1.The Effect of Taekwondo Training on Aerobic Capacity, MVO2, ST Slope and QRS Duration.

The purpose of this study was to investigate the effects of taekwondo training on VO2max, MVO2 and electrocardiogram ST slope, QRS duration in college woman. Subject were divided into two groups, one is taekwondo group (n=11) and the other is control group (n=10). They participated in graded exercise test (Balke protocol) and measured aerobic capacity and electrocardiogram. The aerobic capacity (VO2max and AT) was measured by autogas analyser. The resting and exercise rate-pressure product (RPP) could be measured with HR and systolic BP for MVO2 measurement. The electrocardiogram ST slope, QRS duration was calculated using automated software program (QMC). The measured data were analyzed by using paired t -test. The results of this study were as follows : There was a significant difference of VO2max and AT between taekwondo group and control group. VO2max of taekwondo group was 4~5 mL/kg/min higher than control group (p<0.01) and AT also taekwondo group was 3.4% higher than control group (p<0.05). There was no significant difference of maximal exercise MVO2 between taekwondo and control group. There was no significant difference of electrocardiogram ST slope between group but rest QRS duration in taekwondo group was longer than the control group (p<0.01). Conclusively, these data suggest that regular taekwondo training will strengthen on aerobic capacity and resting slow heart beats.

3.Torsion of Appendix Testis: 14 Cases.

In spite of today's striking development of radiologic studies, the differential diagnosis of acute scrotal diseases is difficult. We studied 14 cases of appendix testis torsion that had visited our hospital from May 1991 to October 1994 in order to be diagnosed accurately and made appropriate treatment. The torsion of appendix testis occurred during prepubertal ages, and ages of our 14 cases were from 3 to 13, and average age was 9.6 years old. In all of our cases the chief complaint was scrotal pain. The localized pain and palpable tender mass of testicular upper pole were present in only 8 of 14 cases and 5 cases had blue dot sign. These were very helpful in diagnosis of torsion of appendix testis. Doppler ultrasonography was done in 9 cases and radionuclide scrotal imaging was done in 3 cases, but none of these studies were diagnosed as torsion of appendix testis definitely. But in 8 cases that had positive blue dot sign and/or localizing tender mass in upper pole of testis, we could diagnose torsion of appendix testis certainly. We operated 11 of l4 cases( scrotal exploration and excision of torsed appendix testis) and treated conservatively 3 of 14. Most 11 operated cases were revealed that scrotal pains were relieved within 24 hours of postoperative period and secondary complications did not occurred in 3 cases treated conservatively. In summary, the early surgical exploration is necessary to improve maximal testicular salvage when testis torsion cannot be ruled out completely in acute scrotal diseases. But if testis torsion can be ruled out completely, conservative treatment may be considered in torsion of appendix testis.

4.Anthropological Study on the Facial Flatness of Korean from the Historic to the Modern Period.

The facial flatness was studied by the metric method which involves six measurements of breadth and projection of the midfacial skeleton, the calculation of three indices from the measurements. The indices of the modern Korean adult crania, Korea & Chosun Dynasty crania, the Iron Age Korean crania, and the late Neolithic Korean crania were investigated. The indices were compared to those of other published data on the Korean crania and those of neighboring asian races as well. All indices for facial flatness of Korea & Chosun Dynasty were similar to modern Korean. However, zygomaxillary index was different between modern and late Neolithic crania. The difference of zygomaxillary index was prominent in males than females. The difference of facial flatness between modern and ancient seems to be mainly due to zygomatic breadth. The frontal indices of modern Korean and Korea & Chosun Dynasty crania were relatively low and the simotic indices were relatively high among neighboring asian races. The zygomaxillary index is intermediate. Clustering analysis represented that the flatnesses of the modern Korean crania and Korea & Chosun Dynasty crania were closely related to those of the northern Chinese, modern Japanese, however, somewhat different from that of ancient Korean, Mongol, Siberian etc.

5.Overexpression of bone morphogenetic protein 4 in STO fibroblast feeder cells represses the proliferation of mouse embryonic stem cells in vitro.

