1.Treatment of fresh femoral cervical fracture by closed reduction with percutaneous knowledpins.

Management of fresh femoral neck fractures by closed reduction with percutaneous knowles pins in the past 17 years in CTO. From 1983 to this year, 2,412 patients with fresh femoral cervical fractures were treated by closed reduction with percutanous knowles pins through 3 periods. Percutaneous pining become the main procedure in treament of fresh femoral neck fractures in Center for Traumatology and Orthopaedics at least 1-4 cases a days with good results especially for the elderly patients. They can wake up the days after surgery. This method was also realized in developed countries at the end of the years 70. With very small incisions, minimal influence for the body, percutanous pinning enhanced the recovery, shortened the hospital time and lowered treatment fees.

2.Prevalence of epilepsy in Phu Linh \ufffd?Soc Son \ufffd?Ha Noi, 2003.

Background: Epilepsy is a chronic disease, the incidence of epilepsy was from 5/1000 to 10/1000 population. In developed countries the rate of epileptic patients are managed much higher than in developing countries due to the differences in socioeconomic conditions as well as understanding of epilepsy. In Vietnam, few epidemiological studies about epilepsy were published. Objectives: To reveal the prevalence of epilepsy in a rural community in Viet Nam and the relation between the prevalence and age, career, level of education, economic condition of the patient. Subjects and method: This was a cross-sectional described study. Door \ufffd?to \ufffd?door survey based on WHO\u2019s questionnaires was conducted by neurologists and student of Faculty of Public healthy of Hanoi Medical University on people at Phu Linh commune, Soc Son district, Hanoi city. Data was processed by SPSS 10.0 program. Results: Periodic prevalence was 7.5\ufffd?(5.5\ufffd?was active epilepsy). Generalized epilepsy: 79.66%, focal epilepsy: 16.95% indeterminate epilepsy: 3.39%. There was a significant relation between education and prevalence of epilepsy. Conclusion: Periodic prevalence of Epilepsy in Phu Linh (2003) was 7.5\ufffd? Toninco-colonique seizures were dominant. A significant relation between number of epileptic patients and level of education was found.

3.Real situation of blood glucose control in type 2 diabetic patients admitted to Department of Endocrinology, Bach Mai Hospital

Background: High glucose level can cause many serious complications, especially for type 2 diabetic patients. Poorly blood glucose control in type 2 diabetic patients usually induces serious complications. Objective: (1) To evaluate blood glucose control state and (2) To describe affected factors to blood glucose control in type 2 diabetic patients. Subjects and method: We carried out a survey on blood glucose control in 104 type 2 diabetic patients (based on HbA1c results), admitted to Dept of Endocrinology, Bach Mai Hospital from December 2005 to April 2006. 73.1 % of patients is female, average age is 63.4 years old, 57.7% has hypertension, and 62.5% has diabetes for \ufffd?5 years. Results:Blood glucose is poorly controlled in 80.8% (HbA1C > 7.5%), and good controlled in only 12.5% patients (HbA1C < 6.5%). Several affected factors are identify, such as patients were not educated and/or do not perform correctly nutritional and exercise therapy (48.1 %); not compliant with drug treatment; or not measure regularly blood glucose; and long duration of diabetes (p < 0.01). Conclusions: (1) Blood glucose in majority of type 2 diabetic patients is poorly controlled. (2) Factors affected are non - compliance with therapy, irregularly blood glucose measurement and long duration of diabetes.

4.Ultrasound kidney dimension and the correlation of kidney length and age, weight and height of children aged from 6 to 8 years old

Background:The dimension and function of kidney have close relationship. In the developing countries, the index on kidney dimension of children has rather fully studied. In Viet Nam, most of studies only conducted on adults. Objectives:This study aims to determine the ultrasound kidney dimension and the correlation of kidney length and age, weight and height of children aged from 6 to 8 years old. Subjects and method: The data was obtained from a prospective study on 180 normal children at age from 6 to 8 years. Each kidney was measured length, width , thickness, and parenchymal thickness, and the mean values were recorded. The relationship of kidney length with age, body weight, body height were also taken. Results: Kidney dimension was increased by age. The average ultrasound kidney dimension:+ Right kidney: At 6 years old: Boy: length: 72.8\xb14; width: 31.2\xb13 ; thickness: 33.9\xb12 ; Girl: length: 69.8\xb15; width: 30.4\xb12 ; thickness: 32.7\xb12. At 7 years old: Boy: length: 77.7\xb13; width: 33.8\xb13 ; thickness:34.7\xb12; Girl: length: 76.9\xb17; width: 32.0\xb12; thickness: 33,9\xb15; The kidney length, kidney weight, kidney volume of boys were bigger than girls, and the difference was significant (p<0.05). Conclusion: The kidney length correlated strongly and positively with body weight, body height and age.

Background: The method of immunoelectron microscopy has been found more than 20 years. 6.Situation of cancer and various environmental factors in 4 communes \u2013 Nha Trang city

Background: Nha Trang city located in central Viet Nam, has had dramatic economic development in recent years. The economic development has also leads to changes in the environment. Previous studies showed that cancers were closely related to environmental factors, but there was no study on this issue in Nha Trang city. Objectives: The study aimed to describe the cancer cases that were recorded in the cancer registration and the related environmental factors in Vinh Luong commune and the three neighborhood communes of Nha Trang city. Subjects and method: 192 patients who were diagnosed with cancer in the provincial general hospital and other hospitals from 1/1/2000 to 30/9/2006 and have resided in 4 above communes. Water samples collected from 14 water-wells from these communes were tested for chemical pollution and bacteria index. 20 sites in Van Dang village were tested for radioactive pollution levels. Results: Average crude morbidity of cancer per 100,000 inhabitants per year in Van Dang village and Vinh Luong commune and in the 4 communes as a whole, were 64.9, 51 and 53.3, respectively. The rates for male and female were 65.0 and 41.0, respectively. The most common cancers in women were cervical and breast cancers (11.8% and 9.2%, respectively). For men, leading cancers were liver cancer (31.9%) and lung cancer (14.7%). Almost all of the water samples did not meet the hygiene standard for nitrate (NO3) and coliform. Natural radioactive levels in these 4 communes were normal. Conclusion: Van Dang village had the highest cancer prevalence per 100,000 habitants compared to the other villages of Vinh Luong commune, but it was still lower than that of some other provinces. Most of the water samples collected from the water-wells did not met standard for nitrate levels and coliform.

