2.A Clinical Statistics of Apgar Scoring System of Newborn.

This clinical observation was made on 6206 newborn delivered at Ewha Womans University Hospital in 1973~1977 to figure out any relations between Apgar score and problems such as maternal age, birth weight, labor duration, gestational period, SPRM, delivery type, meconium were stained baby, mortality. The reults were summarized as followings: 1) Percentage distribution of Apgar score for 6206 newborns was 2.7% for score 0~3, 7.9% for score 4~6, 89.4% for score 7~10. 2) There was a trend to lower score in the newborns in the less or more than the full term of gestation. (38~42wks) 3) There was declining to lower score in the newborns of prolonged labor. (24hr) 4) There was no significant difference between maternal age and Apgar score. 5) There was a trend to lower scores in the newborns of meconium staning, prolonged membrane rupture, not received antenatal care. 6) Apgar score was greatly influenced by the modes of the deliveries : orderly lowered : spontaneous vertex & vaccum delivery, c-section, and breech delivery. 7) Death rate within 3 days of hospitalization after birth was 1.5% of all new born.

4.Diagnosis of Malaria Using Automatic Hematology Analyzer.

BACKGROUND: For the diagnosis of malaria, examination of blood smear slides by light microscopy is used as standard, and commercial kits detecting malarial antibodies and antigens are available, and molecular methods such as polymerase chain reaction (PCR) are used additionally. But, these diagnostic methods can be performed when clinicians request them, so problems of misdiagnosing the patients who are not suspected malaria may be occurred. METHODS: In 42 Korean patients with malaria, the author analyzed the characteristic signals of malaria using granularity (90 degrees depolarized) versus lobularity (90 degrees polarized) graph of Cell-Dyn 4000 (CD4000) automatic hematologic analyzer. And, the author examined the presence of malaria in 421 random samples by CD4000 and Giemsa stain. RESULTS: The usefulness of CD4000 in diagnosing malaria are as follows, 93.0% sensitivity, 99.3% specificity, 93.0% positive-predictive value, and 99.3% negative-predictive value. CONCLUSION: CD4000 automatic hematology analyzer has high diagnostic sensitivity and specificity in diagnosing malaria. Because complete blood count (CBC) is the routine test for most patients, this method has advantage of time and cost effectiveness and can even detect malaria in unsuspected cases.

5.A Case of Localized Skin Infection Caused by Cephalotheca foveolata.

We report a case of localized skin infection caused by Cephalotheca foveolata in a 67-year-old male patient, who had had erythematous verrucous plaques on the right dorsum of his foot, big toe, heel and sole for 10 years. Histopathologically, chronic granulomatous inflammation and fungal elements were observed. Yellowish brown, velvety colonies and black cleistothecia were noted on the culture of biopsied tissue specimens, which were cultured on Sabouraud's dextrose agar for 8 weeks. We confirmed a new Cephalotheca species by colony, microscopic morphology and molecular analysis. The patient was treated with surgical excision by skin graft.

6.A Case of Herpes Zoster in a 9-month-old Infant.

We report a case of herpes zoster in a 9-month-old female infant, who showed multiple, grouped, erythematous papulovesicles on the right upper chest, axilla and upper back along the T4 dermatome. The patient had no history of varicella or varicella vaccination, and her mother had no contact history with varicella during pregnancy. However, the mother had been infected with the herpes zoster virus about 3 years before. Multinucleated, giant cells were shown on a Tzanck smear, and histopathologic findings were consistent with the herpes viral infection. Laboratory findings revealed that the anti-Varicella-Zoster virus IgG was positive and IgM was negative. The patient was treated with acyclovir for 7 days and papulovesicles healed without sequelae.

7.A Case of Ganglion on the Sole.

Ganglion is a relatively-common, benign, soft tissue tumor that is almost always located near a joint capsule or tendon sheath. The common location is around the wrist, ankle or foot dorsum. We report a case of ganglion in a 53-year-old woman who presented with a solitary, tender, 1.0x1.0 cm sized, normal skin-colored, deep-seated nodule on the left sole. She had been treated by total excision of the tumor and recurrence has not been observed to date.

8.The Genotyping of the Secretory Gene (Sec2) in the Korean Population.

BACKGROUND: The expression and secretion of ABH antigens in epithelial cells of glands are controlled by secretor type alpha (1,2)fucosyltrasnferase activity and the human secretor alpha (1,2)fucosyltransferase gene (Sec2) determines the ABH secretor status and influences the Lewis phenotype of an individual. Homozygosity of the mutation for this allele is responsible for the nonsecretor phenotype in nonsecretor individual. The aim of this study was to investigate the status and the distribution of the Sec2 genotype in the Korean population. METHODS: In order to explore the secretory genotypes of the Korean population, the 158 specimens were analyzed by the PCR-RFLP (polymerase chain reaction-restriction fragment length polymorphism) method designed for the detection of the A385T, the C357T and the G428A mutations of Se alleles. RESULTS: The frequencies of Se1, Se2 and sej among 316 alleles examined in a random sample of 158 Korean individuals were 11.1%, 40.5% and 48.4%. The frequencies of Se1/Se1, Se1/Se2, Se2/Se2, Se1/sej, Se2/sej and sej/sej among 158 genotypes were 3.2%, 3.2%, 20.3%, 12.7%, 37.3% and 23.4%. The G428A nonsense mutation discovered in the Sec2 gene of nonsecretors in Caucasian was not found in any of 158 Korean population. CONCLUSIONS: The distribution of the genotypes of the Sec2 gene in the Korean population showed a rather wide distribution of the sej allele than the Caucasian population and was similar to the Japanese population. PCR-RFLP method can be effectively used for the genotyping of the Sec2 gene.

9.Plasma Exchange in a Patient with Antineutrophil Cytoplasmic Antibody-associated Vasculitis.

Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis is a systemic autoimmune disease with a poor prognosis in untreated patients, and combination therapy with steroidsand cyclophosphamide is the current standard treatment. Treatment with immunosuppressive agents results in high remission rates; however, there is a considerable morbidity related to the immunosuppressive state, such as life-threatening infections. To reduce serious immunosuppressant-related adverse effects, potentially promising treatment options, including plasma exchange, cytapheresis, administration of intravenous immunoglobulin and TNF-alpha blocking agents have been developed. We performed plasma exchange with steroid pulse therapy for a 52 year-old female patient with pulmonary hemorrhagewith ANCA-associated crescentic glomerulonephritis. After combination therapy, the pulmonary hemorrhage and renal function recovered and myeloperoxidase (MPO)-ANCA decreased.

10.A Case of Tinea Corporis Caused by Trichophyton verrucosum Showing Tinea Imbricata-like Clinical Appearance.

We report a case of tinea corporis with unusual clinical features in a 7-year-old boy. He had a 7.5 x 7.0 cm-sized, well-defined, erythematous patch with concentrically arranged rings of scales on the right thigh, and two 2.5 x 2.0 cm and 1.0 x 1.0 cm-sized satellite lesions beside the lesion. Histopathologically, PAS staining revealed fungal hyphae in the horney layer of the epidermis. Fungal culture of scales of the lesions on Sabouraud's dextrose agar showed typical Trichophyton verrucosum. He was treated daily with 125 mg of terbinafine for 1 month and with topical lanoconazole. Skin lesions improved after a month of treatment, and recurrence was not observed.
