1.Clinical effects of Isotretinoin erythromycin gel in treatment of mild to moderate acne vulgaris

Objective To explore the clinical efficacy and safety of isotretinoin erythromycin gel in the treatment of mild to moderate acne vulgaris. Methods A randomized,open,parallel,controlled clinical study was conducted.A total of 200 patients were enrolled in this study,in which 3 patients were excluded and 24 patients droped off during the study.Efficacy analysis was carried out in 197 patients and safety analysis in 173 patients.The patients were classified into trial group (n=87)and control group (n=86) to be treated with isotretinoin erythromycin gel or tretinoin cream once a night for 8 weeks,respectively.The clinical effects and adverse drug reactions were observed.Results The effective rates were 64.37 % (56/87) in trial group and 58.14 % (50/86) in control group with no statistical significance (P>0.05).The rate of adverse reactions were 15.31% in trial group and 14.14 % in control group with no statistical significance (P>0.05).Conclusions Isotretinoin erythromycin gel is safe and effective in the treatment of mild to moderate acne vulgaris.
2.Role of aqueous extract from Lishi No.5 formula on concentration of intracellular free-calcium in neurons

Objective Effects of aqueous extract from Lishi No.5 formula on concentration of intracellular free-calcium fluorescent intensity in neurons. Methods Hippocampus neurons of 1-day newborn SD rats were cultured with conventional culture technique. The cells cultured for 9-12 days were used for experiment. Intracellular free calcium fluorescent intensity of neurons cultured under different conditions was assayed with confocal microscopic calcium image technique after loading of Fluo-3/AM. Results Free-calcium concentration was enhanced by aqueous extract from Lishi No.5 formula, this concentration was up to 126.35?9.35nmol/L, but the concentration is at normal scope; L-type calcium channel blocker Nifedipine may block the effect of aqueous extract from Lishi No.5 formula enhancing intracellular free-calcium concentration in part, it make intracellular free-calcium concentration down to 90.75?10.15nmol/L, but Nifedipine itself also decreased markedly free-calcium concentration to 40.65?5.65nmol/L. NMDA increase Markedly calcium fluorescent intensity, the effects of NMDA was decreased notably after pre-treating with aqueous extract from Lishi No.5 formula. The effect of MK-801, an antagonist of NMDA receptor, on inhibition of NMDA increasing free calcium fluorescent intensity was significantly reduced after pretreatment of aqueous extract from Lishi No.5 formula. Conclusion Aqueous extract from Lishi No.5 formula maybe bidirectional adjust intracellular free calcium concentration via enhancing L-type calcium channel activity and blocking NMDA receptor in part.
3.Effects of minocycline on the cognition and expression of BDNF, Bcl-2 and Bax in hippocampus of Alzheimer’s disease model rats

Objective To investigate the effect of minocycline on the cognition and expressions of brain-derived neurotrophic factor (BDNF), apoptosis related factor Bcl-2 and Bax in hippocampus of rats with Alzheimer’s disease (AD).
Methods The rat model was established by microinjection of Aβ25-35 into lateral ventricle. Thirty healthy male SD rats were randomly divided into three groups:control group, model group and minocycline treatment group. Normal saline 1 mL/(kg·d) was intraperitoneally injected in control group and model group. The minocycline treatment group was intraperitoneally injected with minocycline 50 mg/(kg · d) for 14 days. Morris water maze was used to detect the behaviors of animals. The expressions of BDNF, Bcl-2 and Bax in hippocampus were measured by Western blotting and enzyme linked immunosorbent assay (ELISA). The apoptosis of neurons was detected by TdT-mediated dUTP nick-end labeling (TUNEL). Results Minocycline greatly improved the behaviors of AD rats, up-regulated the expressions of BDNF and Bcl-2, and down-regulated the expression of Bax in hippocampus, and reduced cell apoptosis. Conclusion Minocycline plays a protective role in neural function by promoting the growth of neurons and inhibiting the neuronal apoptosis.
4.Detection of serum antibodies to polymorphic membrane proteins (Pmp) in patients with urogenital Chlamydia trachomatis infection

