1.Therapeutic effect compared between laparoscope and open neoplasty of gastroduodenal ulcer perforated

Objective To compare the safety and therapeutic effect between laparoscope and open neoplasty of gastroduodenal ulcer perforated.Methods Among 93 patients with gastroduodenal ulcer perforated,54 patients were given laparoscope neoplasty of gastroduodenal ulcer perforated (laparoscope group),39 patients were given open neoplasty of gastroduodenal ulcer perforated (open group).The informations during the period of operation in the 2 groups were compared.Results There was no death in the 2 groups,2 cases in laparoscope group converted to open operation.The operation time of laparoscope group was significantly longer than that of open group [(74.85 ± 15.80) min vs.(62.97 ± 12.14) min],there was statistical difference (P < 0.01).There were no statistical differences in the time of postoperative indwelling gastric tube,hospitalization time and complication rate between the 2 groups (P > 0.05).The rate of postoperative analgesia of laparoscope group was significantly lower than that of open group [20.4% (11/54) vs.74.4% (29/39)],there was statistical difference (P < 0.01).Conclusion Laparoscope neoplasty of gastroduodenal ulcer perforated is effective and safe,and has high application value.
2.Assessment of Left Ventricular Systolic function in Elderly Isolated Systolic Hypertension

Aim To explore the hemodynamic change of elderly isolated hypertension(EISH). Methods Color doppler ultrasonography were applied to examine thirty EISH patients and twenty eight elderly healthy subjects. The left ventricular systolic function as well as hemodynamic parameters were measured. Results The indices of left ventricular systolic function in EISH group such as SV,CO,EF and CI were obviously higher than those of control group(P<0.05),Linear relevant analysis show that pulse pressure was remarkable related to SV,CO,EF and CI in EISH group,with r value 0.43,0.56,0.46,0.45 respectively.Conclusion EISH patients apparently had ventricular systolic dysfunction and hemodynamic abnormality. Pulse pressure may be associated with development of sysytolic dysfunction.

4.Role of ERK/FoxO3a signal axis in inhibitory effect of vitexin 1 (VB-1) in HepG2 cell proliferation.

OBJECTIVETo investigate whether the ERK/FoxO3a signal axis could induce the inhibitory effect of vitexin 1 (VB-1) in HepG2 cell proliferation.
METHODThe MTT method was adopted to observe the effect of different concentrations of VB-1 on human hepatoma carcinoma cell line HepG2 and immortalized human embryo liver cell line L-02. The cell growth was assessed by the clone formation assay. The protein phosphorylation levels of ERK1/2 and FoxO3a were measured by the western blot.
RESULTVB-1 inhibited the viability of HepG2 cell line in a concentration-dependent manner, with a weak effect on L-02 cell line. VB-1 could effectively inhibit the anchorage-dependent growth of HepG2 cells, and reduce the expression levels of pERK1/2 and pFoxO3a in a concentration-dependent manner. MEK1/2 inhibitor PD98059 could enhance VB-1' s effect in inhibiting HepG2 cell proliferation and ERK1/2, FoxO3a phosphorylation.
CONCLUSIONVB-1 inhibits the proliferative activity of hepatoma carcinoma cell line HepG2 by blocking the ERK/FoxO3a signal axis.

5.Immunotherapy for hepatocellular carcinoma

In the past 10 years, the progress in systemic therapy for hepatocellular carcinoma (HCC) is attributed to the application of molecular target therapy and the improvement in immunotherapy. The immunosuppressive microenvironment of HCC enables HCC cells to avoid attack by the immune system, which is also an important reason for the progression of HCC. Improving immune killing of HCC and correcting immunosuppressive conditions are important strategies for immunotherapy for HCC. Tumor vaccine therapy based on HCC specific antigen, genetically engineered T lymphocytes, and basic research and bench-to-bedside translation of immune checkpoint inhibitors have significantly improved the outcome of immunotherapy. Further studies should be performed for immunotherapy combined with other antitumor therapies such as local ablation, molecular targeted therapy, and tumor vaccine therapy.
6.Treatment of mesh infection after tension-free repair of inguinal hernia

Objective To analyze the treatment methods of the mesh infection after tension-free repair of inguinal hernia. Methods The clinical data of 6 patients with mesh infection after tension-free repair of inguinal hernia from January 2012 to December 2017 were retrospectively analyzed. The materials of the meshes were standard knitted polypropylene. Transabdominal preperitoneal technique (TAPP) was performed in 1 case, modified Kugel repair was performed in 1 case, and Lichtenstein repair was used in 4 cases. Five patients accepted reoperation, and the infected meshes were removed combined with closed drainage. One patient accepted conservative treatment. Results The patients who received the reoperation all healed and were discharged without perioperative death; the patients were followed up 6 to 40 months (average 19 months), and there were no recurrence patients. The patient with conservative treatment developed incision chronic infectious sinus formation. Conclusions There are many reasons for mesh infection after tension-free repair of inguinal hernia, and the management should be individual. Once the infection occurs, the infected mesh should be removed timely so as to shorten the course of disease, but the key is prevention.
7.Protection of penile vascular endothelial function: a new strategy for the management of erectile dysfunction.

Erectile function is a typical neurovascular process. Penile vascular endothelial dysfunction is indicated to be one of the important mechanisms of ED. Protective agents to improve penile vascular endothelial function show significant benefits to erectile function by decreasing the damage of oxidative stress and optimizing the related mediators. The protection of penile vascular endothelial function is a new approach to the treatment of ED.

