1.Influence of daily goal health education on cognitive attitude to the disease and treatment compliance of patients with stroke

Objective To apply daily goal health education in the inpatients with stroke,to observe the influence on cognitive attitude to the disease and treatment compliance of patients.Methods A total of 133 cases of inpatients with ischemic stroke were included,which were divided into experimental group (n=67) and control group (n=66),experimental group conducted with daily goal health education mode,while the control group with conventional health education mode.Cognitive attitude to the disease and treatment compliance were all compared in patients when discharged.Results After health education in two ways,scores of disease treatment attitude and social participation attitude of the experimental group [(12.15±2.61) points and (5.70±1.60) points] was higher than the control group [(11.33±1.91) points and (5.02±1.49) points],the difference was statistically significant (t=2.059,2.561,P<0.05),treatment compliance was higher than control group (x2=6.130-7.063,P<0.05).Conclusions Daily goal health education can significandy improve the cognitive attitude and the treatment compliance of patients with stoke,which is a good health education mode to the inpatients with stroke.
2.Discussion of stroke unit management in nursing based upon international standards in neurology department

Objective To discuss the practice of nursing management model of stroke unit based upon the international standards implemented in the neurological unit.Methods The experiences of the stroke unit management were summarized and applied to instruct the clinical practice.Results After the implementation,the average hospitalization days,the proportion of drugs,medical cost,and mortality rate decreased.The satisfactory degree of patients with the stroke unit increased.Conclusions The nursing management model of stroke unit not only improved the quality of nursing service to stroke patients,but also decreases the mortality and disability rate,shortened the length of hospital stay,and enhanced the satisfaction degree of patients and their family members.
3.A clinico-pathological study of primary adrenal lymphoma(report of 2 cases)

Objective To study the clinico-pathological features of primary adrenal lymphoma (PAL). Methods 2 cases of PAL were reviewed and studied. Results The age of the 2 patients was 45 and 57.The presenting symptoms were nonspecific.Histologically,the neoplasm was composed of numerous medium-sized cells with round nucleus,usually 1-2 in number and with relatively abundant basophilic cytoplasm.The neoplastic cells showed frequent mitosis.Immunohistochemically,the tumor cells were strongly positive for LCA,L26 and CD74 while negative for UCHL-1,S-100 protein and CK. One case was discharged 3 months after operation because of dyscrasia and the other was lost on follow up.Prognosis of PAL was poor. Conclusions PAL is extremely rare with a high grade of malignancy.The diagnosis could be made on histopathological studies and immunohistochemical studies.
4.Uterine artery embolization for the treatment of uterine adenomyosis:comparison study of different embolic agents

Objective To investigate the clinical effects and complications of uterine artery embolization (UAE) therapy using different embolic agents in treating uterine adenomyosis. Methods During 2004—2011, a total of 45 female patients with uterine adenomyosis were admmitted to authors’ hospital to receive uterine artery embolization therapy. The embolic agents used in the procedures included domestic iodized oil, sodium alginate microspheres and Embosphere microspheres. The patients were randomly divided into group A (n=15, using domestic iodized oil as embolic agent), Group B (n=13, using sodium alginate microspheres as embolic agent) and group C (n = 17, using Embosphere microspheres as embolic agent). After the treatment, all the patients were followed up for over 2 years, focusing on the observation of the clinical symptoms such dysmenorrheal, changes of MRI signs and severe complications. The results were analyzed and compared among the three groups. Results After UAE, the clinical symptoms such as the degree of dysmenorrhea, menstrual quantity, etc. were improved in all patients. Six months after UAE, the obvious remission rate of dysmenorrheal in group A, B and C was 33.3% (5/15), 30.8% (4/13) and 41.1%(7/17) respectively. Twelve months after UAE, MRI scanning showed that the uterine sizes in group A, B and C were reduced to 49.19%, 48.25% and 50.05% respectively. Follow-up examination at 24 months after UAE showed that recurrence of dysmenorrheal in group A, B and C was seen in 2, 4 and 2 cases respectively, and amenorrhea was seen in 2 cases of group A and one case in group C. Conclusion The use of domestic iodized oil, sodium alginate microspheres or Embosphere microspheres as embolic agents in performing super-selective uterine artery embolization for adenomyosis can effectively relieve the degree of dysmenorrheal, decrease the uterine size. In addition to ovarian dysfunction, the uterine cavity adhesion may be also a possible cause of amenorrhea occurring after UAE may. In order to ensure a similar clinical efficacy, the use of granular solid embolic agent is preferred as its safety is possibly higher than the liquid embolic agents.
5.USPIO enhanced 7.0T MRI research of inflammation in rat ischemic stroke

