P < 0.01), V(z)/F increased 23.1% (P > 0.05) of dextromethorphan. Our data provided that borneol speed up dextromethorphan's elimination in vivo. Since the activity of CYP2D can be induced by borneol, the metabolic interactions might happen when borneol and the substrate drug CYP2D are used together.
