In order to prevent the teaching reform of organ system curriculum from fall of teaching quality, the teaching effect of organ system curriculum and subject curriculum system in teaching the basic knowledge of pathophysiology was compared. In organ system curriculum teaching, there was no difference between the grades of students in Batch 2011 and Batch 2012 who conformed to the teaching principle of "gradual improvement" and those of the students taught with subject curriculum system. On the contrary, the students of Batch 2013 and Batch 2014 with insufficient curriculum content integration had a decreasing trend or a significant reduction in the teaching effect of organ system curriculum compared with that of subject curriculum system. After the supplementary for curriculum knowledge was made, the teaching effect of the organ system was significantly improved, which was better than that of the subject curriculum system. In conclusion, we have summarized and reflected on the effectiveness of the teaching reform of organ system curriculum, once again proved that basic medical teaching must also follow the inherent law of medical education, which is the teaching principle of "gradual improvement."

Objective To investigate the characteristics of subcutaneous panniculitis-like T cell lymphoma ( SPTCL) in order to facilitate prompt identification and proper treatment of this rare and heterogeneous disease entity. Methods For 8 patients who had been misdiagnosed as rheumatic diseases but eventually confirmed as SPTCL though pathology and immunohistochemistry, a retrospective chart review was made with regard to their clinical symptoms, laboratory test results, pathological features, responses to therapy as well as outcomes. Results These 8 patients with a male to female ratio of 1: 1 were initially misdiagnosed as a variety of rheumatic diseases such as erythema nodosa, nodular panniculitis, systemic vasculitis, etc. The period from the onset of symptoms to the confirmation of diagnosis as SPTCL was 28. 6 months on average (range, 4-84 months). All the cases presented with multiple subcutaneous nodules, plaques or tumors which involved various anatomic sites including the head and neck, the trunk, and the extremities. Fever was the most frequently accompanying symptom (7/8) , followed by lymphadenopathy (4/8) , hepatomegaly (3/8), splenomegaly (3/8). Hemophagocytic phenomenon was seen in 3 cases.A total of 22 times of biopsy involving multiple anatomic sites were performed on these 8 cases with 2. 75 times on average (range, 1-5 times). All cases demonstrated a dense lymphoid infiltrate with significant cellular heteromorphism located in the subcutaneous tissue. CD_3 was positive in the majority of the cases.Immunostaining for γδTCR was positive in one case. The anti-rheumatic therapy including steroids and immunosuppressants administered before the identification of SPTCL attained minimal therapeutic effect. In contrast, 6 cases gained partial response after chemotherapy except that the other 2 cases died of fatal pulmonary infiltration and subsequent infection. Conclusions SPTCL is a rare and heterogeneous entity which is unseldomly misdiagnosed as rheumatic disease. The anti-rheumatic therapy including steroids and immunosuppressants can attain minimal therapeutic effect. Early identification by means of histology and immunohistochemistry as well as immunostaining for PCR is critical for proper treatment.

In this study, modification technology by surface coating was used to improve the flowability of powder of Chinese herbs extracts. Seven kinds of powder of Chinese herbs extracts were coated with 1% silica nanoparticles using an under-driven Comil. The powder characteristics tester was used to evaluate the flowability of uncoated and coated powders. Surface morphology and particle size distribution of powders were compared by scanning electron microscope (SEM) and optical microscope. The powder hygroscopicity was also investigated. The results showed that, after modification, angle of repose, angle of spatula, compressibility and adhesiveness extremely decreased, and flowability index substantially increased, the powder flowability was significantly improved, especially Gegen and Dahuang extracts powders. Scanning electron microscopy images showed the distribution of silica nanoparticles on the host drug particle surfaces. There were no remarkable changes in the particle size distribution and hygroscopicity of all powders after coating. Therefore, Comilling for surface coating modification technology was an effective method to improve the flowability of Chinese herbs extracts and suggested a possible way forward to enhance the quality of Chinese drugs pharmaceutics in their study and manufacture.

