2.Effects of different target effect-site concentrations of sufentanil on MACBAR of desflurane in patients undergoing abdominal surgery

Objective To evaluate the effects of different target effect-site concentrations (Ces) of sufentanil on the minimum alveolar concentration of desflurane inhibiting stress responses to skin incision (MACsAR) in 50% of patients undergoing abdominal surgery.Methods Eighty-three patients,aged 20-60 yr,scheduled for elective abdominal surgery with an expected incision longer than 10 cm,were enrolled in the study.All the patients were randomly allocated to one of three groups using a random number table:control group (group C,n =28),sufentanil with target Ce of 0.1 ng/ml group (group S1,n =27),and sufentanil with target Ce of 0.3 ng/ml group (group S2,n =28).After tracheal intubation,desflurane inhalation was started,and sufentanil was infused at the preset target Ces in S1 and S2 groups.The initial end-tidal concentration of desflurane was 9.0% in group C,and 6.0% in S1 and S2 groups.The target concentration was maintained at least for 15 min.The modified up-and-down method was used to perform the test.The end-tidal concentration of desflurane was adjusted in the next patients according to the response to skin incision.The MACBAR and 95% confidence interval of desflurane was calculated according to the up-and-down method.Mean arterial pressure and heart rate were monitored and recorded and rate-pressure product was calculated.Results The MACBAR (95% confidence interval) ofdesflurane was 11.2% (11.1%-11.3%),7.8% (7.7%-7.9%),and 4.2% (4.1%-4.3%) in C,S1 and S2 groups,respectively.MACBAR and rate-pressure product of desflurane were significantly lower in S1 and S2 groups than in C group,and in S2 group than in S1 group (P < 0.05 or 0.01).Conclusion The optimum target Ce of sufentanil is 0.3 ng/ml when combined with desflurane in the patients undergoing abdominal surgery.
3.Clinical features of invasive pneumococcus with resistance to antimicrobial agents in Pediatric Intensive Care Unit

Objective To study the clinical features of invasive pneumococcus disease (IPD) with resistance to antimicrobial agents in children,and to improve the diagnosis and treatment of this disease.Methods The clinical data from 21 IPD patients younger than 13 years old were collected from January 2008 through December 2010 in Pediatric Intensive Care Unit in Beijing Children's Hospital for retrospective analysis. Specimens of blood,pleural effusion,cerebrospinal fluid and soft tissue aspirated were collected from these children,and 23 strains of streptococcus pneumonia (SP) were cultured,isolated and confirmed,and the antibiotics susceptibility to penicillin and other antibiotics of these strains were assayed.Results Among the 21 IPD children,the ratio of male to female was 0.9∶1,and the age was 5 months to 13 years,with 61.9% of them under 2 years.Of them,12 patients (57.1% ) had purulent pleurisy,and 1 (4.8% )patient had an underlying disease diagnosed to be X - linked agammaglobulinemia (XLA).There was no seasonal difference in the occurrence rate of this disease. Eight (38.1%) patients were cured,11(52.4% ) were improved,while 2 (9.5% ) patients not improved without death.There was no statistically significant difference in the annual detection rate of invasive SP (x2 =3.711,P =0.156).The incidences of penicillin-intermediate susceptibility SP (PISP) and penicillin-resistant SP (PRSP) were 47.8% and 26.1% respectively.The rate of resistance to multiple antibiotics was 91.3%.Conclusions Children aged less than 5 years,especially younger than 2 years,are prone to IPD,and purulent pleurisy and septicemia are often seen in this disease. Some patients had the underlying diseases.The complications included hemophagocytic syndrome,acute respiratory distress syndrome,septic shock,bronchial pleural fistula and so on.The multidrug resistance rate was 91.3%.It is important to put great emphasis on the monitoring antibiotics resistance to invasive pneumococcal disease.
4.Endovascular therapy for Stanford type B aorta dissection

Objective To evaluate the methods,applicability and results of cndovascular therapy for Stanford type B aortic dissection.Methods The clinical data of 36 cases of Stanford type B aortic dissection were analysed retrospectively.The stent-grafts were introduced via femoral arteriotomy.Stent-grafts were implanted to blockade tears of aortic dissection under the guidance of DSA.Results In all cases,the aortic dissection tears were blockaded,true lumens were opened,and organs function was restored.There were not complications such as internal hemorrhage due to trauma or stent displacement on CTA 3 and 12 months after endovascular therapywhile 2 cases developed new tears.The caliber of the 36 true lumens was restored to near normal.Twenty-two pseudocoeloms were obliterated,while the others were reduced is size and had varying degrees of thrombosis.Conclusions Endovascular thcrapy is safe and effectivc for the patients with Stanford type B aortic dissection.Its clinical application has a favorable outlook.
5.Application of airway pressure release ventilation in severe pneumonia-related acute respiratory distress syndrome in children