Embryonic stem cells (ESCs) can be propagated in vitro on feeder layers of mouse STO fibroblast cells. The STO cells secrete several cytokines that are essential for ESCs to maintain their undifferentiated state. In this study, we found significant growth inhibition of mouse ESCs (mESCs) cultured on STO cells infected with adenovirus containing a dominant-negative mutant form of IkappaB (rAd-dnIkappaB). This blockage of the NF-kappaB signal pathway in STO cells led to a significant decrease in [3H]thymidine incorporation and colony formation of mESCs. Expression profile of cytokines secreted from the STO cells revealed an increase in the bone morphogenetic protein4 (BMP4) transcript level in the STO cells infected with adenoviral vector encoding dominant negative IkappaB (rAd-dnIkappaB). These results suggested that the NF-kappaB signaling pathway represses expression of BMP4 in STO feeder cells. Conditioned medium from the rAd-dnIkappaB-infected STO cells also significantly reduced the colony size of mESCs. Addition of BMP4 prevented colony formation of mESCs cultured in the conditioned medium. Our finding suggested that an excess of BMP4 in the conditioned medium also inhibits proliferation of mESCs.

7.The Relation between Tourniquet Hypertension and Bispectral Index in Patients Undergoing a Total Knee Arthroplasty.

BACKGROUND: Tourniquet hypertension (TH) may develop from an autonomic reflex consequent to the sensitization of the central nervous system (CNS) due to ischemia of the affected limb and noxious surgical stimulation. TH could be preventable when anesthetic depth is sufficient enough to block plastic changes of the CNS during an operation. This study was aimed to evaluate the efficacy of the bispectral index (BIS) to predict development of TH by assessing correlations between BIS changes and hemodynamic changes during the skin incision. METHODS: Sixty patients who had undergone a total knee arthroplasty with a pneumatic tourniquet during isoflurane anesthesia were selected, and the samples were randomly divided into two groups: BIS 30 (30 - 39) and BIS 40 (40 - 49). Several variables such as BIS, mean arterial pressure (MAP), heart rate (HR) and end tidal concentraion of isoflurane were monitored during the operation. TH was defined as more than a 30% increase in MAP compared with the baseline values during the skin incision. The correlation between BIS changes and MAP and HR changes, and the incidence of TH between BIS groups were compared. RESULTS: The MAP and HR were meaningfully increased in both the BIS 30 and the BIS 40 groups (P < 0.05), but no significant correlations were found between the BIS changes and the MAP and HR changes during the skin incision. The incidence of intraoperative hypertension was not different between the BIS groups. The BIS of the patients who had TH was not different from those who did not have TH. CONCLUSIONS: BIS may not reflect the changes of the CNS and autonomic nervous system induced by noxious surgical stimulation during an operation, and BIS changes during a skin incision cannot be a predictor of TH.

8.Anesthetic Management of Myotonic Dystrophy: A case report.

Myotonic dystrophy is a muscle disorder characterized by progressive muscle wasting and weakness associated with myotonia. It is a systemic disease transmitted in an autosomal dominant pattern. Myotonia may be precipitated by hypothermia, shivering, and mechanical or electrical stimulation. Furthermore, sensitivity to sedative, anesthetic and neuromusclular blocking agents may result in intraoperative and early postoperative cardiovascular and respiratory complications, as well as prolonged recovery from anesthesia. Therefore, a patient with myotonic dystrophy is at high risk for complications from anesthetic management. We report the successful anesthetic management of such a patient undergoing a total abdominal hysterectomy, in which we used an epidural technique for intraoperative anesthesia and postoperative analgesia.

10.PI3K-Akt-Wnt Pathway Is Implicated in Exercise-Induced Improvement of Short-term Memory in Cerebral Palsy Rats.

PURPOSE: Maternal lipopolysaccharide (LPS) injection induces neurodevelopmental disorders, such as cerebral palsy. Exercise activates phosphatidylinositol 3-kinase (PI3K)-protein kinase B (Akt) signaling pathway that enhances neurogenesis. Wnt ligands are also implicated in the hippocampal neurogenesis and synaptic plasticity. Glycogen synthase kinase-3β (GSK-3β) is a downstream molecule of Akt, and GSK-3β is known to modulate hippocampal neurogenesis negatively. METHODS: Cerebral palsy was made by maternal LPS-injection. On the 5 weeks after birth, treadmill running was applied to the rat pups of the exercise groups, for 30 minutes, 5 times a week during 6 weeks. RESULTS: Treadmill running alleviated short-term memory impairments of the cerebral palsy rat pups. Hippocampal cell proliferation was increased and hippocampal apoptosis was suppressed by treadmill running in the cerebral palsy rat pups. Hippocampal phosphorylated-PI3K/PI3K ratio, phosphorylated-Akt/Akt ratio, and Wnt expression were enhanced by treadmill running in the cerebral palsy rat pups. In contrast, hippocampal phosphorylated-GSK-3β/GSK-3β ratio and β-catenin expression were suppressed by treadmill running in the cerebral palsy rat pups. CONCLUSIONS: The results of this study showed that short-term memory improvement due to treadmill running in cerebral palsy occurs via activation of the PI3K-Akt-Wnt pathway.