8.Role of chemokine CCL28 in hypoxia-induced migration of hepatocellular carcinoma.

OBJECTIVETo investigate the role of CCL28 in hypoxia-induced cell migration of hepatocellular carcinoma (HCC).
METHODSResected liver tissues from 50 HCC patients were subjected to real-time (rt)-PCR analysis to evaluate the mRNA expression levels of the hypoxia-induced factor HIF-1a and the chemokine CCL28. Patient data on treatment and outcome were analyzed. The human HCC cell lines HepG2 and HCCLM3 were used to investigate effects of hypoxic conditions on HIF-1a and CCL28 expressions by rt-PCR, western blotting, and enzyme-linked immunoassay. The CCL28-mediated effects of hypoxic conditions on cell mobility and invasion were assessed by trans-well and matrigel assays, respectively, in HCCLM3 with CCL28 expression silenced by small-interfering (si)RNA transfection. Spearman's rank test was used to assess the correlation between CCL28 and effects on disease- and treatment-related factors.
RESULTSThe mRNA levels of CCL28 (0.025 +/- 0.075) were found to be strongly correlated with HIF-1a(0.065 +/- 0.098) in human clinical samples of HCC (r = 0.595, P less than 0.01), with higher expressions of both related to recurrence after surgery (P = 0.011 and 0.019, respectively). In vitro hypoxic conditions stimulated HIF-1a and CCL28 expression in a time-dependent manner in both HepG2 (HIF-1a: F = 873.5; CCL28: F = 151.6) and HCCLM3 (HIF-1a: F = 964.5; CCL28: F = 285.8) (all P less than 0.01). siRNA inhibition of CCL28 in HCCLM3 cells led to a significant reduction in hypoxia-induced invasion and migration (all P = 0.011).
CONCLUSIONChemokine CCL28 expression is up-regulated in human HCC and under in vitro hypoxic conditions, and may play an important role in hypoxia-induced HCC migration and invasion.

9.Comparison of different threshold ~(18)FDG PET with computer tomography for defining gross tumor volume in non-small cell lung carcinoma

Objective Under different standard uptake value(SUV),to assess gross tumor volume (GTV) definition for non-small cell lung cancer(NSCLC) with 18-fluoro-deoxy-glucose positron emission tomography (~(18)FDG PET) both under definite threshold (42 percent threshold) and various relative threshold (threshold SUV/maximum SUV) derived from the linear regressive function,threshold SUV=0.307?(mean target SUV)+0.588,with computer tomography (CT).Methods Of 20 patients with non-small cell lung cancer,the CT GTV (GTV_(?)).PET GTV with 42 percents threshold (GTV_(42%)) and PET GTV with relative threshold (GTV_(?)) were obtained and compared.Results The mean GTV_(42%),,mean GTV_(?) and mean GTV_(CT) was (13 812.5?13 841.4),(24 325.3?22 454.7) and (28 350.9?26 079.8)mm~3,respectively,with the difference in mean GTV among these three methods significant (F =10,P<0.01).The GTV_(42%) was smaller than the GTV_(?) and the GTV_(CT)(P<0.01),with i(?)significant difference between GTV_(?) and GTV_(CT)(P=0.125).Conclusion The relative threshold is more suitable to define the gross tumor volume than the definite threshold.
10.Bacterial distribution and drug resistance in blood samples in Ningxia Hui Autonomous Region, 2018-2020

Abstract: Objective To understand the distribution and drug resistance of bacteria in clinical blood culture specimens in Ningxia in recent years, and to provide a basis for the prevention and treatment of bloodstream infection diseases. Methods The blood culture isolation bacteria and drug resistance of Ningxia bacterial resistance monitoring network hospitals from 2018 to 2020 were statistically analyzed by WHONET5.6 software. Results In the past three years, a total of 6 757 strains of bacteria were isolated from blood samples, including 3 697 strains (54.7%) of gram-negative bacteria and 3 060 (45.3%) of gram-positive bacteria. Among the gram-negative bacteria, Escherichia coli (2 074 strains,30.7%), Klebsiella pneumoniae (696 strains), Pseudomonas aeruginosa (139 strains), and Acinetobacter baumannii (121 strains). Among the gram-positive bacteria, coagulase-negative Staphylococcus (1 691 strains,25.0%), Staphylococcus aureus (442 strains), Streptococcus spp. (431 strains), Enterococcus spp. (379 strains). Resistance to Escherichia coli and Klebsiella pneumoniae was 56.6% and 22.6% against third-generation cephalosporins, and resistance to carbapenems was 1.0% and 3.7%, respectively. Pseudomonas aeruginosa and Acinetobacter baumannii were resistant to carbapenems at 9.0%(12/139) and 80.7%(71/121). Methicillin-resistant Staphylococcus aureus (MRSA) was detected at 26.8%, methicillin-resistant coagulase-negative Staphylococcus was detected at 70%, and no Staphylococcus bacteria resistant to vancomycin and linezolid were found. For three years, only 1 strain of vancomycin-resistant Enterococcus faecalis was detected, and no linezolid-resistant Staphylococcus and Enterococcus were detected. Conclusions Ningxia clinical blood specimen isolates of Escherichia coli, coagulase-negative Staphylococcus, and Klebsiella pneumoniae are more common. Among them, the resistance rate of Escherichia coli and Klebsiella pneumoniae to the third generation of cephalosporins is relatively stable, and the resistance rate to carbapenems is low. Acinetobacter baumannii is highly resistant to carbapenems, and methicillin-resistant Staphylococcus aureus detection rates are on the rise and should be closely monitored.