Objecfive To detect serum antibodies to Pmp in patients with urogenital Chlamydia trachomatis infection and to assess the relationship between Pmp and urogenital C.traehomatis infection.Methods Twenty healthy adults and 77 patients with urogenital C. trachomatis infection were recruited into this study.A 3-month foilow-up was carried out in 43 patients,who were classified into persistent infection group(n=19)and negative-conversion group(n=24).Western-blot was performed to detect serum antibodies to Pmp in all subjects.Results The positivity rate of anti-Pmp antibodies was 90.20% (71/77) in patients,significantly higher than that in the normal controls[20% (4/20),P<0.05].All the 9 types of anti-Pmp antibodies were detected in patients with a varying positivity rates,which were 61.04% (47/77),88.31% (68/77),63.63% (49/77),28.57% (22,77),63.63% (49/77),75.32% (58/77),62.34% (48/77),77.92% (60/77)and 70.13% (54/77) for antibodies against PmpA,PmpB,PmpC,PmpD,PmpE,PmpF,PmpG,PmpH and PmpI respectivelyThe prevalence was highest for anti-Pmp B antibodies and lowest for anti-Pmp D antibodies.There was no significant difference in the positivity rate of anti-Pmp antibodies between persistent infection group and negativeconversion group.Conclusions Anti-Pmp antibodies could be generated in patients infected with C. trachomatis.The immunogenicity of different Pmps is different,and the immunoprotective activity of Pmps is rather weak.Individual differences exist in serum anti-Pmp antibodies among patients.Nine types of Pmps are expressed in patients with urogenital C. trachomatis infection.
5.The diagnostic value of ProGRP for small cell lung cancer

Objective To explore the clinical significance of serum ProGRP in diagnosis of SCLC.Methods Serum ProGRP were detected in twenty-three SCLC patients,forty NSCLC patients and forty-three patients with benign pulmonary disease whose were definite diagnosis collected from April 15,2016 to July 19,2016,and a contemporary cohort of 40 healthy controls were while recruited.Results The levels of ProGRP in SCLC group were significantly higher than those in NSCLC group,benign pulmonary disease group and healthy control group(P <0.05).The levels of ProGRP in extensive disease group were significantly higher than those in 1imited disease group(P <0.05).There was no significant difference in the positive rates of ProGRP between the sex group and age group (P > 0.05 ).The boundary value of diagnosis of SCLC through ProGRP identified through ROC curve was 65.66 ng/L.The sensitivity and specificity of the ProGRP for the diagnosis of SCLC were respectively 90.9% and 89.9%.Conclu-sion Serum ProGRP can be use as a sensitive and specific index for diagnosis of SCLC,and the level of ProGRP can also be used for the clinical staging of SCLC.
6.Genetype distribution of mycoplasma pneumoniae by denaturing high-performance liquid chromatography

Objective To investigate the genetype distribution of mycoplasma pneumoniae(MP) by denaturing high-performance liquid chromatography(DHPLC).Methods A total of 300 cases nasopharyngeal aspirate were collected from our hospital.The MP genes of standard strains and clinical specimens isolates were amplified by PCR followed by DHPLC and genetype determination.Results A total of 110 cases were positive after 24 hours fermentation from 300 cases with pharyngeal swab.By the specific primers of MP-129,MP-FH standard strain and specimens,2 280 bp and 2 580 bp gene fragments were made out respectively.One hundred and ten strains of clinical isolates were detected by DHPLC.One hundred and seven strains of P1-Ⅰ were 1b subtype,3 were type P1-Ⅱ which were all 2a subtype.Conclusion The genetype of MP infection in children from our hospital is P1-Ⅰ,1b subtype by using DHPLC technology.
7.Outcomes of treatment of Chlamydia trachomatis infection with azithromycin: an evaluation by different criteria

ObjectiveTo compare the recovery rates calculated according to different criteriain patients with urogenital Chlamydia trachomatis (Ct) infection after treatment with azithromycin. Methods Clinical data on outpatients who were diagnosed with urogenital Ct infection and treated with azithromycin in the sexually transmitted disease(STD) outpatient clinic of Tianjin Medical University General Hospital were retrospectively reviewed. Recovery rates were calculated according to the improvement of symptom and (or) reexamination results of Ct at 1,5 and 9 weeks after the end of treatment.ResultsSignificant differences were observed between the recovery rates calculated according to symptom improvement and those according to laboratory reexamination results.No obvious correlation existed between the presence of symptom and positive reexamination results.The recovery rates calculated according to the second reexamination result differed significantly from those according to the first reexamination result,but were similar to those according to the third reexamination result. ConclusionsThe cure of Ct infection should be determined according to laboratory test results,and two times of reexamination at 1 and 5 weeks after the final treatment are recommended.
8.Pyodermatitis-pyostomatitis vegetans: a case report

