1.Study on acid-base disturbance in patients with post-traumatic multiple organ dysfunction syndrome.

OBJECTIVE: To investigate the classification and incidence of acid-base disturbance (ABD) in the patients with post-traumatic multiple organ dysfunction syndrome (MODS). METHODS: A total of 119 patients with MODS were examined with arterial blood gas analysis and serum electrolytes detection for 675 times in this study. RESULTS: Different types of ABD existed in 647 times out of 675 times (95.9%) of blood-gas analyses. There were 270 times (41.7%) of simple ABD, 271 times (41.9%) of double ABD and 106 times (16.4%) of triple ABD. Among which, 404 times (62.4%) were in respiratory alkalosis (RAL), 332 times (51.3%) in metabolic acidosis (MA), 227 times (35.1%) in metabolic alkalosis (MAL) and 167 times (25.8%) in respiratory acidosis (RA). In this study, 79 cases (66.4%) out of 119 cases with MODS died from these kinds of ABD. CONCLUSIONS: It suggests that in the early stage of MODS, RAL with or without hypoxemia may exist, and later on, MA or even triple ABD may occur. In order to detect and correct the primary disorders as early as possible, it is important to keep the balance of hydrolyte. The treatment of primary diseases is also important. Disorders of acid-base balance were corrected according to pH standard values, anion gap (AG) and the potential [HCO(3)(-)] were also calculated simultaneously. When pH was more than 7.50 or lower than 7.20, it is necessary to give drugs of acidity or alkalinity to the patients with ABD to maintain pH value within a normal range.
2.Study on the correlation of eosinophils and sinus CT findings in histology of chronic rhinosinusitis

OBJECTIVE To investigate the correlation between eosinophils(Eos) and CT findings in patients with chronic rhinosinusitis(CRS).METHODS The clinical data of 355 CRS patients who were treated at the First Affiliated Hospital of Wenzhou Medical University from January 2013 to June 2015 were studied.Friedman stage and Lund score and pathophysiological Eos count were recorded in all cases.The results were divided into eosinophilic CRS(ECRS) group and non-eosinophilic CRS(non-eosinophilic CRS,NECRS) group.The scores of CT findings of the two groups were compared and analyzed,and the correlation of the results of CT scores and the number of Eos in the tissue were explored.RESULTS There was a significant difference in total CT scores between ECRS and NECRS groups(Z=1.674,P<0.01),but there was no significant difference in CT scores among T1,T2,T3 and T4 stages(Z=1.011,0.892,0.612,0.80,P<0.01).And CT scores were positively correlated with Eos counts and percentages in the tissues (correlation coefficients r were 0.216,0.223,P<0.01).CONCLUSION 1.The difference of Eos in the tissue of the CRS is the cause of different CT manifestations.2.The more Eos count or the percentage,the higher the CT scores of patients with chronic sinusitis,which means the more severe of the CT imaging.
3.The efficacy and safety analysis of endoscopic resection for infratemporal fossa benign mass

Objective:To investigate the efficacy and safety of endoscopic resection of infratemporal fossa mass and to determine the indications for surgery.Methods:A retrospective case series study was conducted, including a total of 29 patients who underwent endoscopic surgery to treat infratemporal fossa mass in the Department of Rhinology of Beijing Tongren Hospital, Capital Medical University, from April 2008 to December 2021. Ten males and 19 females were included in the study, with age of (46.5±13.7) years. Pre-and post-operative sinus CT, sinus or nasopharyngeal enhanced MRI were evaluated, respectively. The main outcome measurements were the total resection of mass and the incidence of surgery-related complications.Results:Among the 29 cases of infratemporal fossa mass, 22 were schwannomas, 3 were cysts, 2 were neurofibromas, 1 was pleomorphic adenoma and 1 was basal cell adenoma. Preoperative imaging showed well-defined lesion boundaries, and postoperative pathology confirmed the benign nature of all cases. The endoscopic transnasal approach was used in 28 patients, while the combination of the transnasal approach and the transoral approach was used in 1 patient. Complete tumor removal was achieved in all cases with a 100% resection rate. The average follow-up time was 38 months (7-168 months), and no tumor recurrence was observed.Conclusions:The Endoscopic transnasal approach is a safe and effective surgical approach for the treatment of benign tumors or masses in the infratemporal fossa.
4. Predictive study on recurrence of chronic sinusitis with nasal polyps by tissue eosinophils and sinus CT

To investigate the correlation between postoperative recurrence and clinical parameters in patients with chronic rhinosinusitis with nasal polyps (CRSwNP), and to study predicitve value of total scores of eosinophils (EOS) and sinus CT for postoperative recurrence.

A retrospective analysis of 264 patients with CRSwNP who were admitted to the First Affiliated Hospital of Wenzhou Medical University from June 2013 to December 2015 was performed. Objective:
To evaluate the efficacy of treatment at 1 year after completing the 2-year specific sublingual immunotherapy in patients with allergic rhinitis (AR).
Methods:
A retrospective analysis of 124 patients with AR who were allergic to dust mite and treated in the First Affiliated Hospital of Wenzhou Medical University since 2012 was performed. All patients achieved sublingual immunotherapy (SLIT) for 2 years and had a regular follow-up over 1 year. Efficacy evaluation index such as VAS score, total nasal symptom score and total medication score were compared between the time of SLIT for 2 years, and 1 year after termination of SLIT. Paired t test was used for the comparison of scores.
Results:
Compared with those before treatment, the VAS score, total nasal symptom score and total medication score at the time of SLIT for 2 years were significantly decreased in all patients with AR [(9.40±5.96)
