3.Mutagenicity of Organic Extract From Drinking Water in a Certain City

Objective To have a knowledge of the mutagenicity of contaminates in drinking water in a certain city. Methods Mutagenic activity of organic extracts of source water chloridized water in water plant and tap water from the city were detected with single cell gel-electrophoresis mouse primary hepatocytes in culture were used and Ames test. Results For Ames test the organic extract in 3 liters chloridized water in water plant and tap water samples produced positive result and that in 6 liters source water sample still gave a negative result. For single cell gel-electrophoresis test the organic extract in 0.1 liter chloridized water in water plant tap water and 0.5 liter source water produced positive result. Conclusion The sensitivity of single cell gel-electrophoresis with primary hepatocytes in detection of genotoxicity of cloridized water and source water is much higher than Ames test.
4.Effects of community-based nursing intervention in people with prehypertention

Objective To evaluate the effects d community-based nursing intervention in people with prehypertention. Methods A total of 120 residents with prehypertension were randomly divided into intervention group (60 cases) and control group (60 cases). The intervention group received 1-month commuity-based nursing intervention including group education, individual living manner intervention and exercise guiding. No special measures were given to the control group. Behavior improvement scores, blood pressure and bodyweight index were measured before and after the intervention. Results The score of the intervention group was significantly lower than before intervention,and was significantly lower than that of the control group as well (T=18.01,1g 89,21.60,9.44;t =21.05,20. 48,10. 50,5. 57;P <0. 01). Behavior improvement scores,blood pressure and bodyweight index in the intervention group were significantly lower than those before intervention and those of the control group (T=4. 18,4.11,1.88;t =4.20,5.08,2. 15;P<0. 01 or P<0.05).Conclusions Community-based nursing intervention can lower blood pressure of people with prehypertension. It also has a great significance in preventing high blood pressure.
5.Early results of coronary endarterectomy combined with coronary artery bypass grafting in patients with diffused coronary artery disease.

BACKGROUNDIt is still a challenge for the cardiac surgeons to achieve adequate revascularization for diffused coronary artery disease (CAD). Coronary endarterectomy (CE) offers an alternative choice of coronary artery reconstruction and revascularization. In this study, short-term result of CE combined with coronary artery bypass graft (CABG) was discussed in the treatment for the diffused CAD.
METHODSFrom January 2012 to April 2014, 221 cases of CABG were performed by the same surgeon in our unit. Among these cases, 38 cases of CE + CABG were performed, which was about 17.2% (38/221) of the cohort. All these patients were divided into two groups: CE + CABG group (Group A) and CABG alone group (Group B). All clinical data were compared between the two groups, and postoperative complications and in-hospital mortality were analyzed. The categorical and continuous variables were analyzed by Chi-square test and Student's t-test respectively.
RESULTSDiabetes mellitus, hypertension, hyperlipidemia, and peripheral vascular disease were more common in group A. In this cohort, a total of 50 vessels were endarterectomized. Among them, CE was performed on left anterior descending artery in 11 cases, on right coronary artery in 29 cases, on diagonal artery in 3 cases, on intermediate artery in 2 cases, on obtuse marginal artery in 5 cases. There was no hospital mortality in both groups. The intro-aortic balloon pump was required in 3 cases in Group A (3/38), which was more often than that in Group B (3/183). At the time of follow-up, coronary computed tomography angiogram showed all the grafts with CE were patent (50/50). There is no cardio-related mortality in both groups. All these patients were free from coronary re-intervention.
CONCLUSIONSCoronary endarterectomy + CABG can offer satisfactory result for patients with diffused CAD in a short-term after the operation.

6.Prospective randomized controlled trial of antibiotic prophylaxis for newly placed peritoneal dialysis catheter to prevent postoperative peritonitis and wound infection

Objective To assess the efficacy of two antibiotic prophylactic regimens in a prospective randomized trial in 1 year for patients undergoing insertion of catheters,and to provide the evidence for uniform consensus existing on the timing,route,and choice of antibiotic.Methods During a period of 12 months,78 patients,who consecutively entered the peritoneal dialysis programme,[45 women and 33 men,mean age (48.2?15.7)years] were included.The prophylactic regimens were a single dose of ceftriaxone (1.0 g) given intravenously 30 minutes before surgery (Group A) and given cefazolin (0.25 g/L) i.p.in the each dialysis bag for 3 days postoperatively (Group B).All operations were performed in one room.The wound was observed every day,and body temperature,Count of white blood corpuscle and type,dialysate were examined every day. Results In Group A and B,none of the patients showed peritonitis or wound infection during the post-operative period (within 10 days).One of 39 patients(2.5%) in the group A,and 2 of 39 patients (5.1%) in the group B had exit site infection (P>0.05).Conclusions There is no significant difference in the incidence of peritonitis and wound infection between two groups. Prophylactic preoperative single-dose antibiotics intravenously do as well as antibiotics given intraperitoneally for peritoneal dialysis catheter insertion,but is much more convenient.
7.Relationship between mannose-binding protein gene polymorphisms and disease progression and HBV DNA in patients with chronic HBV infection

