Objective Based on the radiographs to analyze the generic and the developing factors of hallux valgus.Methods Two hundred and forty-one cases were divided into two groups according to the symptoms suffered,which were normal group and hallux valgus group.All cases were female,aged 10 to 90 years old.The three parameters-the first intermetatarsal angle,sesamoid inclination angle and hallux valgus angle,were taken full weightbearing radiographs,analyzed by non-paired t test and Pearson correlation analysis according to age.Results In normal group,the first intermetatarsal angle was correlated with age ( correlation coefficient r =0.125,P =0.003 ).Sesamoid inclination angles in both groups were correlated with age( normal group r =0.384,P <0.001 ;hallux valgus group r =0.359,P =0.0002 ).However,hallux valgus angle had no correlation with age ( normal group r =0.066,hallux valgus group r=0.080).All 3 parameters in hallux valgus group were statistically larger than normal group.Conclusion Aging is a possible reason to cause the 1 st metatarsophlangeal joint deformity,but larger parameters were found in valgus group,suggesting the major cause of hallux valgus is congenital factor rather than aging.

Objective To investigate the changes of activation and apoptosis of T-lymphocyte in peripheral blood of patients with chronic obstructive pulmonary disease(COPD) at the attack and relieved stage and their clinical significance. Methods 64 cases of COPD were divided into 2 groups: the acute stage(n=34)and the relieved stage groups(n=30). The T-lymphocytes were marked by fluoromonoantibodies CD 3 +, CD 4 +, CD 25 + , CD 95 +, CD 3 +/CD 25 +, CD 8 +/CD 28 +,CD 8 +/CD 28 - and was counted by flow cytometry . Results Compared with the relieved stage, in patients with COPD at the acute attack period,the CD 3 +, CD 95 +, CD 3 +/ CD 25 +, CD 8 +/ CD 28 - were increased, CD 4 + ,CD 25 + had not obvious change, but CD 8 +/ CD 28 + was increased. Conclusions In COPD at the acute attack stage, the immune function was disordered as the result of the change of T subtype of lymphocytes and the reduction of total T lymphocyte.So except for treatment in accordance with their symptoms,immunodrug of cells should be used in order to raise the immune function of the patients.

8.The prognostic value for cardiovascular events of elevated NT-proBNP in coronary heart disease patients with metabolic syndrome

Objective To explore the prognostic value for cardiovascular events of elevated NT-proBNP in coronary heart disease(CHD) patients with metabolic syndrome(MS).Methods The level of NT-proBNP was measured in 165 CHD patients with MS diagnosed by coronary angiography.After 2 years follow-up,we recorded the corresponding cardiovascular events in those patients.Results The NT-proBNP level was much higher in ACS group than that in SCHD group( P < 0.01 ).There were totally 45 events after 2 years.After adjustment of traditional risk factors including hypertension,diabetes,smoking history and high sensitivity C reactive protein( hs-CRP),Cox proportional hazard analysis indicated that high NT-proBNP level( > 111.58ng/L)was an independent risk factor for future events (relative risk 2.67,P <0.01 ).Conclusion Elevated NT-proBNP level might be one of the independent risk factor for cardiovascular events in CHD patiens with MS.
9.Free-hand cervical pedicle screw fixation for upper cervical fracture and instability

Objective To evaluate the clinical effect of the free-hand cervical pedicle screw fixation in treatment of the upper cervical fracture and instability.Methods A retrospective review was performed on 15 patients with upper cervical fracture and instability treated with cervical pedicle screw fixation and fusion from September 2006 to January 2009.There were 11 males and 4 females,at average age of 41.2 years(range,18-60 years).Of all,there were five patients with atlas fracture and dislocation(including three simple anterior arch fractures and two Jefferson fractures),three with axis fracture and dislocation,one with dens fractures plus nonunion,two with C2,3 fracture and dislocation and four with atlantoaxial instability without fracture.The main clinical complaints included local neck pain and/or tetraplegia.Halo traction was recommended to restore the cervical sequence preoperatively in all patients.All 15 patients were treated by cervical pedicle screw-rods internal fixation and bone graft fusion.During the operation,the point and angle of the implanted pedicle screws were determined by preoperative X-ray and CT scan and the bony channel drilled with free-hand before implantation of the Summit or Vertex pedicle screws(22-26 mm long)and posterior interlaminar autologous or allogeneic bone fusion.Patients could get out of bed with neck collar at days 1-2 after operation.Results A total of 64 cervical pedicle screws were implanted in all 15 patients,with no vertebral artery injury,spinal cord injury or cerebrospinal fluid leakage.Postoperative X-ray and CT scan confirmed satisfactory internal fixation.The clinical symptoms were improved significantly.Fourteen patients were followed up for 12-36 months,which showed bony fusion,with no looseness or breakage of the screws.Neurologic impairment was improved in all patients,with no complications associated with the cervical pedicle screw.Conclusions Cervical pedicle screw internal fixation can reestablish the upper cervical vertebrae stability and help to recover the spinal cord and nerve function and hence is a reliable method for upper cervical fracture and/or instability.
10.Comparison of transthoracic contrast echocardiography and transesophageal contrast echocardiography for detecting right to left shunt in patients with petent foramen ovale

Objective To compare the effect of transthoracic contrast echocardiography (cTTE) with transesophageal contrast echocardiography (cTEE) for detecting right to left shunt (RLS) in patients with patent foramen ovale (PFO). Methods The prospective study was conducted in 29 consecutive patients with PFO who suffered from cryptogenic stroke and/or migraine. Contrast echocardiography was performed in all 29 patients. The cTTE was performed using transducer with second harmonic imaging modality (transmitting frequency 1.7 MHz, receiving frequency 3.4 MHz). The cTEE was performed using transducer with frequency 7 MHz. Ten milliliter saline solution of contrast were rapidly administrated through an antecubital vein. According to whether microbubble (MB) appearing in left atrium after complete opaciifcation of right atrium within the ifrst 3 circles, the results were classiifed by a four-level semi-quantitative categorization:Level 1 (no PFO-RSL), no MB in left atrium; Level 2 (small PFO-RSL) 1-10 MBs; Level 3 (medium PFO-RSL) 10-30 MBs;Level 4 (large PFO-RSL)>30 MBs. Results The total detection rate of PFO-RSL was signiifcant different between cTTE and cTEE (86.2%vs 55.2%,χ2=5.711, P=0.017). In cTTE there were 4 cases at level 1, 1 case at level 2, 5 cases at level 3 and 19 cases at level 4. In cTEE there were 13 cases at leverl 1, 2 cases at level 2, 6 cases at level 3 and 7 cases at level 4. The comparison of semi-quantitative grading derived from cTTE and cTEE was also signiifcant different (Wilcoxon signed ranks test showed Z=-3.789, P=0.000). Conclusions The efifciency in detection of PFO-RLS with cTTE was better than with cTEE. Compared with cTEE, cTTE was easier in practice and brought less discomfort and complications to patients.
