1.Correlation between blood-stasis tongue figure and platelet activating factor (PAF) and acetyl hydrolase of PAF (PAF-AH) in patients with primary dysmenorrhea.

OBJECTIVETo explore the pathologic mechanism of blood-stasis tongue figure (BSTF) formation in patients with primary dysmenorrhea.
METHODSBlood levels of platelet activating factor (PAF) and acetyl hydrolase of PAF (PAF-AH) in 41 patients with primary dysmenorrhea and 20 healthy subjects were detected by enzyme linked immunosorbent assay (ELISA).
RESULTSThe level of PAF in the 22 patients with BSTF was 252. 214 +/- 37. 568 ng/L, which was higher than that in patients without BSTF (19 patients, 212.348 +/- 22.794 ng/L) and healthy subjects (182.126 +/- 18.306 ng/L) respectively, while level of PAF-AH showed an opposite sequence in them, i.e., 3.090 +/- 1.483, 5.382 +/- 1.873, and 5.607 +/- 2.073 ng/L, respectively (P < 0.05). Patients without BSTF showed only a higher level of PAF when compared with that in healthy subjects (P < 0.05). No significant difference in PAF or PAF-AH levels was shown among patients with BDTF of different Chinese medical syndrome types (P > 0.05).
CONCLUSIONPAF level obviously increased and PAF-AH level obviously decreased in primary dysmenorrhea patients of BSTF, suggesting that the imbalance of PAF and PAF-AH was correlated with the pathologic mechanism of the BSTF formation in primary dysmenorrhea patients.

2.Three cases of enterovirus 71 infection with pulmonary edema or pulmonary hemorrhage as the early clinical manifestation

Objective To investigate the clinical features of the enterovirus 71 ( EV71 ) infection complicated with pulmonary edema or pulmonary hemorrhage as a fulminant and often fatal illness.Methods The medical records of three cases with EV71 infection were retrospectively reviewed for clinical manifestations, laboratory data, medications, and outcome.Results All the cases were infants and died of the infection. These infants had no skin or mucosal lesions, however, they had sudden onset of cyanosis and tachypnea 1 to 2 days after the onset of the febrile disease with vomiting. All these 3 cases were misdiagnosed and were treated for shock on admission. Pulmonary hemorrhage was not considered in any of the cases on admission. All the cases received tracheal intubation when foamy secretions were discharged from the mouth and nose of the patients and notable cyanosis occurred. After intubation, pink foamy fluid flew out from the endotracheal tube in all the 3 cases. The patients had hyperglycemia and limb weakness, two had tachycardia, and hypertension was found in one case. Chest X-ray showed bilateral or unilateral widespread air space opacity, but the cardiac size and shape were normal. All the patients had leukocytosis. Enterovirus 71 infection was confirmed by detection of specific nucleic acid sequences of the virus from throat swab and tracheal secretions samples and in one case in cerebrospinal fluid.Conclusions Pulmonary edema or pulmonary hemorrhage occurred in the 3 cases with EV71 infection. The initial presentation was nonspecific with fever and vomiting, and sudden appearance of cyanosis, tachypnea, tachycardia, hypertension or hypotension, limb weakness, which may suggest pulmonary edema or hemorrhage. Excessive fluid resuscitation may deteriorate the illness, on the contrary, fluid restriction and inotropic agents, and early intubation with positive pressure mechanical ventilation may be the proper treatment.
3.Forty two cases infected with swine streptococcosis

Objective To investigate the clinical characteristics,diagnosis,treatment and prognosis of human swine streptococcosis occurred in some areas of Jiangsu Province from late summer to autumn since 1998.Methods The epidemiologic and clinical features of 42 cases were collected and analyzed.The bio- chemical features of strains isolated from patient's blood or cerebrospinal fluid(CSF) were tested,and the homogeneity were compared among 15 Streptococcus suisⅡ.Results All patients had acute infection toxe- mic symptoms such as chill,fever,headache and malaise etc.Toxic shock syndrome or meningitis syndrome were the major clinical manifestations.Forty two cases of human swine streptococosis were classified into 3 types:the rates of general,shock and meningitis type were 7.1% (3/42),38.1% (16/42) and 54.7%(23/42),respectively.Ten patients were died of shock type,32 were cured.Strain isolated from patients was identified as Streptococcus suisⅡby API-Strep,the biochemical reactional code was 0641473,and appraised result was 99.9%.There was highly homogeneity in the strains of Streptococcus suisⅡisolated from patients and sick pigs identified by genomic fingerprinting.Com- bined therapy of large doses of penicillin G and ceftriaxone was effective in these patients.Conclusions Human swine streptococosis is zoonosis caused by Streptococcus suisⅡand the clinical manifesta- tions are variable.In the cases of shock type,the onset of disease is stormy and the fatality rate is very high.While the prognosis of general and meningitis type is good and the majority of the cases are cured by effective antibiotic therapy.
4.Dynamic changes of plasma VEGF, SDF-1 and peripheral CD34+ cells in patients with acute myocardial infarction.