Objective To investigate the effects of airway pressure release ventilation (APRV) in children with severe pneumonia-related acute respiratory distress syndrome(ARDS).Methods Ten children suffering severe pneumonia-related ARDS with APRV were included in Pediatric Intensive Care Unit, Beijing Children's Hospital,Capital Medical University from March 2011 to October 2014.Ventilation variables, changes of airway pressure and Ramsay scores were collected and compared with that in conventional ventilation (CV).Clinical variables were measured at CV before APRV and at 1,4,12,24 hours after transition to APRV.Results High airway pressure(Phigh) at each time point during APRV was significantly lower than peak airway pressure (Ppeak) or plateau airway pressure (Pplat) in CV[(26.00 ±2.94) cmH2O(1 cmH2O =0.098 kPa) ,(24.40 ±3.34) cmH2O,(23.30 ±3.46) cmH2O,(23.00 ± 3.80) cmH2O vs (31.80 ± 5.59) cmH2O, P < 0.01].Mean airway pressure (Pmean) at each time point during APRV was significantly higher than that in CV [(23.00 ± 2.86) cmH2 O, (21.69 ± 3.12) cmH2 O, (20.89 ± 3.31) cmH2 O, (20.46 ± 3.48) cmH2 O vs (17.50 ± 2.37) cmH2 O, P < 0.05].Fraction of inspired oxygen (FiO2) were significantly decreased at 4, 12 and 24 hours after APRV than that in CV [(73.00 ± 22.39) %, (63.50 ± 20.16) %, (63.00 ± 21.11) % vs (88.00 ± 15.49) %, P < 0.05].Ramsay scores were significantly decreased at each time point during after APRV than that in CV [(3.90 ± 0.74) scores, (2.90 ± 0.88) scores, (3.00 ± 1.15) scores,(3.50 ± 0.71) scores vs (4.60 ± 0.52) scores, P < 0.05].Conclusions Compared with CV, APRV had a lower Phigh and FiO2 ,a higher Pmean and more shallow sedation.APRV may be an effective ventilation mode in children's severe pneumonia-related ARDS.
6.The study of near and long term changes of bone cement and cancellous bone after percutaneous vertebroplasty

Objective To observe the research of the biologically performance,interface micro-structure and nano-indentation situation when polymethyl methacrylate bone cement-PMMA injected into rabbit in the near future and far future individually.Methods New Zealand white rabbits were used ovariectomy plus dcxamethasone intramuscular injection to establish osteoporosis animal model.All animal models were divided into 3 groups with 35 in each osteoporosis group,testing group,and normal group.Execute the lumbar centrum of New Zealand rabbit as osteoporosis bone defect model,injected bone cement,with the help of material dynamo experiment machine to do the axial compression tests,three point bending test,and torsion test in order to observe the biological dynamic changes.Optical microscope,fluorescence microscope and scanning electron microscope were used to observe the changes of micro bone cement and the changes of bone looseness.Nano-indentation testing was applied by the triboindenter to study the critical load.Results PMMA can rapidly establish the strong support with stable function in the near future.Biomechanical experiments showed that biomechanical properties of bone cement group was significantly higher than those in the other two groups.Biomechanical properties of bone cement group may decline with the time,but which were still better than those of OP in the control group.Histo-morphological observation result shows that under osteoporosis state the bone grows slower,bone's rebuilding time also extended.And in the later period,main bone's continuous osteoporosis had some impact on the interface.Nano-indentation testing showed that the young modulus and stiffness of the interface among bone,material and interface were significantly differences.Bone cement had showen the best nano indentation hardness,then was interface and bone tissue.Conclusion The polymethyl methacrylate bone cement llⅢ has a good effect in filling,on account of it has great plasticity and liquidity and can penetrate into the minuteness aperture between trabecula,then after solidifying,it will combine firmly with the host bone.PMMA can rapidly establish the strong support with stable function after operation.The performance is decreasing in the far future but still with satisfaction.
7.Construction and validation of a finite element model of pelvis after subtotal sacrectomy