Objective To observe signal changes induced by USPIO accords with iron swallowed by macrophages in brain tissue sections in rats. Methods Thirty-eight SD rats were divided into two groups randomly. Three of them were involved in sham operation group, other thirty-five rats were divided into five subgroups averagely according to 7.0T MR scanning time (6 h, 12 h, 24 h, 48 h and 72 h). After establishment of MCAO models, USPIO was injected to tail intravenous and monitored with high resolution MRI at different time point, while rats in control group were injected with the same dose of sodium chloride. Brain tissue wax section was acquired after MR scanning. Cell necrosis, iron particle and activated macrophages were observed with HE dying, Prussian Blue dying and CD68 immunochemistry staining respectively. Results The ischemic lesion was detected as hyperintense area on T2WI after occlusion and perfusion of MCA. The accumulation of USPIO appeared as hypointense on T2WI but hyperintense on T1WI. The maximum signal change was observed at 48-72 h in both T1WI and T2WI (P>0.05). The iron particle accumulation was found in the boundary of ischemic lesion and necrotic area with Prussian Blue dying. Activated microglia was manifested with CD68 immunochemistry staining, the number of microglia at 72 h was more than those of the other time points. Conclusion USPIO can be used as a contrast agent to monitor rat ischemic stroke in vivo, and the signal changes induced by USPIO approximately accord with iron swallowed by macrophages in brain tissue sections. The cells which swallow USPIO are mainly activated macrophages.
6.The effect of butylphthalide on plasma brain-type creatine kinase isoenzyme,endothelin and calcitonin gene related peptide in patients with acute cerebral infarction

ObjectiveTo explore the correlation between butylphthalide and plasma brain-type creatine kinase isoenzyme (CK-BB),endothelin (ET),calcitonin gene related peptide (CGRP) in patients with acute cerebral infarction (ACI).MethodsSixty patients with ACI were divided into treatment group and control group with 30 cases each by random digits table method.Both groups received normal treatment continuously for 7 days,and butylphthalide of 200 mg was added to treatment group for 3 times per day.The levels of plasma CK-BB,ET and CGRP of two groups before and after treatment were measured and compared.ResultsThe levels of plasma CK-BB,ET of two groups both decreased significantly after treatment,but CGRP rose obviously.It showed significant differences before and after treatment of both groups (P <0.01 or < 0.05).The levels of plasma CK-BB,ET of treatment group after treatment were lower than those of control group in the same period [(216.48±36.95) U/L vs.(333.07±54.03) U/L,(83.33±26.48)ng/L vs.(98.46±31.46) ng/L,P < 0.05 ].The level of plasma CGRP of treatment group after treatment was significantly higher than that of control group in the same period [ (44.16±13.28 ) ng/L vs.(36.42±12.31 )ng/L,P < 0.05 ].ConclusionButylphthalide can reduce the plasma enzyme activity,balance ET and CGRP,extend the cerebral arteries to antagonize ET,improve cerebral ischemia and cerebral hypoxia,which can protect brain cells and endothelial cells.
7.The association between tumor necrosis factor receptor 2 gene polymorphism and development of coronary artery disease

