1.Studies on B cell differentiation from cord blood CD34~+CD19~- hematopoietic stem/progenitor cells in vitro

Objective:To investigate the condition of B cell differentiation from cord blood CD34+CD19-hematopoietic stem/progenitor cells in vitro.Methods:CD34+CD19-cells from cord blood were isolated and purified by using immunomagnetic beads separation system.CD34+CD19-cells supported by murine S-17 stromal cells were stimulated in co-culture with T3 and cytokines.Differentiated B cells were analyzed by flow cytometry.Results:The amplification of the B cells derived from CD34+CD19-hematopoietic stem/progenitor cells in co-culture with T3 and IL-7 reached 198-fold of increase,most of the induced cells expressed CD10 and CD19.Conclusion:In the experimental conditions selected,co-culture of CD34+CD19-cells with T3,IL-7 and murine S-17 stromal cells could stimulate differentiate toward to B cells in vitro.
2.Incontinence-associated dermatitis′ characteristics and the analysis of risk factors in Neurology Department

Objective To find out the clinical features and risk factors of incontinence-associated dermatitis ( IAD) in neurology patients. Methods A total of 413 cases of neurology patients were selected by cross-sectional descriptive survey in a neurology hospital in Nanjing. Participants′ IAD status, nursing interventions and risk factors were collected. The univariate analysis and Logistic regression analysis were applied to analyze the factors that potentially associate with the occurrence of IAD. Results 4. 1% of neurology patients had IAD, 88. 2% of them had gatism, and IAD was more likely happened on patients with cerebrovascular disease. According to the univariate analysis, there were 15 factors which included GCS, Braden and PAT score affected IAD occurrence (P<0. 05); logistic regression analysis showed the PAT score could predict IAD (OR=15. 528, P<0. 05 ). Conclusions IAD in Neurology Department is more concentrated on gatism patients. The occurrence of IAD is affected by multi-factors;PAT score can be seen as a predict factor of the occurrence of IAD.
3.Risk prevention and oral care in patients with gingival bleeding caused by oral administration of warfarin

Objective To observe effect of TCM oral care solution in patients with gingival bleed-ing caused by oral administration of warfarin.Methods A total of 150 hospitalized patients in the car-diology department were selected as study objects,and were randomly divided into control group 1,control group 2 and experimental group,with 50 patients per group.Patients in the three groups were given medication care of warfarin and health guidance for mouth care.The control group 1 increased the use of normal saline gargle;the control group 2 increased the use of silver ion gargle,and the ex-perimental group was given the use of traditional Chinese medicine oral care solution gargle.After the intervention,the number of gingival bleeding,gingival sulcus bleeding index(SBI)and halitosis score in the three groups were compared.Results The number of gingival bleeding during gargling(10 d)and within 4 weeks after discharge in the experimental group were less than that in the control group 1 and control group 2,the difference was statistically significant(P<0.05).The SBI of the experimen-tal group was(1.53±0.69),which was lower than(2.12±0.79)in the control group 1 and(2.09± 0.76)in the control group 2(P<0.05).The score of halitosis during gargling in the experimental group was(1.70±0.68),which was lower than(2.28±0.74)in the control group 1 and(2.23± 0.79)in the control group 2(P<0.05).Conclusion The TCM oral care solution for patients with gingival bleeding caused by oral administration of warfarin can significantly reduce the number of gingi-val bleeding,reduce SBI,alleviate bad breath,improve oral health and drug safety.
4.Protective effect of J-Valve transapical aortic Valve replacement in patients with aortic stenosis with low coronary ostium

Objective:To investigate the safety and efficacy of J-valve transapical catheter aortic Valve replacement(TA-TAVI) in the early treatment of aortic stenosis with low coronary ostium.Methods:From January 2020 to April 2022, 20 patients with aortic stenosis with coronary opening height ≤10 mm who underwent TA-TAVI treatment in the Valve Surgery Center of Beijing Anzhen Hospital Affiliated to Capital Medical University were enrolled retrospectively. All patients underwent preoperative imaging screening, and 5 patients underwent preoperative coronary artery protection: The height of coronary artery opening was less than 5 mm in 3 cases. 2 cases had stent stenosis & LT after coronary artery stenting 50%, and the position of the coronary artery opening was less than 8mm. The postoperative complications, mortality and cardiac function prognosis of TA-TAVI were analyzed.Results:There were 5 males and 15 females. The average age of the whole group was(73.00±6.20) years. Transthoracic echocardiography showed that all patients had severe aortic valve stenosis, and 85% of the patients had NYHA grade Ⅲ-Ⅳ, and 80% of the patients had aortic valve stenosis with insufficiency. The mean height of left coronary artery opening was(9.07±3.70) mm. RCA(11.39±3.00) mm; The opening height of coronary artery was ≤5 mm in 3 cases(left 7.5%), 5-8 mm in 5 cases(12.5%), and 8-10mm in 16 cases(40.0%). Mean aortic sinus(valsalva) diameter: Left(30.06±5.90) mm; Right(28.50±5.68) mm; Non(29.96±6.15) mm. J-valve Valve was successfully implanted through apical catheter in all patients, most of whom were size 23. CPB was performed in 2 patients at the same time, permanent pacemaker was inserted in 2 patients, and moderate or above perivalvular leakage was found in 0 patients. All patients had TNI( P=0.12) and MYO( P=0.03) before and 24 hours after operation. None of the 5 patients underwent coronary artery stenting. None of the 5 patients died within 30 days after the operation. Ta-tavi is effective 30 days after operation, with low complication rate and good prognosis of cardiac function. Conclusion:J-valve transapical catheter aortic Valve replacement is safe and effective in the treatment of aortic stenosis with low coronary ostium.