OBJECTIVETo investigate the changes in plasma levels of vascular endothelial growth factor (VEGF) and stromal cell-derived factor-1 (SDF-1) and in peripheral CD34(+) cells in patients with acute myocardial infarction (AMI), and explore their role in AMI.
METHODSEnzyme-linked immunoassay (ELISA) was employed for measuring the levels of VEGF and SDF-1 in AMI patients on days 1, 3, 7, 10, and 14 of onset and in normal control subjects. The absolute counts of CD34(+) in the peripheral blood were measured on days 1, 7, and 14 by flow cytometry in AMI patients, with their myocardial enzyme and troponin I detected and electrocardiography (ECG) and echocardiography (UCG) recorded.
RESULTSPeripheral CD34(+) cells obviously increased on day 7 after AMI onset (2.35-/+0.72/microl vs 1.48-/+0.49/micro, P<0.05). VEGF levels were significantly higher in AMI patients than in the control subjects, reaching the peak level and on day 14 (197.56-/+39.87 vs 53.79-/+18.12 pg/ml, P<0.01). SDF-1 level obviously decreased on day 1 after AMI onset (1683.12-/+224.79 vs 2178.67-/+265.34 pg/ml, P<0.01), followed by gradually increased to the control level. Obvious correlation was noted between the level of VEGF on day 7 and the peak level of peripheral CD34(+) cells, and the peak plasma VEGF level was obviously associated with the peak serum CK-MB and troponin I levels.
CONCLUSIONThe stem cells are mobilized into the peripheral blood in the event of AMI. Obviously increased VEGF level following AMI may persist for at least 2 weeks, whereas SDF-1 level undergoes temporary decrement after AMI. The dynamic changes of VEGF and SDF-1 can be related to the mobilization and homing of the stem cells to the injured myocardium.

5.A study on the correlation between serum ENA -78 expression and pelvic inflammatory adhesion and pain grading among patients with endometriosis

Objective To observe the changes of ENA -78 expression levels in patients with endometriosis (EM)before and after treatment,and the role of ENA -78 expression in pain and pelvic inflammation.Methods A total of 160 patients with EMwere selected as the research subjects for I phase,II phase,III phase,IV phase patients.In the same period,40 cases of non EMpatients were selected as control group.Serum ENA -78 expression levels were detected among patients with EM before treatment,3 months after treatment and 6 months after treatment,and the control group before treatment by the method of ELISA .The correlation between ENA -78 expression and pelvic inflammatory adhesion and pain grading among patients with EMwas analyzed.Results There was significant difference in the level of serum ENA -78 expression between the two groups at different time points (P <0.01).The expression level of serum ENA -78 of case group decreased with time (P <0.01),while the expression level of serum ENA -78 of EM patients with different pain grading was different (P <0.05).The expression level of ENA -78 in severe group was higher than that in mild group and moderate group (P <0.05).The degree of pelvic adhesion in EMpatients was positively correlated with the level of ENA -78 expression (rs =0.675,P =0.000).The expression of ENA -78 in serum of EMpatients with different degrees of pelvic inflammation was different (P <0.05 ),and the expression of V -grade group ENA -78 was higher than other
groups (P <0.05).The level of ENA -78 expression was positively correlated with the degree of pain in patients with EM (rs =0.601,P =0.000).Conclusion Serum ENA -78 expression levels among EM patients were high,and positively correlated with pelvic inflammatory adhesion and pain grading.Serum ENA -78 decreased after GnRh -a treatment.