Objective To study the association of biallelic polymorphism at position 196 in exon 6 of tumor necrosis factor receptor 2(TNFR2) gene and coronary artery disease(CAD).Methods A retrospective case control study was designed in this investigation.Patients with angiographically proven CAD (n=142)and coronary atherosclerosis(n=215)were compared to healthy volunteers(n=631). Genotyping was performed by ShineRoar probes analysis.Results People with GG genotype at position 196 in exon 6 of TNFR2 gene have more risks of development of CAD(OR=2.556,95% CI:1.051~6.218, χ2=4.57,P=0.033)and CA(OR=2.547,95% CI:1.162~5.579,χ2=5.81,P=0.016) than those with TT genotype.Further analysis after adjustment for sex and age demonstrated that GG genotype was not associated with CAD(OR=0.614,95% CI:0.166~2.279,χ2=0.53,P=0.466)and coronary atherosclerosis(OR=O.644,95%CI:0.200-2.069,χ2=0.55,P=0.459).Conclusion Tumor necrosis factor receptor 2 gene polymorphism at position 196 in exon 6 was not an independent risk factor of development of CAD and coronary atherosclerosis among Han Chinese.
8.Clinicopathologic features and prognosis of the young pulmonary primary salivary gland tumor

Purpose To study the clinicopathologic features retrospectively and to explore the prognosis of 15 cases of young pulmonary primary salivary gland tumor. Methods The 15 cases of young pulmonary primary salivary carcinoma were analyzed respectively. Re-sults The 15 cases showed 7 cases with adenoid cystic carcinoma and 8 cases with mucoepidermoid carcinoma. Cough and bloody sputum were main symptoms, while chest pain, chest distress and fever were less seen. There were 10 cases performed with partial pul-monary lobectomy, 3 cases performed with unilateral total lobectomy, 1 case performed with artery perfusion treatment and bronchial ar-tery embolization and 1 case performed with conservative treatment for pancreatic metastasis after diagnosis. The mean follow-up time was 28 months ( from 4 to 96 months) . Conclusion The primary salivary gland-type lung tumor belonged to benign or low-grade ma-lignant tumor, mainly accompanied with hacking cough and bloody sputum, which was commonly found in trachea and bronchus. Final diagnosis could be made through bronchoscopy and biopsy. Complete surgical excision is the primary method and the prognosis is usual-ly good, especially in the young.
9.Detection of ALK protein in lung squamous cell carcinoma with immunohistochemistry and FISH

Purpose To investigate the occurrence of echinoderm microtubule-associated protein-like 4 (EML4) gene and the anaplas-tic lymphoma kinase (ALK) gene rearrangements in lung squamous cell carcinomas (SCC), and to provide with a test for targeted therapy of SCC. Methods In this study, we analyzed ALK protein expression with a specific rabbit monoclonal Ig antibody ( D5F3 clone) in 219 cases of lung SCC. The positive cases were confirmed with ALK fluorescence in situ hybridization ( FISH) . Results 4 out of 219 (1. 8%) cases of lung SCC were ALK positive detected by immunohistochemistry (IHC) staining, which were confirmed by ALK FISH. Conclusions There are SCC having EML4-ALK gene rearrangement. The response of SCC patients with ALK expression to targeted therapy of Crizotinib should be explored. Strong positive expression of ALK protein can be interpreted as EML4-ALK gene rearrangement.
10.Electromagnetic noise blocks the gap-junctional intercellular communication suppression induced by 50 Hz magnetic field.

OBJECTIVETo explore whether the superposition of an electromagnetic noise can block gap-junctional intercellular communication(GJIC) suppression induced by 50 Hz 0.4 mT extremely low frequency magnetic field(ELF MF).
METHODSFibroblast cells of mice(NIH 3T3) were exposed to 0.4 mT ELF MF or(and) electromagnetic noise with the same intensity of MF for 24 h, and the GJIC was determined by using fluorescence recovery after photobleaching(FRAP) analysis, which was performed with a laser-scanning confocal microscope(Leica, Germany).
RESULTSELF MF exposure significantly inhibited GJIC with fluorescence recovery rate of 27.67% +/- 5.12% as compared with the control group(45.57% +/- 9.72%) (P < 0.01), while that of ELF MF plus noise group was (52.61% +/- 8.30%), which was significantly different from ELF MF group(P < 0.01), but not from control(P > 0.05).
CONCLUSIONElectromagnetic noise could block the GJIC suppression induced by 50 Hz 0.4 mT MF.