Objective To compare the static standing balance of stroke patients after different biofeedback protocols. Methods Thirty-two stroke patients were randomly divided into a knowledge of performance (KP) group,a knowledge of results (KR) group and a control group. All 3 groups received 4 weeks of conventional rehabilitation training plus another 30min of static standing balance training per day.The KP group received audio-visual feedback in real time during the training. The KR group received section result feedback. The control group received no feedback during the extra balance training. Before and after the training, the performance of the 3 groups was evaluated using Berg's Balance Scale (BBS) and a portable biofeedback device. Results Average BBS performance improved significantly more in the KP group (3.08± 1.08) than in KR group (1.30±0.67) and control group (1.20± 0.79) (P<0.05).No significant difference was detected between the KR and control groups (P>0.05).The average improvements of the KP group in terms of Standing with Eyes Closed and Tandem Standing (0.92±0.79 and 0.83± 0.39) were significantly highcr than those in the KR (0.30± 0.48 and 0.20± 0.42) and control groups (0.01 ± 0.01 and 0.40±0.52) (P<0.05).Average trunk angular displacements in all four directions [Anterior (2.83±0.93;6.15± 1.85),Posterior (2.56±0.88;5.97±1.74);Left (2.86±1.16;6.49±2.42),Right (2.68±1.43;5.98±2.05)] in the KP group was significantly higher than in the others (P<0.05).No significant differences were detected between the KR and control groups in BBS results or in posture. Conclusions Static standing training should incorporate real time biofeedback. It is then more effective than conventional standing training or training with section results feedback. It is worth spreading in clinical applications.

Objective To explore the difference in pulmonary function between overlap syndrome and chronic obstructive pulmonary disease (COPD) in patients. Methods One-hundred-ninty-four patients came from respiratory department in Tianjin Medical University General Hospital, from October 2012 to August 2014, were included in this study. After inquisition of medical history, evaluation of lung function and polysomnography (PSG), patients were divided into overlap syndrome (OS, n=78) group, COPD group (n=76) and healthy control group (n=40). The indexes of lung function and the pulse oscillation index of pulmonary function were compared between three groups. Results Values of the peak expiratory flow rate (PEF) and the remaining 75%FVC when expiratory flow rate (MEF75) were significantly decreased in OS group than those of COPD group (P<0.05). Patients with sleep apnea hypopnea index (AHI)≥30 times/h showed significantly decreased MEF25 and the maximum mid expiratory flow (MMEF) in OS group than those of COPD group (P<0.05). Values of the total impedance (Zrs), the resonant frequency (Fres), viscosity resistance when oscillation frequency was 5 Hz (R5), R20 were significantly increased in OS group than those of COPD group (P<0.05). Patients with AHI≥30 times/h were increased significantly than COPD group in R5-R20 and X5 besides Zrs, Fres, R5, R20. Conclusion There is more serious airway obstruction in patients with OS than that in patients with COPD. Severe sleep disordered breathing can aggravate small airway obstruction in patients with COPD.

Tianjin Medical Journal 2015;(1):109-112
The key treatment for overlap syndrome should focus on maximizing the therapeutic effect of each condition and concentrate of interventions that have showed benefit in both diseases. The goal of therapy includes the improvement in objective data including the reduction in sleep fragmentation, exacerbation rate, hospitalization frequency and mortality, and subjective data such as daytime functioning, quality of life and sleep quality. At present, the treatment of overlap syndrome is mainly rely on the positive airway pressure to improve ventilatory capacity and gas exchange function. Oxygen therapy as auxiliary treatment can alleviate hypoxemia. The key of drug therapy is to dilate the bronchus, clear secretions, reduce in?flammatory response to improve the ventilation function. Meanwhile, daily exercise and dietary habits of life activity are indis?pensable in the treatment of the disease as important supporting role to improve the motor function and the quality of life in patients.