BACKGROUND:For the sacroiliac joint has complex structure and locates deep,its biomechanical properties have not been fully understood,even less about the study of reconstruction.OBJECTIVE:To construct a finite element model of pelvis after subtotal sacrectomy,and validate its effectiveness,which is supposed to be useful for evaluating reconstructions of sacroiliac joint.TIME AND SETTING:The establishment and validation of three-dimensional finite element model was performed in the Orthopaedic Laboratory of Changhai Hospital,Second Military Medical University between January and March 2008.MATERIALS:The CT scan images of adult males who had no pathological changes in lumber and pelvis were downloaded from CT work station in Changhai hospital.The CT scan included low lumber and whole pelvis,and got 132 DICOM images in all.METHODS:Two three-dimensional finite element pelvis models were established:① an intact pelvis model,② a defective pelvis model on which subtotal sacrectomy was performed cephalic to the S1 neural foramina.According to the biomechanical experiment of Hugate RR Jr,axial loads of 3 000 N and 1 000 N were applied on intact and defective pelvis models,respectively.The max axial displacements and stiffness on the L5 of two models were calculated and following comparison with the result in the literature.MAIN OUTCOME MEASURES:The max axial displacements and stiffness on the L5 were observed and considered as the index reflecting sacroiliac joint stability.RESULTS:The max axial displacements of the L5 on intact and defective pelvis models were 7.99 mm and 7.68 mm,respectively.The stiffness of the L5 on intact and defective pelvis models were 375 N/mm and 130 N/mm,respectively.The max axial displacements of the L5 on intact and defective pelvis models in the literature were(10.73?5.10) mm and(11.71?5.74) mm,and the stiffness were(353?231) N/mm and(101?49) N/mm,respectivly .CONCLUSION:In this experiment,we were successful to establish the finite element model of pelvis after subtotal sacrectomy.The calculated result was similar with the reference.The model could be used to evaluate different ways to reconstruct the sacroiliac joint.
8.Combined use of RGD-peptide modified PLGA and TGF-beta1 gene transfected MSCs to improve cell biobehaviors in vitro.

In order to improve the surface properties of PLGA polymer for a better material/cell interface to modulate the cells behaviors, we prepared a novel three-block copolymer, PLGA-[ASP-PEG], and immobilized an RGD-containing peptide, Gly-Arg-Gly-Asp-Ser-Pro-Cys (GRGDSPC) on the surface of it. Transforming growth factor-beta1 (TGF-beta1) was transfected into bone marrow stromal cells (MSCs) employed as seeded cells. Cell adhesion, spreading, proliferation and differentiation on this material were investigated. The results showed that the cell adhesive ratio on RGD-modified materials was higher than on un-modified materials (P<0.05). The extent of cell spreading was also wider on RGD-modified materials than on un-modified materials. Cell proliferation indices of transfected MSCs were increased as compared with the un-transfected MSCs (P<0.05). The ALP activities in the MSCs cultured with RGD-modified materials were higher than on un-modified materials after 14 days (P<0.05), and those in transfected MSCs were higher than in un-transfected MSCs (P<0.05). It was suggested that the combined use of RGD-modification and TGF-beta gene transfection could improve the interaction of biomaterial and cells.
9.Correlation of elevated serum exosomal miR-27a levels with drug resistance and adverse outcomes in pulmonary tuberculosis

Abstract: Objective To investigate the correlation of serum exosomal microRNA (exomiR)-27a with drug resistance and adverse outcomes in pulmonary tuberculosis (PTB), to provide new evidence for the development of anti-tuberculosis treatment. Methods From May 2018 to June 2020, 326 patients with PTB in the Eighth Affiliated Hospital of Xinjiang Medical University were selected and divided into 228 patients with active TB (active group) and 98 patients with latent TB infection (latent group), and 100 healthy subjects were included as controls. The serum exosomes of all subjects were extracted and identified by transmission electron microscopy, nanoparticle particle size analyzer and flow cytometry. The expression level of serum exomiR-27a was detected by real-time quantitative PCR. The diagnostic value of serum exomiR-27a in PTB was analyzed by receiver operating characteristic (ROC) curve. Drug resistance in patients with active PTB was recorded after standard treatment. The effect of serum exomiR-27a on the incidence of adverse outcomes was analyzed. Results The characterization of serum exosomes confirmed that exosomes were successfully isolated from serum. Compared with the control group 0.92(0.63, 1.17), the expression of serum exomiR-27a was up-regulated in the active group 1.55(1.18, 2.09) and the latent group 1.27 (0.96, 1.65), and the expression of exomiR-27a in the active group was higher than that in the latent group (P<0.05). PTB patients with high serum exomiR-27a expression (≥1.50) had poorer chest radiography and a higher proportion of chronic hepatitis comorbidities (P<0.05). ROC curve analysis showed that the area under the curve (AUC) of serum exomiR-27a distinguishing control and active group was 0.838 (95%CI: 0.795-0.880) and the AUC of serum exomiR-27a distinguishing normal and latent group was 0.766 (95%CI: 0.701-0.830). Patients in the active group received standard antituberculosis therapy (≥6 months). The expression level of serum exomiR-27a in drug-resistant patients was higher than that in sensitive patients (P<0.001), and the expression level of serum exomiR-27a in multi-drug resistant tuberculosis patients was higher than that in single-drug resistant tuberculosis patients (P<0.001). Logistic regression analysis showed that elevated serum exomiR-27a was an independent predictor of PTB resistance (P<0.001). High expression of serum exomiR-27a indicated a higher incidence of adverse outcomes (log rank=15.725, P<0.001). Conclusions High expression of serum exomiR-27a is helpful in the diagnosis and prediction of adverse outcomes of PTB.

The content, procedures and methods of the video data collecting have been related and analyzed in this article to provide reference for the Chinese medicine resources survey.