Objective To determine the influences of Mannose binding protein (MBP) gene polymorphisms on HBV DNA loads and on the progression of liver disease in patients with chronic HBV infeclion.Method The Codons on 54 MBP gene polymorphisms and HBV DNA loads in a cohort of 395 patients with chronic HBV infection,including 244 with chronic hepatitis B (CHB),151 with liver cirrhosis(LC) and 88 normal controls were examined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and fluorescent quantitative PCR (FQ-PCR).Result The MBP genotype frequencies of GGC/GAC and alleles genetic frequencies of GAC in CHB group showed no significant differences comparing to the normal control group ( P > 0.05 ).The MBP genotype frequencies of GGC/GAC and alleles genetic frequencies of GAC on CHB group (severe),compensation phase of LC group and decompensation phase of LC group were higher than those in the normal control group (P < 0.05 ),the genetic polymorphism of decompensation of LC was 36.5 %,highest of all.The MBP genotype frequencies of GGC/GAC and alleles genetic frequencies of GAC of patients with chronic HBV infection were not changed with the differences of HBV-DNA loads.Conclusion The codes on 54 MBP gene polymorphisms is not closely related to HBV DNA loads,but was associated with the progression of hepatitis B infection.
8.Apoptosis peripheral of T lymphocytes in patients with acute exacerbration of chronic bronchitis

OBJECTIVE: To compare peripheral T lymphocyte apoptosis of chronic bronchitis during an acute episode with that during a stable period. METHODS: Seventeen Chronic Btonchitis patients who presented to the ER with an acute episode were compared with 10 stable Chronic Bronchitis patients seen in the out-patient clinic. Peripheral venous blood was obtained from each patient and T lymphocytes were separated by fluorescent monoclonal antibodies. CD3, CD95 and CD25 were measured by flow cytometry. RESULTS: In the acute episode patients, the total quantity of T lymphocytes with CD3 was (42.63 +/-16.37) and with CD95 (14.15 +/-11.06). In the stable group T lymphocyte CD3 was (64.22 +/-23.90) and CD95 (34.88 +/-8.92) (t=-4.238, P <0.001; t=-8.796, P <0.001). The total quantity of Lymphocyte CD25 did not differ significantly between the two groups (P > 0.05). Total quantity of active T lymphocytes with CD3/CD25 was lower in the acute episode group (6.26 +/-3.94) compared to the stable group (10.52 +/-3143) (t=-1.068, P <0.05). CONCLUSION: T lymphocyte apoptosis increased in Chronic Bronchitis patients who presented with an acute episode. They also experienced a significant disturbance in immune function.
9.Outcome evaluation on rebuilding stove and health education in preventing coal-burning fluorosis and arsenic poisoning in Shaanxi Province

Objective To evaluate the effects of rebuilding stove and health education on preventing coal-burning fluorosis and arsenic poisoning in Shaanxi in 2005.Methods According to "Scheme of Impmving Stove in Preventing Coal-burning Fluorosis and Arsenic Poisoning of Shaanxi in 2005",the initial meeting was convened,while liability contracts were signed,leading and technical guiding groups were established,professional training was carried out.On the basis of the epidemiologic data,stoves were improved in 7 chosen counties in Ankang and Hanzhong City where the health education in several modalities was carried out.The project was checked and accepted when the work was completed.Thirty children in fourth grade were randomly selected in one primary school of each county.Fifleen adults aged 16 years old were chosen randomly in each village in each country.They were asked to answer the questionnaire about the health knowledge.Results There were 955 322 stoves improved in 7 countries in Ankang and Hanzhong City accounting for rebuilding stove was 100%(95 322/95 314).The awareness rates of health knowledge were 88%(444/508)in the adults and 100%(210/210)in children.Conclusions The government mangement leadership,the cooperation between the related departments, the participation of residents and the assufance of fund are the essentials in long lasting control of endemic diseases.