OBJECTIVETo investigate the chemical constituents of Dendrobium chrysotoxum.
METHODThe chemical constituents were isolated by various column chromatographic methods and structurally elucidated by spectral evidences.
RESULTTen compounds were obtained and identified as (+)-syringare sinol (1), 5alpha, 8alpha-epidioxy-24( R)-methycholesta-6, 22-dien-3beta-ol (2), trans-3-(4-hydroxy-3-methoxyphenyl)-acrylic acid octacosyl ester (3), defusin (4), 3, 4-dihydroxy benzoic acid (5), 3, 4-dimethoxy-benzoic acid (6), vanillic acid (7), 3, 4-dimethoxy-benzoic acid methyl ester (8), 3, 5-dibromo-2-aminobenzaldehyde (9), heptadecanoic acid 2, 3-dihydroxy-propyl ester (10).
CONCLUSIONCompounds 1, 2 and 6-10 were isolated from this plant for the first time.

8.Study on optimization parameters of electroacupuncture at Fenglong (ST 40) for adjusting blood lipids.

OBJECTIVETo study the effects of different parameters (frequency, intensity, needle-retained time and treatment interval) of electroacupuncture at Fenglong (ST 40) for adjusting blood lipids, so as to find out the optimization parameter.
METHODSFifty-four cases meeting the criteria for hyperlipoidemia were randomly divided into 27 groups with orthogonal design L27 (3(13) ). According to the orthogonal design program they were treated with electroacupuncture at Fenglong (ST 40). Ten sessions constituted one course with a one week's interval between two course. The treatment was given for 2 courses.
RESULTS(1) The parameters of EA at Fenglong (ST 40) for regulating blood lipids in primary and secondary orders are: frequency, needle-retained time, interval of treatment, intensity. (2) The parameters of EA at Fenglong (ST 40) for various programs in regulating various blood lipids are: for TG, frequency AM 50 Hz, needle-retained time 20 ain, intensity 1 mA, twice each week; for TC, frequency AM 100 Hz, needle-retained time 30 min, intensity 1 mA, once every other day; for LDL-C, frequency Am 100 Hz, needle-retained time 30 min, intensity tolerable and comfortable, once every other day.

9.Clinicopathologic analysis of spermatocytic seminoma.

OBJECTIVETo study the clinicopathologic features and biological behavior of spermatocytic seminoma.
METHODSA retrospective analysis of patients diagnosed as seminoma, spermatocytic seminoma between January 2003 and May 2011, was performed. Clinical data, HE stained section and immunohistochemical staining (SP method) were reviewed with follow-up.
RESULTSSixty-six cases of seminoma and 5 cases of spermatocytic seminoma were identified. The average age at the diagnosis of 5 cases of spermatocytic seminoma was 53 years, and no patient had a history of crytorchidism or germ cell tumor. All five patients had stage pT1 tumor. Immunohistochemical studies showed that spermatocytic seminoma was negative for CK, vimentin, OCT3/4, PLAP, and LCA, and PAS staining was also negative. All five patients were well after operation. In contrast, the average age at diagnosis of the 66 cases of seminoma was 37 years, in which 12% had a history of crytorchidism and 11% were in stage pT2 or the above. Immunohistochemical studies showed that seminoma was positive for OCT3/4, PLAP, and CD117. During the follow-up, 2 patients developed metastasis and 3 patients died of the disease.
CONCLUSIONSSpermatocytic seminoma is rare and appears to follow a benign clinical course Due to its favourable prognosis, further treatment is not necessary after orchidectomy. Accurate pathologic diagnosis is critical for patient management and for avoiding over-treatment.

10.Liver resection in hepatolithiasis: 20-year's evolution.

OBJECTIVETo analyze operative and perioperative factors associated with hepatectomy in hepatolithiasis.
METHODS245 consecutive hepatolithiasis patients undergoing hepatectomy from January 1986 to December 2005 at Chinese People's Liberation Army General Hospital were investigated retrospectively according to medical documentation.
RESULTSHepatolithiasis accounted for 29.6% (245/827) in all benign liver diseases treated with hepatectomy during this time period. There were 88 cases in male and 157 cases in female, the average age was (46.9 +/- 11.3) years. Cases of right liver resection and hepatic segments resection were much more than that in 1963 - 1985. Blood transfusion during operation was given in 45.3% of cases. Complication incidence was 16.3%, with infection 3.3% and bile leakage 2.4%. Length of stay after operation was (15.7 +/- 9.2) days. Perioperative mortality rate was 0.4% (1/245).
CONCLUSIONSIndividualized hepatectomy is the important surgical treatment of hepatolithiasis. Hepatectomy can be performed safely with low mortality and low complication incidence, provided that it is carried out with optimized perioperative management and innovative surgical technique.
