1.Training model construction and education reform exploration of eight-year medical education

Objective To construct and implement the training model of eight-year medical education with characteristics of Shanghai Jiaotong University. Methods Based on survey,discussion and consultation,the experiences of long schooling medical education in Shanghai Jiaotong University School of Medicine were summarized.Training plan and education reform scheme were established. Results Training objective,guideline and major reform measures had been clarified.The training plan and reform scheme were under process of implementation. Conclusion The training objective of eight-year medical education should be further confirmed.The curriculum should be in accordance with the training objective,and education reform is important and necessary for the eight-year medical education.
2.Analysis of appropriate ecological environment of Himalayan marmot based on remote sensing and geographic information system in Qinghai province

Objective To assess the quantitative relationship between the distribution of Himalayan marmot and its ecological environment,the terrain,the temperature and the precipitation,using remote sensing and geographic information system in Qinghai province.Methods The distribution of Himalayan marmot was located by Google Earth and ArcGIS software and by using field survey data provided by Chinese Center for Disease Control and Prevention.The corresponding ecological environment of marmot including terrain,temperature and precipitation were derived from the spatial information datasets.All results were processed according to the overlay and statistics analysis using ArcGIS software.Results Seventy-seven point twenty-seven percent(153/198) of Himalayan marmot were distributed in the area of elevation between 3000 and 4000 meters.The number of marmot reached the highest when the slope was between 0 and 17 degrees,and aspect range was between 91 and 270 degrees,180 degree was as south direction.During the period with the maximum temperature of the warmest month of 14.3-17.5 ℃,17.6-20.8 ℃ and 20.9-24.0 ℃,the distribution of marmot reached 95%(186/198) of the total area.Meanwhile,most of the marmot were presented in the area with average precipitation of 46-108 mm.Conclusions A quantitative analysis of appropriate ecological environment of Himalayan marmot in a large scope is carried uul successfully using remote sensing and geographic information system.The study indicates that spatial information technology has important applications in plague prevention and control.
3.Effects of human-computer interaction intelligent compression package for heparin injection in patients with orthopedic surgery

Objective To test the effects of human-computer interaction intelligent compression package for heparin injection in patients with orthopedic surgery.Methods From July 2015 to June 2016,108 patients with orthopedic surgery were enrolled in the study,and were divided into the experimental group (54 cases) and the control group(54 cases) by random number table.The experimental group was given heparin injection with human-computer interaction intelligent compression package for 3 min,while the control group was given manual compression for 3 min.We evaluated the incidence and severity of subcutaneous hemorrhage and nurses' operating time of two groups.Results The incidence of subcutaneous hemorrhage was 3.9% in the experimental group,12.4% in the control group(P<0.05).The operating time was(100.4±8.7 s) for the experimental group and(233.8±15.3 s) for the control group (P<0.01).Conclusion Using human-computer interaction intelligent compression package can reduce the incidence of subcutaneous hemorrhage.It can also reduce the working hours of nurses and optimize human resources.
4.Risk factors of the failure in digit replantation.

OBJECTIVETo explore the relative factors on the failure in digit replantation in order to take preventions to control the risk factors.
METHODSFrom January 2013 to December 2013, 236 consecutive patients (311 fingers) underwent digit replantation were collected to analyze retrospectively, involving 183 males and 53 females with an average age of 34.5 years old ranging from 2 to 62 years old (6 cases under 6 years old and 230 cases elder than 6 years old). There were 51 thumbs, 87 index fingers, 78 middle fingers, 63 ring fings and 32 little thumbs. Forty cases(forty fings) who were failured as the observation group, the others as the control group. The factors of age, gender, finger, cause of injury, smoking history, ischemia duration, plane of division, condition of venous drainage and condition of arterial repair we assessed.
RESULTSAll 236 cases with 311 fingers were replanted, 40 fingers were failured after operation. The relative factors on the failure in digit replantation included smoking history, cause of injury, plane of division, condition of venous drainage and condition of arterial repair (P< 0.05). There were no significant correlation between the failure and age, gender, finger and ischemia duration (P>0.05).
CONCLUSIONSmoking history, causes of injury, plane of division, condition of venous drainage and condition of arterial repair are risks of failure in digit replantation. Before choosing the type of operation, it should be think about the patient's general conditions, injury status, grasp firmly the operative indications and actively carry out surgical treatment.

5.Comparative observation of the efficacy on acute gouty arthritis between acupuncture combined with infrared irradiation and western medicine.

OBJECTIVETo compare the efficacy difference in treatment of acute gouty arthritis between acupuncture combined with infrared irradiation and Indomethachin as well as observe the impacts on liver function.
METHODSOne hundred and sixty cases of gout were randomized into an acupuncture group (80 cases) and an Indomethachin group (80 cases). In acupuncture group, acupuncture was applied to Zusanli (ST 36), Sanyinjiao (SP 6), Quchi (LI 11), Xuehai (SP 10), Yanglingquan (GB 34) and Ashi points. Additionally, infrared irradiation was used in the local area. The treatment was given once daily. In Indomethachin group, Indomethachin was taken orally, 25 mg each time, three times per day. The treatment lasted for 5 days in either group. Separately, before and after treatment, pain severity and the levels of blood uric acid (BUA), erythrocyte sedimentation rate (ESR), alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were detected. Additionally, the efficacy and the impacts on liver function were assessed.
RESULTSThe curative rate was 52.5% (42/80) in acupuncture group, which was superior to 22.5% (18/80) in Indomethachin group (P < 0.01). In acupuncture group, the analgesia efficacy was better than that in Indomethachin group (P < 0.01) and the effect on reducing BUA and ERS levels was same as that in Indomethachin group (all P > 0.05). After treatment, ALT and AST levels had no changes in acupuncture group, but they increased apparently in Indomethachin group (all P < 0.01).
CONCLUSIONAcupuncture combined with infrared irradiation achieves superior efficacy on acute gouty arthritis as compared with oral medication of Indomethachin and the therapy provides a significant effect on analgesia and does not bring the damage of liver function.