Background: Noise pollution has seriously negative effects on worker\u2019s health, especially their hearing ability. Occupational hearing loss is one of 21 insured occupational diseases. Occupational hearing loss is common in workers in transpostation, construction, energy heavy and light industry areas. Objectives: 1. To determine the degree of noise pollution in Thua Thien Hue Unity Auto Mechanical Joint Stock Company. 2. To evaluate occupational failing hearing and occupational hearing loss among workers in the company. 3. To promote preventive measures which imnimize occupational failing hearing and occupational hearing loss. Subjects and method:A cross-sectional, descriptive study was carried out in 254 employees in Thua Thien Hue Unity Auto Mechanical \r\n', u'Joint Stock Company. Results and Conclusion: 72.7% the employees of Thua Thien Hue Unity Auto Mechanical \r\n', u'Joint Stock Company have exposed to environmental noise at the level higher than the permitted one. The rate of occupational failing hearing among these people is rather high (33.1%), and occupational hearing loss rate is 11.8%. 8.Clinical characteristics of poststroke dementia patients with age of 60 years and older

Background: Dementia is one of the major causes of dependency after stroke. The prevalence of poststroke dementia (PSD)defined as any dementia occurring after stroke is likely to increase in the future.Objectives: This study have two purposes: 1) Clinical study of MCI and dementia after the first stroke of patients with age of 60 years and older; 2) Overview on clinical characteristics of memory disorders. Subjects and method: 30 patients with were diagnosed with the first ischemic stroke in Huu nghi hospital together with the same number in the control group were involved in this study. The subjects in the two groups were all satisfied with included/excluded criteria diagnosis. Clinical diagnosis of new - onset dementia or other mental disorders was determined using neuropsychological tests. Results: Many functions of the brain were impaired including: logical memory, visiospatial skills, executive function were statistically reduced in the research group compared to the control. However, language function was also impacted but not as much as others. The frequency of the poststrocke dementia in this study was 12.3% while the poststrocke mild cognitive impairment rate was 47%. Conclusions: Global cognitive functioning together with memory state was significantly declined in the ischemic stroke compared to the control group.

9.Application of neuropsychological battery tests in post-stroke dementia diagnosis among patients over 60 years old

Background: In Vascular Dementia (VaD) patients, the causes of blood vessels were common, and preventable and treatable, so that it is very important to detect and diagnose in the early stages of the disease. Diagnosis of dementia is based on clinical symptoms, and neuropsychological tests are useful tools. Objectives: (1) To evaluate the severity of VaD and Vascular Cognitive Impairment (VCI) after the 1st ischemic stroke in patients over 60 years old. (2) To make observations on the clinical features of post stroke dementia in these patient groups using neuropsychological battery. Subjects: 94 patients with 1st acute ischemic stroke, who were over 60 years old, conscious and literate, and cooperated well with physicians. A standard evaluation protocol was conducted at one month after an ischemic stroke for all the patients. Method: Prospective study. Data was analyzed by using SPSS software version 13.0. Results and conclusions: The rates of VCI and VaD after the first ischemic stroke were 21.3% and 25.5%, respectively. Clinical determinants of dementia were: visuoconstruction (65% patients), visual motor speed (50%), memory disorders (more than 40%, in which visual memory 45.8% and verbal memory 41.6%), executive function (37.5%), and language skill (37.5%). The attention and language functions were less affected (only 25% of the patients). Mini mental state examination score can be used to evaluate and classify clearly 3 groups: VaD, VCI patients and normal people.

10.Lead level in blood and urine of fishermen in Canh Duong village, Phu Loc, Thua Thien Hue province

Background: In Viet Nam, there were many researches on determining lead level in blood, food and environment. But until now, there were not researches on determining lead level in fishermen community. Objectives: Determine trace amount of lead by von-ampe dissolve anot using electrode BiFE. Analysis on trace amount of lead in blood and urine of fishermen in Canh Duong village. Propose solutions for preventing lead poisoning in studied community. Subjects and method: Fishermen in Canh Duong village, Phu Loc, Thua Thien Hue province. The method differential pulse-anodic stripping voltametry (DP-ASV) using Bismuth Film Electrodic in situ was successfully applied to measure lead levels in blood and 24h urine of fishermen in Canh Duong village. Results: The mean level of lead in blood of the suspected group (fishermen) in Canh Duong village (34,7 \xb5g/dL) was higher than that of the control group (16,3 \xb5g/dL). For both groups, 37 out of 40 blood samples (92,5%) had lead level higher than WHO\u2019s recommended level (10 \xb5g/dL). Especially, 6 of 40 blood samples (15%) had lead level higher than 50 \xb5g/dL. The lead level in 24h urine of suspected group had slightly higher (about 1,2 times) than that in the control group. The lead level of the two groups was much higher than the normal level. Conclusion: The method DP-ASV/BiFE in situ was applied successfully to determine lead level in blood and 24h urine. The investigative results of related information showed that: the risk of chronic lead poisoning in studied community very high.