A 58-year-old female was admitted to the hospital for a 3-month history of erythema,pustules and vegetating plaques on the lips and scalp,as well as in intertriginous areas.She had a 2-year history of ulcerative colitis,which had been aggravated before the development of skin lesions.Skin examination demonstrated diffuse thickening of both lips covered with multiple granule-to mung bean-sized confluent pustules.Yellow crusts were observed at the lip margins.A 1.5 cm × 2 cm vegetating plaque surrounded by anular pustules was found in the left buccal mucosa.There were multiple irregularly-sized vegetating plaques with erosions and yellow crusts on the scalp,neck,as well as in the periumbilical and left inguinal and axillary region,and annular vesicles and pustules were observed in the center and margin of these plaques.The lesions subsided leaving hyperpigmented macules.There were four fresh pustules in the right axillary region.Histological examination of a biopsy specimen taken from the lower lip margin revealed mild hyperplasia of and neutrophil microabscess in the epidermis,as well as a dense inflammatory infiltrate composed mostly of neutrophils and eosinophils with microabscess formation in the dermis.Direct immunofluorescence examination of the normal-appearing skin next to the lesions was negative.The percentage of peripheral blood eosinophil was slightly elevated.Bacterial and fungal cultures of pustular contents were negative.A diagnosis of pyodermatitis-pyostomatitis vegetans was made.
9.Regulation of activated genes of fibroblasts and their improvement of skin aging

Objective To investigate the possible mechanisms using different chemicals to improve the skin aging by the effect of estrogen,vitamin E,vitamin C on the transcriptional level of MMP-1 and HAS-2 mRNAs in cultured human skin fibroblast in vitro.Methods The different concentrations of estrogen,vitamin E,vitamin C were put into cultured human skin fibroblasts.After 24 hours,RT-PCR was used to detect the effects of the different chemicals on the transcriptional levels of the HAS-2 and MMP-1 mRNAs in fibroblasts.Results The effects of estrogen and vitamin C on the MMP-1 mRNA had significant difference between the high dose group and blank control group,and between the low dose group and blank control group (P<0.05) ; the difference between the low dose group and high dose group was not statistically significant (P>0.05).The effects of vitamin E and vitamin C on Has-2 mRNA also had significant difference between the high dose group and the blank control group,and between the low dose group and blank control group (P<0.05),but the difference between the low dose group and high dose group was not statistically significant (P>0.05).Conclusions Estrogen and vitamin C can reduce the transcription of MMP-1 gene,which decreases the degradation of collagen in the skin; vitamin E and vitamin C can increase the transcription of HAS-2 gene,which may possibly increase the synthesis of hyaluronic acid to alleviate the skin aging.
10.The early diagnostic value of corneal laser confocal microscopy for small neuropathy in elderly patients with type 2 diabetes mellitus

Objective To investigate the early diagnostic value of cornel confocal microscopy for the screening of small neuropathy in elderly patients with type 2 diabetic mellitus.Methods In the prospective study,96 elderly patients with diabetes as study group and 46 patients with non-diabetes as the control group were continuously collected from our hospital endocrinology and ophthalmology out patients during May 2014 to February 2016.The 96 cases of type 2 diabetes were subdivided into 47 patients with diabetic peripheral neuropathy (DPN)and 47 patients with nowdiabetic peripheral neuropathy(non DPN).Results The diabetes course was shorter in non-DPN group than in DPN group(P=0.000).The levels of glycosylated hemoglobin and urine albumin were lower in the nonDPN than in the DPN(P =0.072,0.007,respectively).The corneal nerve fiber density was lower in the DPN group than in NDPN group (P =0.000).Corneal nerve fiber density was higher in control group than in DPN and NDPN group.The differences in number of corneal nerve fibers showed no statistical significance between DPN and NDPN group (x2 =2.391,P =0.314).But the number of corneal nerve fibers was significant less in DPN and NDPN group than in control group(x2 =16.014,P =0.000).The negative correlation was found between the course of disease and corneal fibrous density by using single factor linear regression analysis.The number of corneal nerve fibers was lower in smoking group than in non-smoking group(P=0.003).The multiple linear regression analysis showed that duration of diabetes was a risk factor for diabetic neuropathy.Conclusions In some elderly diabetic patients with non-neuropathy,corneal nerve fiber density and number have been significantly decreased before nerve conductive velocity is reduced.Therefore,corneal confocal microscopy can be used to detect and diagnose small diabetic neuropathy in elderly patients with diabetes mellitus.