6.Median effective target effect-site concentration of remifentanyl inhibiting cardiovascular response to tracheal intubation in women with preeclamp-sia under intravenous injection of labetalol

Objective To determine the median effective concentration (EC50) of remifentanyl inhibiting cardiovascular response to tracheal intubation in women with preeclampsia. Methods Thirty women with preeclampsia were enrolled in this study. The target Ce was set at 3 ng/mL. The positive cardiovascular response was defined as the blood pressure≥160/110 mmHg. The EC50 (95%confidence interva1) of cardiovascular responses to tracheal intubation was calculated by Probit analysis. Results EC50 (95% confidence interval) of remifentanyl inhibiting cardiovascular response to tracheal intubation was 1.93 (1.89~1.95) ng/mL. Conclusion EC50 of remifentanyl inhibiting cardiovascular response to tracheal intubation is 1.93 ng/mL in women with preeclampsia.
7.The effects of triiodothyronine on energy metabolism and myocardial protection

Objective To observe the effects of triiodothyronine (T3) on myocardial energy metabolism and its protection of the myocardial cells in rat ischemic reperfusion myocardial model with different thyroid functions. Methods The rats were categorized into group A (with normal thyroid function) and group B (with hypothyroidism), and each group were divided into blank control group, ischemic group, control reperfusion group and T3 reperfusion group. An isolated rat heart working model was established. The contents of ATP, ADP, AMP in the myocardial tissue in different groups were measured by high-performance liquid chromatographic method. Results The contents of ATP, ADP, AMP declined markedly in hypothyroidism group (P<0.01). Thirty minutes of ischemic arrest induced significant decrease of, ATP, ADP and AMP contents both groups A and B, which further declined when the hearts were subjected to 20 min of reperfusion. However ,the content of ATP increased markedly in T3 reperfusion groups compared with the control reperfusion groups in both group A:[(1.03±0.02) vs (0.62±0.02) mmol/g and group B:[(0.85±0.02) vs (0.40±0.01)] mmol/g (P<0.01). Conclusions Hypothyroidism or ischemic reperfusion injury may badly damage myocardial energy metabolism; T3 coold markedly increase the capacities of synthesizing and reserving ATP, improving myocardial protection.

9.Expression of green fluorescent protein gene in mouse T lymphocytes mediated by lentiviral vector.

This study was purposed to constructe the three-plasmid system of the lentiviral vector carrying the green fluorescent protein (GFP) gene and to investigate the expression of GFP in T lymphocytes of the mouse. The polypurine tract (PPT) element, ubiquinone promoter (PUB) and GFP were ligated to plasmid pLO134 using subcloning technology to construct plasmid pTK153. Human kidney 293T cells were co-transfected with the three-plasmid system containing packaging plasmid DeltaNRF, plasmid pTK153 and envelope plasmid VSV-G by using calcium phosphate DNA precipation and the expression of GFP was observed under fluorescence microscope after 12 hours. The viral particles were collected after transfection 72 hours, were frozen at -80 degrees C and were used to infect mouse T lymphocytes at multiplicity of infection (m.o.i.) of 3. The expression of GFP in mouse T lymphocytes was observed by fluorescence microscopy and fluorescence-activated cell sorting (FACS). The results showed that the transfection efficacy was 63.04 +/- 7.24% in 293T cells analysed by FACS and the viral titer was (3.09 +/- 0.61) x 10(6) U/ml. The expression of GFP was also evident in mouse T lymphocytes and the transduction efficacy was (37.98 +/- 6.26)%. It is concluded that the three-plasmid system of lentiviral vector containing GFP gene is successfully constructed and the transduction efficacy is high in mouse T lymphocytes.

10.Rapid tooth movement through distraction osteogenesis of the periodontal ligament in dogs.

BACKGROUNDAnimal models are needed for the study of rapid tooth movement into the extraction socket through distraction osteogenesis of the periodontal ligament.
METHODSModified distraction devices were placed on eight dogs between the first and third mandibular premolars on the left sides; similar placement of traditional straight wise appliances on the right sides served as the control. The experimental distractors were activated (0.25 mm/d) twice a day and the control devices were activated (100 g) for two weeks with consolidation periods at weeks two, three, six, and ten. Two dogs were sacrificed at each consolidation time point; rates and patterns of tooth movement, loss of anchorage, and periapical films were evaluated, and the affected premolars and surrounding periodontal tissues were decalcified and examined histologically. General observations, X-ray periapical filming and histology examination were performed.
RESULTSDistal movement ((3.66+/-0.14) mm) measured two weeks after modified distraction exceeded that achieved using the traditional device ((1.15+/-0.21) mm; P<0.05). Loss of anchorage was minimally averaged (0.34+/-0.06) mm and (0.32+/-0.07) mm in the experimental and control sides, respectively. By radiography, apical and lateral surface root resorptions on both sides were minimal. Alveolar bone lesions were never evident. Fibroblasts were enriched in periodontal ligaments and bone spicules formed actively along directions of distraction.
CONCLUSIONSThe canine model is suitable for the study of rapid tooth movement through distraction osteogenesis of the periodontal ligament. The technique accelerates tooth movement, periodontal remodeling, alveolar bone absorption, and may induce fibroblast formation, as compared to the traditional orthodontic method, without adversely affecting root absorption, bone loss, tooth mobility and anchorage loss.
